Does bronchoalveolar lavage lymphocytosis predict prognosis in fibrotic hypersensitivity pneumonitis, and is this relationship influenced by low-dose immunomodulatory therapy at the time of BAL?

BACKGROUND: Bronchoalveolar lavage (BAL) lymphocytosis is associated with improved prognosis in fibrotic hypersensitivity pneumonitis (fHP), although its prognostic value is unclear in patients receiving immunomodulatory treatment. We investigated whether treatment tapered to the lowest tolerated level at the time of BAL modifies associations with disease severity, one-year lung function, and transplant-free survival. METHODS: We analysed 247 fHP patients undergoing BAL, including 162 untreated and 85 receiving immunomodulatory therapy at the time of BAL. Associations between BAL lymphocytosi

Reduced post-transplant survival in patients bridged with durable LVADs following the 2018 US heart allocation policy change: OPTN/UNOS registry analysis.

PURPOSE: In 2018, the US implemented heart allocation changes to address evolving trends in waiting list mortality. This study examines the impact of the allocation changes on post-transplant survival in patients bridged with durable left ventricular assist devices (LVAD) compared to non-LVAD heart transplant recipients. METHODS: We used the OPTN/UNOS registry to identify adult (≥18 years), first-time, heart-only transplant recipients from 2014 to 2023. Recipients were categorized by date of transplant into PRE- (1/1/14-17/10/18) and POST- (18/10/18-6/10/23)groups and LVAD status at transplan

Improved peripheral airway reactance is associated with clinical remission after biologic therapy in severe asthma.

BACKGROUND: Small airways dysfunction (SAD) is increasingly recognized as a treatable trait in severe asthma and may influence the likelihood of achieving clinical remission. OBJECTIVE: To investigate whether oscillometry-defined SAD parameters were associated with remission after biologic or standard-of-care treatment. METHODS: Data from 237 adults with Global Initiative for Asthma-defined severe asthma were retrospectively collected at a tertiary specialist center. A total of 127 individuals received add-on biologic therapy (benralizumab, dupilumab, or tezepelumab) and 110 received standar

Real-World Evaluation of Recombinant Zoster Vaccine Uptake and Outcomes in Solid Organ Transplant Recipients: A Retrospective Cohort Study.

Herpes zoster incidence is increased after solid organ transplantation. We analyzed adult recipients of liver, heart, lung, kidney, pancreas or dual-organ transplants between January 2018 and December 2025. We assessed recombinant zoster vaccine uptake and the association between pre-transplant vaccination and post-transplant herpes zoster in 2595 patients. Median follow-up was 2.8 years [IQR 1.6-4.8]. Thirty-two percent of patients completed the two-dose recombinant zoster vaccine series before transplant. A total of 110 herpes zoster cases occurred. By vaccine status, incidence rates (per 1,

Identification of a Rejection Signature and Distinct Inflammatory Alveolar Macrophage Populations Post-lung Transplantation.

BACKGROUND: The long-term outcome of lung transplantation remains inferior to that of other solid organ transplants, primarily due to rejection/infection. This pilot study investigates longitudinal changes in the bronchoalveolar lavage (BAL) leukocyte transcriptomes and phenotypes, and their impact on clinical outcomes. METHODS: BAL samples were collected at baseline and 1-12 mo posttransplantation. Leukocyte phenotypes, activation status, and biomarkers were characterized alongside their gene expression profiles. RESULTS: We identified distinct BAL leukocyte transcriptional signatures assoc

Infectious consequences of antimicrobial change from vancomycin-piperacillin/ tazobactam to vancomycin-cefepime in the prevention of AKI during orthotopic heart transplantation.

BACKGROUND: Empiric antimicrobial selection in orthotopic heart transplantation (OHT) is critically important as post-operative infections are a significant cause of morbidity and mortality. While variability of antimicrobial selection exists between centers, both combinations of vancomycin and piperacillin-tazobactam (VPT) and vancomycin and cefepime (VC) are frequently used. VPT drug combination has been associated with increased risk of acute kidney injury (AKI) in multiple reports. Our study aimed to evaluate the infectious impact of changing the empiric antimicrobial selection from VPT to

The Impact of Preexisting Diabetes Mellitus on Chronic Lung Allograft Dysfunction After Lung Transplantation.

BACKGROUND: Chronic lung allograft dysfunction (CLAD) remains a major cause of mortality after lung transplantation. Diabetes mellitus (DM) may contribute to CLAD through metabolic dysregulation. We evaluated post-lung transplant outcomes in recipients with DM. METHODS: Adults undergoing first-time, isolated lung transplant (November 2017 to December 2024) with at least 6 months of follow-up were identified in the United Network for Organ Sharing registry and stratified by DM diagnosis at waitlist registration. CLAD was defined as ≥20% forced expiratory volume in 1 second and/or forced vital

Longitudinal Mediastinal Lymph Node Dynamics Associate With Mortality and Fibrosis Progression in Fibrotic Interstitial Lung Disease.

BACKGROUND: Although mediastinal lymph node (MLN) enlargement is common in fibrotic interstitial lung disease (ILD), its dynamic behavior and prognostic value are unclear. RESEARCH QUESTION: Do MLN size, count, and progression on high-resolution chest CT imaging associate with physiological decline, fibrosis progression, and transplant-free survival, and do these factors improve risk models? STUDY DESIGN AND METHODS: This study included a prospective cohort of patients with fibrotic ILD from a tertiary center (N = 1,122) with independent replication (n = 181). MLNs were scored by using stand

Impact of Acute Kidney Injury on Hospitalization Outcomes in Patients with Chronic Kidney Disease Undergoing Cardiac Procedures with Cardiopulmonary Bypass in the United States.

BACKGROUND: Acute kidney injury (AKI) is a common complication after cardiac surgery and is associated with more infections, prolonged recovery, longer hospital stays, increased readmission rates, greater costs, and higher mortality. Patients with pre-existing chronic kidney disease (CKD) are at particularly high risk, with this risk increasing as renal function worsens. Damage caused by AKI may be irreversible, despite a return to baseline GFR, compounding stress on remaining nephrons. Understanding risk in these populations may ultimately help to improve prevention and outcomes. OBJECTIVE:

Left Ventricular Venting Strategies for Cardiogenic Shock in Patients Treated With Extracorporeal Cardiopulmonary Support V-A ECMO: From Left Ventricular Unloading to Offloading.

While veno-arterial extracorporeal membrane oxygenation (V-A ECMO) has become central in the management of patients in advanced stages of cardiogenic shock due to its relative ease of deployment, there is no standardized practice for left ventricular venting. This study evaluated the relative impact of distinct left ventricular venting strategies-no venting (control), unloading (intra-aortic balloon pump or Impella CP), and offloading (Impella 5.0/5.5)-on successful device weaning and overall hospital survival. A retrospective analysis of 232 patients (2018 to 2021) with refractory cardiogenic

Pediatric lung transplantation in Japan across legislative and allocation reforms: A nationwide registry analysis of waitlist and post-transplant outcomes.

BACKGROUND: Pediatric lung transplantation is a life-saving option for end-stage lung disease, but national data from Japan, particularly regarding the impact of recent policy changes, are limited. METHODS: Nationwide registries of candidates (deceased-donor waitlist) and recipients in Japan (1998-2024) were analyzed with follow-up through July 2025. Outcomes were evaluated across three eras defined by the 2010 legislative revision and the 2020 pediatric priority rules. Waitlist outcomes were assessed within 2 years of listing and post-transplant survival within 2 years of transplantation (ad

General anaesthesia versus monitored anaesthesia care and postoperative outcome in patients with elevated right ventricular systolic pressure undergoing low-risk surgical procedures: a retrospective single-centre cohort study.

BACKGROUND: Pulmonary hypertension, often indicated before surgery by elevated right ventricular systolic pressure (RVSP) on echocardiography, is associated with increased perioperative risk. For low-risk procedures in which either general anaesthesia or monitored anaesthesia care may be selected, comparative data on postoperative outcomes are limited. METHODS: We conducted a single-centre retrospective observational cohort study of 4962 adults with RVSP >50 mm Hg who underwent low-risk procedures. Eligible procedures included endoscopy, bronchoscopy, catheterisation laboratory interventions,

Outcomes of Lung Transplantation From Asphyxiation Donors: Reassessing Hypoxic Injury and Implications for Donation After Circulatory Death.

OBJECTIVES: With persistent donor shortages limiting lung transplantation (LT), optimizing the use of extended-criteria donors is essential. Asphyxiation (AXP) and donation after circulatory death (DCD) donors both experience hypoxic-ischaemic injury, raising concern for graft performance. We evaluated the long-term outcomes of AXP donor LT and the effect of ex vivo lung perfusion (EVLP) on survival. METHODS: Adult recipients (≥18 years) of primary LT (2014-2024) in the United Network for Organ Sharing registry were stratified by donor cause of death: AXP vs non-AXP. Survival was estimated by

Risk Factors and Outcomes of Reintubation Following Combined Heart-Lung Transplantation.

BACKGROUND: Unplanned reintubation after combined heart-lung transplantation (HLT) significantly affects morbidity and mortality, yet national data are limited. We evaluated the incidence, predictors, timing, and outcomes associated with reintubation after HLT. METHODS: We retrospectively reviewed adults undergoing primary HLT in the UNOS registry from January 2004 to September 2024. Patients were stratified by reintubation status (defined as re-established mechanical ventilation (MV) after initial extubation). Propensity matching (1:1) balanced recipient and donor characteristics. Multivaria

Acute lung allograft dysfunction predicts lung allograft failure: Construct validity of the 2026 International Society for Heart and Lung Transplantation spirometric definition.

INTRODUCTION: Acute lung allograft dysfunction (ALAD) has been proposed as a screening criterion to identify lung transplant recipients at increased risk for graft loss. ALAD was defined formally in a 2026 International Society for Heart and Lung Transplantation (ISHLT) Delphi consensus statement but has not been empirically evaluated. We sought to test the construct validity of the ISHLT ALAD definition by testing the hypothesis that ALAD is associated with increased risk of graft failure. METHODS: We included 86,885 spirometry measurements from 2,877 participants in the 8-center Lung Transp

Molecular Heterogeneity and Prognostic Implications of Low-Grade Acute Cellular Rejection (1R/1A) in Heart Transplantation.

INTRODUCTION: International Society for Heart & Lung Transplantation (ISHLT) grade 1R/1A cellular rejection (ACR) is generally considered clinically low risk and does not routinely prompt treatment. Whether this histological finding reflects biologically meaningful alloimmune activation and confers prognostic risk remains uncertain. We sought to define the molecular and clinical significance of low-grade ACR by integrating intragraft transcriptomic profiling with circulating biomarkers and post-biopsy outcomes. METHODS: We analyzed consecutive for-cause endomyocardial biopsies (EMBs) with pai

Precision Diagnosis in APOL1 Kidney Disease With the p.N264K M1 Protective Variant.

IMPORTANCE: The APOL1 M1 (p.N264K) variant protects against G2-associated APOL1 focal segmental glomerulosclerosis (FSGS) and chronic kidney disease (CKD). However, the utility of knowing an individual's M1 status in guiding kidney disease diagnosis and other clinical scenarios remains underexplored. OBJECTIVE: To test 2 hypotheses: (1) in patients with APOL1 high-risk (HR) genotype kidney disease with at least 1 G2 allele, M1 can distinguish APOL1 CKD from non-APOL1 CKD; (2) in people with APOL1 low-risk (LR) genotypes, M1 is independently associated with protection against FSGS and CKD. Beyond Donor-Specific Antibodies: Molecular Characterization of Antibody-Mediated Rejection Using Tissue-Based Transcriptomics in Heart Transplantation.

BACKGROUND: Antibody-mediated rejection (AMR) contributes to graft dysfunction after heart transplantation, yet histopathology and circulating donor-specific antibodies (DSA) may incompletely capture intragraft biology. We characterized molecular ABMR by human leukocyte antigen (HLA)-DSA status and, within DSA-negative cases, non-HLA antibody status. METHODS: We analyzed 579 for-cause endomyocardial biopsies with paired Molecular Microscope Diagnostic System (MMDx) and histopathology. Molecular scores, transcripts, donor-derived cell-free DNA (dd-cfDNA), and gene-expression profiling (GEP) we
