Effects of gonadal steroids in women with a history of postpartum depression.

OBJECTIVE: Endocrine factors are purported to play a role in the etiology of postpartum depression, but direct evidence for this role is lacking. The authors investigated the possible role of changes in gonadal steroid levels in postpartum depression by simulating two hormonal conditions related to pregnancy and parturition in euthymic women with and without a history of postpartum depression. METHOD: The supraphysiologic gonadal steroid levels of pregnancy and withdrawal from these high levels to a hypogonadal state were simulated by inducing hypogonadism in euthymic women-eight with and eig

Prevalence and clinical significance of postpartum endometritis and wound infection.

OBJECTIVE: To correlate clinical variables (gestational age, severe pregnancy-induced hypertension, gestational diabetes mellitus, history of previous cesarean sections, fetal distress, perinatal mortality, postpartum anemia, Apgar score < or = 3 at 1 minute and < or = 7 at 5 minutes, and instrumental delivery) with postpartum endometritis (PPE) and wound infection. METHODS: Descriptive cross-sectional study of the outcome of 75,947 term and preterm singleton deliveries; vaginally and by cesarean section from 1989-1997. RESULTS: The prevalence of PPE after vaginal deliveries was 0.17% (120/6

[Anesthetic management of parturients with cerebrovascular diseases].

From January 1992 to December 1997, 13 parturients with cerebrovascular diseases had childbirth at our institution. Among them, 8 patients received anesthesia for delivery. Five patients had a history of ruptured arteriovenous malformation (AVM), cerebral aneurysm, or intraventricular bleeding due to moyamoya disease, and they had radical operations. Of these 5 patients after radical operations, three had a repeat cesarean section under spinal anesthesia, and two had a vaginal delivery under epidural anesthesia to avoid excessive hypertension and hyperventilation. There were two patients with

[Porphyric crisis: experience of 30 episodes].

The experience of 30 porphyric crisis is reviewed in 25 patients attended since 1967: 21 patients had 1 crisis, 3 had 2, and 1 had 3 of these episodes. In all patients, porphyria was diagnosed in relation to one crisis, even though many of them had family histories and/or previous clinical symptoms of this disease. There was clear predominance (80%) of women, but they are also a majority among acute porphyrias. The most frequent symptoms were: abdominal pain, tachycardia, dark urine, neurological and psychiatric alterations and arterial hypertension. The neurological alterations required the u

[Psychiatric disorders and illnesses after childbirth].

After childbirth, from about a quarter up to nearly one half of all puerperae develop a short-lasting, mild affective distress, the so called blues. During the first months after delivery, about 10-15% of all young mothers suffer from a longstanding depression which is so severe that they are in need of treatment. In one or two out of 1000 women even a psychotic disorder becomes manifest. These postpartal disturbances and diseases are often not diagnosed by doctors-on the one hand because the women concerned often hide their complaints due to shame and a sense guilt regarding their supposed fa

[Postpartum psychiatry].

The four important syndromes that constitute postnatal psychiatry are chronologically: 1) Post-Traumatic Stress Disorder (PTSD), 2) Baby-blues, 3) Puerperal psychosis and 4) Postnatal depression. The PTSD (prevalence 1%) occurs from 24 to 48 hours after a difficult childbirth. Anxiety predominates in association with nightmares and recurrent images of the experience. Baby-blues (prevalence 80%) is a feeling of sadness that doesn't need to be treated. It occurs around the third day of post-partum. Its intensity is a predictive factor of postnatal depression. Puerperal psychosis (prevalence 0.2%

The effect of normal childbirth on eyes with abnormalities predisposing to rhegmatogenous retinal detachment.

BACKGROUND: Pregnant women who have high myopia, a history of retinal detachment or retinal holes, or have known lattice degeneration are frequently referred to an ophthalmologist for advice concerning the management of pregnancy and labor, i.e. whether a spontaneous vaginal delivery can be allowed and whether prophylaxis for high-risk retinal pathology is indicated. Many obstetricians still believe that pregnant women with ocular abnormalities predisposing to rhegmatogenous retinal detachment should have an instrumental delivery, and a few even advocate cesarian section. Very little has been

[Psychoses in puerperium: symptoms, course and long-term prognosis].

86 women who suffered from a psychotic disorder for the first time in their life within 6 weeks after parturition, were investigated regarding sociodemographic and gynaecological parameters, time of onset and initial symptomatology, psychopathological symptoms during hospitalisation, treatment and short-term outcome of the illness. The majority of patients became ill after the first (75%), full-term normal (63%) delivery and within the first two postpartum weeks (78%). The most frequent initial symptoms were anxiety, restlessness, depressive mood, sleep disturbances, behaviour disturbances, ca

[Genital herpes and childbirth].

Neonatal herpes is a serious infection due to the risk of diffusion and damage to the central nervous system. Depending on the author, the incidence of herpetic infection reported ranges from 1/7,500 to 1/1,000,000 neonates. type II herpes virus is responsible for three quarters of infections. The risk or in-utero damage is low, and does not exceed 5 percent of cases. Contamination usually occurs during delivery or during the immediate post-partum period. The clinical forms of neonatal infection consist essentially of diffuse forms, located in the central nervous system, and localized forms in

The bible illustrates some interesting views on the matter of Gynecology and Obstetrics. Beside Old and New Testaments, some Apocryphas have to be discussed for their contents about this subject. This article concerns the following points: impregnation, formation of the human embryo, Eve's creation, indirect fecundation and spermatic pollution, bi-sexuality of the first man, libido, signs of virginity, female infertility, incest, multiparity, post-menopausal conception, the fructifying powers of amulets, plants and seeds, the course of pregnancy and antenatal care, normal and difficult labour,

Maternal mortality in pre-Columbian Indians of Arica, Chile.

A study of female mummies representing seven pre-Columbian Andean populations of Arica, Chile, dating from 1300 B.C. (Azapa phase) to A.D. 1400 (Gentilar phase), disclosed that 18 (14% of the total) had died from childbirth-complicated death (CCD). The majority of these individuals appear to have died during puerperium, three of them without complete delivery. Higher rates of CCD were found in the earlier cultural phases (1300 B.C.-A.D. 600) after which the maternal mortality rate decreased. The precise causes of this mortality are not known, but septic conditions, acute diseases, and cultural

Maternal mortality in Indonesia and Egypt.

Twenty-three percent of deaths to women of reproductive age (15-49 years) in Bali, Indonesia and Menoufia, Egypt were due to maternal causes. Among the younger women, the percentage was even higher. In both areas complications of pregnancy and childbirth were a leading cause of death (the first cause in Bali, the second in Menoufia). In both sites, postpartum hemorrhage was the most common cause of maternal death. Relative to the United States, the number of maternal deaths per 100,000 live births was 20 times higher in Menoufia and 78 times higher in Bali. Families of women of reproductive ag

Risk of multiple sclerosis exacerbation during pregnancy and breast-feeding.

Studies in the past have reported an increased risk of exacerbations in multiple sclerosis during the postpartum period; it is not known whether breast-feeding alters this risk. We interviewed 435 women regarding pregnancy and breast-feeding history, providing for analysis 191 pregnancies that had occurred during a nonprogressive phase of the disease. The exacerbation rates during the nine-month postpartum period (34%) were more than three times the exacerbation rate during the nine months of pregnancy (10%). The exacerbation risk was highest in the three-month period following childbirth and

Epidemiology of puerperal psychoses.

Computer linkage of an obstetric register and a psychiatric case register made it possible to investigate the temporal relationship between childbirth and psychiatric contact in a population of 470 000 people over a 12-year period: 54 087 births resulted in 120 psychiatric admissions within 90 days of parturition [corrected]. The 'relative risk' of admission to a psychiatric hospital with a psychotic illness was extremely high in the first 30 days after childbirth, particularly in primiparae, suggesting that metabolic factors are involved in the genesis of puerperal psychoses. However, being u

Cyclothymic disorder and bromocriptine: predisposing factors for postpartum mania?

Women are most susceptible to psychotic reactions during the postpartum period, a time of intense psychological and physiological stress. Mania and depression are particularly common at this time, especially in women with past or family histories of major or minor affective disorders, specifically cyclothymia and dysthymia. Close attention after childbirth is warranted to alleviate and prevent these episodes in such women. Sympathomimetic drugs such as bromocriptine and the over-the-counter diet aids, which are linked to the induction of manic episodes, are frequently used in the puerperal per

Osteoporosis associated with pregnancy and lactation: bone biopsy and skeletal features in three patients.

Case reports of three young patients who developed vertebral fractures and skeletal complications during pregnancy and/or lactation are presented. Radiologic features are described. All three had severe disease with three to nine vertebral fractures at presentation postpartum. In two patients, follow-up for 5-7.8 yr (including further pregnancy in each) revealed no further fractures. In general, serum and urine features were normal, the exceptions being a low serum 25-hydroxyvitamin D level (plus intermittent elevation of serum parathyroid hormone) in one, a tendency to low plasma alkaline pho

Galactorrhea in the adolescent.

Inappropriate lactation--that which occurs beyond the usual postpartum or breast-feeding period or in the male--has been of medical interest for centuries. There is a suggestion that the first recorded case occurred in the Bible in the Book of Esther wherein Mordecai may have breast-fed his niece Esther [1,2]. The Talmud describes a man who nursed his infant after his wife's death during childbirth [3]. Lactation in virgin women was noted by the early Greeks [4], and Aristotle observed lactation in men [5]. The phenomenon of "witch's milk," seen in the newborn, was described as early as 1684 b

[Coagulation disorders in obstetric patients at gynaecological hospital of Leipzig University, 1961 through 1978 (author's transl)].

Intrapartum coagulopathy was secured from 41 in 69,336 obstetric patients, between 1961 and 1978. Haemostatic disorders, such as v. Willebrand-Jürgens syndrome, idiopathic thrombocytopenia, plasmatic coagulation defects, and hyperfibrinolysis, had been recorded from ten patients even before parturition. The courses of pregnancy, childbirth, and puerperium of those patients are reported, with reference being made to the need for prepartum hospitalisation as well as for target-oriented substitutional therapy and non-invasive delivery control. - Acute intrapartum haemorrhagic syndrome occurred to

Prevention of maternal death in a Nigerian village.

In a Nigerian village, the expected maternal mortality would be between 6 and 12 per 1000 live births, the most common causes of maternal death being obstructed labor, hemorrhage and eclampsia. Yet, over an 8 1/2-year period, when 2324 births occurred, the rate was 1.72 per 1000, of which 0.43 were due to obstetric complications. The four deaths were associated, respectively, with typhoid fever, tuberculosis and typhoid fever, infectious hepatitis and postpartum hemorrhage with dysentery. Using the experience gained in the village, recommendations are made for reducing maternal mortality in ru
