The case is reported of a woman who died at the age of 36 years from obstructive respiratory failure due to diffuse tracheobronchial amyloidosis which had caused symptoms for six years. When first seen her symptoms of wheezing cough and mucopurulent sputum sometimes streaked with blood were of recent onset, but on bronchogram and bronchoscopy her disease was already widespread. Appearances at bronchography were interpreted as malformation and at bronchoscopy as tracheobronchitis, possibly tuberculous. Bronchoscopic biopsy was necessary to make the diagnosis of amyloidosis. The amyloid gave the

The appearance on and spread of Group A streptococci among different body sites in relationship to the development of impetigo were studied prospectively in 31 children in five families. During July and August 1969 intensive clinical, bacteriological, and serological observations were made, including cultures taken at least every other day. In individual children, site sequence of spread of Group A streptococci was from normal skin to lesions and finally to respiratory tract. Streptococci were recovered from normal skin before development of lesions (mean interval of 10 days) in 74% of episode

Prognosis for life in the Landry-Guillain-Barré-Strohl syndrome is dependent upon the development of respiratory and non-respiratory "complications" and their successful management. Review of the literature, a case history, and a study of 14 patients with this syndrome at the University Hospital, Edmonton, indicate that "complications" can be anticipated in virtually all areas of acute care management, including respiratory, gastrointestinal, urinary tract, central and autonomic nervous systems, metabolic, cardiovascular, and infectious disease. The proper management of patients with the Landr

Pulmonary alveolar microlithiasis is a rare condition of unknown aetiology which is emerging from obscurity and being more frequently recognized. The case histories of two patients, one of whom is the oldest reported case in the literature, are described. In one case extensive fibrosis of interstitial tissue was present and many calcospherites appeared to lie outside the alveolar spaces. Calcospherites were also found in the bronchial wall of this patient, and this has not previously been reported. The lungs were analysed for calcium, phosphate, and iron, and detailed respiratory function test

In order to determine the cause of a perinatal death it is important to correlate the postmortem findings with the clinical histories of both mother and infant. The postmortem examination of a newborn infant differs from that of an adult in a number of ways, especially in the method of removal and dissection of the brain, the examination of the cervical spine, the dissection of the cardiovascular system for congenital abnormalities, and the examination of the placenta. A systematic technique for performing a perinatal necropsy is described. Stress is laid on measurement of the size of the infa

a survey of respiratory disease among male physicians of London, Ontario, resulted in a 96.3% response. The age-standardized rates of chronic bronchitis were not very different from others reported in the recent medical literature, taking into account smoking habits, but the overall prevalence of bronchial asthma was high (7.4%), with a low prevalence in the category "obstructive lung disease". The possibility of overlap or interchange in these diagnoses is raised, although the diagnosis of bronchial asthma in this particular group is believed to be well established in every case.A history of s

A new method was used to measure immunoglobulin and total protein concentrations in nasal secretions of normal infants and infants with acute respiratory infections and with allergies. A measured quantity of inulin was added to the administered wash solution in order to determine the dilution of the nasal secretions washed from the infants. In normal infants IgA concentration was less than IgG concentration. In nasal secretions from infants with acute respiratory infections IgA concentration was higher and IgG concentration was lower than in secretions from normal infants. Nasal secretions wer

Pleural plaques were found in 644 (6·6%) of 9,760 photofluorograms taken in 1965 in a region of Pelhřimov district; the incidence was highest in the age group 66-70 years. The advanced age of those affected may be explained by the greater frequency of the causative agent in the past. The disorder was known in Pelhřimov district as early as 1930; it was then thought to be posttuberculous. The past history of the cases was uninformative; as a rule, the only common previous disease was pleurisy with effusion, occurring in 9·7%. The general condition of those affected was excellent; only 8% were a

The formation of posterior pharyngeal pouches is generally attributed to dysfunction of the cricopharyngeal sphincter. The reason for this dysfunction and its exact nature have not been established. Observations in the Royal Victoria Hospital, Belfast, suggested that an association exists between pharyngeal pouch and hiatus hernia. Barium studies performed in patients with a pharyngeal pouch using a described technique demonstrated the presence of a hiatus hernia in 32 out of 34 patients. Illustrative case histories and a review of published work on cricopharyngeal function are provided to sup

As is known, the inhalation of animal hairs can provoke immunological reactions in the respiratory tract affecting the naso-tracheo-bronchial sector and giving rise to asthma-like syndromes. Another form of disease, found in furriers with long exposure to `hair dust', is described. It is characterized by a granulomatous interstitial pneumonia, of the tuberculoid type, very similar to that described in other diseases related to the inhalation of organic dusts, both vegetable and animal, such as `farmer's lung' and `bird fancier's lung'. This new disease—which we experimentally reproduced—can be

The history and the clinical and necropsy findings are presented of a patient who died from Aspergillus fumigatus pyaemia occurring as an opportunistic infection complicating a dissecting aortic aneurysm. The diagnosis was made (and treatment instituted) during life: it rested upon repeated isolation of the organism from tracheal aspirations, a positive blood culture and positive serum precipitin reactions to A. fumigatus. Debilitating disease along with large doses of antibiotics and corticosteroids provided the conditions necessary for the normally saphrophytic fungi to become pathogenic. As
