Arthroscopic Findings After Osteochondral Allografts of the Knee: Proposed Classification Scheme and Associations With Graft Failure.

BACKGROUND: Limited studies exist on the gross appearance of osteochondral allografts (OCAs) at the time of second-look arthroscopy to determine whether the graft and adjacent cartilage have signs of degeneration contributing to failure. PURPOSE: To describe the gross appearance of OCAs and the adjacent cartilage at the time of a second-look knee operation to help propose an accurate definition of structural and clinical OCA failure and to standardize and classify the intraoperative findings associated with structural graft failure. STUDY DESIGN: Case series; Level of evidence, 4. METHODS:

Radiobiological Effects of Low-Dose Radiation in Normal Fibroblasts of Patients with Head and Neck Cancer Treated with Induction Chemotherapy Combined with Low-Dose Fractionated Radiation.

The aim of the study was to define radiobiological effects of single and fractionated low doses in normal fibroblasts in 40 patients with squamous cell carcinoma of the head and neck (HNSCC) treated with induction chemotherapy combined with low-dose fractionated radiation (LDFR) and to answer the question regarding the role of low-dose hyper-radiosensitivity (HRS) in these effects. HRS status was determined using flow cytometry-based clonogenic survival assay (cells were irradiated with doses 0.1-4 Gy of 6 MV X-rays). Radiobiological effects (cell kill, kinetics of DSB recognition and repair,

Impact of Pre-Transplant Hypomethylating Agents on Reduced-Intensity Conditioning Allogeneic Hematopoietic Cell Transplantation in Myelodysplastic Syndrome With Good or Intermediate-Risk Cytogenetics.

Allogeneic stem cell transplant (Allo-SCT) remains the only curative option for patients with myelodysplastic syndrome (MDS). The benefit of cytoreduction with hypomethylating agents (HMAs) prior to reduced-intensity conditioning (RIC) transplant, particularly in patients with less than 10% bone marrow blasts, remains uncertain. To evaluate the impact of pretransplant HMA in a large registry-based cohort of patients with MDS who underwent RIC Allo-SCT. This retrospective study utilized registry data to analyze transplant outcomes among patients with MDS who received pretransplant HMA and those

Total marrow irradiation-based conditioning for allogeneic hematopoietic stem cell transplantation in patients with hematologic malignancies: A multicenter real-world study.

Currently, there is very limited large-scale real-world data on the use of total marrow irradiation (TMI) as conditioning for allogeneic hematopoietic stem cell transplantation (allo-HSCT). The primary objective of this study was to evaluate the real-world feasibility, efficacy, and safety of TMI-based conditioning in a large multicenter cohort. We retrospectively included consecutive patients undergoing allo-HSCT with TMI-based conditioning across four Chinese centers (2017-2024). The primary endpoint was overall survival (OS); secondary endpoints included disease-free survival (DFS), graft-v

Discontinued therapies for sickle cell disease: status and future directions.

INTRODUCTION: Over the past decade, the therapeutic landscape for sickle cell disease (SCD) has expanded beyond hydroxyurea to targeted small molecules, monoclonal antibodies, and transformative cellular and gene therapies. However, not every investigational approach survives the rigor of randomized trials, post-marketing surveillance, and commercial realities. High-profile discontinuations and trial failures have important implications for a disease community with longstanding unmet needs and a historical mistrust of biomedical research. AREAS COVERED: This review analyzes the proximate caus

A novel full digital workflow for dental autotransplantation: surgical accuracy and clinical outcomes in a prospective cohort study.

INTRODUCTION: Dental autotransplantation is a biologically conservative alternative to prosthetic or implant-supported rehabilitation, though achieving predictable outcomes remains challenging. The use of digital workflows integrating CBCT-based planning, 3D modelling, and customized surgical guides may improve surgical precision and reproducibility. This study aimed to evaluate the accuracy and clinical outcomes of a full-digital workflow for dental autotransplantation. METHODS: Patients with a non-restorable tooth and a healthy third molar suitable for donation were included. Preoperative C

Refined detection of CD34⁺CD38⁻CD45RA⁺ leukemic stem cells using a single-tube flow cytometry assay and its strong association with measurable residual disease in acute myeloid leukemia: a retrospective cohort study.

BACKGROUND: Leukemic stem cells (LSCs) are the cellular reservoir most strongly implicated in relapse of acute myeloid leukemia, yet their operational detection by multiparameter flow cytometry remains challenging because of immunophenotypic overlap with normal progenitors and variability across assays. Including CD45RA in the CD34⁺CD38⁻ gating strategy substantially improves discrimination between malignant and normal stem/progenitor populations and thus enables more precise LSC enumeration in a single-tube format. Given the clinical importance of accurately quantifying the LSC compartment, w

Clinical implications of variant allele frequencies of genes in patients with acute myeloid leukemia.

PURPOSES: Although next-generation sequencing (NGS) has allowed for the detection of mutations in acute myeloid leukemia (AML), the clinical relevance of variant allele frequency (VAF) for the majority of mutations is unknown. This study aimed to investigate whether the VAFs of AML-associated mutations could improve the prognostic classification in AML. METHODS: Bone marrow samples from 254 AML patients were included for targeted sequencing. Univariate Cox regression analyses identified mutations affecting survival, and optimal VAF cutoffs were determined by X-tile software. Multivariate Cox

Suicidal Ideation Experienced by Long-Term Survivors of Blood or Marrow Transplantation: A Blood or Marrow Transplant Survivor Study Report.

High-intensity therapeutic exposures and prolonged immune suppression place blood or marrow transplant (BMT) survivors at a high risk of long-term physical and psychological morbidity that could lead to suicidal ideation. We examined suicidal ideation in BMT survivors who received autologous or allogeneic BMT between 1974 and 2014 at one of three transplant programs and survived ≥2 y after BMT. Overall, 3841 BMT survivors and 1261 siblings who completed a 255-item survey at age ≥18 y were included. The survey assesses sociodemographics, chronic health conditions, and psychological health. Evaluation of smoking status as a prognostic factor under immune checkpoint inhibitor treatment: A single center analysis.

Smoking may influence cancer treatment outcomes; however, there is conflicting evidence on the efficacy of immune checkpoint inhibitors (ICIs) in active smokers. It has been suggested that smokers receiving ICIs may achieve improved outcomes compared with non-smokers. The present study aimed to challenge this assumption by investigating the prognostic impact of smoking status and other characteristics in patients with metastatic or advanced cancer undergoing ICI therapy. In the present study, patients with predetermined solid tumors who received ICIs either as a monotherapy or in combination r

MRD conversion during oral azacitidine maintenance correlates with outcomes in predominantly favorable-risk AML.

BACKGROUND: Oral azacitidine improves survival in patients with AML in first complete remission who are not candidates for allogeneic transplantation. Most favorable-risk AML patients harbor molecular markers enabling sensitive measurable residual disease (MRD) monitoring. While MRD is a well-established prognostic factor in AML, the clinical significance of MRD dynamics during maintenance therapy remains incompletely defined. METHODS: We conducted a multicenter, real-world retrospective study of 30 patients with AML, predominantly ELN favorable-risk, treated with oral azacitidine maintenance

Treosulfan-based conditioning regimen for allogeneic HSCT in children with AML: study of the Polish Pediatric Study Group for HSCT.

BACKGROUND: Allogeneic HSCT (allo-HSCT) is indicated in high risk pediatric AML patients in CR1 and in all patients in CR2. The myeloablative busulfan-based conditioning for HSCT has been accepted commonly as the standard in them, but its short- and long-term complications forced the development of alternative regimens. Treosulfan started to be used in pediatric HSCT patients in the year 2000 due to its better toxicity profile along with sufficient myeloablative, antileukemic and immunosuppressive effects. OBJECTIVE: The objective of the study was to evaluate the long-term results of the firs

Real-world treatment patterns of patients with large B-cell lymphoma over time and into a post-CAR T approval era.

A significant proportion of patients with large B-cell lymphoma (LBCL) experience relapsed or refractory (R/R) disease after first-line (1L) therapy. Chimeric antigen receptor T-cell (CAR T) therapy is approved for R/R LBCL, yet real-world use remains unclear. This retrospective study evaluated treatment patterns and survival outcomes in patients potentially eligible for CAR T in the Flatiron Health Research Database. Patients diagnosed with LBCL from 2011 to 2024 who initiated 1L therapy were assessed. Predictors of mortality before the opportunity to initiate second-line (2L) therapy were ev

RUNX1 Alterations in Pediatric Myeloid Malignancies: Divergent Germline and Somatic Trajectories.

RUNX1 alterations contribute to pediatric myeloid malignancies through both germline predisposition syndromes and somatic leukemogenic events, but their clinical and biological significance in children remains incompletely defined. This retrospective single-center study evaluated six pediatric patients with myelodysplastic syndromes or acute leukemias harboring RUNX1 variants, integrating clinical, cytogenetic, and targeted next-generation sequencing data, with germline status confirmed using non-hematopoietic tissues. Three patients carried germline RUNX1 variants, characterized by antecedent

Risk Assessment for Venous Thrombosis in Lymphoma and Emerging Biomarkers.

Venous Thrombosis is a frequent and clinically significant complication in lymphoma patients, resulting in increased morbidity, mortality and therapeutic challenges. The pathophysiological mechanisms underlying lymphoma-associated thrombosis are multifactorial, involving patients' clinical characteristics, tumour biology, systemic inflammation, endothelial dysfunction and therapy-induced prothrombotic changes. Traditional predictive tools for cancer-associated thrombosis (CAT) have shown suboptimal application in lymphoma patients due to disease-specific heterogeneity. The ThroLy score was dev

Pediatric cancer survival in the United States from 2001 to 2021.

BACKGROUND: Approximately 15,000 United States (US) children and adolescents, 0-14 and 15-19 years old, respectively, are diagnosed with cancer annually. Although overall survival now exceeds 80%, cancer remains the second leading cause of death in this population, and improvements have not been uniform across subgroups. METHODS: Five- and 10-year relative survival (RS) for cancer cases diagnosed during 2001-2021 among individuals <20 years old at diagnosis were calculated using the National Program of Cancer Registries database, covering 87.4% of the US population. Results were stratified by
