Comparison of long-term outcomes between opioid-free and opioid-based anesthesia in head and neck cancer surgeries: A pilot study.

UNLABELLED: Background, aim, and objective: Literature is replete with studies evaluating opioid-sparing strategies in head and neck cancer (HNC) surgeries; however, no study has attempted opioid-free anesthesia (OFA). Various long-term outcomes have not been evaluated in any cancer surgeries. This study aimed to compare OFA with standard opioid-based anesthe-sia (OBA) in HNC surgeries. The primary objective was the incidence of chronic post-surgical pain (CPSP), and the sec-ondary objective included Numeric Rating Scale (NRS)-Pain scores, Neuropathic Pain Symptom Inventory (NPSI), Douleur Neu

STEMI in a Patient With Aspirin-Induced Angioedema: A Case for Early Desensitization.

We report a 47-year-old male presenting with inferior ST-segment elevation myocardial infarction (STEMI) and a history of aspirin-induced angioedema. Despite the challenges of antithrombotic management in the context of aspirin allergy, the patient underwent successful primary percutaneous coronary intervention (PCI), followed by early in-hospital aspirin desensitization. This case highlights the feasibility and safety of early desensitization, allowing timely initiation of guideline-recommended dual antiplatelet therapy in a high risk setting.

Gabapentinoids and risk for asthma exacerbations: a nationwide retrospective cohort study.

INTRODUCTION: Despite warnings from the US Food and Drug Administration about respiratory adverse events associated with gabapentinoids, direct evidence of their risk in patients with asthma is lacking. METHODS: Using a national administrative claims database, we prepared two active comparators-new user cohorts of patients with a history of neuropathic or chronic pain and asthma-initiating gabapentinoids and comparator drugs (tricyclic antidepressants (TCAs) or serotonin norepinephrine reuptake inhibitors (SNRIs)). Overlap propensity score weighting was used to control for potential confounde

Evaluating the Safety of a Ketamine Sedation Protocol In a Pediatric Oncology Population in Tanzania: A Quality Improvement Project.

This project aimed to reintroduce and evaluate a pediatric ketamine sedation protocol at a Zonal Referral Center in Mwanza, Tanzania, where nonanesthesia clinicians provide sedation when trained anesthesia personnel are unavailable. The objectives were to assess knowledge retention, confidence, and protocol compliance. The study design used a pre- and postintervention design. Nonanesthesia clinicians at a > 1,000-bed hospital were surveyed to assess their knowledge and confidence about ketamine sedation. Over 10 days, 50 clinicians received training that included lectures and cognitive aid res

Tapentadol-induced urticarial rash in a patient with acute pancreatitis.

Tapentadol, a dual-action opioid, is increasingly used for pain management in acute pancreatitis (AP). Although its common side effects are well-documented, cutaneous reactions are rare. We report a 30-year-old gentleman with AP who developed generalized urticaria following oral tapentadol use. The patient had no prior history of atopy or drug allergies. The rash resolved after discontinuation of tapentadol and administration of a short course of corticosteroids and antihistamines. Cutaneous reactions to tapentadol are extremely rare but should be recognized promptly to ensure timely managemen

Drug hypersensitivity leading to emergency department visit: a comprehensive analysis of clinical features and management.

BACKGROUND/AIMS: Drug hypersensitivity reactions (DHRs) are unpredictable adverse drug reactions that can present with varied clinical manifestations, including anaphylaxis. However, data on DHRs initially diagnosed in the emergency department (ED) and subsequently confirmed by allergy specialists remain limited. This study investigated the clinical characteristics of ED-diagnosed DHRs and the outcomes of culprit-drug identification during outpatient allergy follow-ups. METHODS: We retrospectively analyzed adverse drug reaction reports from the ED of a university hospital in Korea between Jan

Performance of the I3A Scoring System for Risk Stratification in a Retrospective Cohort of Children With Ibuprofen-Induced NSAID Hypersensitivity.

BACKGROUND: Two clinical history-based risk-stratification tools for nonsteroidal anti-inflammatory drug (NSAID) hypersensitivity have recently been developed: the I3A score, a pediatric ibuprofen-specific tool based on age and index reaction characteristics, and the CA4TCH score, a broader multivariable tool applicable across NSAIDs and age groups. OBJECTIVE: To externally validate the I3A score, and compare its performance with the CA4TCH score. METHODS: We retrospectively analyzed data of consecutive pediatric subjects (<18 years) presenting between February 2001 and December 2020 at the

Diagnosing Nonsteroidal Anti-Inflammatory Drug-Hypersensitivity/Allergy and Nonsteroidal Anti-Inflammatory Drug-Exacerbated or Induced Food Allergy Phenotypes in Children and Adolescents.

BACKGROUND: Nonsteroidal anti-inflammatory drugs (NSAIDs) are widely used in children. Hypersensitivity reactions (HRs) to NSAIDs represent a diagnostic challenge, particularly because NSAIDs may act as aggravating factors or cofactors as in two phenotypes recently named NSAID-exacerbated food allergy (NEFA) and NSAID-induced food allergy (NIFA). OBJECTIVE: To diagnose NSAID hypersensitivity/allergy (NH/A) and NEFA/NIFA phenotypes in children and adolescents, classify HRs to NSAIDs according to updated international guidelines, and identify clinical and immunologic predictors of true NH/A. Successful management of protracted anaphylaxis-induced severe acute respiratory distress syndrome masquerading as non-ST elevation myocardial infarction: a rare case report.

BACKGROUND: Prolonged anaphylaxis, especially symptoms that last several days, can lead to misdiagnosis and delays in treatment, especially when patients present with complications such as acute respiratory distress syndrome (ARDS) and secondary myocardial injury. A thorough review of medical history is very important for accurate diagnosis. Early recognition and intervention are essential for managing both the allergic response and pulmonary-cardiac injury. CASE PRESENTATION: We present the case of a 70-year-old woman with no significant medical history who developed rapid respiratory failur

Utility of basophil activation tests in the diagnosis of cross-intolerance to nonsteroidal anti-inflammatory drug reactions.

Background: Nonsteroidal anti-inflammatory drugs (NSAID) are the second most frequent cause of drug-induced hypersensitivity after β-lactam antibiotics. Diagnosing cross-intolerance reactions to NSAIDs remains challenging because conventional allergy tests are not useful and drug provocation tests (DPT), the current criterion standard, are resource-intensive and carry risk of severe reactions. Basophil activation testing (BAT) has emerged as a potential ex vivo diagnostic alternative. Objective: This study aimed to evaluate the diagnostic utility of the BAT for cross-intolerance reactions to N

Older Age Is Associated With Long-Term Retention in Buprenorphine Treatment for Opioid Use Disorder.

BACKGROUND: Opioid use disorder (OUD) among older adults is a fast-growing public health problem. However, little is known about treatment outcomes among older adults in office-based buprenorphine programs. Thus, our objective was to examine how age is associated with buprenorphine treatment outcomes among adults with OUD who initiate buprenorphine treatment in primary care. METHODS: This was a retrospective cohort study of all adults with OUD who initiated buprenorphine at an office-based treatment program in the Bronx, NY between June 1, 2015 and December 31, 2017. Using cox proportional ha

Systematic allergological evaluation enables NSAID allergy delabeling and identification of safe alternatives in adults.

BACKGROUND: Hypersensitivity reactions to nonsteroidal anti-inflammatory drugs (NSAIDs) are a frequent reason for allergy referral and a major diagnostic challenge. In everyday practice, overdiagnosis contributes to unnecessary drug avoidance, restricting therapeutic access to first-line analgesic and anti-inflammatory treatments and impacting clinical care. OBJECTIVE: To determine the true prevalence of confirmed NSAID hypersensitivity in adults with suspected reactions through systematic allergological evaluation and assess its clinical impact on therapeutic access through safe delabeling a

Reclassification of NSAID hypersensitivity according to the WAO 2025 guidelines: insights from a cohort of 527 patients.

BACKGROUND: Nonsteroidal anti-inflammatory drugs (NSAIDs) are among the most common culprits of drug hypersensitivity. Previous classifications, including the European Academy of Allergy and Clinical Immunology (EAACI) and the European Network for Drug Allergy framework, left a subset of patients unclassified under the "blended" category, particularly those with anaphylaxis not fitting into standard groups. The World Allergy Organization (WAO) 2025 guidelines introduced new subcategories, such as mixed NSAID-exacerbated cutaneous disease (NECD) and mixed NSAID-exacerbated respiratory disease (

Genetic Variants Associated with Non-Steroidal Anti-Inflammatory Drug-Induced Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis.

Background/Objectives: Non-steroidal anti-inflammatory drugs (NSAIDs) are widely prescribed to help alleviate pain and treat inflammation, but they are also recognized as common causes of severe cutaneous adverse reactions (SCARs), including Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). Despite their clinical importance, pharmacogenetic markers to predict individual susceptibility to NSAID-induced SJS/TEN remain insufficiently defined. This study investigated associations between HLA class I and II alleles, CYP2C9 polymorphisms, and NSAID-induced SJS/TEN in a Thai popula

Patch Testing in Paediatric Drug Hypersensitivity Reactions: Current Evidence and Clinical Implications.

Drug hypersensitivity reactions (DHRs) are common in children and are diagnostically challenging, with cutaneous manifestations being the most common presentation. Patch testing (PT) is a well-established tool for investigating delayed T-cell-mediated reactions and is widely used in patients with contact dermatitis. However, its role in paediatric DHRs remains unclear. Available data indicate that PT is safe and particularly useful when drug provocation testing is contraindicated, such as in patients with severe cutaneous adverse reactions. However, diagnostic yield varies considerably among d

Anesthetics and cancer biology: Translational evidence from perioperative modulation to drug repurposing (Review).

BACKGROUND: Reports suggest that children with mobility impairment represent a significant proportion of the population living with a disability. Footwear is considered to be the key extrinsic factor affecting children's gait and footwear modifications have been historically postulated to assist with locomotory difficulty. Although therapeutic footwear has been considered within the literature, there is a lack of consistency on terminology and paucity on the overall understanding. A scoping review was performed to chart the key concepts in children's footwear and to establish the range of stud
