Obstetricians' experiences caring for pregnant people experiencing incarceration who request permanent contraception.

BACKGROUND: Given the historic and contemporary legacy of non-consensual sterilization and concern for coercion among incarcerated populations, routine provision of permanent contraception is discouraged. However, as with non-incarcerated patients, the 58,000 pregnant people who are incarcerated every year have diverse contraceptive goals, including the desire for permanent contraception. Clinicians caring for incarcerated patients must navigate this ethical tension. The perspectives and experiences of obstetricians who perform surgical permanent contraception procedures and who care for incar

The Intersection of Incarceration and Reproductive Health: Contraceptive Perspectives of Women in Rural Maine Jails.

The U.S. carceral health care system presents a unique opportunity to expand reproductive health services. Evidence suggests that women incarcerated in jails want to initiate contraception, but access varies across facilities and is limited by concerns about cost, reproductive coercion, and a lack of consistent policies. To better understand the reproductive health needs of rural incarcerated women, a cross-sectional study of women incarcerated in four rural Maine jails was conducted using a 59-question survey on contraceptive history, needs, and preferences. Seventy-three women with complex h

Randomized Controlled Trial of a Tubal Sterilization Decision Aid Among Medicaid Enrollees: Effects on Intended Contraceptive Method and Decision-Making Satisfaction.

INTRODUCTION: Tubal sterilization is a permanent contraceptive method, yet misconceptions about its permanence are prevalent, and its history is marked by both coercion and barriers to access-particularly among Medicaid enrollees. Decision aids may help to support more informed, value-concordant decision-making. METHODS: From March 2020 to November 2023, English- and Spanish-speaking pregnant Medicaid enrollees at less than 24 weeks' gestation were randomized to either receive or not receive a novel web-based tubal sterilization decision aid. Participants completed surveys after randomization

Factors that impede women's contraceptive care needs during and immediately after incarceration in New South Wales, Australian. Qualitative perspectives of professional staff working with incarcerated women.

INTRODUCTION: Women with a history of incarceration report higher rates of pregnancy and abortion, lower contraception use and poorer birth outcomes compared to other women in the community. This suggests family-planning and reproductive-health needs that are not well served by current models of care. Despite this, little is known about how contraception is provided within Australian carceral settings, or the structural barriers experienced by professional staff working in these environments. We report findings from a qualitative study conducted in New South Wales (NSW), Australia, describing

Mitochondrial metabolic plasticity in cancer: hybrid bioenergetic states and translational targeting in surgical oncology.

Mitochondrial metabolism is increasingly recognized as an active determinant of tumor growth and cancer-cell adaptation rather than solely a source of cellular energy. Mitochondrial metabolism supports tumor anabolism, while mitochondrial DNA (mtDNA) alterations can affect respiratory chain function, redox homeostasis, and adaptation to changing tumor environments. At the same time, tumor metabolism is heterogeneous and flexible and cannot be explained by a uniform Warburg model. Tumor cells may use glycolysis, oxidative phosphorylation (OXPHOS), or hybrid glycolysis/OXPHOS states, with the re

Association of Levonorgestrel-Containing Emergency Contraceptive Pills with Breast Cancer in Premenopausal Latin American Women: The PRECAMA Study.

BACKGROUND: The emergency contraceptive pill (ECP) contains progestin, which can have proliferative effects when used intermittently and thus increase the risk of breast cancer. METHODS: We investigated the association between the use of ECPs and breast cancer risk in premenopausal Latin American women from 2012 to 2023. We hypothesized that ECP use increases breast cancer risk. We used data from a case-control study (the PRECAMA study) conducted among premenopausal Latin American women. We evaluated variables such as having ever used an ECP, sociodemographic factors, anthropometric and lifes

SYSTEMIC THROMBOEMBOLIC OUTCOMES IN YOUNG PATIENTS AFTER RETINAL VEIN OCCLUSIONS.

PURPOSE: To evaluate the long-term incidence of hypercoagulable complications in young patients with retinal vein occlusions and negative workups compared with matched controls. METHODS: A retrospective cohort study was conducted using the TriNetX Analytics Network. Patients aged 18 to 50 with newly diagnosed retinal vein occlusion and no history of hypertension, glaucoma, thrombophilia, or recent oral contraceptive use were compared with age-matched controls. Propensity score matching was performed to balance baseline demographics and comorbidities. Primary outcomes included the cumulative i

A Hybrid Modeling Framework for Predictive Digital Twins of CHO Cell Culture.

Digital twins of mammalian cell cultures hold great potential for predictive bioprocess modeling, yet their development is challenged by the nonlinear dynamics and metabolic complexity of these systems. We present a hybrid computational framework that integrates mechanistic and data-driven modeling to construct predictive digital twins for Chinese hamster ovary (CHO) cell cultures producing monoclonal antibodies. The framework couples ordinary differential equation (ODE) models with constraint-based metabolic modeling and machine learning components trained on Bayesian-estimated metabolic rate

Evolving care pathways for women with migraine in Italy: results from a national survey.

BACKGROUND: Migraine predominantly affects women with prolonged and more severe episodes that severely compromise their quality of life. Despite its burden, diagnostic-therapeutic pathways specifically designed for women are currently lacking, and care for women with migraine remains fragmented. By expanding the results obtained from a recent Delphi consensus study, we aim to identify key unmet needs and propose expert-driven recommendations to address female-specific migraine care in Italy. METHODS: We conducted a national survey in which the items used in the previous Delphi study were admi

Historical Neighborhood Redlining and Fertility in a Cohort of US Black Women.

BACKGROUND: Structural racism can manifest in contemporary neighborhoods via historical policies or programs. For example, the Home Owners' Loan Corporation, a government-backed program from the 1930s, systematically diverted wealth away from Black neighborhoods. The reproductive health consequences of this racist program remain understudied. We evaluated associations between residence in a historically redlined neighborhood and fecundability, the per-cycle probability of conception. METHODS: We used data from the Black Women's Health Study, a US cohort of Black women who were aged 21-69 year

Lipoprotein(a) and incident venous thromboembolism in pre- and postmenopausal women, and in men.

BACKGROUND AND AIMS: Elevated lipoprotein(a) [Lp(a)] levels are an established risk factor for atherosclerotic cardiovascular disease, but the association between Lp(a) and venous thromboembolism (VTE) remains unclear. Sex and hormonal status may modify the relationship between Lp(a) and VTE. METHODS: The present study included participants from the UK Biobank with available baseline Lp(a) data. Individuals with a history of VTE or cancer, as well as those using anticoagulants, were excluded. Multivariable-adjusted Cox models were used to assess the association between Lp(a) levels ≥125 nmol/

Impact of Laparoscopic Sleeve Gastrectomy on Menstrual Regularity and Spontaneous Pregnancy in Morbidly Obese Women: A Retrospective Cohort Study.

Background and Objectives: Obesity is a major contributor to female reproductive dysfunction, frequently resulting in menstrual irregularity, anovulation, and subfertility. Bariatric surgery improves metabolic health; however, its effect on reproductive outcomes-particularly the shift from assisted to spontaneous conception-remains incompletely defined. This study aimed to evaluate the impact of laparoscopic sleeve gastrectomy (LSG) on menstrual cycle regularity and spontaneous pregnancy rates in women of reproductive age. Materials and Methods: This retrospective observational study included

Characterizing endometriosis and adenomyosis symptom clusters and their impact on quality of life in the All of Us Research Program.

STUDY QUESTION: Are there distinct symptom clusters in women with endometriosis and adenomyosis, and how do these clusters predict quality of life? SUMMARY ANSWER: Latent class analyses of 22 438 women with endometriosis identified four symptom phenotypes, including high pain-gastrointestinal and psychological-neurological clusters; adenomyosis cases were concentrated in high‑pain classes without a minimal‑symptom group, and high‑burden phenotypes had poorer quality of life. WHAT IS KNOWN ALREADY: Endometriosis and adenomyosis are heterogeneous conditions with diagnostic delays of 4-11 years

Association of reproductive factors and chronic kidney disease in postmenopausal women: results from the U.S. National Health and Nutrition Examination Survey 1999 - 2020.

Chronic kidney disease (CKD) poses a growing global health burden, and reproductive history may influence CKD risk via hormonal pathways. We conducted a population-based cross-sectional analysis using ten cycles of the National Health and Nutrition Examination Survey (NHANES, 1999-March 2020), including 12,912 postmenopausal women with complete reproductive and kidney data. CKD was defined as estimated glomerular filtration rate <60 mL/min/1.73 m2 (CKD Epidemiology Collaboration 2021) or urinary albumin-to-creatinine ratio ≥30 mg/g. Survey-weighted logistic regression and restricted cubic spli

Market Integration and Reproductive Transitions Among Indigenous Shuar and Neighboring Nonindigenous Ecuadorians in Amazonian Ecuador.

OBJECTIVE: Global reproductive transitions are well documented, yet less is known about how neighboring populations with divergent histories and exposure to market integration (MI) experience them. This study examines how sociocultural and economic proxies of MI predict variation in reproductive outcomes among indigenous Shuar and nonindigenous Ecuadorian mestizos (Colonos) from Amazonian Ecuador, focusing on menarcheal age, age at first birth, parity, breastfeeding duration, and contraceptive use. METHODS: Reproductive histories were collected from 360 Shuar and 205 Colono women (ages 15-90)

Socio-behavioral factors linked to willingness to use new male contraceptives: A cross-sectional survey of men and female partners across four low- and middle-income countries.

OBJECTIVE: Numerous population-level surveys support willingness to use new male contraceptives (MC) among men and their female partners across a range of cultures and geographies, despite unproven efficacy and uncertain risk. Whether such willingness is related to attitudes about gender equity and/or key reproductive experiences warrants further exploration. We explore socio-behavioral factors linked to variations in willingness to use new MCs in Colombia, Ghana, Morocco, and Togo. STUDY DESIGN: We conducted approximately two thousand surveys examining new MC attitudes with sexually active,
