Ulnar Epiphysiodesis for Distal Radius Physeal Arrest: Comparing Early Detection to Late Presentation Management.

BACKGROUND: Premature physeal arrest of the distal radius (PPAR) is a rare but serious complication following pediatric fractures that may result in ulnar impaction syndrome. The purpose of this study was to compare outcomes following distal ulnar epiphysiodesis between those identified without deformity (early cohort) and those identified with deformity (late cohort). The hypothesis was that treating children before deformity development would improve patient-reported outcomes (PROs). METHODS: All patients younger than 18 years of age who underwent ulnar epiphysiodesis due to PPAR from 2011

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Radial Longitudinal Deficiency: Description of a Novel Surgical Technique and Clinical Cases.

Radial longitudinal deficiency III and IV present as a short upper limb, functional elbow, and wrist with severe radial and palmar angulation, where the carpus articulates with the radial and palmar edge of the ulna, allowing limited mobility in a nonfunctional position. Surgical treatment aims to correct radial angulation and flexed carpal position, often altering carpal positioning over the distal ulna and impacting wrist mobility. In addition, fixation through distal ulnar epiphysis affects its growth. Although these procedures improve appearance, functionality remains suboptimal. This stud

Implant survival at four years for hemiarthroplasty and total shoulder arthroplasty in the treatment of atraumatic avascular necrosis of the humeral head.

AIMS: Hemiarthroplasty (HA) and total shoulder arthroplasty (TSA) are often the preferred forms of treatment for patients with atraumatic avascular necrosis of the humeral head when conservative treatment fails. Little has been reported about the survival of HA and TSA for this indication. The aim of this study was to investigate the differences in revision rates between HA and TSA in these patients, to determine whether one of these implants has a superior survival and may be a better choice in the treatment of this condition. METHODS: Data from 280 shoulders with 159 primary HAs and 121 TSA

Humeral Head-Split Fracture in Two Dogs.

Two skeletally immature female dogs were each investigated for chronic weight-bearing thoracic limb lameness. The first patient was lame for 2 months following a tumble whilst playing, and the second patient had been intermittently lame since 3 weeks of age. In both cases, radiographic examination of the shoulder revealed fissuring of the caudal humeral head consistent with an incomplete proximal humeral Salter-Harris type IV fracture with an Enoki-mushroom-like appearance of the caudal fragment, where two heads rise from a common stem. There was secondary neoarthrosis of the caudal humeral he

Short-term outcomes of gradual ulnar lengthening in multiple hereditary osteochondromas of the forearm with monolateral external fixator.

Hereditary multiple exostoses is an autosomal dominant genetic condition primarily affecting long bones. Forearm deformities, including wrist ulnar deviation, ulnar shortening, radial or ulnar bowing, and radial head dislocation, are common manifestations. Gradual ulnar lengthening is suggested as a viable treatment option for managing these deformities. This retrospective single-center study included 15 forearms treated with gradual ulnar lengthening using a monolateral external fixator by a single surgeon. Clinical and radiological characteristics were recorded preoperatively and during foll

Advancing HR-pQCT-based homogenised FE models with smooth structured hexahedral meshes.

Nonlinear homogenised finite element (hFE) models can accurately predict stiffness and strength of ultra-distal sections of the radius and tibia using in vivo HR-pQCT images. Recent findings showed good stiffness prediction at these distal sections but a limited ability to reproduce experimental strain localisation. The coarseness of voxel-based meshes reduces the computational effort at the cost of heavily simplifying the underlying geometry of the cortex, the gradient of material properties, and the resulting strain distribution. To overcome these limitations, we present a comprehensive appr

Reliability of unicortical plate fixation for metacarpal shaft fractures: A preliminary clinical report with short-term follow-up.

BACKGROUND: Unicortical plate fixation offers several theoretical advantages in the treatment of metacarpal shaft fractures compared to bicortical fixation. This approach avoids the potential hazard of excessive drilling into the volar cortex, thus minimizing damage to surrounding soft tissues and helping prevent complications related to improperly sized screws. These benefits prompted our team to conduct a preliminary clinical study to investigate the effectiveness and safety of this approach. In this study, we present our initial experience assessing the efficacy of utilizing unicortical pla
