Haemophilia Management in Natural Disasters-From Field to Consensus Report.

OBJECTIVE: The earthquake in Türkiye had a profound impact on both haemophilia patients and healthcare services in the region. This experience highlighted the need for an effective action plan for patients with bleeding disorders in extraordinary situations. METHODS: This is a cross-sectional survey study designed to identify the needs of patients and physicians, as well as problems in health services. Two questionnaires were prepared and analysed. The first questionnaire consists of 26 questions for patients, including physical problems, difficulties in accessing doctors/treatment, and stora

Design Strategies to Target Joint Resident Mesenchymal Stem Cells for Osteochondral Regeneration.

Restoration of the osteochondral unit remains a major challenge in regenerative orthopaedics, largely due to the limited intrinsic healing capacity of articular cartilage and the complex, multilayered nature of the cartilage-bone interface. Osteochondral regeneration must accommodate differences in cellular composition, vascularization, metabolic demand, and mechanical properties between cartilage and bone, while simultaneously recreating a stable, functional interface. While exogenous mesenchymal stem cell (MSC) therapies have dominated the field, their clinical translation has been hindered

Total body irradiation across conditioning intensities: historical context, dose-fractionation strategy, and the role of the chemotherapy backbone.

Total body irradiation (TBI) is a form of radiotherapy that provides a uniform dose of ionizing radiation to the entire body. It has been a mainstay of the conditioning regimen for allogeneic hematopoietic stem cell transplantation (allo-HSCT) for many decades. The clinical usefulness of TBI is based on the balance of its cytotoxic and immunosuppressive effects, which allow for the eradication of remaining malignant cells, including sanctuary sites, as well as the establishment of a successful transplant. Early "supra-lethal" single-dose schedules of TBI were highly effective but also came wit

Chimeric antigen receptor (CAR) T-cell therapy in dermatology: A narrative review.

Chimeric antigen receptor (CAR) T-cells are autologous T-cells genetically engineered to express an antigen receptor that can recognise and kill cells expressing that target antigen. Originally developed for refractory or relapsed B-cell and plasma cell malignancies, CAR T-cell therapy is now being explored as a promising treatment for B-cell-mediated autoimmune diseases. CAR-T cells produce 'deep' B-cell depletion and cause 'resetting' of the immune system, thereby achieving long-lasting remissions and potentially even 'cure', addressing some major limitations of current immunosuppressive and

Quality of life of patients with Kaposi Sarcoma: a cross-sectional study.

BACKGROUND: Kaposi sarcoma (KS) is an angioproliferative tumor linked to Human Herpesvirus 8 (HHV8) infection, presenting in five clinical variants. While much research has focused on its clinical characteristics and treatment, limited data exist on its psychological impact and effect on quality of life (QoL). This study aims to evaluate the emotional burden of KS in a large cohort of Italian patients. METHODS: A cross-sectional observational study was conducted at a tertiary care center in Milan, Italy, from January to December 2023. Patients with KS were recruited and completed the European

Delayed Full-Thickness Skin Grafting After Skin Cancer Resection: A Systematic Review.

BACKGROUND: Reconstruction after Mohs micrographic surgery (MMS) or wide local excision (WLE) requires careful consideration of defect size, location, depth, margin status, and patient preferences to optimize cosmetic, functional, and oncologic outcomes. The role of delayed full-thickness skin grafts (FTSGs) during reconstruction after skin cancer removal remains less defined. OBJECTIVE: To characterize the indications, technique, and efficacy of delayed FTSG after MMS or WLE for skin cancer. MATERIALS AND METHODS: Databases were searched for studies on delayed FTSG after MMS or WLE for skin

The role of a multidisciplinary approach in non-melanoma skin cancer management.

A multidisciplinary team (MDT) is fundamental for properly managing non-melanoma skin cancer (NMSC). Surgery is the first treatment choice for early-stage disease; however, in cases with a high risk of recurrence or when demolitive approaches would not achieve safe margins, radiotherapy (RT) or an MDT approach is required. We retrospectively revised the population evaluated at our weekly MDT meeting, and all patients were discussed. A case series of 130 patients visited from July 2021 to March 2024 was collected. In-person visits were performed. The male/female ratio was 69.5/31.5%. Elderly pa

Intent to Treat Analysis of the Primary and Secondary Outcomes for the ODINN Intact Fish Skin Graft for Deep Diabetic Foot Wounds Trial.

There is a significant need for trials that evaluate the treatment of University of Texas (UT) grade 2 and 3 diabetic foot ulcers (bone, joint, or tendon exposed wounds). We undertook a trial looking at the effect of intact fish skin graft (IFSG) on these deep and difficult-to-heal ulcers. 262 patients Intent to Treat (ITT) patients with UT grade 2 and 3 DFUs were randomised to receive intact fish skin graft (IFSG) or a standardised treatment (SOC) that adhered to the International Working Group on the Diabetic Foot (IWGDF) guidelines. The secondary endpoints that were measured included wound

Desmosine as a Novel Biomarker Candidate for Pulmonary and Cutaneous Chronic Graft-Versus-Host Disease.

Pulmonary and cutaneous fibrotic manifestations are common in chronic graft-versus-host disease (cGVHD) and can significantly reduce patients' quality of life and functional status. Plasma desmosine (pDES), a byproduct of elastin breakdown, may serve as a biomarker for fibrotic tissue involvement in cGVHD. The primmary objective was to evaluate the association between plasma desmosine levels and pulmonary and cutaneous manifestations in patients with cGVHD. We measured pDES levels in 81 cGVHD patients enrolled in the NIH cGVHD Natural History Study using liquid chromatography-tandem mass spect

Therapeutic Outcomes in VEXAS Syndrome: A Multicenter Comparative Cohort of Allogeneic Hematopoietic Stem Cell Transplantation and Hypomethylating Agents.

Hypomethylating agents (HMA) and allogeneic hematopoietic stem cell transplantation (alloHSCT) have both demonstrated remissions in VEXAS; however, comparative data is lacking. We conducted a multicenter, retrospective analysis of 66 patients diagnosed with VEXAS syndrome treated with HMA (n = 35) or alloHSCT (n = 31). Baseline characteristics such as genetics, co-morbidities, and performance status were balanced between the groups, except older age in the HMA group. Median follow-up from therapy initiation was 18 months (95% CI: 11-26), and 14 (21%) deaths were reported (alloHSCT n = 3; HMA n

Reconstruction of a Complex Pretibial Wound Using Dermal Regeneration Template and Split-Thickness Skin Graft.

The reconstruction of the distal leg is challenging due to watershed vascularity and scarce soft-tissue bulk. While traditional reconstruction guidelines rely on vascularized flap coverage, such interventions are higher-risk options in certain patients with significant medical co-morbidities. This case report describes non-microsurgical reconstruction of a distal-third pre-tibial wound in a patient with a history of renal transplant, type 2 diabetes, coronary artery disease, and peripheral vascular disease. In this patient, chronic immunosuppression induced blunt wound healing, while concurren

Beyond the First Graft: Skin Cancer Risk and Risk Stratification in Retransplanted Solid Organ Recipients.

Solid organ transplant recipients (SOTRs) face a markedly elevated risk of post-transplant skin cancer, particularly cutaneous squamous cell carcinoma (cSCC), driven by chronic immunosuppression, including the direct oncogenic effects of some immunosuppressive drugs, and cumulative carcinogenic risk factors including ultraviolet radiation exposure. Retransplanted solid organ transplant recipients (R-SOTRs) represent a growing yet under-recognised subgroup, carrying additional vulnerability as the second or third transplant will often require more profound immunosuppression. Despite this, no re

Sustained-release oxymatrine hydrogel enhances the survival of transplanted placental mesenchymal stem cells and promotes wound healing in diabetic mice.

Chronic diabetic wounds represent a significant clinical challenge due to impaired healing processes characterized by persistent inflammation, compromised angiogenesis, and a hostile microenvironment. Despite the therapeutic potential of mesenchymal stem cells (MSCs), their post-transplantation survival remains suboptimal in such pathological conditions. Here, we developed a sustained-release oxymatrine-loaded hyaluronic acid methacryloyl (HAMA) hydrogel to enhance placental mesenchymal stem cell (PMSC)-mediated repair in diabetic wounds. In a streptozotocin-induced diabetic mouse model with f

Endothelin: A Potential Universal Systemic Biomarker.

BACKGROUND: Endothelins is a family of vasoconstrictive peptides known for their high potency. They are mainly synthesized and secreted by the endothelial cells lining the blood vessels in response to various stimuli. Their main physiological role is the regulation of vascular tone, affecting blood pressure and tissue perfusion. OBJECTIVE: The aim of this review was to evaluate the importance of Endothelin-1 (ET-1) plasma levels as a marker in diagnosis, disease burden, or development, due to its vascular effects. METHODS: Data was collected and grouped, from several studies in different org

Algorithm to reduce inter-rater variability in assessment of intermacs patient profiles.

BACKGROUND: Intermacs Patient Profiles (IPP) are subjective assessments of patient acuity of illness assigned at the time of durable left ventricular assist device implantation (dLVAD). Previous studies have demonstrated provider variability with IPP assignment. We developed a novel algorithm to derive a patient's IPP acuity of illness to assist clinicians with IPP assignment. METHODS: Twenty standardized clinical vignettes were developed characterizing a patient's status at time of evaluation for dLVAD implantation. Vignettes included the full range of patient acuity of illness represented b

Machine Learning Models for Posttransplant Lymphoproliferative Disorder (PTLD) Risk Prediction in Thoracic Transplantation.

Posttransplant lymphoproliferative disorder (PTLD) is the second most common malignancy in thoracic transplant recipients and is associated with poor survival. Accurate risk prediction is essential for prevention and management. Adult (≥18 years) heart, lung, and heart-lung transplant recipients were extracted from the Scientific Registry of Transplant (SRTR) and the United Network of Organ Sharing (UNOS) databases. A total of 160 donor and recipient pretransplant variables, including demographics, laboratory tests, induction therapy, and human leukocyte antigens (HLA), were analyzed. Risk sco

Improving 10-year cardiovascular risk prediction in patients with established cardiovascular disease: flexible addition of risk predictors on top of the SMART2 risk score.

BACKGROUND: Guidelines recommend using the SMART2 model, estimating the risk of recurrent cardiovascular (CV) events, to support treatment decisions in patients with established atherosclerotic CV disease (ASCVD). They further outline that adding biomarkers, comorbidities, anthropometric, and social factors may improve these predictions. AIMS: To investigate the added predictive value of guideline-outlined factors including biomarkers, comorbidities, anthropometric, and social factors on top of the SMART2 model using an approach enabling their use as add-on predictors. Risk factors when considering heart transplants with donors aged 45 or above: Development of a Novel Mortality Risk Score using UNOS data.

BACKGROUND: Current International Society for Heart and Lung Transplantation guidelines support the use of donor hearts aged ≥45 if significant coronary artery disease is excluded and ischemic time remains <4 hours. This study aimed to identify additional risk factors affecting patient survival and develop a risk score for recipients of older donor hearts. METHODS: We performed a retrospective cohort study of adult heart transplants using donors aged ≥45 from January 2000 to June 2024, using United Network for Organ Sharing data.
