A male patient in his early 30s underwent minimally invasive repair of pectus excavatum. According to standard Nuss bar procedure, a 30-degree thoracoscope was introduced through a right midaxillary 10 mm trocar in the 4th intercostal space. Two bars and five stabilisers were placed in a retromuscular position. After discharge, the patient experienced right upper back pain requiring prolonged opioid usage for three months and right scapular winging limiting functional activities. After conservative treatment with physiotherapy for 11 months, the patient still suffered from residual scapula ala

BACKGROUND: Acquired acromial compromise, including thinning (less than 30% of the normal acromion) or fragmentation resulting from acromiohumeral impingement or previous acromioplasty, is a concern in reverse shoulder arthroplasty (RSA). This condition may lead to shoulder pain and difficulties in arm elevation because of acromial insufficiency fracture.
QUESTIONS/PURPOSES: (1) Do patients with acromial compromise (thinning less than 30% of normal acromion or fragmentation) have worse functional outcome scores, ROM, and strength after RSA compared with patients without acromial compromise? (

The Latarjet procedure was first described in 1954. It is a nonanatomic procedure that requires transfer of the coracoid process, together with the conjoint tendon, to the anterior glenoid margin. The aim is to prevent recurrent anterior shoulder dislocation through a "triple blocking" effect, which includes restoring bone loss, providing a dynamic sling effect, and performing capsulolabral repair. Despite the long history of the Latarjet procedure, studies evaluating its impact on scapulothoracic kinematics are sparse. However, there is a concern that scapulothoracic dyskinesia may occur owin

Bursal acromial reconstruction/resurfacing, acromiograft, and biologic tuberoplasty are all terms used to describe the concept of fixing a graft (human dermal allograft or fascia lata) to the undersurface of the acromion or onto the greater tuberosity with the aim of achieving pain relief and improved function in patients with massive irreparable cuff tears. The rationale for these procedures is based on the biomechanical concept that an interpositional spacer can reverse superior migration of the humeral head, increase acromiohumeral distance, reduce subacromial contact pressure, and limit pa

CASE: We report a unique case of dynamic scapular winging due to compression of the long thoracic nerve by a ventral scapular osteochondroma, representing a combination of mechanical and neural causes. Arthroscopic resection of the lesion was performed, which led to complete resolution of the symptoms.
CONCLUSION: By reporting this case, we aimed to increase awareness of the importance of a correct etiological diagnosis of dynamic scapular winging, so that targeted treatment can be addressed. Arthroscopic resection seems ideal for this indication because it reduces the risk of complications a

BACKGROUND: Augmented baseplates can be effective at addressing eccentric glenoid wear in reverse total shoulder arthroplasty. However, these implants often come in a limited number of predetermined shapes that require additional reaming to ensure adequate glenoid seating. This typically involves complex instrumentation and can have a negative impact on implant stability. Modular baseplate augmentation based on intraoperative measurements may allow for more precise defect filling while preserving glenoid bone. The purpose of this investigation was to assess the stability of a novel ringed base

BACKGROUND: Weber rotational osteotomy that increases humeral retrotorsion in patients with anterior shoulder instability has become unpopular because of recurrence of instability and high rates of early-onset osteoarthritis (OA). However, the wear pattern in patients after rotational osteotomy remains unknown. The aim of this study was to determine the influence of surgically increased humerus retrotorsion on glenohumeral and scapulohumeral centering in a long-term follow-up.
METHODS: The data of 18 shoulders in 18 patients diagnosed with a unilateral chronic recurrent anterior shoulder inst

BACKGROUND: Severe posterior glenoid bone loss with glenohumeral osteoarthritis with an intact rotator cuff can be managed with reverse shoulder arthroplasty but requires lateralization and version correction to avoid potential complications, such as instability, notching and implant failure. Angled bone grafting with humeral head autograft can provide durable glenoid bone stock, but results have been mixed. The purpose of this study was to evaluate patient-reported and objective outcomes as well as complication and failure rates for patients who underwent angled humeral head autografting for

BACKGROUND: Posterior humeral subluxation (PHS) in B2 and B3 glenoid is a cause of asymmetric long-term stress on the glenoid and the potential reason for glenoid loosening in anatomic total shoulder arthroplasty and painful glenoid erosion in hemiarthroplasty with metallic heads. We hypothesized that corrective and concentric (C2) reaming of the glenoid associated with pyrocarbon hemiarthroplasty (HA-PYC) could improve the centering of the humeral head and decrease the risk of persistent painful glenoid erosion in young and active patients with B2 and B3 glenoid.
METHODS: Between 2014 and 20

BACKGROUND: Optimal management of retroversion in anatomic total shoulder arthroplasty (aTSA) remains controversial, and limited attention has been directed to the impact of glenoid inclination. Prior biomechanical study suggests that residual glenoid inclination generates shear stresses that may lead to early glenoid loosening. Combined biplanar glenoid deformities may complicate anatomic glenoid reconstruction and affect outcomes. The goal of this matched-cohort analysis was to assess the relationship between biplanar deformities and midterm radiographic loosening in aTSA.
METHODS: The stud

BACKGROUND: Despite improved visualization, the use of arthroscopic surgery to perform the Latarjet procedure has not decreased the rates of complications and glenohumeral osteoarthritis (OA) in the long term. Many of the reported complications are related to the use of screws for bone block fixation with freehand drilling.
PURPOSE: To evaluate the long-term (at a minimum 10-year follow-up) clinical and radiological outcomes of the arthroscopic Bristow-Latarjet procedure using a posterior guided drilling technique and suture button for coracoid bone graft fixation.

BACKGROUND: To date, long-term results of the arthroscopic repair of glenoid rim fractures are missing. The aim of this study was, to evaluate clinical and radiographic results following arthroscopic repair of anteroinferior glenoid fractures using anchors or bioabsorbable compression screws after a mean follow-up period of 10 years.
METHODS: Clinical outcome measures included evaluation of recurrent instability, the Constant Score, Subjective Shoulder Value, Rowe Score (RS), Western Ontario Shoulder Instability Score, and Melbourne Instability Shoulder Score. X-ray images were obtained for a

In theory, extrinsic anterolateral acromial impingement of the rotator cuff can contribute to cuff tearing. Thus, acromioplasty may be performed concomitantly with arthroscopic rotator cuff repair (ARCR). A review of recent randomized controlled trials confirms that patient long-term outcomes after ARCR are superior when acromioplasty is performed when compared to no concomitant acromioplasty, while complication and retear rates are similar. This is in contrast to previous reviews identifying no significant clinical differences in postoperative functional outcomes, patient-reported outcomes, o

Septic nonunion after vertebral fusion can lead to significant patient disability. The management of septic nonunions usually involves surgical debridement, bone fixation, and antibiotic therapy. Particularly challenging is lumbosacral vertebral nonunions, which necessitate a difficult surgical approach. We present a novel approach using a scapula tip free flap through an intra-abdominal approach to reconstruct a L5-S1 vertebral defect after a septic nonunion. Our patient, 31-year-old man, with no medical conditions, had a fusion of L5-S1 due to severe lower back pain secondary to isthmic spon

The term "dyskinesia" has often been used interchangeably with "winging," leading to ambiguity in the literature. To address this, the broader term "scapulothoracic abnormal motion (STAM)" was introduced to describe any abnormal position or movement of the scapula on the chest, resulting in pain and dysfunction. Scapulothoracic abnormal motion has a wide range of causes, including musculoskeletal imbalances such as pectoralis minor hyperactivity, neurological impairments such as long thoracic nerve palsy, and genetic conditions like facioscapulohumeral muscular dystrophy (FSHD).
