PTP1B deficiency in myeloid cells increases susceptibility to Candida albicans systemic infection by modulating antifungal immunity.

Invasive candidiasis, primarily caused by Candida albicans, poses a significant threat to immunocompromised patients, with high mortality rates. Understanding how immune responses to Candida albicans are mounted and controlled is fundamental to developing new therapeutic strategies. Protein tyrosine phosphatase 1B (PTP1B) is a regulator of immunoreceptor signaling and downstream inflammatory and metabolic responses and a pharmaceutical target. Here, we reveal a critical role for myeloid cell-intrinsic PTP1B in antifungal immunity. Mice lacking PTP1B in myeloid cells (LysM PTP1B-/-) were signif

Dosimetric evaluations using cycle-consistent generative adversarial network synthetic CT for MR-guided adaptive radiation therapy.

BACKGROUND: Magnetic resonance (MR) guided radiation therapy combines high-resolution image capabilities of MRI with the precise targeting of radiation therapy. However, MRI does not provide the essential electron density information for accurate dose calculation, which limit the application of MRI. In this presented work, we evaluated the potential for Deep Learning (DL) based synthetic CT (sCT) generation using 3D MRI setup scans acquired during real-time adaptive MRI-guided radiation therapy. METHODS: We trained and evaluated a Cycle-consistent Generative Adversarial Network (Cycle-GAN) us

The innate thermogenic capacity of brown adipose tissue develops independently of sympathetic signaling.

Brown adipose tissue (BAT) dissipates energy as heat in response to β-adrenergic signaling induced by the sympathetic nervous system (SNS). While this pathway is essential for the cold-induced remodeling and metabolic activity of BAT, its role in developmental programming is unclear. Here, we show that brown adipocytes acquire thermogenic identity during embryogenesis independently of sympathetic innervation and β-adrenergic signaling. Genetic sympathectomy or disrupted β-adrenergic signaling had minimal effects on thermogenic gene expression or tissue morphology during either embryonic or pos

Acquired Diaphragmatic Hernia Following Pediatric Liver Transplantation: Incidence, Risk Factors, and Surgical Outcomes.

Acquired diaphragmatic hernia (ADH) is an uncommon yet relevant complication after pediatric liver transplantation (pLTx). True incidence and risk factors remain poorly defined, largely due to limited screening, heterogeneous imaging practices, and the absence of validated predictive models. This study aimed to determine the incidence, presentation, and risk factors for ADH and to compare surgical repair techniques and short-term outcomes of thoracoscopic versus open approaches. We performed a retrospective review of all children undergoing pLTx at our institution (2014 and 2024). Demographic a

In addition to hemorrhagic cystitis, Fanconi Syndrome is a serious clinical side effect during ifosfamide (IFO) therapy. Fanconi syndrome is a generalized dysfunction of the proximal tubule which is characterized by excessive urinary excretion of glucose, phosphate, bicarbonate, amino acids and other solutes excreted by this segment of the nephron including L-carnitine. Carnitine is essential cofactor for β-oxidation of long-chain fatty acids in the myocardium. IFO therapy is associated with increased urinary carnitine excretion with subsequent secondary deficiency of the molecule. Cardiac abn

Fatal Influenza B-MRSA Coinfection in a Healthy Adolescent: Necrotizing Pneumonia, Cytokine Storm, and Multi-Organ Failure.

BACKGROUND: Influenza B usually causes mild illness in children. Severe and fatal cases can occur when complicated by secondary Staphylococcus aureus (S. aureus) pneumonia, including community-acquired methicillin-resistant Staphylococcus aureus (MRSA). We present a rare, rapidly progressive fatal case in an adolescent with no known medical history to highlight diagnostic and therapeutic pitfalls. CASE PRESENTATION: A 16-year-old boy with no known underlying conditions (unvaccinated for influenza) presented critically ill at "Sf. Ioan" Clinical Emergency Pediatric Hospital in Galați after one

Impact of community-acquired invasive Escherichia coli disease on mortality and readmissions in elderly patients: a multi-center study in Germany.

BACKGROUND: Extraintestinal pathogenic Escherichia coli (ExPEC) causes invasive E. coli disease (IED), resulting in significant morbidity and mortality, particularly among the elderly. IED can present as bacteremia or sepsis and poses serious health risks for adults aged ≥ 60 years, compounded by increasing antimicrobial resistance. However, real-world data on IED's clinical burden and risk factors in Europe are sparse. This study systematically reviews medical charts across 11 hospitals in Germany to address this gap by evaluating the impact of IED on patient outcomes, mortality rates, and ho

A neoantigen vaccine generates antitumour immunity in renal cell carcinoma.

Personalized cancer vaccines (PCVs) can generate circulating immune responses against predicted neoantigens1-6. However, whether such responses can target cancer driver mutations, lead to immune recognition of a patient's tumour and result in clinical activity are largely unknown. These questions are of particular interest for patients who have tumours with a low mutational burden. Here we conducted a phase I trial (ClinicalTrials.gov identifier NCT02950766) to test a neoantigen-targeting PCV in patients with high-risk, fully resected clear cell renal cell carcinoma (RCC; stage III or IV) with

Cognitive impairment assessed by static and dynamic changes of spontaneous brain activity during end stage renal disease patients on early hemodialysis.

BACKGROUND: Compared with the general population, patients with end-stage renal disease (ESRD) undergoing maintenance hemodialysis (ESHD) exhibit a higher incidence of cognitive impairment. Early identification of cognitive impairment in these patients is crucial for reducing disability and mortality rates. Examining the characteristics of static and dynamic regional spontaneous activities in ESHD cases may provide insights into neuropathological damage in these patients. METHODS: Resting-state functional magnetic resonance images were acquired from 40 patients with early ESHD (3 or 4 times/w

Coronary Artery Disease and Intradialytic Myocardial Ischemia in Hemodialysis: An Exploratory Study Using Intradialytic Imaging.

RATIONALE & OBJECTIVE: Recurrent segmental myocardial ischemia (myocardial stunning) is a well-recognized consequence of hemodialysis leading to fixed contractile deficits and increased morbidity and cardiac mortality. Although epicardial coronary artery disease (CAD) is not a prerequisite, hemodialysis patients commonly have significant arterial plaque burden, and the interaction with established CAD and hemodialysis-induced myocardial ischemic injury is currently unknown. STUDY DESIGN: A single-center cross-sectional study. SETTING & PARTICIPANTS: Thirteen patients on maintenance hemodialy

Comprehensive dietary patterns explain acquired cystic kidney disease risk through genetic and metabolomic mechanisms.

BACKGROUND: Acquired renal cysts (ARC) are associated with kidney function decline, necessitating novel dietary pattern (DP) analyses in large cohorts. METHODS: This UK Biobank prospective cohort study (2006-2010) included participants with ≥2 dietary records, excluding those with severe kidney damage. The constructed comprehensive dietary pattern integration (CDPI) utilized reduced rank regression (RRR) and latent profile analysis (LPA). ARC cases (ICD-10: N28.1) were assessed via Cox regression for risk and dose-response, with NMR metabolites examined as mediators. RESULTS: Among 119,709 p

Imaging of Pediatric Renal Cysts: An Update-AJR Expert Panel Narrative Review.

Pediatric renal cysts can be isolated, hereditary, or related to acquired disease processes, and they may be congenital or develop over time. Given the heterogeneity of pediatric renal cysts, their increasing detection, and the absence of unified pediatric-specific guidelines, additional practical imaging-based recommendations are needed. This AJR Expert Panel Narrative Review seeks to address such gaps by providing guidance for radiologists in the evaluation of renal cysts, focusing on imaging characteristics, diagnostic considerations, and management recommendations. The authors comprise a m

Brucellosis-associated hemophagocytic lymphohistiocytosis: a case report and literature review.

INTRODUCTION: Hemophagocytic lymphohistiocytosis (HLH), a rare and deadly disease, is typically classified as either primary (familial) or secondary (acquired), depending on the etiology and underlying cause. Secondary HLH often develops in the presence of infectious, malignant, rheumatologic, or metabolic conditions, with infections, especially Epstein-Barr virus (EBV) infection, being among the leading causes. Brucella infection-induced HLH is relatively rare, with only eight cases reported in the past decade, all of which had a favorable prognosis following timely diagnosis and treatment.

Shiga Toxin-Producing Escherichia coli-Associated Hemolytic Uremic Syndrome in Adult Kidney Transplant Recipients.

INTRODUCTION: Shiga toxin (Stx)-producing Escherichia coli (E coli)-associated hemolytic uremic syndrome (STEC-HUS) is an acquired form of thrombotic microangiopathy (TMA) caused by food and waterborne infection. It remains the leading cause of acute renal injury (AKI) in children. In adults, STEC-HUS is less common but often more severe. Data on STEC-HUS in kidney transplant recipients (KTRs) are limited. METHODS: We conducted a retrospective study involving adult KTRs diagnosed with STEC-HUS between January 2012 and April 2022 across 15 French nephrology centers. We aimed to characterize th
