OBJECTIVE: Biological reconstruction of extensive meta-/diaphyseal bone defects using autologous fibular graft. Aim is stable defect bridging with preservation of the limb and restoration of function.
INDICATIONS: Intercalary bone defects caused by joint-preserving tumor resection of bone tumors, failed osteosyntheses with pseudarthrosis, chronic osteomyelitis after debridement, posttraumatic or congenital bone loss. Vascularized grafts are particularly indicated in cases of compromised soft tissue, previous radiotherapy, defects > 10-12 cm, or anticipated delayed healing.

CASE: A 15-year-old girl presented with isolated anterolateral tibial bowing without fracture, an unusual manifestation. OBJECTIVES: Meier-Gorlin syndrome (MGORS) is a rare primordial dwarfism characterized by microtia, patellar hypoplasia/aplasia, and short stature. Additional features may include skeletal, respiratory, urogenital, and endocrine abnormalities. 13 genes have been implicated, with DONSON, essential for replication fork stability and intra-S phase checkpoint activation, being the most recently identified. Only six patients with DONSON-related MGORS have been reported. This study expands the phenotype by presenting long-term follow-up and prenatal data in two affected siblings.
