Interference of high 50/60-Hz electric and magnetic fields with the normal functioning of active cardiac implants: implications for newly implanted workers returning to or entering work in the utility sector.

The 50/60-Hz electric and magnetic fields in substations or under high-voltage lines may interfere with the operation of cardiac implantable electronic devices (CIEDs). This review of the literature over the last 20 years provides guidance to occupational physicians who give orientations when newly implanted workers return to or enter work. For magnetic fields, CIEDs are generally not affected up to 100 μT. For electric fields, CIEDs set to nominal sensitivity and in bipolar mode are unaffected up to 5 kV/m. Clinically significant or symptomatic disturbances are unlikely up to 10 kV/m. Specifi

Procedural Complexity and Bleeding Risk in Patients Undergoing Percutaneous Coronary Intervention.

The impact of complex coronary anatomy on outcomes in patients at high bleeding risk (HBR) undergoing percutaneous coronary intervention (PCI) remains unclear. Patients from a large PCI referral center were labeled as HBR in presence of ≥1 major or ≥2 minor Academic Research Consortium HBR criteria and undergoing complex PCI if they had any of the following: ≥3 vessels treated, ≥3 stents implanted, ≥3 lesions treated, stent length >60 mm, bifurcation lesion with ≥2 stents, PCI of chronic total occlusion, left main, saphenous venous graft or requiring the use of atherectomy. Primary outcomes at

The association of mechanical versus manual chest compressions with neurological outcomes at hospital discharge among out-of-hospital cardiac arrests treated with intra-arrest transport.

AIM: Mechanical chest compression (MCC) devices provide an opportunity to deliver high quality chest compressions during times when manual chest compressions may be challenging. Among non-traumatic out-of-hospital cardiac arrests (OHCA) treated with intra-arrest transport (IAT), we investigated the association of MCC, versus manual chest compressions, and patient outcomes. METHODS: Utilizing the British Columbia Cardiac Arrest Registry, we included adult OHCA cases (January 2019 - June 2025) who were transported to hospital with ongoing chest compressions. We classified patients as MCC or man

Early right ventricular failure following HeartMate 3 left ventricular assist device implantation: Risk factors and outcomes.

BACKGROUND: Right ventricular failure (RVF) remains a major determinant of early morbidity and mortality after left ventricular assist device (LVAD) implantation. The long-term consequences of early RVF in contemporary HM3 recipients remain incompletely characterized. METHODS: We analyzed all HM3 recipients enrolled in the MOMENTUM 3 trials, including the randomized investigational device exemption and the continued access protocol. Patients were stratified by presence or absence of early RVF, according to the 2020 MCS-ARC criteria. Severe RVF was defined as need for right ventricular assist

Optimizing Aesthetic Results in Breast Surgery and Cardiac Implantable Electronic Device (CIED) Placement Using Acellular Biological Matrices.

The increasing number of younger patients requiring cardiac implantable electronic devices (CIEDs) necessitates not only survival but also a preserved quality of life. Aesthetic considerations for CIED placement-particularly in the context of the female breast-are increasingly important, yet current guidelines lack standardized recommendations for ideal generator pocket positioning, especially in the presence of breast implants. We report the first use of bovine pericardium mesh to reposition a migrated, protruding subcutaneous CIED in a patient with prior breast implant reconstruction, who pr

Mechanical Circulatory Support on Coronary Artery Bypass Grafting for Advanced Ischemic Cardiomyopathy: State of the Art.

Ischemic cardiomyopathy is defined as coronary artery disease accompanied by left ventricular dysfunction with an ejection fraction equal to or less than 40%. The substrate of ischemic cardiomyopathy is heterogeneous, characterized by the coexistence of normal, stunned, hibernating, and scarred myocardium within the same myocardial region. Altogether, these components may represent different phases of a single pathological process. It is well-established that the assessment of isolated myocardial viability and ischemia alone has failed to reliably guide the indication for coronary artery bypas

Drug-Coated Balloon Angioplasty vs Drug-Eluting Stents in Noncomplex Coronary Bifurcation Lesions: The REC-CAGEFREE I Trial.

BACKGROUND: Drug-coated balloons (DCBs) are attractive for treating de novo coronary lesions, especially when involving bifurcations; however, their efficacy compared with drug-eluting stents (DES) remains uncertain. OBJECTIVES: The aim of this study was to assess the prognosis of DCBs vs DES in patients with noncomplex bifurcation and nonbifurcation lesions. METHODS: This was a prespecified subgroup analysis of the REC-CAGEFREE I (Paclitaxel-Coated Balloon for Treatment of De-Novo Non-Complex Coronary Artery Lesions) trial, which was an investigator-initiated, noninferiority trial conducted

Image quality assessment in mammography of women with cardiac implantable electronic devices: An insight into radiographers' experiences in Australia.

INTRODUCTION: To report radiographers' experiences of the impact of cardiac implantable electronic devices (CIEDs) on mammographic image quality and assessment; to identify differences between the screening and diagnostic settings. METHOD: A survey was conducted to investigate Australian radiographers' experiences of assessing image quality during mammograms of women with a CIED. Closed Likert scale responses of 'never', 'sometimes', 'often' or 'always' were analysed descriptively using percentages. A Pearson's chi-squared test, with a significance level of p < 0.05, was used to determine dif

Totally endoscopic beating-heart re-redo tricuspid valve repair using smartcanula.

Tricuspid valve surgery inherently carries an increased peri-operative risk. In the redo setting, impaired right ventricular function and the frequent presence of hepatic dysfunction are further amplified by extensive adhesions. Conventional re-sternotomy, although well-established, requires substantial tissue trauma. Therefore, strategies that minimize surgical trauma-such as a minimally invasive approach on the beating heart with limited access-may be advantageous in this high-risk population. In this video tutorial, we present a minimally invasive (totally endoscopic) second re-operation in

Evidence-Based Strategies for the Prevention of Cardiac Implantable Electronic Device Infections: An Up-to-Date Narrative Review.

Background and Objectives: Cardiac implantable electronic device (CIED) infections remain among the most serious complications of pacemaker, implantable cardioverter-defibrillator, and cardiac resynchronization therapy procedures. They are associated with substantial morbidity, mortality, prolonged hospitalization, system extraction, long-term antimicrobial therapy, and increased healthcare costs. As most infections arise from perioperative contamination or procedure-related complications, prevention has become a major priority in contemporary electrophysiology practice. This review aimed to s

Impact of Operating Room Efficiencies on Patient Outcomes Following Primary Coronary Artery Bypass Surgery.

OBJECTIVES: Prolonged cardiopulmonary bypass (CPB) time during coronary artery bypass grafting (CABG) is associated with poor outcomes, however, the association of other operating room (OR) times is less understood. We studied the impact of OR times on outcomes and resource utilization after CABG. METHODS: Patients undergoing isolated primary CABG from a large multicentre regional collaborative were analysed. The impact of risk-adjusted total OR, surgery, non-surgery, CPB, and off-CPB times on morbidity, extubation time, ICU and hospital length of stay (LOS), cost, and mortality, was studied.

A Tale of Two LADs (Left Anterior Descending Arteries): Type IV LAD as a Rare Coronary Anomaly.

A dual left anterior descending (LAD) artery is angiographically defined as the presence of two distinct coronary arteries supplying the LAD territory. It represents a rare coronary artery anomaly in the general population but may have important implications during the evaluation and management of acute coronary syndromes (ACS), percutaneous coronary intervention (PCI), and coronary artery bypass grafting (CABG). Accurate recognition of this variant is essential to avoid misinterpretation of coronary angiograms, incomplete revascularisation, and procedural complications. The objective of this

Development and Validation of the LIFE-5S Score for Predicting 5-Year Mortality After Transvenous Lead Extraction.

INTRODUCTION: Long-term mortality after transvenous lead extraction (TLE) remains substantial despite high procedural success. However, simple tools for estimating long-term prognosis after successful TLE are lacking. METHODS AND RESULTS: This retrospective multicenter study included 3368 consecutive patients undergoing TLE between 2006 and 2020 with complete 5-year follow-up. Patients were assigned in a 1:1 ratio to derivation and validation cohorts (n = 1684 each). Independent predictors of mortality were identified using multivariable Cox proportional hazards analysis and incorporated into

Outcomes following major cardiac procedures in socially vulnerable patients are not improved at high-volume centers.

BACKGROUND: The association between social determinants of health and adverse perioperative outcomes is well established across surgical specialties. While treatment at high-volume hospitals has been historically linked to improved survival and outcomes following cardiac surgery, it remains unclear whether these volume-associated benefits extend to socially vulnerable populations. METHODS: The 2016-2022 National Inpatient Sample was queried to identify all adult (≥18 years) hospitalizations that included elective coronary artery bypass grafting and isolated or concomitant valve procedures. We

Frequency and Extent of Leadless Pacemaker Encapsulation.

BACKGROUND: Leadless pacemakers (LPs) are increasingly used for permanent pacing. Although infrequent, retrieval of chronic implants may be necessary but are hindered by encapsulation and tissue adherence. To date, LP encapsulation has been described only in animal models and postmortem specimens. OBJECTIVES: The objective of this study was to assess the presence, extent, and anatomical characteristics of LP encapsulation using contrast-enhanced computed tomography (CT). METHODS: We conducted a retrospective study of all patients with LPs implanted at our center who underwent postimplantatio

A Comprehensive Review of Acute Coronary Syndrome and Bypass Surgery: Recent Advances, Timing, and Indicative Considerations.

BACKGROUND: Acute coronary syndrome (ACS) continues to be a major contributor to morbidity and mortality worldwide. While percutaneous coronary interventions (PCIs) have significantly evolved, coronary artery bypass grafting (CABG) has retained a role in emergency revascularization. Nevertheless, ongoing debate persists about how to select candidates for surgery, when to operate, and which surgical techniques offer the greatest safety and efficacy. METHODS: A comprehensive literature search was conducted, yielding 2302 records, of which 25 studies met predefined screening criteria and were in

One short circuit, two novel subcutaneous implantable cardiac defibrillator complications.

BACKGROUND: Short circuits from cardiac implantable electronic device (CIED) systems leak current to surrounding tissues, rendering them malfunctional and resulting in unpredictable behaviour. CASE SUMMARY: A 74-year-old gentleman was admitted electively for subcutaneous implantable cardiac defibrillator (SICD) generator change. The generator was exchanged like-for-like without incident. During defibrillation testing (DFT), both shocks were unsuccessful, with the second shock causing a malfunction in the self-adhesive pads attached to the external defibrillator.
