Cutaneous insulin allergy responsive to oral desensitisation and aspirin.

A diabetic man with no previous history of allergy began to suffer itchy, painful swelling at the sites of injection after three months' treatment with bovine insulin. Insulin specific IgE concentrations (1.2-2.0 U/ml) were higher than in non-allergic diabetics (mean 0.4 (SD 0.06) U/ml) but lower than in most other patients allergic to insulin (1.0-19.0 U/ml). Standard approaches failed to overcome the allergic reaction, and four separate attempts at desensitisation were unsuccessful. The patient was then given oral insulin 800 U thrice daily together with enteric coated aspirin 1300 mg thrice

Laryngeal edema as the only symptom of hypersensitivity to salicylic acid and other substances.

Idiosyncrasy to salicylic acid and related substances is well known, the most common symptoms being asthma, rhinorrhea and urticaria. We here describe two cases whose only symptoms were hoarseness and in which inspection revealed laryngeal edema when the patients ingested any of these substances. In cases of chronic recurring hoarseness, a history should be taken with respect to such hypersensitivity and suspect cases should be challenged, since a strict diet may improve the symptoms.

[Urticaria--angioedema induced by non-steroidal anti-inflammatory analgesics].

Forty-two adult patients had been evaluated with a history of Urticaria and Angioedema (U-AE) associated with ingestion of non-steroidal anti-inflammatory drugs (NSAID). None developed attacks of bronchospasm. None had chronic urticaria. The same reaction was repeated in 36 individuals on at least two different occasions following administration of these drugs. Based on patients'history we distinguished three groups: Group A: twenty-three patients who had presented U-AE with the administration of pyrazolone drugs. Thereafter, sixteen of them had taken aspirin without adverse reactions. Group B

Problems of medication with the pediatric patient.

During the course of dental treatment, when it is necessary to prescribe pharmacologic agents for the pediatric patient, a drug should be selected that is not only effective but also available in a readily acceptable form. Dosages should be considered carefully. Rather than attempting to adjust adult recommendations, established guidelines for children should be utilized. Manufacturer's doses should be followed when available. If a child frustrates efforts toward treatment and does not respond to nonpharmacotherapeutic management approaches, premedication should be considered. The practitioner

Acetaminophen and aspirin challenges in subgroups of asthmatics.

To determine whether the different modes of action of aspirin and acetaminophen are associated with different incidences of bronchoconstriction, and whether there is cross-reactivity in terms of intolerance, selected asthmatics were challenged with both analgesics. Clinical profiles were observed to characterize more precisely the aspirin-intolerant asthmatic. Inhalational challenges with PGF2 alpha were then performed on nine selected subjects. It was determined that a positive history of aspirin intolerance is a reliable indicator of potential for anaphylactoid reactions but is not associate

Headaches.

Although classification can be difficult, determining the type of headache, such as tension, migraine or cluster, aids in the formulation of a therapeutic approach. Treatment involves regulation rather than cure. Acute and prophylactic pharmacotherapy and control of stress through behavior modification, relaxation training and biofeedback are methods of management.

Adverse pulmonary responses to aspirin and acetaminophen in chronic childhood asthma.

Because aspirin (ASA) is often reported to have an adverse effect on pulmonary function in children with chronic asthma, acetaminophen is commonly used as an ASA substitute in these children. To study acetaminophen effects on pulmonary functions, double-blind, oral challenges of ASA (600 mg), acetaminophen (600 mg), or lactose were administered on separate days to 25 chronic asthmatics, ten boys and 15 girls, ranging in age from 8 to 18 years (mean age +/- 1 SD: 12.5 +/- 2.8 years). No patient had a past history of adverse reactions to either drug. Forced expiratory volume in 1 second (FEV1),

Asthma improved by aspirin-like drugs.

In six adult patients with bronchial asthma aspirin in a single dose of 0.3-1.2 g produced bronchodilatation with a marked improvement in both clinical symptoms and pulmonary function tests. In these patients asthma was characterized by the following traits: onset after a flu-like infection, perennial course, lack of relation to specific allergenic exposures, negative skin tests with common aeroallergens, and history of rhinorrhoea or sinusitis. Oral challenge with 12 nonsteroidal anti-inflammatory drugs performed in two patients showed that bronchodilatation could be induced not only by aspir

Aspirin-sensitive rhinosinusitis/asthma: spectrum of adverse reactions to aspirin.

In order to determine the types of respiratory responses observed during aspirin-induced reactions, 50 consecutive asthmatic patients with a history of aspirin sensitivity underwent prospective oral aspirin challenges between 1979 and 1981. Oral aspirin challenges produced 36 asthmatic responses (33 combined with rhinitis and three purely asthmatic) and six acute rhinoconjunctivitis responses (three combined with mild asthma and three purely rhinoconjunctivitis) but failed to stimulate any reaction in eight patients. The results produced by these challenges were then compared with results reco

[Contact urticaria caused by skin test in pyrazolone allergy].

About one third of patients with anaphylactic reactions to pyrazolones showed contact urticaria induced by to at least one pyrazolone after 30-60 min in patch test. This reaction was never observed in patients with various exanthemas or allergic contact dermatitis to these medicaments. Contact urticaria in patch test was mostly caused by propyphenazone, aminophenazone, and metamizole. The suspicious pyrazolone provoked positive results in only about 70%. No correlation could be found between contact urticaria in patch test and severity of allergic history, short interval before skin test and a

Aspirin desensitization in aspirin-sensitive asthma: failure to maintain a desensitized state during prolonged therapy.

A patient with a history of asthma induced by acetylsalicylic acid (ASA) was found to be ASA sensitive when orally challenged with ASA. She was successfully desensitized using incremental doses of ASA given orally and maintained on ASA or other nonsteroidal antiinflammatory (NSAI) agents for the treatment of arthritis. After 6 months of uninterrupted therapy the patient developed asthmatic symptoms that were related to ASA and NSAI drug therapy. Although desensitization may be achieved in patients with ASA-sensitive asthma, sensitivity may recur despite continuous therapy.

[Rhinosinusitis polyposa and intolerance to analgesics (aspirin intolerance)].

Although the signs or symptoms of rhinitis, bronchial asthma, urticaria and hypersensitivity to non-steroidal antiphlogistics, antirheumatics and analgesics as well as to some foods, seem to point towards an anaphylactic reaction, this intolerance--known in the U.S.A. as "aspirin intolerance"--is of non-allergic aetiology. In most cases the disease begins in the upper airways. Hence, early diagnosis by the ENT specialist is essential. The medical examinations and case histories of 20 patients suffering from rhinosinusitis complicated by aspirin intolerance, are discussed.

An approach to urticaria.

As the problem of chronic urticaria is frustrating both to the physician and patient, an attempt to study its aetiology was made. 100 cases of chronic urticaria was studied in 1979. Patients having daily urticaria for more than 6 weeks, were included. There were 52 males and 48 females. A detailed history of the pattern of urticaria, drugs, physical causes, diet and illnesses were asked for, followed by physical examination. Each were then screened through a Diagnostic Tray including basic haematological tests, urinalysis, stools analysis, physical allergy tests, food provocation battery and f

Specific desensitization in 'aspirin-sensitive' urticaria; plasma prostaglandin levels and clinical manifestations.

Six out of eight patients with a history of aspirin-provoked urticaria/angioedema responded with adverse reactions, including urticaria and bronchospasm, to provoking doses of oral aspirin from 30-515 mg. The other two patients did not react to 1.2 g of aspirin on three occasions. Five of the six patients who had reacted became desensitized after their initial aspirin reaction, tolerating 650 mg on the second day. They then took 650 mg day-1 of aspirin for three weeks, during which time the ingestion of foods which had previously caused a variety of moderate or severe reactions caused no sympt

Recent advances in Gilles de la Tourette syndrome: implications for clinical practice and future research.

The clinical presentation and natural history of Gilles de la Tourette syndrome (TS) are reviewed. The waxing and waning of symptoms, the rostrocaudal progression of areas of motor involvement, complex stereotypies, and the familial aggregation of TS and chronic multiple tics are suggestions of underlying neurochemical disturbance; whereas attentional problems, impulsivity and obsessive-compulsive behaviors create interesting links with other disorders. The most robust metabolic findings are the lowered CSF HVA in many TS patients, and the positive response of symptoms to dopamine blockers and

Tartrazine and benzoate challenge and dietary avoidance in chronic asthma.

This study undertook to determine the usefulness of tartrazine and benzoate challenge and dietary avoidance in the management of patients with chronic asthma. Double-blind ingestion-challenge tests were performed on separate days with lactose, tartrazine, benzoate and acetylsalicyclic acid (ASA). Of the twenty-eight subjects challenged, one responded to tartrazine and one to benzoate. Two additional subjects responded to ASA and a further eight were not tested with this material because of a definite history of sensitivity. Twenty-four subjects completed 1 month periods of observation while fi

Near-fatal bronchospasm in an asthmatic patient following ingestion of flurbiprofen. A case report.

Aspirin and other inhibitors of prostaglandin synthetase are known to precipitate bronchospasm in sensitive individuals. We describe a 42-year-old patient with mild asthma who had a near-fatal attack of bronchospasm and required intermittent positive pressure ventilation and resuscitation following ingestion of a single tablet of flurbiprofen (Froben; Boots). When prescribing such agents, an appropriate history should be taken, and when asthma or hypersensitivity to related drugs is suspected, oral challenge with a small test dose and administration of the first dose under supervision are esse

Sphenoethmoidectomy: its role in the asthmatic patient.

Fifty patients who underwent intranasal sphenoethmoidectomy at the St Louis University Medical Center between July 1977 and April 1980 have been evaluated with respect to preoperative extent of disease and medication requirement as well as postoperative course and reduction in medication requirements, if any. These patients were followed jointly by the Departments of Allergy and Otolaryngology. Most of these patients were not allergic, and over half had intrinsic asthma. Fifteen of the 50 patients, all asthmatic, gave a history of aspirin sensitivity. Many had had previous nasal surgical treat
