An 8 week old infant with severe heart failure from type 1 truncus arteriosus underwent successful corrective surgery employing the Rastelli procedure with use of deep hypothermia and total circulatory arrest. Postoperative hemodynamic studies showed complete closure of the septal defect, disappearance of truncal stenosis, but presence of mild porcine valve stenosis. This procedure is possible even in very small subjects and is preferable to palliative pulmonary arterial banding.

The parturient with a cardiac valve prosthesis presents a twofold problem of management: (1) that of a rising cardiac workload in the presence of serious heart disease, and (2) that of chronic anticoagulant therapy. Pain relief, a shortened 2nd stage of labor, and avoidance of bearing-down efforts reduce stress. Vaginal delivery is preferred to cesarean section, and segmental lumbar extradural block is the optimal anesthetic method. Oxytocics must be used with caution and heparin is preferred to longacting oral anticoagulants.

Sixteen cases of chronic Q fever are described. In eight there was a history of exposure to infection from farms or farm products. All had valvular heart disease, involving the mitral valve in nine and the aortic valve in seven. Infection occurred on a prosthetic valve in two patients. Arterial embolism was common. The presence of an arteriovenous hemodialysis fistula. 2. Hemodynamic alterations associated with aortic insufficiency. 3. Choice of anesthetic agents, fluid, and electrolyte balance in the presence of renal failure. 4. Postoperative management of hypertension and hyperkalemia. The problem of the patient who has had one operation for coarctation of the aorta and who then requires another because of a late complication at or near the coarctation site is a demanding one. In the course of doing routine echocardiograms on patients with mitral prosthetic valves, we observed peculiar intracavitary echoes within the left ventricle. Of the 36 valves studied in 31 different patients, 14 echocardiograms demonstrated the abnormal intracavitary echoes and 22 did not. A case of left atrial myxoma causing mitral insufficiency treated with valve replacement and diagnosed preoperatively as mitral stenosis is presented. The presence of mitral regurgitation is very rare in atrial myxomas and requires mitral valve replacement. In our case mitral regurgitaion was due to the mechanical trauma of the valve by the tumor mass because the patient had no history of rheumatic fever and the pathologic examination of the valve displayed neither rheumatic nor myxomatous degeneration.

The results of 249 mitral commissurotomies in calcification of the mitral valve are analysed. The authors have differentiated three grades of calcification of the valve and two groups according to its functional state. Late results were followed up in 177 patients within the terms up to 10 years and longer. Four patients with heart valve prostheses developed Candidate endocarditis. At autopsy the foci of infection were confined to patches of neoendocardium that appeared to have developed as ingrowths of host endocardium onto the sewing cloth of the artificial valve. This suggested that one mechanism of fungal infection is related to the presence of the neoendocardium rather than to the cloth or metallic substances of the valve itself. Future studies might be directed toward inhibiting the development of the neoendocardium or to agents preventing its susceptibility to infection.

Five hundred and eighty six patients with mitral valve disease were studied with cardiac catheterization between 1961 and 1972; 48 (8.2%) had extreme pulmonary hypertension (resting systolic pulmonary artery pressure of 80 mmHg or above and pulmonary vascular resistance of 10 units or greater) and of these patients, 27 underwent cardiac surgery. Hemodynamics of 12 patients with tetralogy of Fallot were monitored during the first 72 hours after surgical repair. This is an account of experience with valve replacement in patients with active endocarditis. In four patients the aortic valve was replaced, and in one, the mitral valve. Indication for surgery in all five cases was heart insufficiency.
