Loss of the ferripyochelin receptor FptA drives reduced cefiderocol susceptibility and impairs fitness in Pseudomonas aeruginosa PA14.

Pseudomonas aeruginosa is an opportunistic human pathogen and a frequent cause of multidrug-resistant infections. This organism continues to evade antimicrobial therapy despite the clinical introduction of new antipseudomonal antibiotics over the past several years. One of these agents is cefiderocol (FDC), a novel siderophore-cephalosporin conjugate antibiotic that was designed to overcome both intrinsic and acquired β-lactam resistance mechanisms in P. aeruginosa. However, studies have demonstrated that inactivation of energy transducer protein (TonB)-dependent receptors, most notably the ca

Helios is critical for Treg effector differentiation in adipose autoimmunity.

The deletion of the transcription factor Helios in regulatory T cells (Tregs) in mice leads to systemic Th1 immune activation, decreased T follicular regulatory cells, and enhanced germinal center formation. We report here these mice also exhibit acquired lipodystrophy, hepatic steatosis, and insulin resistance. Adipose tissue exhibited lymphocytic infiltration followed by CD8+ T cell-mediated destruction of adipocytes. Helios-deficient Tregs were more frequent in adipose tissue and showed an enhanced effector Treg (eTreg) phenotype (CD62Llo Tcf1lo). The Helios-deficient eTregs in adipose tiss

Sepsis in multimorbidity: a domain-based framework for systemic vulnerability and precision care.

Sepsis is a leading cause of morbidity and mortality, amplified by multimorbidity. This narrative review synthesizes epidemiological, pathophysiological, and immunological evidence to show how prevalent conditions-type 2 diabetes and obesity, heart failure and cerebrovascular disease, COPD, chronic kidney disease, cancer/HIV, and severe mental illness-reshape sepsis biology and outcomes. Convergent mechanisms include low-grade inflammation, impaired innate and adaptive immunity, endothelial injury with immunothrombosis/NETosis, barrier disruption with dysbiosis, and neuroendocrine maladaptatio

Determinants of Suboptimal Breastfeeding Techniques and Maternal Knowledge in Northwest Ethiopia: A Cross-Sectional Observational Study.

BACKGROUND: Ineffective breastfeeding techniques are associated with nipple trauma and the early discontinuation of exclusive breastfeeding. Despite frequent contact with healthcare services, many mothers do not acquire the practical skills needed for effective breastfeeding techniques. This study assessed the prevalence of ineffective breastfeeding techniques, identified associated factors, and evaluated maternal knowledge among lactating mothers attending public hospitals in northwest Ethiopia. METHODS: A facility-based cross-sectional study was conducted among 420 systematically selected l

The "ADC-after-ADC" paradigm: navigating cross-resistance and sequencing in breast cancer therapy.

Antibody-drug conjugates (ADCs) are entering earlier lines of breast cancer therapy, but evidence guiding treatment after a prior ADC remains limited. We performed a structured narrative synthesis of prospective post-prior-ADC studies, peer-reviewed retrospective cohorts, human translational analyses, functional preclinical studies, regulatory documents, and trial registries through July 23, 2026. No completed trial has randomized clinically comparable patients between alternative ADC sequence orders. Clinical characteristics of immune-related adverse events after immune checkpoint inhibitor therapy with or without chemotherapy in patients with antinuclear antibody-positive malignancies.

BACKGROUND: Antinuclear antibodies (ANA) are detected in cancer patients before ICI therapy, but their association with immune-related adverse events (irAEs) and outcomes is uncertain. This study aimed to characterize irAEs and tolerability in ANA-positive patients receiving ICI ± chemotherapy, and explore implications for resistance. METHODS: We retrospectively analyzed 121 solid tumor patients who received ICI therapy ± chemotherapy at the China-Japan Union Hospital (2021-2026). By baseline ANA titer (≥1:80 positive), patients were divided into positive (n=60) and negative (n=61) groups. We

From intestinal immune niches to systemic inflammation: programming cellular plasticity and immune redistribution across organs.

Immune-mediated inflammatory diseases have traditionally been investigated within organ-specific frameworks. Although increasing evidence has revealed extensive communication along individual gut-organ axes, these interactions are often described through separate mechanisms, including microbial dysbiosis, barrier disruption, soluble mediators, and immune-cell redistribution. A central challenge remains to determine how intestinal immune states are established and how they are translated into organ-specific inflammatory responses. In this review, we propose that the intestine functions as a dyn

EZH2 directs HER2+ breast cancer progression through the modulation of epithelial plasticity.

Breast cancer remains a leading cause of death among women, with the HER2+ subtype being particularly aggressive due to acquired resistance to HER2-targeted therapies. Enhancer of Zeste Homolog 2 (EZH2), the catalytic subunit of Polycomb Repressive Complex 2, represses the expression of genetic programs crucial for differentiation, proliferation, and apoptosis. To investigate the role of EZH2 in HER2+ tumor progression, we crossed a genetically engineered mouse model of HER2-driven breast cancer with a conditional Ezh2 knockout strain and showed that Ezh2 is essential for accelerating tumor in

Reprogramming the immunosuppressive breast cancer microenvironment: integrating cellular, metabolic, and stromal targets for rational immunotherapy.

Breast cancer, a highly heterogeneous malignancy and a leading cause of cancer-related mortality among women worldwide, is profoundly shaped in its initiation, progression, and therapeutic response by the tumor immune microenvironment (TIME). This review consolidates recent advances in deciphering the cellular, molecular, and metabolic complexity of breast cancer TIME and highlights mechanisms of immune suppression that impede durable treatment efficacy. We critically appraise current and emerging immunotherapeutic approaches, with a focus on strategies that aim to transform immunologically "c

MiRNAs: a call to arms that shapes the plasticity of tumor associated macrophages in breast cancer.

Breast cancer (BC) remains a leading cause of cancer-related mortality, and a major contribution to tumor progression and resistance to therapies arise from tumor microenvironment (TME). Tumor is indeed able to shape a self-permissive TME, reprogramming the cellular components into allies. Tumor-associated macrophages (TAMs), abundant in BC TME, mainly acquire an immunosuppressive M2-like phenotype able to fuel tumor progression, immune evasion, metastasis and therapy resistance through a dynamic crosstalk with cancer cells. MicroRNAs, transferred via extracellular vesicles and exploited by th

Cellular remodeling of ovarian follicular epithelial cells transmits an obligate nutritional endosymbiont in a scale insect.

Many insects show complex associations with vertically transmitted endosymbionts. Here, we describe unique cellular remodeling of the oocyte's follicular epithelial cells into endosymbiont-bearing tubular structures in the ensign scale insect Insignorthezia insignis (Hemiptera: Ortheziidae). Vitellogenic oocytes develop a bulge-like distension at the posterior pole. Here, follicular epithelial cells undergo extensive cellular reorganization, remodeling their actin cytoskeleton and plasma membrane to produce cellular protrusions. These tubular protrusions, which are densely packed with the endo

Leveraging Representation Learning for Bi-parametric Prostate MRI to Disambiguate PI-RADS 3 and Improve Biopsy Decision Strategies.

OBJECTIVES: Despite its high negative predictive value (NPV) for clinically significant prostate cancer (csPCa), MRI suffers from a substantial number of false positives, especially for intermediate-risk cases. In this work, we determine whether a deep learning model trained with PI-RADS-guided representation learning can disambiguate the PI-RADS 3 classification, detect csPCa from bi-parametric prostate MR images, and avoid unnecessary benign biopsies. MATERIALS AND METHODS: This study included 28,263 MR examinations and radiology reports from 21,938 men imaged for known or suspected prostat

Predicting response to neoadjuvant therapy in breast cancer using longitudinal DCE-MRI deep learning integrated with tumor microenvironment data.

OBJECTIVE: This study developed and validated a multimodal fusion model to enable the early and accurate prediction of pathological complete response (pCR) to neoadjuvant therapy (NAT) in breast cancer. The model integrates deep learning (DL) features derived from longitudinal dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) acquired early during treatment, peripheral blood inflammatory (PBI) indices, and baseline levels of tumor-infiltrating lymphocytes (TILs). METHODS: A total of 262 breast cancer patients receiving NAT were retrospectively enrolled and divided into a training

Editing Candida: Origins and Advances of CRISPR Tools.

Pathogens causing candidiasis encompass a diverse group of ascomycetous yeasts that have become essential models for studying fungal adaptability, pathogenicity, and host-pathogen interactions. Although many candidiasis-promoting species exist as commensals within host microbiota, several have acquired virulence traits that enable opportunistic infections, positioning them as a leading cause of invasive fungal disease in humans. Deciphering the molecular and genetic determinants that underpin the biology of organisms responsible for candidiasis has long been a central objective in medical and

Interleukins in community-acquired pneumonia: from biomarkers to precision medicine.

Community-acquired pneumonia (CAP) is still a leading cause of death due to infection globally, yet precise severity assessment remains a significant clinical problem. More than any other group of cytokines, interleukins are central to the regulation of inflammation and shed light on this intricate pathology. In the present review we summarize the biological and clinical characteristics of some of the principal interleukins (ILs) in CAP, classified primarily according to their physiological activity as pro-inflammatory (IL-2, IL-6, IL-8 and IL-12), anti-inflammatory (IL-7, IL-10 and IL-37), du

Identification and validation of a seven-gene metastasis-associated prognostic model in breast cancer.

INTRODUCTION: Metastasis is the primary cause of mortality in patients with breast cancer. This study aimed to develop a prognostic signature based on metastasis- and cancer-associated differentially expressed genes (M-CA-DEGs) and to identify potential novel therapeutic genes. MATERIALS AND METHODS: Data were acquired from the TCGA-BRCA, AURORA US Network, SCAN-B, and GEO databases. M-CA-DEGs were identified, and a prognostic risk model was constructed via univariate Cox and LASSO regression analyses. The prognostic value was verified using calibration curves and decision curve analysis (DCA

Immunological characterization of neuronal intranuclear inclusion disease with kidney injury: an exploratory analysis in a multi-center cohort.

BACKGROUND: Neuronal intranuclear inclusion disease (NIID) is a neurodegenerative disorder caused by GGC repeat expansions in NOTCH2NLC, leading to uN2CpolyG protein deposition. Although immune-mediated renal lesions have been described in NIID, the systemic immunoinflammatory profile associated with kidney injury and its relationship with genetic burden remain undefined. This study investigated peripheral immune alterations in NIID-related nephropathy and their correlation with repeat expansion size. METHODS: This multicenter retrospective study enrolled 150 genetically and pathologically co
