Dual-scale assessment of motor dysfunction in older adults with locomotor dysfunction and its association with gait and muscle activation.

OBJECTIVE: This study examined the reliability and validity of a combined 25-question geriatric locomotive function scale (GLFS-25) and Locomotion Dysfunction Grade scale (LDG) classification protocol in older adults with locomotor dysfunction, and explored which objective gait and muscle activation parameters were most strongly associated with GLFS-25 scores. METHODS: The GLFS-25 and LDG were used to assess 289 older adults with locomotor dysfunction, of whom 60 underwent gait analysis and surface electromyography (sEMG). RESULTS: Reliability analysis yielded a Cronbach's α of 0.702 for the

Asymmetric metaphyseal cones versus medial modular block augment with central cone for medial tibial defects : a biomechanical comparison in varying bone quality.

AIMS: Managing large medial tibial bone defects during total knee arthroplasty (TKA) remains technically demanding, particularly in elderly patients. This study quantitatively compares two reconstructive strategies: 'bridging' the defect using an asymmetric metaphyseal cone versus 'anatomical reconstruction' employing a symmetrical cone with a metallic block augment, and assesses their sensitivity to osteoporotic bone quality. METHODS: A finite element model of a tibia with a standardized medial defect (Anderson Orthopaedic Research Institute (AORI) Type 2a) was adapted to simulate both confi

Dual GIP/GLP-1 receptor agonist versus selective GLP-1 receptor agonist in patients undergoing primary total knee or hip arthroplasty.

PURPOSE: Tirzepatide is a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist, whereas semaglutide is selective for the GLP-1 receptor. We compared post-operative outcomes after primary total knee or hip arthroplasty (TKA/THA) between patients prescribed tirzepatide and those prescribed semaglutide. METHODS: A retrospective cohort study was performed using the TriNetX US Collaborative Network. Adults undergoing primary TKA or THA with at least two coded prescription fills for tirzepatide or semaglutide within six months of surgery were

[Translated article] Comparison of peroneus longus and hamstring autografts in ACL reconstruction: Graft diameter and donor-site morbidity analysis.

BACKGROUND: Surgeons increasingly consider the peroneus longus tendon an effective alternative to hamstring autografts for anterior cruciate ligament reconstruction (ACLR). Choosing the right graft critically impacts long-term knee stability, prompting surgeons to strictly avoid undersized tissues. This study retrospectively compared clinical outcomes, knee stability, and comprehensive donor-site morbidity in ACLR utilizing peroneus longus versus hamstring tendon autografts. METHODS: We evaluated 30 patients undergoing arthroscopic ACLR in this retrospective review. The cohort was divided equ

Effect of 8-form Tai Chi vs health education on mobility in older adults with sarcopenic knee osteoarthritis: A randomized clinical trial.

OBJECTIVE: To evaluate whether a 12-week simplified 8-form Tai Chi program improves mobility in older adults with sarcopenic knee osteoarthritis (SPKOA) compared with health education plus progressive home exercise. METHOD: This assessor-masked, 2-site trial randomized 96 adults aged 65-85 years with SPKOA 1:1 to Tai Chi or health education plus home exercise. The primary outcome was change in Timed Up and Go (TUG) time at week 12. Site-adjusted categorical-time linear mixed-effects models were used. A TUG improvement of at least 2.8 s defined individual response. RESULTS: At week 12, the ad

Asymmetric cement distribution in the coronal plane increases the risk of prosthetic loosening following total knee arthroplasty: an in-silico study.

BACKGROUND: Total knee arthroplasty (TKA) is the standard treatment for end-stage knee osteoarthritis (KOA). Aseptic loosening of the tibial component is a common complication following TKA. Previous studies have indicated that reduced cement volume and thinner cement mantles can lead to stress concentration in the cancellous bone beneath the tibial plateau, which serves as an etiological basis for prosthetic loosening. Our preliminary study has shown that variations in cement distribution can alter the local biomechanical environment. Asymmetric cement distribution is a frequently observed cl

Retrieval of Thrombosed Inferior Vena Cava Filter and Mechanical Thrombectomy of Associated Caval Thrombosis Using FlowTriever and ClotTriever Systems With the ClotTriever as Embolic Protection.

A 55-year-old Chinese male with anti-phospholipid syndrome presented with left lower limb deep venous thrombosis (DVT) on warfarin and had an inferior vena cava filter (IVCF) implanted before a radical resection of a perineal sarcoma. The inferior vena cava (IVC) filter removal was attempted 2 weeks after the surgery, but thrombus was found at the filter. It was decided to leave the filter and to continue with anticoagulation. He presented with progressive right lower limb swelling, erythema and pain, with a CT venogram showing caval thrombosis extending from the IVC filter to the right below-

Successful Use of Peripheral Nerve Blocks for Emergent Trauma Surgery in a Patient With Perioperative Myocardial Infarction: A Case Report.

Patients with concurrent trauma-related surgical emergencies and ST-segment elevation myocardial infarction (MI) present a unique anesthetic challenge, as damage control surgery and rapid resuscitation must take priority over timely coronary revascularization. During these surgeries, the physiologic insult from general anesthesia can be detrimental to a failing heart, so regional anesthesia techniques and their implications in trauma should be considered. We present a 62-year-old male motorcyclist with hypertension, diabetes, and coronary artery disease and a history of percutaneous coronary i

Stretchable h-BN/TPU Fibers for Passive-Active Dual-Mode Thermal Management and Wearable Haptic Interfaces.

Wearable thermal interfaces require rapid, spatially confined thermal feedback together with mechanical compliance, yet existing passive thermal-management textiles and active thermal-haptic devices remain largely separate. Here, a stretchable h-BN/TPU composite fiber is developed by coupling microfluidic spinning with non-solvent-induced phase separation. Rectangular-channel confinement aligns hexagonal boron nitride plates within a microporous thermoplastic polyurethane matrix, constructing directional heat-conduction pathways while preserving large deformation. The optimized 30 wt.% h-BN/TP

Needle-Free Bedside and Anthropometric Measures Show Weak Discrimination of an Adverse Blood-Biomarker Profile in Middle-Aged and Older Mexican Adults: An Internally Validated Machine-Learning Study.

Background/Objectives: Needle-free measures such as grip strength, gait speed, and anthropometry are attractive as potential substitutes for blood-based biomarkers in low-resource settings, yet whether they can predict an adverse blood-biomarker profile remains unknown. This study aimed to determine whether machine-learning algorithms can predict such a profile from a defined set of needle-free bedside and anthropometric measures in Mexican adults. Methods: Using the Mexican Health and Aging Study (MHAS) 2012 biomarker subsample (n = 2000; mean age 62.4 years; 59.8% women), we defined a blood-

Testing the Avrami-Dobrzyński model of cancer transformation using mutation-count clinical data across multiple tumour types.

The Avrami-Dobrzyński (A-D) model interprets malignant transformation as a phase-transition-like process, mathematically equivalent to the Johnson-Mehl-Avrami-Kolmogorov (JMAK) equation from condensed-matter physics. In this framework, the cumulative probability of neoplastic transformation follows a Weibull-type sigmoidal function of the somatic mutation burden, parametrised by a shape constant a and a critical scaling exponent k.
Purpose: To test the generalisability of the A-D model by fitting its cumulative distribution function to clinical mutation-count data spanning 24 cancer types,

Dual-purpose Laparoscopy and Bipedicled Anterolateral Thigh Flap for Abdominal Wall Reconstruction After High-voltage Electrical Burn.

High-voltage electrical burns involving the abdominal wall pose complex management challenges, including delayed bowel perforation, progressive tissue necrosis, and durable reconstruction in compromised tissue. To our knowledge, no standardized approach has been described that combines visceral surveillance with abdominal wall reconstruction in this setting. We report a case of a 43-year-old man who sustained a 22,000-volt electrical injury with 30% total body surface area burns and a full-thickness lower abdominal wall defect (16 × 13 cm) exposing the intact peritoneum.
