[The surgical treatment of scapular fractures].

Scapular fractures are predominantly managed non-operatively. However, a subset of patients develops persistent pain, weakness, and functional impairment due to malunion, non-union, rotator cuff dysfunction, or scapulothoracic dyskinesia. Early identification of patients at risk for ongoing symptoms is essential, as timely surgical intervention may improve outcomes. Owing to the lack of randomized controlled trials comparing operative and non-operative management, surgical indications remain controversial. This article reviews the biomechanics of the scapula, mechanisms of injury, fracture typ

Concurrent Long Thoracic and Lateral Cutaneous Intercostal Nerve Entrapment After Chest Tube Thoracostomy Causing Scapular Winging and Chest Wall Pain: Diagnosis With Ultrasound and Treatment With Hydrodissection.

BACKGROUND: Chest tube thoracostomy may cause iatrogenic nerve injury, resulting in persistent pain and functional impairment. Long thoracic nerve (LTN) palsy after chest tube placement has been described, but concurrent involvement of the LTN and the lateral cutaneous branch of an intercostal nerve (LCIN) appears to be rarely reported. We describe a patient with persistent post-thoracostomy chest wall pain and scapular winging in whom musculoskeletal ultrasonography identified entrapment of both nerves. CASE PRESENTATION: A 43-year-old man presented with severe left shoulder and chest wall p

The long thoracic nerve: Cadaveric study of a potential site of dynamic compression?

PURPOSE: Neuropathies of the long thoracic nerve (LTN) are rare pathologies whose etiology is still subject to discussion. Anatomical study of the nerve from its origin to its distal innervation could provide information about this condition. METHODS: We dissected 12 nerves from their origin in the brachial plexus to their terminal innervation branches. We identified all potential compression sites along the nerve pathway. Each nerve has been classified anatomically into its supraclavicular and infraclavicular parts. We have collected various biometric data. RESULTS: We identified three pote

Subscapular Tunneling to Increase the Reach of Latissimus Dorsi Myocutaneous Flap: A Case Report.

The latissimus dorsi muscle or myocutaneous flap (LDMF) has long been a mainstay in the plastic surgeon's armamentarium, serving as a versatile and reliable tool for reconstructive procedures. Tunneling the LDMF through the subscapular route increases the reach to cover defects as far as nape of the neck. In this article, we report a case of LDMF routed through a subscapular route to cover impending hardware exposure at the nape region. This article provides a comprehensive review of the subscapular tunneled LDMF technique while discussing its indications, surgical anatomy, operative technique

B2 Glenoid in the Active 50-Year-Old With Severe Glenohumeral Osteoarthritis: It's So Confusing! What Should I Do?

In the young, active patient with osteoarthritis, the optimal treatment of B2 glenoid morphology remains a subject of continued debate. Current treatment options have specific advantages and disadvantages. These treatments include hemiarthroplasty with or without glenoid reaming, total shoulder arthroplasty (TSA) with or without eccentric reaming, TSA with bone graft or posteriorly augmented glenoid implant, and reverse TSA. Eccentric glenoid reaming can be effective but has the risk of excessive reaming of subchondral bone for greater retroversion deformities, compromising implant purchase wi

Bony integration of a hybrid glenoid component in anatomical shoulder arthroplasty : short-term CT scan analysis.

AIMS: Loosening of the glenoid component in the long term remains an important complication of the anatomical total shoulder arthroplasty (aTSA). The aim of this study was to explore the bony integration of a hybrid glenoid component based on an analysis of CT scans. METHODS: In a prospective multicentre study, patients who underwent primary aTSA, whose hybrid design of glenoid component included a fully-polyethylene flanged upper peg and a porous-coated titanium lower peg, and who were reviewed with CT scans between 12 and 24 months postoperatively, were included. Editorial Commentary: Open Bankart Procedure for Anterior Shoulder Instability Is Recommended for Contact Athletes With Bony Bankart Fractures or Approximately 10% Glenoid Bone Loss.

Recurrent anterior shoulder instability with subcritical bone loss is a challenging clinical scenario. Many treatment options have been described, including arthroscopic Bankart repair, arthroscopic Bankart repair plus remplissage, open Bankart repair, and the Latarjet procedure. Arthroscopic Bankart repair alone has higher rates of failure in high-risk patient populations, especially at long-term follow-up. These high-risk populations include patients with significant glenoid bone loss (>20%), bipolar bone loss (including subcritical bone loss and off-track or "near-track" [8-10 mm] Hill-Sach

Short-term outcomes of anatomic total shoulder arthroplasty with nonaugmented glenoid component for Walch B2 and B3 glenoid morphology.

BACKGROUND: The optimal management of posterior glenoid bone loss in glenohumeral osteoarthritis with anatomic total shoulder arthroplasty (aTSA) remains unknown, as the degree of bone loss and retroversion can vary across a wide spectrum in glenoids with Walch B2 and B3 morphology. The objectives of the current study were to evaluate the clinical and radiographic outcomes of utilizing standard, all-polyethylene, nonaugmented anchor-peg glenoid (APG) components in aTSA for patients with mild to moderate B2 or B3 morphology. METHODS: Between January 2010 and September 2019, we identified 79 sh

A multi-centre, participant-blinded, randomized, 3-year study to compare the efficacy of Virtual Surgical Planning (VSP) to Freehand Surgery (FHS) on bony union and quality of life outcomes for mandibular reconstruction with fibular and scapular free flaps: study protocol for a randomized phase II/III trial.

BACKGROUND: Advanced head and neck malignancies with underlying bony involvement often require aggressive oncological resection of large segments of the oral cavity including the mandible. These patients require vascularized donor osseous free tissue transfer to reconstruct significant defects. Traditionally, the donor bone is harvested on its vascular supply and shaped to the defect in a free hand fashion (FHS). However, virtual surgical planning (VSP) has emerged as a method to optimize reconstructive outcomes and decrease operative time. The goals of this study are to assess superiority of

A cause not to be underestimated in the development of winged scapula: sports-related injuries.

Winged scapula (WS) is characterized by the medial and/or inferior border of the scapula protruding from the thoracic wall. The aetiologies include sports activities, trauma, surgical complications, infections and muscular dystrophies. In our study, we aim to describe the electrophysiological and clinical features of patients referred to our clinic with a preliminary diagnosis of winged scapula and to identify the causes of WS. Between 2009 and 2021, a retrospective analysis was performed of the neurophysiological studies and clinical data of patients who presented with a preliminary diagnosis

Five-year outcomes of anatomic total shoulder arthroplasty with Aequalis Perform+ posterior augmented glenoid implants in Walch B2 and B3 glenoids.

BACKGROUND: Posterior glenoid wear and persistent static posterior subluxation of the humeral head (SPSH) are associated with early failure of anatomic total shoulder arthroplasty (aTSA). In patients with Walch B2 or B3 glenoid types, posterior augmented glenoid (PAG) implants have been reported to successfully correct SPSH on short-term follow-up. However, the medium-term and long-term clinical and radiological outcomes of PAG remain unclear. METHODS: We prospectively followed 10 consecutive patients (mean age, 67.9 ± 8.9 years; 5 females) with primary glenohumeral osteoarthritis and B2 or B

Predictors of functional outcome after extra-articular scapular fracture stabilization with Brodsky approach.

PURPOSE: Although surgical stabilization for displaced extra-articular scapular fractures is increasingly utilized, there remains limited evidence on predictors of functional recovery. This study aimed to evaluate long-term functional and radiographic outcomes after stabilization using the Brodsky posterior approach and to determine whether glenopolar angle or lateral border medialization independently predicts postoperative function. We hypothesized that improved radiographic parameters would be associated with better outcomes. METHODS: This retrospective cohort included 16 patients with dis

Preliminary Results of Coracoid Excision for Anterior Shoulder Pain in Brachial Plexus Birth Injury.

BACKGROUND: Secondary shoulder deformities are the most frequent sequelae of upper trunk-type brachial plexus birth injury (BPBI). Coracoid process dysplasia was recently described as being associated with glenoid retroversion and was recognized to block successful humeral head relocation. There are little to no reports on coracoid process deformity causing anterior shoulder pain. Our hypothesis is that coracoid process impingement causes anterior shoulder pain in BPBI, which is relieved by coracoid excision. This study presents the short-term results of this new technique in BPBI patients. M

Long Thoracic Nerve Decompression and Transfer: Single-Center Case Series of Clinical and Electromyographic Outcomes.

BACKGROUND: Long thoracic nerve (LTN) decompression is considered in recalcitrant scapular winging secondary to chronic LTN palsy. Nerve transfer, typically from the thoracodorsal nerve (TDN), is suggested if, despite adequate decompression, intraoperative nerve stimulation demonstrates no improvement. Literature concerning transfer is scarce. To evaluate and compare these 2 procedures' clinical and electrical outcomes, we performed a single-center, retrospective case series of all LTN decompression patients with or without transfer for chronic LTN palsy, examining postoperative adapted Medica

Preferred screw placement area identified by imaging based quadrant and heatmap analysis for long screw fixation in Ogawa I coracoid process fractures.

Ogawa I coracoid process fractures are relatively rare. Surgical fixation requires precise determination of the screw length and direction to avoid iatrogenic injuries. Identifying the preferred screw placement area (PSPA) through imaging techniques is crucial for enhancing surgical safety and stability. This study utilized shoulder computed tomography (CT) data from 100 adult patients, employing three-dimensional (3D) modelling and simulated surgeries to analyze the PSPA for long screw fixation in Ogawa I coracoid process fractures. Scapular Y-view two-dimensional (2D) fluoroscopic images wer
