A study of anaphylactoid reactions (AR) observed between September 1982 and September 1983 was carried out in the surgical departments of a French regional hospital. The patients who had presented clinical symptoms suggesting an AR (bronchospasm, collapse, tachycardia, with or without skin rash) during a general anaesthesia were included in this study. A precise history of previous anaesthesias and allergy was taken; allergological testing was carried out six to eight weeks after the AR. We analysed the case histories of 22 patients with life threatening asthma retrospectively. Fifteen patients needed mechanical ventilation. Three patients, all of them in cardiac arrest before or at the moment of admission, died. In five patients, halothane therapy was successfully used as a last resort. No major complications occurred. Two case-reports are presented.

Plasma histamine levels during enflurane anaesthesia were studied in 14 oral surgery patients without any history of allergy and general disease except for the oral region. Plasma histamine was measured using a fluorometric method combined with high speed liquid chromatography. The difference between the histamine levels measured at each time before and during oral surgery was not significant.

A 32-year-old female was admitted for a minor elective surgical procedure. Her past history included at least six uneventful general anaesthetics. Anaphylaxis developed shortly following induction of anaesthesia with thiopentone, Innovar and gallamine. Resuscitation was successful but was complicated by ventricular fibrillation. Full recovery followed. Subsequent allergy skin tests revealed hypersensitivity to thiopentone. Recommendations for investigation of suspected hypersensitivity to anaesthetic agents are included, as are guidelines for the recognition and treatment of anaphylaxis.

A 57-year-old woman with a history of allergy to local anesthetics was referred to us to be admitted for a general anesthetic for removal of a sliver from her finger. Because allergic reaction to local anesthetics often is caused by an anesthetic other than lidocaine, or is due to the carrier vehicle in the diluent, a skin test with pure lidocaine was performed. After showing no reaction, the patient was given the standard dose of lidocaine and the sliver was removed in the emergency department. The patient was discharged home with no sequelae.

A 67-yr-old female patient gave a history of allergy to lignocaine. When she was challenge-tested with the intradermal injection of 0.5% bupivacaine 0.2ml she had a systemic reaction. This reaction was accompanied by a decrease in the concentration of complement C4 in the plasma, which indicated that the reaction was immunologically-mediated. This is the first report of allergy to a local anaesthetic drug which has been documented by concurrent immunological changes.

Severe anaphylactoid reactions during anaesthesia in 116 patients are described. The majority of patients who reacted to induction agents had previous exposure to the drug, while the majority of patients who reacted to muscle relaxants had not. There was a statistically significant increased incidence of allergy, atopy, asthma and previous reactions in patients who had reactions compared with a control group undergoing uneventful anaesthesia. The patient described here experienced an acute anaphylactic reaction after receiving parenteral lidocaine, oral aspirin, codeine, and penicillin following routine dental treatment. The patient had taken all of the drugs previously with no untoward effects. The incidence and mechanisms of drug-induced hypersensitivity reactions are discussed. The importance of monitoring the patient following drug administration, even in the case of a negative medical and drug history, is discussed.

Systemic antihistamines were administered prior to dye injection in 50 patients undergoing fluorescein angiography. The patients were monitored for side effects. Venous blood samples were obtained before and at three, ten and thirty minutes after intravenous administration of sodium fluorescein and analyzed for histamine levels. A 26-year-old man experienced a Gell-Coombs type I allergic reaction to tetracycline hydrochloride, which had been given as therapy for nongonococcal urethritis. The patient had a history of allergy to penicillin and of asthma. The allergic response to tetracycline resolved after administration of epinephrine and diphenhydramine. Since tetracycline is often used for treatment of sexually transmitted diseases, physicians should be aware of the possibility of an allergic response to this drug.

A study population of 1,158 paid adult volunteers was obtained. Prior to patch testing, a history of previous exposure to four allergens also was obtained. Three cases of hypersensitivity to an anaesthetic induction with fazadinium and thiopentone are described. One reaction was fatal. Two patients had documented and the third presumptive previous exposure to thiopentone, and all had a history of allergy (skin rash) to penicillin. None of the patients had received fazadinium before. The possibility that fazadinium contributed to the severity of the reactions is conjectural.

The clinical history and biological investigations of a patient presenting an immune complex disease induced by Peroben are reported. Biological signs were those of a drug-induced lupus syndrome. A provocation test allowed disclosure of its pathomechanism, since during Peroben intake a high C1q binding activity occurred and later regressed, while deposits of IgM and C3 were evidenced in the vessel walls. Complete or partial thrombosis succeeded accompanying a leukocytoclastic vasculitis.

Patients with a history of anaphylactoid reaction to radiographic contrast medium (RCM) are at increased risk for subsequent reaction.
