Penicillin Allergy De-Labeling in Older Adults and Long-Term Care Residents: A Clinical Review.

BACKGROUND: Penicillin allergy labels are common in older adults and long-term care (LTC) residents, contributing to inappropriate antibiotic use, adverse outcomes, and increased healthcare costs. This review examines the prevalence, impact, safety, and barriers to penicillin allergy evaluation and de-labeling in these populations. METHODS: This review synthesizes findings from studies on penicillin allergy prevalence, evaluation, and de-labeling practices in older adults and LTC settings. Articles were identified using searches in PubMed, Embase, and Cochrane databases. Key data presented in

Evaluation of drug provocation tests without prior skin testing in children with suspected penicillin allergy and correlation with PEN-FAST: A single-center study.

PURPOSE: To evaluate patients with suspected penicillin allergy in whom skin testing was omitted during immediate and delayed reactions before drug provocation test (DPT). METHODS: This retrospective study analyzed patients aged 0-18 years with suspected penicillin allergy between 2020 and 2023. Data on hypersensitivity reaction history, laboratory tests, PEN-FAST scores, and DPT records were collected. RESULTS: We evaluated 75 patients (male: 61.3%; median age at index reaction: 4 [range: 1-15] years) with suspected penicillin allergy. Nearly all reactions occurred at home (98.7%) following

Description of the combined evidence-based, theory-based and person-based approaches used to develop a behavioural intervention package to support non-allergist healthcare workers to remove incorrect penicillin allergy records from medical and surgical adult inpatients in a UK hospital.

OBJECTIVES: To develop a behavioural intervention package to support non-allergist healthcare workers (HCWs) to remove incorrect penA records from medical and surgical adult inpatients. This paper describes the development of the penicillin allergy de-labelling (PADL) intervention and the implementation intervention that will support non-allergist-delivered PADL. DESIGN: We combined evidence-based, theory-based and person-based approaches. Qualitative research with healthcare professionals and patients explored barriers and enablers to implementation of the proposed PADL pathway. Key interven

Patients With Nonanaphylactic Penicillin Allergy Are Not at Increased Risk of Allergic Events After Receiving Prophylactic Preoperative Cefazolin Prior to Closed Fracture Repair.

BACKGROUND: The most commonly used prophylactic antibiotic for orthopaedic surgery is cefazolin. As the cefazolin molecule shares beta-lactam structural similarities with penicillin, it is common practice to avoid cefazolin administration to patients with penicillin allergy. We aimed to explore the risk of allergic reactions in patients with penicillin allergy who received cefazolin prophylaxis prior to fracture fixation surgery. QUESTION/PURPOSE: In patients with nonanaphylactic penicillin allergy, is administration of cefazolin associated with increased risk of intraoperative adverse events

Beyond the skin: immunological profiles and infectious complications in ALOX12B-associated autosomal recessive congenital ichthyosis.

BACKGROUND: Pathogenic variants in ALOX12B, a crucial enzyme involved in epidermal lipid processing, are among the most common causes of autosomal recessive congenital ichthyosis (ARCI). Although traditionally considered a cutaneous disorder, the systemic immunological implications of ALOX12B deficiency remain poorly understood. OBJECTIVES: We aimed to broaden the dermatologic and immunologic spectrum of ALOX12B-associated ARCI by characterizing the clinical, immunologic, and genetic features of six patients from three consanguineous families. METHODS: This prospective study included six pat

Penicillin Allergy Delabeling in Pediatric Primary Care: A Multisite Qualitative Study.

OBJECTIVE: Allergy societies recommend penicillin allergy (PA) delabeling via history alone or following direct challenge in children at low risk for true PA. Recent studies demonstrate the safety of PA delabeling in pediatric primary care. We aimed to identify primary care practitioners' attitudes toward PA delabeling and perceived barriers and enablers to PA delabeling. METHODS: We conducted semistructured interviews (n = 11) and focus groups (n = 3) with 29 primary care practitioners of 2 health systems in the northeast United States. We coded transcripts using content analysis and mapped

Effectiveness of a penicillin allergy delabeling program-real-world data from Croatia.

Penicillin allergy is the most commonly reported drug allergy, often leading to unnecessary avoidance of beta-lactam antibiotics, increased use of alternative broad-spectrum antibiotics, and higher healthcare costs. However, studies indicate that over 90% of penicillin allergy labels are erroneous. This study presents real-world data from a penicillin allergy delabeling program conducted at the Special Hospital for Pulmonary Diseases in Zagreb, Croatia. A total of 132 adult patients with a reported beta-lactam allergy were evaluated with a stepwise diagnostic protocol, including medical histor

Breaking the barrier: Transforming the diagnosis and management of penicillin and cephalosporin allergy in China.

OBJECTIVES: β-lactam allergy labels are common in routine care but are variably verified. This manuscript assessed prevalence, clinical features, verification practices, and penicillin-cephalosporin cross-occurrence across multiple hospitals in China. METHODS: We used multicenter cross-sectional questionnaires in outpatient clinics at 22 institutions (n = 6070). Adjudication relied on routine records and prior testing performed in care. "Clinically-confirmed" allergy required a chart-documented reaction and/or a previously positive skin test; indeterminate entries were coded "uncertain." Cros

Efficient pair, hydroxychloroquine monotherapy for tattoo-associated sarcoidosis.

Sarcoidosis is a systemic granulomatous disease which may affect the skin in about one-third of patients. It is characterised by the histopathological finding of non-caseating granulomas. There is a predilection for cutaneous sarcoidosis towards prior trauma sites. Tattoo-associated sarcoidosis may be caused by the injected foreign pigment or its by-products. These reactions are commonly observed with black and red dyes, although other colours have been implicated. Currently, there are no evidence-based treatment regimens of tattoo sarcoidosis; however, the most commonly used treatments leadin

Dissecting the Impact of Anatomy Lab Policies on Pregnant Medical Students.

There has been little research into the unique occupational hazards facing pregnant medical students, particularly surrounding their exposure to formaldehyde during cadaveric dissection. Formaldehyde is potentially teratogenic and mutagenic and has been associated with spontaneous abortion and birth defects. This project was undertaken to investigate the prevalence and content of policies in United States medical schools concerning pregnant students' participation in gross anatomy laboratories. The U.S. News and World Report's top 50 medical schools were systematically reviewed for policies pe

Validation of the PEN-FAST Penicillin Allergy Clinical Decision Rule in an Asian Cohort.

INTRODUCTION: A false penicillin allergy label often leads to the unnecessary avoidance of beta-lactam antibiotics, contributing to antimicrobial resistance and suboptimal clinical outcomes. The PEN-FAST clinical decision rule is a simple, point-of-care tool with a high negative predictive value (NPV) designed to identify low-risk patients who may not require extensive allergy testing. This study aims to validate the performance of the PEN-FAST clinical decision rule in an Asian population. METHODS: A retrospective review of medical records from January 2006 to February 2023 was conducted at

Prospective Multiregional Evaluation of Protocol-Based Penicillin Allergy Delabeling in China: Real-World Outcomes Challenge Mandatory Preemptive Skin Testing Policies.

BACKGROUND: Penicillin allergy is frequently mislabeled, leading to the unnecessary use of second-line antibiotics and contributing to antimicrobial resistance. Preemptive penicillin skin tests (PST) remain mandatory in mainland China and is associated with high false-positive rates. In contrast, Hong Kong follows evidence-based, standardized allergy assessment protocols under the Hong Kong Drug Allergy Delabelling Initiative (HK-DADI). OBJECTIVE: To evaluate longitudinal outcomes of protocol-based penicillin delabeling in real-world clinical practice across mainland China and Hong Kong. MET

Severe allergic reaction to contrast dye during emergent coronary angiogram.

We present a case of a middle-aged woman with an extensive history of adverse reactions to iodinated contrast media (ICM) despite premedication. The patient presented from an outside hospital with an ST elevation myocardial infarction in the inferior and anterior leads. She received thrombolytics and was transferred to our hospital. Due to persistent chest pain and ST elevations consistent with failure of reperfusion with thrombolytics, we elected to pursue emergent catheterisation. Despite administration of intravenous glucocorticoid and diphenhydramine at our hospital, the patient had an ana
