Hypersensitivity to phosphodiesterase-5 inhibition in post-traumatic headache: Evidence of cGMP-dependent signaling.

BackgroundPhosphodiesterase-5 (PDE-5) inhibition increases intracellular cyclic guanosine monophosphate (cGMP), a second messenger molecule implicated in migraine pathogenesis. Given the clinical overlap between migraine and post-traumatic headache (PTH), we investigated whether pharmacologic elevation of cGMP induces migraine-like headache in individuals with persistent PTH.MethodsAdults with persistent PTH and no pre-trauma history of migraine were enrolled in a randomized, double-blind, placebo-controlled, 2-way crossover study. Each participant received a single 100-mg oral dose of sildena

[Dyspnea, pulmonary mosaic sign and pulmonary hypertension].

This case report presents a young female patient whose chief complaint was exertional dyspnea. Right heart catheterization confirmed severe pulmonary hypertension. Concurrently, chest CT showed atypical pulmonary parenchymal and interstitial lesions, and pulmonary function tests indicated severe impairment of diffusion capacity. To clarify whether an association existed between the lung disease and pulmonary hypertension, transbronchial lung cryobiopsy was performed, which revealed multinucleated giant cell aggregates within the alveolar spaces, with cholesterol clefts in the cytoplasm of some

Pharmacogenomics of antibacterial and antiviral therapies: Clinical actionability, evidence gaps, and future directions.

Anti-infective drugs have profoundly transformed the history of medicine. Yet, with the presence of approximately 4.1-5 million interindividual genomic variants in human genome, patients are expected not to respond equally to the same anti-infective drug. This genetic variability, together with nongenetic factors, influences therapeutic outcomes and contributes to drug-induced adverse events in predisposed individuals. Historically, the identification of HLA-B∗57:01 as a predictor of abacavir hypersensitivity in patients with HIV represented the first successful clinical application of pharmac

Clinical and inflammatory determinants of successful abrocitinib dose reduction in atopic dermatitis: a real-world cohort study with implications for treatment optimization.

BACKGROUND: Abrocitinib, a selective Janus kinase 1 inhibitor, is an effective therapy for moderate-to-severe atopic dermatitis (AD). However, evidence guiding dose reduction during long-term maintenance in real-world practice remains limited, and determinants of successful dose tapering are unclear. OBJECTIVE: To evaluate the feasibility of abrocitinib dose reduction and identify clinical and immunological features associated with tapering. METHODS: In this prospective real-world study, 66 adults with moderate-to-severe AD received standard-dose abrocitinib for 12 weeks, followed by individ

Multiple Antibiotic Allergy Evaluation Strategy (MAAES): A Comparative Effectiveness Study.

BACKGROUND: Inaccurate multiple antibiotic allergy labels to first-line agents (penicillins, cephalosporins, and sulfonamides) restrict optimal therapy and often require sequential evaluations across multiple visits, delaying delabeling and increasing loss to follow-up. OBJECTIVE: To conduct a comparative effectiveness study of the Multiple Antibiotic Allergy Evaluation Strategy (MAAES), comparing its effectiveness, safety, and patient-reported outcomes (PRO) with traditional sequential evaluation (non-MAAES). METHODS: This retrospective cohort study (2014-2024) compared same-day multi-antib

Toxic epidermal necrolysis following a furosemide injection: a case report.

BACKGROUND: Toxic epidermal necrolysis is a life threatening adverse cutaneous reaction mostly following drug administration. It is characterized by extensive detachment of skin and mucous membranes with a more than 30% total body surface area involvement. We document a rare occurrence of Toxic epidermal necrolysis following a furosemide injection. CASE PRESENTATION: A one-year-old female from Eastern Uganda was referred to our facility with history of a generalized skin eruption that started 2 days after a furosemide injection. On admission, she had extensive skin sloughing involving the fac

Factors associated with penicillin and sulfonamide allergy and Stevens-Johnson syndrome/toxic epidermal necrolysis.

BACKGROUND: People with HIV (PWH) are more susceptible to drug reactions than the general population. This study aimed to investigate the prevalence of, and demographic characteristics associated with a history of penicillin and sulfonamide allergy and Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN), and to assess whether HLA-B*57:01 is associated with these drug reactions among PWH followed in a large French multicenter cohort. METHODS: All PWH followed from January 2000 to December 2023 with available results for HLA-B*57:01 were included. Logistic regression models were used

Challenges for the effectiveness of infectious disease interventions during global change.

BACKGROUND: Sequential treatment of ankylosing spondylitis (AS) that includes tumour necrosis factor-α antagonists (anti-TNF agents) has been applied in most of the Western countries. Existing cost-effectiveness (CE) models almost exclusively presented the incremental CE of anti-TNF agents using a closed cohort while budget impact studies are mainly lacking. Notwithstanding, information on impact on total population health and societal budget as well as on actual incremental CE for a given decision time span are important for decision makers. This study aimed at quantifying, for different deci

Cross-Reactive NSAID Hypersensitivity: Clinical Findings From Aspirin Provocation and Alternative Drug Challenge Testing.

Nonsteroidal anti-inflammatory drug (NSAID) hypersensitivity reactions are commonly reported but often overestimated due to reliance on clinical history alone. Accurate diagnosis and identification of safe alternative medications are essential for appropriate management. This retrospective study aimed to evaluate the clinical manifestations of NSAID hypersensitivity, assess the diagnostic value and safety of aspirin oral provocation testing, and investigate the tolerability of alternative medications, including acetaminophen, meloxicam, and celecoxib. Patients diagnosed with NSAID hypersensiti

Prescribing carbonic anhydrase inhibitors to patients with "sulfa" antibiotics allergy: do we dare?

OBJECTIVE: To evaluate if provider characteristics affect attitude toward carbonic anhydrase inhibitors (CAI) prescription for patients with history of sulfonamide antibiotic (SA) hypersensitivity. METHODS: A survey querying providers' attitudes toward CAI prescription in hypothetical patients with prior SA hypersensitivity was distributed to four ophthalmology and optometry organizations. Logistic regression was used to assess the relationship between avoiding CAI and profession, specialty, organizational affiliation, and years in practice. RESULTS: Of 250 respondents, 27% and 52% would avo

The drug allergy history tool (DAHT): Validation of a patient-reported survey instrument.

BACKGROUND: While the reaction history is critical for drug allergy evaluations and is typically self-reported, there is no validated survey instrument to collect drug allergy history from patients. OBJECTIVE: We validated a survey instrument that collects patient-reported drug allergy history. METHODS: The drug allergy history tool (DAHT) was revised after 3 rounds of cognitive testing, assessed for reliability through test-retest comparisons, and assessed for quality and validity through a concordance analysis against electronic health record allergist documentation. 5-Aminosalicylic Acid-induced Colitis in a Patient with Rheumatoid Arthritis without Inflammatory Bowel Disease.

This report presents a rare case of 5-aminosalicylic acid (5-ASA)-induced colitis in a 76-year-old man with no history of inflammatory bowel disease (IBD). The patient, who was undergoing treatment for rheumatoid arthritis, developed persistent diarrhea and fever following sulfasalazine (SASP) administration and later experienced a high fever shortly after taking mesalazine. The drug-induced lymphocyte stimulation test result was positive for mesalazine and weakly positive for SASP, thereby supporting the diagnosis of 5-ASA intolerance. This case emphasizes that 5-ASA-induced colitis may occur

Ocular administration of brinzolamide leading to Stevens-Johnson syndrome/toxic epidermal necrolysis overlap: A case report and review.

RATIONALE: Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are severe, life-threatening hypersensitivity reactions most often induced by systemic medications. Reports of SJS/TEN caused by topical ophthalmic sulfonamide-derived carbonic anhydrase inhibitors, such as brinzolamide, are extremely rare. PATIENT CONCERNS: A 31-year-old man underwent pars plana vitrectomy for traumatic retinal detachment in the right eye and subsequently developed secondary glaucoma. He was treated with topical brinzolamide 1% twice daily. On day 6 of treatment, he developed high-grade fever and

Alternative Drug Safety in Children with Nonsteroidal Anti-Inflammatory Drug Hypersensitivity.

INTRODUCTION: Nonsteroidal anti-inflammatory drugs (NSAIDs) are frequently used in the pediatric age group as pain relievers, antipyretics and anti-inflammatory drugs. Since NSAIDs are used in many medical conditions, there is a need for alternative NSAIDs to be used safely in people with hypersensitivity reactions. Selective and partially selective COX-2 inhibitors and weak COX-1 inhibitors are generally used as safe alternative drugs. The aim of this study was to evaluate safe NSAIDs determined by oral provocation tests (OPTs) according to phenotypes in children with NSAID hypersensitivity r

Preoperative survey to evaluate the patients' allergy list and its relevance to perioperative care.

INTRODUCTION: Perioperative hypersensitivity and allergic reactions can result in significant morbidity and mortality. For routine anesthetic care, allergies are determined from a review of the electronic medical record supplemented by a detailed patient history. Although the electronic medical record is generally assumed to be accurate, it may be that allergies are erroneously listed or not based on sound medical practice. The purpose of the current study is to evaluate allergies listed in the electronic medical record of children presenting for surgery and determine their origin, authenticit
