Normothermic Kidney Perfusion: Current Status and Future Perspectives.

Normothermic machine perfusion (NMP), the most recent advancement in solid organ preservation, enables ex situ maintenance of grafts in a physiologically active state, offering a significant advantage over traditional cold storage methods. Whereas NMP is now widely adopted for clinical preservation of livers, hearts, and lungs, its application in kidney transplantation remains relatively limited. In this context, NMP holds promise for expanding the use of marginal donor kidneys by enhancing viability assessment and potentially restoring function in grafts that might otherwise be discarded. Outcomes of Living Related Kidney Donors Following Donor Nephrectomy in Pakistan.

BACKGROUND: Kidney transplantation is the preferred treatment for end-stage renal disease. In Pakistan, living related kidney donors constitute the primary source of kidney transplantation. However, data on postdonation outcomes in this population remain limited. OBJECTIVE: This study aims to evaluate the short- and intermediate-term effects of unilateral nephrectomy on donor kidney function, physical parameters, and glycemic status among living related kidney donors in Pakistan. METHODS: This retrospective observational cohort study was conducted at the Pakistan Kidney and Liver Institute f

Preconditioning donors with corticosteroids improves early lung graft immunity.

BACKGROUND: Preclinical studies have recently revealed the critical role of innate immunity in determining lung transplantation outcomes. Although the International Society for Heart and Lung Transplantation recommends high-dose corticosteroid administration to donors, this practice is inconsistently applied worldwide. Investigating its impact on the donor lung's innate immune response - an unexplored area - could provide valuable evidence to support adoption of donor preconditioning with corticosteroids, beyond their traditional administration to recipients. METHOD: We used a cross-circulato

Rethinking heart transplant allocation: Aligning urgency, equity, and outcomes.

Since the implementation of the new allocation system for heart transplantation in 2018, various unintended consequences have emerged. In response, a recent policy amendment was proposed that took effect in September 2025 requiring transplant programs to document failure of inotropic therapy prior to the use of percutaneous endovascular mechanical circulatory support or intra-aortic balloon pump. The goal of this new policy is to reduce inflation and waitlist congestion of Status 2 and help to further risk stratify patients within the current Status 2 tier by waitlist urgency. This viewpoint h

Fifty-year pediatric heart transplant outcomes: A pediatric-adult center linked analysis.

PURPOSE: Long-term outcomes after pediatric heart transplant (pHT) are rarely reported owing to young programs and loss of follow-up after transition to adult care. We leveraged our program age, center volume, and linked electronic medical record to analyze 50-year outcomes after pHT, including events after adult care transfer. METHODS: Retrospective review of all pHT over 50 years at Stanford (August 19, 1974 to August 19, 2024). Patient characteristics and posttransplant outcomes, including death, retransplant, and subsequent kidney transplantation, were ascertained from the pediatric and a

Distinct plasma proteome in severe pulmonary hypertension associated with chronic lung disease.

BACKGROUND: The mechanisms underlying the development and severity of pulmonary hypertension (PH) in chronic lung diseases (CLD) remain incompletely understood. To identify relevant pathobiological pathways, we analyzed the protein expression profile in plasma of patients with chronic obstructive pulmonary disease (COPD) and fibrosing interstitial lung disease (ILD), stratified by the presence and severity of PH, and explored similarities with idiopathic pulmonary arterial hypertension (iPAH). METHODS: Plasma samples from 114 patients with CLD-36 without PH (17 COPD, 19 ILD), 33 with nonsever

Lung SBRT Outcomes for Inoperable Early-Stage Lung Cancer Are Impaired in Patients With Solid Organ Transplants.

PURPOSE: To characterize outcomes for solid organ transplant patients with medically inoperable early-stage lung cancer (ES-LC) treated with lung stereotactic body radiotherapy (SBRT). METHODS: We surveyed our institutional review board-approved prospective lung SBRT data registry from 2003 to 2023 for any transplant patients. Patterns of failure were assessed, as well as overall survival (OS) and disease-free survival (DFS). Univariate prognostic factors for OS and DFS were identified with Cox proportional hazards regression. RESULTS: Twenty-eight of 1976 definitively treated patients (1.4%

Impact on post-transplantation outcomes in pediatric patients undergoing heart-lung transplantation by recipient age at the time of transplantation.

OBJECTIVE: The impact of age at transplantation on the outcomes of pediatric heart-lung transplantation (HLTX) recipients is not well described. METHODS: The UNOS database (1987-2023) was retrospectively queried for primary pediatric HLTX in patients ≤18 year of age. The cohort was dichotomized into ages <12 years (Young group [YG]) and ≥12 years (Old group [OG]) due to its impact on the previously used lung allocation score (LAS). The data were extracted for clinic characteristics and outcomes. Standard statistical tests were used. RESULTS: 217 (113 YG, 104 OG) patients underwent HTLX. The

Impact of right ventricular reserve function during exercise on aortic valve opening in patients with left ventricular assist device.

BACKGROUND: Although aortic valve (AV) opening during exercise has fewer adverse events in patients with a left ventricular (LV) assist device (LVAD), factors associated with it remain unclear. We hypothesized that right ventricular (RV) reserve function is associated with the AV opening status. This study aimed to explore the hemodynamic parameters observed at rest and during maximal exercise and investigate the association between the RV reserve function and AV opening status in patients with an LVAD. METHODS: We prospectively examined 25 consecutive patients with an LVAD who underwent inva

OPTN/SRTR 2023 Annual Data Report: Introduction.

The OPTN/SRTR 2023 Annual Data Report presents the status of the solid organ transplant system in the United States from 2012 through 2023. Organ-specific chapters are presented for kidney, pancreas, liver, intestine, heart, and lung transplant. Each organ-specific chapter is organized to present waitlist information, donor information (both deceased and living, as appropriate), transplant information, and patient outcomes. Data pertaining to pediatric patients are generally presented separately from the adult data; however, many chapters now have a Donation section, which includes data on adu

Five-year experience of heart transplantation following donation after circulatory death.

OBJECTIVE: The United States' experience with heart transplantation following donation after circulatory death (DCD HT) has expanded since clinical adoption in 2019. We aimed to examine a large institution's outcomes associated with DCD HT versus HT following donation after brain death (DBD). METHODS: Adult heart recipients and corresponding donors at a single quaternary academic center from January 2019 to October 2024 were included. Recipient and donor data were extracted from the institution's electronic medical record and the United Network for Organ Sharing registry, respectively. The pr

Non-pharmacological Management of Fibrosing Interstitial Lung Diseases.

Non-pharmacological interventions are integral to the multidisciplinary management of fibrosing interstitial lung diseases (ILDs), complementing pharmacological therapies by addressing functional decline, symptom burden, and quality of life. Palliative care is fundamental, offering a structured approach to symptom control-particularly dyspnea- and facilitating advance care planning. Oxygen therapy may provide symptomatic benefit in patients with resting, nocturnal, or exertional hypoxemia, though evidence remains limited and extrapolated mainly from idiopathic pulmonary fibrosis cohorts. Pulmo

BACKGROUND: Venovenous extracorporeal membrane oxygenation (VV-ECMO) is often used for lung rest in critically ill patients, including pregnant patients. OBJECTIVES: We aimed to determine how pregnant and non-pregnant patients requiring VV-ECMO differed from each other and how pregnancy impacted in-hospital mortality. METHODS: We used the National Inpatient Sample to identify a cohort of female patients requiring VV-ECMO from 2016 to 2021, stratified by pregnancy status. Baseline characteristics were compared using Pearson chi-square test. Predictors of in-hospital mortality, including pregn

Clinical validation of an AI-based blood testing device for diagnosis and prognosis of acute infection and sepsis.

Lack of reliable diagnostics for the presence, type and severity of infection in patients presenting to emergency departments with non-specific symptoms poses considerable challenges. We developed TriVerity, which uses isothermal amplification of 29 mRNAs and machine learning algorithms on the Myrna instrument to determine likelihoods of bacterial infection, viral infection and need for critical care interventions within 7 days. To validate TriVerity, the SEPSIS-SHIELD study enrolled 1,222 patients with clinically adjudicated infection status and need for critical care intervention within 7 da

Prognostic Relevance of Inflammatory Subphenotypes in Immunocompromised Patients With Sepsis.

OBJECTIVES: Hyperinflammatory and hypoinflammatory molecular subphenotypes in sepsis and acute respiratory distress syndrome have divergent mortality and treatment responses in secondary analyses of randomized controlled trials. However, the prevalence of immunocompromise is low in these populations, and how preexisting immunocompromise contributes to subphenotypes is unknown. We studied two observational sepsis cohorts to test associations between immunocompromise and the hyperinflammatory subphenotype and to assess whether the prognostic relevance of molecular subphenotypes is generalizable

Understanding Failure to Rescue in Solid Organ Transplantation: A Scoping Review.

BACKGROUND: The role of failure to rescue (FTR) in solid organ transplantation (SOT) is less clear. This scoping review aimed to evaluate the incidence, complications, and risk factors for FTR in SOT. METHODS: We systematically searched PubMed, Scopus, and Web of Science for studies reporting on FTR in SOT. Data on incidence, complications, risk factors, and definitions were extracted and synthesized narratively due to heterogeneity across studies. RESULTS: Seven studies were identified: four in liver transplantation, two in lung transplantation, and one in heart transplantation; none addres

Multiple Left Ventricular Ball Thrombi in a Patient With Nonischemic Cardiomyopathy Bridged to Heart-Lung Transplantation.

CASE SUMMARY: We present a case of a young patient with a history of recurrent venous thromboembolism, stroke status post fibrinolysis, and titin-related dilated cardiomyopathy who developed cardiogenic shock in the setting of decompensated heart failure and concomitant large left ventricular thrombus comprising ∼75% of the cavity. Her shock was managed medically as she was evaluated for surgical heart failure therapies. The patient underwent a combined heart-lung transplant and was discharged after an uneventful postoperative course. TAKE-HOME MESSAGES: This case demonstrates that concomitan
