Safe corridors for external skeletal pin insertion in rabbit hindlimb long bones.

External skeletal fixation (ESF) is a minimally invasive method for fracture repair that requires precise anatomical knowledge to prevent soft tissue and nerve injury. This study aimed to identify and describe safe, hazardous, and unsafe corridors for ESF pin insertion in the hind limb long bones of rabbits, based on detailed topographic and cross-sectional anatomy. A total of ten 12-week-old Hyla hybrid rabbits (5 males, 5 females; avg. weight 2.5 kg) with no prior limb fractures or illnesses were used. A safe corridor was defined as an area where the bone is covered only by skin and subcutan

Short-term and long-term in vivo 3D kinematics of the knee joint during split squat after anterior cruciate ligament reconstruction.

BACKGROUND: Early-onset knee osteoarthritis and high reinjury rates after anterior cruciate ligament reconstruction (ACLR) highlight the need to monitor knee joint mechanics over time. Understanding long-term structural changes post-ACLR can clarify mechanisms underlying early degeneration. This study investigated short- and long-term changes in knee arthrokinematics during split squat post-ACLR. METHODS: Sixteen individuals with ACLR (8 short-term, ∼3 years; 8 long-term, ∼12 years) and 8 healthy controls performed split squats while undergoing single-plane fluoroscopy. Individualized 3D mode

Functional and Radiological Benefits of Adding Fibular Osteotomy to Supramalleolar Osteotomy in Ankle Osteoarthritis.

BACKGROUND Ankle arthritis is a chronic degenerative disease; its typical symptom is pain in the ankle joint. This retrospective study of 65 patients with mid-stage ankle osteoarthritis aimed to compare clinical outcomes from supramalleolar osteotomy (SMOT) and supramalleolar osteotomy combined with fibular osteotomy (SMOT+FO). MATERIAL AND METHODS Based on whether fibular osteotomy was performed during the procedure, the patients were categorized into the SMOT group (n=34) or the SMOT+FO group (n=31). Surgical outcomes were systematically assessed using the Visual Analogue Scale (VAS) for pai

Five-year outcomes of medial open-wedge high tibial osteotomy with lateral supplemental lag screw in patients with obesity.

BACKGROUND: Obesity contributes to the accelerated progression of knee osteoarthritis. Medial open-wedge high tibial osteotomy (MOWHTO) is a joint-preserving surgical intervention for unicompartmental knee osteoarthritis; however, its efficacy in patients with obesity remains controversial. This study aimed to evaluate the 5-year clinical and radiographic outcomes of MOWHTO with lateral supplemental lag screw fixation in patients with obesity. MATERIALS AND METHODS: This retrospective cohort study included patients with obesity who underwent MOWHTO between 2017 and 2020, with a minimum follow

Comparison of Proximal Tibial Endoprosthetic and Allograft-Prosthetic Reconstruction.

INTRODUCTION: The proximal tibia is a common location for bone tumors. Two contemporary reconstruction techniques for limb salvage are endoprosthetic replacement (EPR) and allograft-prosthesis composite (APC). This study aimed to compare the long-term oncologic and functional outcomes of EPR and APC reconstructions. METHODS: Thirty-eight (19 APC and 19 EPR) patients underwent oncologic proximal tibia resection and reconstruction were reviewed. Patients undergoing APC reconstruction were younger than the EPR group (31 vs. 44 years, p = 0.049), and patients in the EPR group were more likely to

Effects of liquid nitrogen treatment on the mechanical properties of canine long bones.

We aimed to investigate the mechanical properties of canine long bones treated with liquid nitrogen for limb-sparing surgery (LSS) as described by Tsuchiya et al. Three-point bending tests were performed on left and right femurs and tibias obtained from healthy beagle cadavers to evaluate the bone mechanical properties. There were no significant differences in mechanical properties between the left and right bones from the same individual. Furthermore, none of the indices differed between liquid nitrogen-treated and untreated bones. Our findings suggest that liquid nitrogen treatment does not

Modification of the coronal plane alignment of the knee classification considering the joint line convergence angle in high tibial osteotomy: a geometric approach.

BACKGROUND: Excessive joint line obliquity (JLO) is associated with poor outcomes in open wedge high tibial osteotomy (OWHTO). The Coronal Plane Alignment of the Knee (CPAK) classification categorizes knee phenotypes by alignment and JLO but overlooks the joint line convergence angle (JLCA), which impacts JLO pre- and postoperatively in OWHTO. This study aims to modify JLO calculation by incorporating JLCA through a geometric approach, with the goal of better reflecting the knee joint orientation under weight-bearing conditions. METHODS: 323 knees that received OWHTO were retrospectively revi

Etiology of Limb Loss after Successful Transmetatarsal Amputation.

BACKGROUND: Transmetatarsal amputation (TMA) is a functional foot reconstruction for limb salvage following extensive forefoot infection and/or ischemic gangrene. Unfortunately, up to 30% of TMAs progress to major amputation. This study evaluates the effects of diabetes and peripheral artery disease (PAD) on long-term TMA outcomes. METHODS: This was a retrospective review of a single-surgeon, single-institution consecutive series of TMAs performed between 2006 and 2024. Patients with PAD and perioperative hemoglobin A1c (HbA1c) ≤ 6.5 (euglycemic PAD), PAD and HbA1c > 6.5 (hyperglycemic PAD),

Reassessing Valgus Lower Extremity Alignment: Is Lateral Condyle Hypoplasia a Myth?

BACKGROUND: Lower extremity coronal alignment influences joint loading, gait mechanics, and the progression of osteoarthritis, yet the relative contributions of hip and knee anatomy remain incompletely defined. Valgus lower extremity alignment has traditionally been attributed to lateral femoral condyle hypoplasia, but the major contributor to valgus alignment and that femoral version and tibial bowing are more strongly associated with valgus deformity. METHODS: We performed a retrospective review of 122 consecutive primary total knee arthroplasties (57 valgus, 65 varus) with preoperative lon

Aneurysmal Bone Cyst Presenting as a Pathological Subtrochanteric Femur Fracture in an Adolescent: A Case Report.

INTRODUCTION: Aneurysmal bone cyst is a locally aggressive as well as benign bony lesion most commonly seen in adolescents. Although it mostly involves the metaphysis of long bones, presents as a fracture which is pathological in the subtrochanteric region of the femur is rare. The subtrochanteric femur is a biomechanically demanding region of the lower limb, and the presence of an underlying cystic pathology can complicate fracture management. Management of subtrochanteric femur fractures due to aneurysmal bone cysts in adolescents is limited, as described in the literature. This case is a ra

Strategic Muscle Coverage in Free Fibular Mandibular Reconstruction: An Osteotomy Approach to Minimize Plate Exposure, A Retrospective Analysis of 580 Consecutive Patients.

BACKGROUND: Free fibula flap reconstruction represents the gold standard for mandibular reconstruction following oncologic resection. However, plate exposure remains a significant complication, particularly in patients receiving postoperative radiotherapy. Traditional reconstructive approaches emphasize precise hardware contouring and basic soft tissue coverage but frequently fail to provide adequate muscle protection over hardware, especially in irradiated tissues. METHODS: A retrospective analysis was conducted of 580 consecutive free fibula flap reconstructions performed between 2018 and 2

Unveiling a Silent Bone Lesion: Pathological Fracture in a Child With Non-Ossifying Fibroma.

Non-ossifying fibroma (NOF) is a benign, self-limiting fibrous bone lesion commonly seen in children and adolescents, generally affecting the metaphysis of long bones. Although typically asymptomatic, larger NOF lesions can predispose patients to pathological fractures, especially in weight-bearing bones. An 11-year-old boy presented to the emergency room following a minor knee trauma that caused pain and an inability to bear weight on the affected leg. Imaging studies identified a large, well-defined lytic lesion in the distal femur, primarily suspected to be an aneurysmal or simple bone cyst

A soft diet attenuates exercise-induced increases in cortical bone density and formation in long bones in growing mice.

Bone and muscle are closely linked through functional interdependence. Training and regular exercise can improve and maintain optimal bone mass and strength. Here, we investigated the effects of exercise combined with soft versus hard diet (pellet) on long bones in mice. Four groups of male C57BL/6J mice (n = 5 per group) were studied: hard diet (pellet) with exercise, soft diet (powdered) with exercise, hard diet (pellet) without exercise, and soft diet (powdered) without exercise. Femur and tibia were analyzed using radiographic, micro-CT, and histomorphometric techniques. Exercise combined

Bone Flaps for Structural Reconstruction of the Lower Extremity.

Fixation of vascularized bone grafts (VBGs) relies upon established osteosynthesis principles. VBGs offer the opportunity to transfer living bone with remodeling and hypertrophy potential for reconstruction of bone defects throughout the lower extremity. In defects of the pelvic ring, femoral head and neck, ankle, and foot, as well as for juxta-articular and intercalary long-bone defects, pedicled and free fibula and medial femoral condyle flaps are used to this end. These options provide an autologous structural donor that allows patients to achieve osseous union and functional success.

Vascularized versus nonvascularized free fibular grafts in reconstruction of post-traumatic critical long bone defects, a comparative study.

BACKGROUND: Reconstruction of critical-sized long bone defects is a complex orthopedic challenge. Free fibular grafts, vascularized (FVFG) and nonvascularized (NVFG), are established reconstructive options, but comparative clinical outcomes remain uncertain. PURPOSE: To compare clinical and radiological outcomes of FVFG and NVFG in patients with post-traumatic critical bone defects more than 10 cm. METHODS: A randomized controlled trial was conducted with 50 patients assigned equally to FVFG or NVFG groups. The primary outcome was time to union, while secondary outcomes included graft hypert

Latissimus dorsi and/or serratus anterior - rib composite flap for posttraumatic long bones reconstruction.

Posttraumatic bone defects or their sequels represent one of the most challenging problems in limbs reconstruction. It is proved that due to their advantages, i.e. regenerative properties, shorter time to obtain bone fusion, better stability, better power to fight against infection, the use of vascularized bone grafts (VBGs) represents the best choice in such cases. If large soft tissue defects are concomitant to bone defects, composite flaps including bone segments should be the choice. This paper provides details about such flaps - vascularized rib (R) as part of the latissimus dorsi (LD) an

Reconstruction of the Proximal Tibia and Extensor Mechanism After Tumor Resection: A Systematic Review and Meta-Analysis.

BACKGROUND: Proximal tibia reconstruction after tumor resection is technically challenging due to the dual requirement of skeletal stability and extensor mechanism restoration. Multiple reconstruction methods exist, but comparative data on complications, survival, and functional outcomes remain limited. This study aimed to compare complication rates and implant survival across different proximal tibia reconstruction methods (megaprostheses, osteoarticular allografts [OAs], intercalary allografts [ICAs], and allograft-prosthetic composites [APCs]) and to evaluate how extensor mechanism reconstr
