Patient-reported outcomes after single-stage spiral preputial graft urethroplasty for panurethral strictures: a single-centre prospective study.

BACKGROUND: Panurethral strictures, involving the entire anterior urethra, present a significant reconstructive challenge. While buccal mucosal grafts are commonly used, limitations in graft length and donor site morbidity persist. The spiral preputial graft (SPG) offers a promising alternative, enabling long, hairless grafts without additional donor morbidity. This study prospectively evaluates patient-reported outcomes (PROs) following single-stage SPG urethroplasty for panurethral strictures. METHODS: A prospective interventional study was conducted between January 2021 and June 2024 at a t

Strength training intervention with creatine/whey protein supplementation vs placebo on frailty in older adults with cirrhosis (STRIVE): A Randomized Controlled Trial.

Background Older adults with cirrhosis are particularly vulnerable to developing frailty with poor outcomes including increased falls, physical disability, hospitalizations, and all-cause mortality. Current management of frailty among older adults with cirrhosis is limited to general lifestyle recommendations including increased exercise and optimized diet, and few studies examining exercise interventions include patients with Child Pugh Class B or C. Whey protein and creatine are well-studied supplements commonly used to improve physical performance in athletes and have recently demonstrated

Staged Management of Extensive Suprapubic Wound Necrosis After Hysterectomy Using Negative Pressure Wound Therapy and Delayed Skin Grafting.

Postoperative wound complications are a major clinical challenge, especially in patients with comorbidities such as overweight and diabetes mellitus. Extensive abdominal wall necrosis after gynaecologic surgery is uncommon but may lead to prolonged healing, increased morbidity and the need for complex reconstructive procedures. Negative pressure wound therapy (NPWT) is an effective option for complex surgical wounds, promoting granulation tissue, improving perfusion and reducing bacterial burden. This report describes the case of a 58-year-old woman with overweight status and Type 2 diabetes m

Stem Cell-Derived Exosomes in Wound Healing and Skin Regeneration: Emerging Therapeutic Strategies and Mechanisms.

Chronic cutaneous wounds and traumatic skin injuries remain a major clinical challenge, characterized by dysregulated healing phases, high susceptibility to microbial infection, and suboptimal response to conventional therapies. Stem cell-derived exosomes (SC-Exos) have emerged as a paradigm-shifting, cell-free nanotherapeutic platform that harnesses the paracrine secretome of stem cells while avoiding the immunological and proliferative complications inherent to direct cell transplantation. Exosomes derived from diverse stem cell sources orchestrate multifactorial wound repair by modulating k

Predictors and consequences of perioperative blood transfusions in deep inferior epigastric perforator flap breast reconstruction.

BACKGROUND: Autologous breast reconstruction with deep inferior epigastric perforator (DIEP) flaps plays a critical role in restoring form and function to the breast after mastectomy. As these operations can involve substantial blood loss, they sometimes require perioperative transfusion. We aimed to identify risk factors for transfusion and associated complications in DIEP flap reconstruction. METHODS: A retrospective cohort study was conducted of all patients who underwent DIEP flap reconstruction at a single academic institution. Patients were stratified based on perioperative transfusion

Clinical and molecular markers of vitiligo: a narrative review of early-stage insights into disease activity and treatment response.

Vitiligo fluctuates between progressive and quiescent states, complicating decisions about when to intervene, how to select therapy and how to monitor response. In this review, we integrate bedside phenotypes with multilevel molecular readouts to summarize emerging evidence linking clinical signs and biologic signals to disease activity and treatment response in clinical studies. For staging, tool-free and standardized assessments such as Koebner phenomenon, confetti-like depigmentation, Wood's lamp and dermoscopy provide visible clues associated with active disease. Measurements from blood, s

Rook to the Rescue: A Case Series on the Novel Use of Eggcrate Foam Bolsters for Skin Grafts in Penile and Genital Reconstruction: Rook Bolster Technique for Genital Reconstruction.

INTRODUCTION: Complex genital reconstruction of acquired buried penis after weight loss, Fournier's gangrene, Hidradenitis suppurativa, or trauma remains challenging due to contaminated wound beds, irregular surfaces, and shear forces that compromise graft survival. Eggcrate foam bolsters elicit successful outcomes in burns and mobile surfaces like the tongue. We present a rook-configured eggcrate foam bolster for stabilizing penile grafts in complex genital reconstruction. We aim to assess outcomes and develop a reproducible dressing protocol tailored to the challenges of this anatomical regi

Gene Therapy for Dystrophic Epidermolysis Bullosa.

Dystrophic epidermolysis bullosa (DEB) is a rare, debilitating genodermatosis caused by loss-of-function variants in COL7A1, resulting in type VII collagen (C7) deficiency and defective anchoring fibrils, which are essential for dermal-epidermal adhesion. COL7A1 is a large gene, and C7 is synthesized by both keratinocytes and fibroblasts, and extracellularly secreted, where it forms a supramolecular network that stabilizes the dermal-epidermal junction. Human genotype-phenotype correlations suggest that residual C7 expression (~10% of normal levels) may confer a milder disease phenotype, where
