Hemoglobin-to-red blood cell distribution width ratio is inversely associated with prevalent gastrointestinal cancers in NHANES and serves as an informative feature in machine-learning models.

BACKGROUND: The hemoglobin-to-red blood cell distribution width ratio (HRR) is an inexpensive index derived from routine blood tests, but its relationship with gastrointestinal (GI) cancers in the general population remains unclear. METHODS: We pooled NHANES 2001-2018 data (adults ≥ 20 years). GI cancer history was screened using MCQ220 and site-confirmed using site-specific follow-up items for colon, rectal, stomach, esophageal, liver, and pancreatic cancers. HRR was calculated as hemoglobin (g/dL) divided by RDW (%). Survey-weighted logistic regression estimated odds ratios (ORs) and 95% CI

Association of a history of type 2 diabetes with short-term mortality in patients with decompensated cirrhosis.

BACKGROUND: The association between type 2 diabetes mellitus (T2DM) and mortality in patients with liver cirrhosis remains controversial. This study aimed to evaluate whether a history of T2DM influences short-term all-cause mortality in hospitalized patients with decompensated liver cirrhosis (DLC). METHODS: We conducted a retrospective study of hospitalized patients with DLC at three medical centers in China between November 2019 and June 2024. The study endpoint was the 6-month mortality. The association between T2DM and mortality was assessed using multivariable logistic regression, and s

Duplicated Appendix With Mucocele Mimicking an Appendiceal Neoplasm in a 45-Year-Old Woman: A Case Report.

BACKGROUND Appendiceal duplication is an extremely rare congenital anomaly, with an incidence of 0.004% to 0.009%. An association with an appendiceal mucocele is even rarer. It can complicate the diagnosis and management of right iliac fossa pathology and remains difficult to identify preoperatively. Failure to recognize it can result in recurrent symptoms, missed pathology, intraoperative challenges, and medicolegal consequences. This report describes the case of a 45-year-old woman with history of a loculated peri-appendiceal abscess managed with percutaneous drainage and intravenous antibio

Multifactorial Contributors to Diagnostic Delay in a High-Risk Patient with Hepatocellular Carcinoma: A Case Report with Clinical Lessons.

BACKGROUND: Patients at high risk for primary liver cancer (PLC) are recommended to undergo PLC surveillance at least every six months. Alpha-fetoprotein (AFP) is a commonly used biomarker; however, approximately 15-30% of PLC cases remain AFP-negative throughout their course. Therefore, surveillance in high-risk populations should be based on liver imaging. Any newly detected or progressive lesion should be further characterized by contrast-enhanced computed tomography (CT), contrast-enhanced magnetic resonance imaging (MRI), or contrast-enhanced ultrasound, following guideline-based classifi

Genetic and functional characterization of a novel SERPIND1 variant in a family with isolated gastric varices and venous thrombosis.

BACKGROUND: Isolated gastric varices type I (IGV1) is a rare but severe manifestation of splenic vein thrombosis. While thrombophilia is a recognized cause, the underlying genetic determinants are often elusive. This study investigates the case of a proband presenting with IGV1, deep vein thrombosis (DVT), and Long QT syndrome Type 2 (LQT2), aiming to elucidate the genetic basis and functional impact of the identified SERPIND1 variants. METHODS: Whole-exome sequencing identified candidate variants in the proband, which were validated by Sanger sequencing. Functional assessment included quanti

Non-adherence to diagnostic colonoscopy after positive faecal screening test.

INTRODUCTION: The Danish Colorectal Cancer Screening Programme has achieved a high adherence, in the 88-90% range, to diagnostic colonoscopy following a positive immunochemical faecal occult blood test (iFOBT). However, a small subgroup does not proceed to colonoscopy. This study investigated factors associated with non-adherence to diagnostic colonoscopy in the North Denmark Region (NDR). METHODS: In 2020, 278 had a positive iFOBT in the NDR and did not undergo subsequent colonoscopy. We conducted a two-part study consisting of 1) a journal audit assessing comorbidity, inflammatory bowel dis

Case Report: mNGS-negative Strongyloides hyperinfection syndrome with small bowel obstruction as the initial manifestation in a glucocorticoid-exposed COPD patient: a fatal case.

BACKGROUND: Strongyloides hyperinfection syndrome (SHS) is a life-threatening condition in immunocompromised hosts with an extremely high mortality rate. Metagenomic next-generation sequencing (mNGS) demonstrates limited sensitivity for parasitic detection, and its clinical value remains to be fully elucidated. The conventional microscopy remains an indispensable tool for the diagnosis of parasitic infections. CASE DESCRIPTION: A 73-year-old male patient with a history of chronic obstructive pulmonary disease (COPD), diabetes mellitus, hypertension, and cerebral infarction presented with abdo

Development and a single-centre temporal validation of a machine learning-based model for early prediction of acute pancreatitis-associated liver injury.

BACKGROUND: Acute pancreatitis (AP) is a common acute abdominal inflammatory disorder characterized by premature activation of pancreatic enzymes, leading to pancreatic autodigestion, edema, hemorrhage, and even necrosis. The liver, as the first portal organ receiving pancreatic venous drainage, is one of the most frequently affected extra-pancreatic organs in AP. AP-associated liver injury (LI) is closely correlated with the intensity of systemic inflammation, the development of multiple organ dysfunction, and increased mortality. While machine learning (ML) models have been successfully appl

Effect of uric acid and estimated glomerular filtration rate on the Fibrosis-4 index: A cross-sectional and cohort analysis.

Hepatic steatosis and fibrosis present a significant public health burden, especially in China. Emerging evidence suggests an association between liver fibrosis and kidney function, indicating a potential relationship among uric acid (UA) levels, estimated glomerular filtration rate (eGFR), and the Fibrosis-4 (FIB-4) index. This retrospective cohort study investigates the correlation between UA, estimated GFR, and FIB-4 index in Chinese adults with metabolic dysfunction-associated steatotic liver disease (MASLD) using cross-sectional and longitudinal analyses. A total of 5051 adults with MASLD

Complete remission of minimal change disease after antiviral monotherapy in a patient with immune-active chronic hepatitis B: A case report.

RATIONALE: Minimal change disease (MCD) is characterized by diffuse podocyte foot process effacement on electron microscopy, minimal structural changes on light microscopy, and the absence of immune complex deposition on immunofluorescence. In adults, MCD accounts for approximately 10% to 25% of nephrotic syndrome cases and is typically idiopathic, although secondary causes such as infections, medications, malignancies, and immune disorders have been reported. The association between hepatitis B virus (HBV) infection and glomerular disease has been well established in membranous nephropathy an
