INTRODUCTION: Minority waitlisted kidney patients are less likely to resolve an inactive status change. We investigate race/ethnicity-based differences in the reasons for and resolution of waitlist inactivation.
METHODS: The Organ Procurement and Transplantation Network database was queried (December 2014-January 2020). Cause-specific Cox regression and competing risks analysis were conducted to determine the association of inactive status and race/ethnicity, stratified by specific reasons a patient was made inactive.
RESULTS: Of the 94,035 patients, 52.5% experienced inactivity. 47.0% were

INTRODUCTION: C3 glomerulopathy (C3G) is a rare kidney disease caused by alternative pathway (AP) overactivation. This analysis describes demographic and clinical characteristics of patients with C3G in the United States (US) and evaluates chronic kidney disease (CKD) stage progression.
METHODS: This retrospective cohort study used electronic health records (EHRs) to identify patients aged ≥ 12 years with a C3G diagnosis (index) and with continuous clinical activity ≥ 12 months before (baseline) and ≥ 6 months after index (follow-up; until death or data end). Patients were stratified by prior

PURPOSE: The past two decades witnessed increased use of risk-adjusted standardized measures such as standardized mortality ratios (SMRs) and standardized incidence ratios (SIRs) in institutional comparisons between healthcare units. Estimating the variance of these standardised measures is necessary for creating false discovery rates (FDRs) in studies that use funnel plots for assessing healthcare providers’ performance. Theoretical delta-method, and approximate approaches such as bootstrapping and Bayesian approaches are commonly used. Using Bayesian hierarchical logistic regression for obta

Background: A decrease in governmental vaccination initiatives and diminishing public enthusiasm for vaccines could jeopardize vaccine uptake, potentially endangering those who are most at risk. In this survey study, we evaluated the current acceptance rates of the newly developed monovalent XBB1.5-adapted COVID-19 vaccine among kidney transplant recipients and dialysis patients in Austria and Israel and identified factors influencing vaccine acceptance. Methods: The survey involved a total of 656 patients aged 18 and older and was carried out from 20 November to 21 December 2023, at the Medic

BACKGROUND: Kidney disease requiring renal replacement therapy is common after lung transplantation and is associated with reduced survival. In 2023, the Organ Procurement and Transplantation Network implemented a safety-net policy that prioritizes kidney offers for lung transplant recipients with renal failure in the first year after transplantation. The provision is likely to increase sequential transplantation. However, the long-term survival benefit of kidney transplantation for lung transplant recipients, whether sequential or simultaneous, is not well established.

In this work we study the lifetime Medicare spending patterns of patients with end-stage renal disease (ESRD). We extract the information of patients who started their ESRD services in 2007-2011 from the United States Renal Data System (USRDS). Patients are partitioned into three groups based on their kidney transplant status: 1-unwaitlisted and never transplanted, 2-waitlisted but never transplanted, and 3-waitlisted and then transplanted. To study their Medicare cost trajectories, we use a semiparametric regression model with both fixed and bivariate time-varying coefficients to compare grou

OBJECTIVES: Comparisons of COVID-19 incidence between kidney transplant recipients and patients who did not receive kidney transplant are underexplored in various geographic regions.
MATERIALS AND METHODS: This Indian, single-center, retrospective study analyzed COVID-19 data of patients hospitalized between May 12, 2020, and January 11, 2021. A propensity matching score was used to compare outcomes between the 2 groups. We also used multivariable Cox proportional hazard analyses to assess association of kidney transplantation with mortality.
RESULTS: Of the 1627 COVID-19 cases, 179 were kid

There are no studies which have compared the risk of severe COVID-19 and related mortality between transplant recipients and nontransplant patients. We enrolled two groups of patients hospitalized for COVID-19, that is, kidney transplant recipients (KTR) from the French Registry of Solid Organ Transplant (n = 306) and a single-center cohort of nontransplant patients (n = 795). An analysis was performed among subgroups matched for age and risk factors for severe COVID-19 or mortality. Severe COVID-19 was defined as admission (or transfer) to an intensive care unit, need for mechanical ventilati

BACKGROUND AND OBJECTIVES: In the general population, sleep disorders are associated with mortality. However, such evidence in patients with CKD and ESKD is limited and shows conflicting results. Our aim was to examine the association of sleep apnea with mortality among patients with CKD and ESKD.
DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: In this prospective cohort study, 180 patients (88 with CKD stage 4 or 5, 92 with ESKD) underwent in-home polysomnography, and sleep apnea measures such as apnea hypopnea index (AHI) and nocturnal hypoxemia were obtained. Mortality data were obtained fr

BACKGROUND: The neuroanatomic basis for cognitive impairment in chronic kidney disease (CKD) is incompletely characterized. We performed advanced quantitative structural magnetic resonance imaging (MRI) to determine whether CKD affects brain structure and whether poorer neurocognitive performance in CKD is associated with structural brain differences.
STUDY DESIGN: Cross-sectional.
SETTING & PARTICIPANTS: 85 individuals with CKD stages 2 to 5 and 63 healthy controls, aged 8 to 25 years PREDICTORS: CKD versus control, estimated glomerular filtration rate (eGFR), and kidney transplant status w

BACKGROUND: Despite remarkable improvement in renal function attributable to kidney transplantation, the burden of cardiovascular disease (CVD) among kidney transplant recipients (KTRs) remains high in the post-transplant period. Aggressive use of statins in KTRs may make lipoprotein ratios correlate better with atherosclerotic vascular disease (AsVD) when compared with traditional lipid profile parameters. We therefore evaluated the clinical and echocardiographic correlates of AsVD among non-diabetic, stable, black KTRs in South Africa.
METHODS: This was a cross-sectional study of 41 adult (

BACKGROUND: Norovirus (NV) infection has been reported as a cause of severe chronic diarrhea in transplant recipients, but this entity remains under-recognized in clinical practice, leading to diagnostic delays. Transplant clinicians should become familiar with this syndrome in order to facilitate early detection and management.
METHODS: Demographic, clinical, and outcomes variables were summarized from a series of transplant recipients with positive stool NV reverse transcription polymerase chain reaction (RT-PCR) assays at Johns Hopkins in 2013-2014. Factors associated with longer duration

BACKGROUND: Cardiovascular complications represent a major cause of morbidity and mortality for patients who received kidney transplantation (KT). However, the impact of KT and chronic immunosuppression on platelet response to clopidogrel in patients undergoing coronary or peripheral revascularization procedures remains unclear. This cohort study compares platelet responsiveness to clopidogrel as assessed byvasodilator-stimulated phosphoprotein (VASP) phosphorylation.
METHODS: The study population was divided between chronic kidney disease (CKD) patients who underwent KT (n = 36) and non-tran
