Myocarditis is a clinically and aetiologically heterogeneous disease that is associated with substantial morbidity and mortality, with approximately a million new cases and 17,000 deaths annually worldwide. However, the incidence of myocarditis varies widely by geographical region, sex and age. The aetiology of myocarditis is multifactorial and shaped by regional, demographic and socioeconomic contexts. In high-income countries, parvovirus B19 and human herpesvirus 6 are the predominant cardiotropic viruses that induce lymphocytic myocarditis, particularly in children. By contrast, in low-to-m

The Organ Procurement and Transplantation Network amended Status 2 heart allocation criteria in September 2025, requiring documented failure of high-dose inotropes before candidates could qualify via intra-aortic balloon pump (IABP) or percutaneous endovascular ventricular assist device (PEVAD) support. Using OPTN data, we compared seasonally matched pre-policy (September 2024-June 2025, n=4,206) and post-policy (September 2025-June 2026, n=4,231) adult candidates. While standard Status 3 high-dose inotrope listings increased 2.0% to 4.1% and standard Status 2 IABP/PEVAD listings decreased 18.

BACKGROUND: Menstrual abnormalities, including heavy menstrual bleeding, amenorrhea, and dysmenorrhea, are frequently encountered among children, adolescents, and young adults with congenital and acquired heart disease, yet remain underrecognized in routine cardiovascular care. Anticoagulation, thrombosis risk, heart failure physiology, and limited high-quality evidence complicate management and contribute to substantial variation in practice.
METHODS: The Advanced Cardiac Therapies Improving Outcomes Network convened a multidisciplinary working group of cardiologists, hematologists, gynecolo

BACKGROUND: Primary graft dysfunction (PGD) is the leading cause of early morbidity and mortality after lung transplantation (LTx). Critically ill candidates may receive priority on the transplant waiting list; however, the relationship between prioritization and the development of severe PGD remains unclear.
METHODS: We performed a retrospective analysis of a prospectively maintained cohort including all consecutive LTx performed at a tertiary center between January 2016 and June 2025. Recipients were categorized according to prioritization status. Severe PGD was defined as grade 3 PGD accor

BACKGROUND: The prognostic value of left ventricular (LV) entropy from T1 mapping in pulmonary arterial hypertension (PAH) remains unexplored.
STUDY DESIGN AND METHODS: This prospective study enrolled patients with PAH who underwent cardiovascular magnetic resonance (CMR) within 72h of right heart catheterization (RHC). Myocardial native T1, extracellular volume fraction (ECV) and entropy were calculated in mid-ventricular short-axis slice. The primary endpoint was defined as all-cause mortality or lung transplantation. Outcomes were further analyzed across groups stratified by survival statu

BACKGROUND: Endothelins is a family of vasoconstrictive peptides known for their high potency. They are mainly synthesized and secreted by the endothelial cells lining the blood vessels in response to various stimuli. Their main physiological role is the regulation of vascular tone, affecting blood pressure and tissue perfusion.
OBJECTIVE: The aim of this review was to evaluate the importance of Endothelin-1 (ET-1) plasma levels as a marker in diagnosis, disease burden, or development, due to its vascular effects.
METHODS: Data was collected and grouped, from several studies in different org

BACKGROUND: Intermacs Patient Profiles (IPP) are subjective assessments of patient acuity of illness assigned at the time of durable left ventricular assist device implantation (dLVAD). Previous studies have demonstrated provider variability with IPP assignment. We developed a novel algorithm to derive a patient's IPP acuity of illness to assist clinicians with IPP assignment.
METHODS: Twenty standardized clinical vignettes were developed characterizing a patient's status at time of evaluation for dLVAD implantation. Vignettes included the full range of patient acuity of illness represented b

Posttransplant lymphoproliferative disorder (PTLD) is the second most common malignancy in thoracic transplant recipients and is associated with poor survival. Accurate risk prediction is essential for prevention and management. Adult (≥18 years) heart, lung, and heart-lung transplant recipients were extracted from the Scientific Registry of Transplant (SRTR) and the United Network of Organ Sharing (UNOS) databases. A total of 160 donor and recipient pretransplant variables, including demographics, laboratory tests, induction therapy, and human leukocyte antigens (HLA), were analyzed. Risk sco

BACKGROUND: Guidelines recommend using the SMART2 model, estimating the risk of recurrent cardiovascular (CV) events, to support treatment decisions in patients with established atherosclerotic CV disease (ASCVD). They further outline that adding biomarkers, comorbidities, anthropometric, and social factors may improve these predictions.
AIMS: To investigate the added predictive value of guideline-outlined factors including biomarkers, comorbidities, anthropometric, and social factors on top of the SMART2 model using an approach enabling their use as add-on predictors.

BACKGROUND: Current International Society for Heart and Lung Transplantation guidelines support the use of donor hearts aged ≥45 if significant coronary artery disease is excluded and ischemic time remains <4 hours. This study aimed to identify additional risk factors affecting patient survival and develop a risk score for recipients of older donor hearts.
METHODS: We performed a retrospective cohort study of adult heart transplants using donors aged ≥45 from January 2000 to June 2024, using United Network for Organ Sharing data. Multivariable Cox regression identified risk factors for mortal

BACKGROUND: Solid organ transplantation is the definitive treatment for end-stage organ failure, yet access is highly inequitable worldwide. Comparable long-term evidence across organs, regions, and development settings remains limited.
METHODS: Data from the WHO Global Observatory on Donation and Transplantation (GODT) for six solid organs (2008-2023) were analyzed. Per-million population (PMP) rates and the estimated annual percentage change (EAPC) were calculated; disparities by Human Development Index (HDI), WHO regions, and Global Burden of Disease 2021 (GBD 2021) regions were quantified

INTRODUCTION: Disparities in heart and lung transplantation persist in the United States, yet most studies focus only on patients already referred or listed. We aimed to evaluate racial and socioeconomic disparities in access and receipt of transplantation among patients with chronic heart or lung disease.
METHODS: We conducted a retrospective analysis using the National Inpatient Sample (2016-2022) to identify adults hospitalized with chronic heart or lung disease. Patients undergoing heart or lung transplantation and combine heart and lung transplantation were identified via ICD-10 codes. M

BACKGROUND: Acute cellular rejection (ACR) following heart transplantation (TPL) compromises graft function and survival. The programmed cell death-1 (PD-1)/PD-1 ligand-1 (PD-L1) pathway represents an immune checkpoint that maintains peripheral immune tolerance, but its expression and significance in human cardiac allografts with ACR remain unclear. Thus, we investigated PD-1/ PD-L1 expression in endomyocardial biopsies from heart TPL recipients to clarify the role of this pathway in the ACR of human cardiac allografts and explore the potential of therapeutic modulation of PD-1/PD-L1 in this s

Professor Yasunaru Kawashima was awarded the Order of Culture for the year of 2025. This marks the first time to be given to a pioneer in the field of cardiovascular surgery, and is etched into the history of thoracic surgery in Japan. Through his long-standing academic works in cardiac surgery at Osaka University, Professor Kawashima has not only dedicated to establish the status of cardiovascular surgical outcomes as to the international level, but also introduced new surgical techniques for several congenital heart malformations where curative surgeries were previously undeveloped. VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is a disorder discovered in 2020 that bridges haematology, immunology and genetics. VEXAS syndrome arises from somatic mutations in UBA1, which encodes an E1 ubiquitin-activating enzyme, acquired in haematopoietic stem cells. These mutations disrupt ubiquitin-dependent protein homeostasis, triggering proteotoxic and inflammatory stress that drives systemic inflammation, cytopenias and clonal haematopoiesis. Clinically, VEXAS syndrome presents predominantly in older men with glucocorticoid-dependent inflammation, neutroph

BACKGROUND: Patients with diabetes experience diminished survival after coronary artery bypass grafting (CABG), despite comparable long-term graft patency to patients without diabetes. Reduced survival may be related to accelerated progression of native coronary disease proximal to the graft in patients with diabetes, reducing native flow reserve in the case of progressive graft failure. This phenomenon has not been previously investigated.
METHOD: Patients referred for CABG at the Royal Melbourne Hospital were included if both their preoperative and postoperative coronary angiography reports

BACKGROUND: People with HIV (PWH) experience disparate access to solid organ transplantation (SOT) but are eligible to be organ recipients and donors in Australia. Clinician decision-making about the allocation and utilization of organs is central to transplant access. This study aimed to determine the influence of HIV infection, and other key attributes, in decision-making about SOT recipients and donors.
METHODS: We conducted 2 discrete choice experiment (DCE) surveys, using a D-efficient design, among Australian transplant providers. Clinicians were presented with 6 hypothetical SOT recipi

BACKGROUND: Advancements in pediatric lung transplantation, including support devices, the Lung Allocation Score (LAS), and the subsequent Composite Allocation Score (CAS), have led to significant improvements in survival on the waiting list. Policy changes, like the removal of the Donation Service Area in 2017, aimed to further improve outcomes, but their impact in more recent years remains unclear. Our study examines recent waiting list survival trends and key factors affecting outcomes for pediatric lung transplant candidates.
METHODS: Pediatric patients (n = 535) listed for lung transplan

BACKGROUND: The United Kingdom (UK) was one of the first countries to pioneer donation after circulatory death (DCD) heart transplants. To establish equitable access to DCD hearts, a nationally-funded DCD program was established in 2020. We report the results of this program and examine retrieval techniques, ischemic times and survival.
METHODS: This multi-center, retrospective, observational study compared DCD versus donation after brain death (DBD) heart transplant outcomes from 2020 to 2024. DCD hearts were retrieved using direct procurement and perfusion (DPP) with/without abdominal normo
