Correction of the univentricular heart having two atrioventricular valves.

Problems associated with the early evolution of corrective operation (septation) for univentricular heart (single or common ventricle) are illustrated in a total experience of 36 such operations, 30 of which were recent. Complete heart block existed preoperatively in five of the 30 patients and developed at operation in another 10; only two of these 15 patients attained a satisfactory result. Associated pulmonary stenosis was present in 16 patients, and this seemed to influence the result favorably. An extracardiac conduit between the right-sided ventricular chamber and the distal pulmonary tr

The long-term outlook for valve replacement in active endocarditis.

This study follows the clinical course of 22 patients in the active phase of infective endocarditis who inderwent valve replacement at North Carolina Memorial Hospital between March, 1966, and March, 1976. At the time of operation, there was gross valve tissue destruction in 16 patients, verrucae in nine, ruptured chordae tendineae in five, and myocardial or annular abscess formation in five. Four patients survived less than 6 months after the initial operation. One survived almost 3 years before dying of recurrent carcinoma of the lung. The remaining 17 patients have been followed an average

Thrombosis on Björk-Shiley aortic valve prosthesis. A case report and review of the literature.

A patient who developed thrombosis on a Björk-Shiley aortic valve prosthesis is reported. The patient presented with severe angina pectoris and the main physical findings were absence of the closing click of the prosthetic valve and the presence of systolic and diastolic aortic murmurs. Echocardiography showed early closure of the mitral valve and depressed left ventricular function. An emergency operation was performed and the clot was removed. The patient's recovery was uneventful and he is well one year after surgery. Awareness of the possibility of valve thrombosis is necessary in view of

The prognosis in Ebstein's disease of the heart. Long-term follow-up of 22 patients.

A follow-up study of 22 patients with Ebstein's anomaly has been performed. Nine patients died 1-21 years (mean 9) after the initial admission while the 13 patients alive at the end of the observation period had been followed for 5-26 years (mean 15). Clinical, ECG, radiological, and haemodynamic features were analyzed with reference to their prognostic significance. The presence or absence of cyanosis due to right-to-left shunt through an atrial septal defect (ASD) distinguished best between a good and poor prognosis. Right-sided heart failure and dyspnoea at rest, often associated with palpi

Isolated replacement of the mitral valve with the Starr-Edwards prosthesis.

Isolated mitral valve replacement with the Starr-Edwards prosthesis has been performed on 657 patients at the Mayo Clinic during the 11 year period ending January, 1972. The most recent subset of that series comprised patients who received the Model 6120 prosthesis. In this group, the operative mortality rate was 9 per cent and the actuarial late death rate at 5 years was 20 per cent. This survival rate is a significant improvement over the natural history of severe mitral valve disease. Factors associated with operative deaths are large left artrial size, advanced functional class, and previo

Massive left atrial thrombus complicating mitral stenosis with atrial fibrillation: results of surgical treatment.

Thirty-five patients with massive atrial thrombus complicating mitral stenosis and atrial fibrillation were treated surgically. The mean age in this series is lower than that reported in the West. Furthermore, the incidence of preoperative embolic manifestations has been less frequent in our experience. We believe it is desirable to document the presence of a thrombus by selective left atrial cineangiography. Surgical treatment was accomplished with an acceptable mortality. The vital role played by the use of low-molecular-weight dextran and early anticoagulation in decreasing the incidence of

The surgical management of bacterial endocarditis: a review.

A total of 239 surgically treated patients with primary endocarditis were reviewed both from the literature and from our own experience. The age range was 10 to 74 years with a male to female ratio of 3:1. A wide variety of organisms was found. However, as a group, gram positive organisms predominate. The onset of congestive failure was the major indication for surgery. The aortic valve was predominantly involved with the mitral valve running a distant second. The hospital mortality rate was 20% and the late mortality rate was 6.7% with an overall mortality of 26.7%. The prognosis in infective

Mitral valve replacement in the presence of severe pulmonary hypertension.

Thirty patients with severe preoperative pulmonary hypertension (pulmonary artery pressure range 90-165 mmHg, mean 118 mmHg) were reviewed following single mitral valve replacement, with prosthetic valves, within the last 10 years (1964-74). The early and late mortality for this group was no different from that of the total series of mitral valve replacements performed over the same period. Marked postoperative clinical improvement was accompanied by corresponding radiological and electrocardiographic changes. In addition there was a statistically highly significant haemodynamic improvement in

Coronary perfusion and myocardial metabolism during open-heart surgery in man.

Myocardial oxygen consumption and extraction of lactate and free fatty acids (FFA) have been measured in six patients during cardiopulmonary bypass at two coronary flow rates differing by 25 per cent (109 and 148 ml. per minute). Significant differences were found between these flows and between natural and artificial coronary perfusion. This fact indicates the presence of anaerobic metabolism, especially at the lower coronary flow. These findings are discussed in relation to criteria for coronary flow and perfusion pressure during open-heart procedures.

The effect of hemodilution on plasma levels of cortisol and free cortisol.

The effect of hemodilution on plasma levels of total cortisol and free cortisol was studied in 12 patients undergoing open-heart surgery. In each case, at the onset of extracorporeal circulation (ECC) the hematocrit value was rapidly reduced to 25 to 30 per cent and was maintained around this level during perfusion. Total plasma cortisol levels fell significantly with the hemodilution (p less than 0.0001), but the plasma concentration of free, biologically active cortisol was maintained at its pre-dilution level because of an increase in the percentage of plasma cortisol existing in the free f

Calcific coronary embolization associated with cardiac valve replacement. Necropsy x-ray study.

The hearts of 14 patients dying early after replacement of a calcified aortic or mitral valve were examined for the presence of calcific coronary emboli. The incidence of embolization as shown by x-ray film was 92-9 per cent. The number of emboli in individual cases ranged from 4 to 119. Replacement of an aortic valve was associated with a more severe embolization than replacement of a mitral valve. Th mechanisms of calcific embolization are discussed and preventive surgical measures suggested.

Echocardiographic diagnosis of thrombus on a heterograft aortic valve in the mitral position.

Echocardiographic studies were used to diagnose a thrombus adherent to the ventricular surface of a stent-mounted porcine aortic heterograft valve in the mitral position. Multiple, intense, irregularly thickened echoes were present within and behind the anterior and posterior heterograft valvular stent. This suggested the presence of adherent biologic material, such as a thrombus. The abnormal echocardiographic findings are compared to the findings in a normally functioning porcine heterograft aortic valve in the mitral position.

Echocardiogram of the porcine aortic bioprosthesis in the mitral position.

Echocardiography was performed in 10 consecutive patients who had a clinically normally functioning porcine aortic bioprosthesis in the mitral position. Strong well defined echoes were recorded from the anterior and posterior aspects of the xenograft stent. The maximal separation of the anterior and posterior stent echoes approximated the diameter of the stent at its base. The maximal excursion of the anterior stent was 5 to 10 mm (mean 7.5) with a mean systolic slope of 15 to 35 mm/sec (mean 22.2) and diastolic slope of 11 to 59 mm/sec (mean 21.5). In all 10 patients it was possible to record

Cardiac valve replacement in the first 21 years of life.

From 1969 through 1974, 25 patients under age 21 years had prosthetic cardiac vlave replacement at the Emory University Hospital. Twenty-nine valves were replaced in this group. Patients ranged in age from 2 to 21 years, with a mean of 14 years. Concomitant correction of associated cardiac lesions was required in 14 (56%). Five (21%) had had prior open valvuoplastic procedures. Operative mortality for the entire group was 12%, with an additional late mortality of 8%. There have been no early or late deaths in the last three years' experience. Best results are obtained when early correction of

Diphtheroid endocarditis. Report of nine cases and review of the literature.

Diphtheroid endocarditis is a rare disease. A large proportion of the reported cases have occurred in the presence of prosthetic heart valves. Nine previously unreported cases of diphtheroid endocarditis are discussed along with 25 others taken from the literature. Eight out of the nine new cases occurred in patients with artificial heart valves. A high mortality was associated with infections occurring on prosthetic heart valves. Microbiological studies were frequently hampered by poor in vitro growth of the organisms. Resistance to penicillin and cephalothin was common. Erythromycin or penic

[Hemodynamics changes in children and adolescents with mitral valve replacement].

The authors analyze the pre and post-operative hemodynamic picture of 30 children and adolescents with mitral valve replacement, which corresponds to a group of 106 patients operated on from 1964 to 1974. At variable time periods, between 9 days and 48 months (average 28 months) from the valve replacement, a new catheterization was done. The majority of the studies were done in the period between 13 and 36 months. Of the 30 cases, 21 were carriers of the Starr-Edwards valve, and the rest of different types. The Starr-Edwards valves produced a lawering of the CVP from 23.0 to 11.0 mm. Hg (avera

Mitral valve replacement: a 10-year follow-up of non-cloth-covered vs. cloth-covered caged-ball prostheses.

The timing of operation and the selection of prosthesis depend upon the evaluation of long-term results at 5 years and beyond. From 1965 to 1975, 290 patients had isolated mitral valve replacement with currently used prostheses, resulting in a 6.6% operative mortality and 64% relative survival at 10 years. In considering mortality, the results obtained with both series of valves were combined and a search made for significant differences in preoperative variables between the operative deaths and operative survivors. The significant variables for operative mortality were valve model, preoperati

Flutter of left ventricular structures in patients with aortic regurgitation, with special reference to patients with associated mitral stenosis.

Echocardiography was performed in 45 patients with aortic regurgitation. Forty showed a high frequency diastolic flutter of the mitral valve, which was holodiastolic in all but the patients with associated mitral stenosis. Of four patients with coexisting mitral stenosis, mitral flutter was absent in two; in the other two, in atrial fibrillation, mitral flutter occurred, but only during a fixed interval after mitral valve opening, irrespective of cycle length. A fine flutter of similar frequency was observed on the left ventricular aspect of the ventricular septum in 12 patients. In six of the

Non-infected false aneurysm of the ascending aorta eight years after aortic valve surgery.

In this report, the development of false aneurysm of the ascending aorta 8 years following the repair of congenital aortic stenosis is described. A 14-year-old male patient was found to have congenital aortic stenosis in 1966 after medical check-up and subsequently operated upon. In the postoperative period, he was complicated by remittent fever of 1 month's duration, which was finally controlled by antibiotics. Arterial blood cultures taken on a few occasions were negative. Eight years and 5 months after surgery, he was noticed to have a mediastinal mass around the base of the heart and the p
