OBJECTIVES: We studied angiographic outcome and its predictors after traditional coronary balloon angioplasty in diabetics. We further examined whether changes in ejection fraction were influenced by the status of the dilated site(s) at follow-up.
BACKGROUND: Recent studies have suggested that diabetics have a particularly poor outcome after balloon angioplasty. The reasons for this observation are not known.
METHODS: We investigated procedural and six-month angiographic outcome, analyzed by quantitative coronary angiography, and left ventricular function in 485 consecutive diabetics (627 le

We prospectively studied 223 patients (288 lesions) who underwent elective or bail out implantation of 309 NIR stents (Scimed, Boston Scientific Corporation, Galway, Ireland). Most lesions (68.4%) had unfavorable characteristics (type B2 or C). Primary success in stent deployment was achieved in 305 (98.6%). There was no Q-wave myocardial infarction. Emergency coronary artery bypass grafting (CABG) was required in 1 patient and 1 death occurred. Subacute thrombosis rate was 0.4%. Reference diameter was 2.65+/-0.67 mm. Minimum luminal diameter (MLD) increased from 0.62+/-0.45 to 2.69+/-0.57 mm

Although adjunctive abciximab therapy improves outcome after angioplasty or atherectomy, there are few data demonstrating its benefit for intracoronary stent implantation. We characterized patients receiving abciximab for stent placement in our practice and determined the impact of abciximab on outcome. Abciximab was introduced to our practice in April 1995 for percutaneous revascularization. Demographic, clinical, and angiographic variables that were independently associated with the use of abciximab for stent placement through 1996 (abciximab era) were examined. We then examined among all pa

Despite the favourable impact of stent implantation on restenosis rates for selected lesion types, in-stent restenosis remains a commonly encountered and difficult to manage entity. Histologically, in-stent restenosis appears to derive almost exclusively from neointimal hyperplasia, which appears to be more abundant following stent implantation than balloon angioplasty. The most powerful predictors of in-stent restenosis include the presence of diabetes, greater lesion length, small vessel caliber, and inadequate stent expansion. Several strategies have been proposed as a means to reduce the i

Sudden cardiac death due to arrhythmic events is the major cause of mortality among early post-myocardial infarction (MI) patients, accounting for > 250,000 deaths annually in the United States alone. Antiarrhythmic drugs can be used in such patients as well as in those who have not had a recent MI but are at high risk for sudden cardiac death (e.g., those with ventricular tachycardia/fibrillation, or who have survived cardiac arrest). Most antiarrhythmic drugs available, however, have limitations arising from their toxic and proarrhythmic potential. Thus, research and development of new agent

We reviewed our experience with 28 unselected, consecutive patients undergoing left main coronary artery (LMCA) angioplasty who had been considered unsuitable for coronary artery bypass graft surgery (CABG). Fourteen patients (50%) had a protected LMCA circulation. Balloon angioplasty was performed in 11 patients (39.3%), and stents were implanted in 17 patients (60.7%). The procedure was elective in 22 patients (78.6%) and acute in the setting of myocardial infarction/cardiogenic shock in 6 (21.4%). The mean follow-up duration was 15.9 +/- 12 months. There were 5 early (before hospital discha

AIM: The presence of intracoronary thrombus was considered a contraindication for stent deployment. Recently, many investigators have demonstrated that the use of stents for thrombus-laden lesions under both elective and bail-out conditions is effective and safe, even in the setting of acute myocardial infarction (AMI).
METHODS AND RESULTS: In this study Palmaz-Schatz stents were implanted to treat suboptimal results and complications of percutaneous transluminal coronary angioplasty (PTCA) in 41 thrombus-containing lesions. Clinical presentation was unstable angina in 24 and AMI in 17 patien

BACKGROUND: This single-center report describes the results of unplanned coronary stenting for the treatment of suboptimal angiographic results after conventional coronary angioplasty (PTCA), Suboptimal results of PTCA were defined as: a) residual stenosis > or = 30% with TIMI flow 3; b) presence of coronary dissection < 15 mm with TIMI flow 3.
PATIENTS: From January 1994 to December 1995, 213 patients with suboptimal result of PTCA underwent coronary stent implantation.
RESULTS: Stenting resulted in a technical and angiographic success in 99.5% of patients. In-hospital complications involve

Intracoronary implantation of stent devices is a new interventional technique with a promising role in the treatment of acute coronary occlusions caused by intimal dissection and suboptimal results of percutaneous transluminal coronary angioplasty. However, the prolonged presence of stent material embedded in the arterial wall may generate unknown late consequences. Major complications consist of subacute to chronic occlusion and restenosis. To elucidate the underlying cause we removed an angiographically occluded intracoronary artery Palmaz-Schatz stent before coronary artery bypass grafting.

Direct percutaneous transluminal coronary angioplasty (PTCA) in the early phase of acute myocardial infarction (AMI) has till now a relatively limited use, mainly because of logistic problems, in comparison with systemic thrombolysis. The aim of this paper was to discuss the role direct PTCA during AMI, based on the most recent international experience. In brief, the major benefits of direct PTCA are the high percentage of recanalization (90%), optimal recanalization quality, the absence of contraindications in most cases; in patients with cardiogenic shock the mortality is lowered from 80% to

BACKGROUND: Abrupt vessel closure after percutaneous transluminal coronary angioplasty (PTCA) is associated with major adverse events. Different surgical and nonsurgical approaches have been advocated to treat or prevent this complication. This study summarizes our 4-year experience with Palmaz-Schatz stenting for the management of 339 patients with present or threatened occlusion after PTCA.
METHODS AND RESULTS: Stent implantation was attempted in a total of 339 and 4959 patients with PTCA during the study period and was successful in 327 (96.5%). During the follow-up, events like death, myo

We studied 32 patients (age 58 +/- 9 years) who had been treated with a Palmaz-Schatz stent after significant dissection complicating percutaneous transluminal coronary angioplasty (PTCA). We attempted to cover the entire site of dissection with prosthesis. The presence of dissection after PTCA was associated with Thrombolysis in Myocardial Infarction grade 0 to 1 flow in 19 patients and grade 2 flow in 13. The stented arteries were: left anterior descending artery in 19 patients, right coronary artery in seven, and left circumflex artery in five. A single stent was implanted in 11 and multipl

BACKGROUND: The balloon-expandable intracoronary stent developed by Palmaz and Schatz is undergoing clinical evaluation for use in unfavorable anatomic situations and in the prevention of restenosis. Because the stent's mechanism of action would suggest effectiveness in salvaging certain percutaneous transluminal coronary angioplasty (PTCA) failures, we retrospectively examined the results of emergency unplanned coronary artery stenting for failed PTCA procedures, including acute occlusion.
METHODS AND RESULTS: The study population consisted of all US patients receiving emergency unplanned st
