Insertion of Provox2 voice prosthesis using the anterograde technique immediately after the secondary tracheoesophageal puncture.

Insertion of a voice prosthesis through the tracheoesophageal puncture (TEP) is one way to restore the voice after total laryngectomy. The second generation Provox voice prosthesis is presently one of the most popular devices. Although TEP can be done primarily, there are many centres that prefer it to be done at a second stage for various reasons. However, secondary TEP for retrograde replacement of prosthesis can be difficult and general anaesthesia is very often necessary. Moreover, the presence of neck stiffness and fibrosis from the surgery or previous radiotherapy could affect the neck e

Artificial nail aspiration masquerading as refractory croup.

Infants with an aspirated foreign body who present with acute onset of obstructed airflow should be readily diagnosed. When aspirated foreign body is overlooked, patients may present with recurrent inspiratory, biphasic, or expiratory stridor. We report an infant who repeatedly presented with varying degrees of inspiratory stridor. The diagnosis of aspirated foreign body was hampered by the presence of respiratory syncytial virus infection with the initial episode.

Laryngeal oedema in neonatal apnoea and bradycardia syndrome (a pilot study).

PURPOSE: Some preterm infants in general good health continue to present recurrent apnoeas, bradycardias and desaturations (ABD) despite usual treatments. These events may lead to transitory brain hypoxia and to further neurological injury. The purpose of this study has been to evaluate the role of laryngeal oedema in this symptomatology and to assess corticoid treatment. METHOD: Twelve preterm babies born at a median age of 28.5 weeks (range: 26-35 weeks) already showed signs of ABD at a median age of life of 28.5 days (range: 9-80 days). Fiberoptic laryngeal endoscopy was performed on these

Biosurfactant from Lactococcus lactis 53 inhibits microbial adhesion on silicone rubber.

The ability of biosurfactant obtained from the probiotic bacterium Lactococcus lactis 53 to inhibit adhesion of four bacterial and two yeast strains isolated from explanted voice prostheses to silicone rubber with and without an adsorbed biosurfactant layer was investigated in a parallel-plate flow chamber. The microbial cell surfaces and the silicone rubber with and without an adsorbed biosurfactant layer were characterized using contact-angle measurements. Water contact angles indicated that the silicone-rubber surface with adsorbed biosurfactant was more hydrophilic (48 degrees) than bare s

Influence of different combinations of bacteria and yeasts in voice prosthesis biofilms on air flow resistance.

Laryngectomized patients use silicone rubber voice prostheses to rehabilitate their voice. However, biofilm formation limits the lifetime of voice prostheses. The presence of particular combinations of bacterial and yeast strains in voice prosthesis biofilms has been suggested to be crucial for causing valve failure. In order to identify combinations of bacterial and yeast strains causative to failure of voice prostheses, the effects of various combinations of bacterial and yeast strains on air flow resistances of Groningen button voice prostheses were determined. Biofilms were grown on Gronin

Metastatic spread of occult papillary carcinoma of the thyroid to the parapharyngeal space: a case report.

OBJECTIVES: We evaluated the types and the results of surgical treatment performed for congenital scoliosis. METHODS: Forty-one patients (26 females, 15 males; mean age 12.8 years) with congenital scoliosis were included. The patients were classified according to the Winter's system. Transpedicular hemiepiphysiodesis, "egg shell" procedure, and anterior hemiarthrodesis and posterior convex fusion were performed in infantile (n=1) and juvenile (n=8) patients. In the adolescent (n=32) group, posterior in situ fusion was performed for rigid curves (n=15), posterior fusion after correction with p

Autologous fat injection to treat leakage around tracheoesophageal puncture.

PURPOSE: To evaluate prospectively autologous fat injection as a treatment for leakage around tracheoesophageal puncture. PATIENTS AND METHODS: For ten patients who exhibited leakage of saliva or liquid around their tracheoesophageal puncture, autologous fat injection was carried out into the tracheoesophageal wall around the maintained prosthesis. Short-term success was evaluated at one month, and long-term follow-up was continued as long as the patients remained alive. The effectiveness of the procedure was evaluated based on the presence or absence of leakage around the puncture as the pat

Combined 'trache-stent': a useful option in the treatment of a complex case of subglottic stenosis.

The authors describe the adaptation of a Montgomery T-tube laryngo-tracheal stent to incorporate an uncuffed fenestrated Shiley tracheostomy tube in the management of a complex case of subglottic stenosis. The combined 'trache-stent' provided a secure, unobstructed airway and optimal phonation. The presence of a removable inner tube facilitated cleaning and reduced patient anxiety regarding the perceived risks of stent obstruction with dried secretions. The combined 'trache-stent' was upsized at four weeks with minimal evidence of local granulation formation. The device was removed entirely si

Colonization of a voice prosthesis by Cryptococcus neoformans.

Tracheoesophageal voice prostheses in laryngectomized patients commonly deteriorate due to the presence of yeasts, particularly Candida species. We describe the first case of colonization of such a device by Cryptococcus neoformans in a patient with a history of glottic carcinoma. The isolate showed an identical genomic pattern with C. neoformans from pigeon excreta in the patient's environment.

Adhesive interactions between voice prosthetic yeast and bacteria on silicone rubber in the absence and presence of saliva.

Biofilms on silicone rubber voice prostheses are the major cause for frequent failure and replacement of these devices. The presence of both bacterial strains and yeast has been suggested to be crucial for the development of voice prosthetic biofilms. Adhesive interactions between Candida albicans, Candida krusei, and Candida tropicalis with 14 bacterial strains, all isolated from explanted voice prostheses were investigated in a parallel plate flow chamber. Bacteria were first allowed to adhere to silicone rubber, after which the flow chamber was perfused with yeast, suspended either in saliv

[Nutrition and cancer: causative, protective and therapeutic aspects].

In prospective cohort studies an association between fat consumption and breast and colon cancer was not confirmed. Alcoholic beverages enhance the risk of cancer of the oral cavity, pharynx, larynx and oesophagus, while the relation is less well established for cancer in the stomach, pancreas, colon, rectum and breast. Consumption of vegetables and fruits reduces the risk of lung, oesophagus, stomach and pancreatic cancer. Which food components are responsible for this observation is unclear, but beta-carotene does not appear to be responsible. Clinical depletion in cancer patients is a resul

Enhancement of electrolaryngeal speech by adaptive filtering.

Artificial larynges provide a means of verbal communication for people who have either lost or are otherwise unable to use their larynges. Although they enable adequate communication, the resulting speech has an unnatural quality and is significantly less intelligible than normal speech. One of the major problems with the widely used Transcutaneous Artificial Larynx (TAL) is the presence of a steady background noise caused by the leakage of acoustic energy from the TAL, its interface with the neck, and the surrounding neck tissue. The severity of the problem varies from speaker to speaker, par

Dysphagia secondary to invasive candidiasis of a jejunal free flap.

The presence of a mass in a jejunal free flap that causes dysphagia less than two years after a pharyngolaryngectomy for carcinoma usually indicates tumour recurrence. We present a case of invasive candidiasis of a jejunal free flap presenting with dysphagia and a mass. To our knowledge this is previously unreported. Such a cause should always be considered in the differential diagnosis, as early recognition and treatment are likely to result in a favourable outcome.

Streptococcus thermophilus and its biosurfactants inhibit adhesion by Candida spp. on silicone rubber.

The adhesion of yeasts, two Candida albicans and two Candida tropicalis strains isolated from naturally colonized voice prostheses, to silicone rubber with and without a salivary conditioning film in the absence and presence of adhering Streptococcus thermophilus B, a biosurfactant-releasing dairy isolate, was studied. Coverage of 1 to 4% of the surface of silicone rubber substrata with adhering S. thermophilus B gave significant reductions in the initial yeast adhesion regardless of the presence of a conditioning film. Mechanistically, this interference in yeast adhesion by S. thermophilus B

Survival times of Provox valves.

This study was performed to assess the survival times of the Provox valve in the Manchester area. Thirty-nine patients from four hospitals, representing 81 valve failures, were studied. The effects of the timing of the tracheo-oesophageal puncture, previous radiotherapy, and the presence and timing of cricopharyngeal myotomy on valve life were analysed. Regression analysis using an extension of the Cox model to allow strata showed that the lifetime of the first valve only is adversely affected by previous radiotherapy. The other covariates do not have a statistically significant effect on valv

[Bacterial flora of the respiratory tract in artificially ventilated patients].

Patients treated in ICUs are at particular risk for nosocomial infections. Within this group a ventilator-associated pneumonia still constitutes a major problem. Highly virulent hospital strains, existing in ICUs, can colonize patients' upper respiratory tract. Therefore this study aimed at analysing the organisms found in patients who required mechanical ventilation in ICUs. The same types of bacteria with the same (high) rate colonized upper airways, trachea and stomach of patients treated at the intensive Care Unit. It is suggested that the presence of Gram negative bacilli may serve as a m

Cardiopulmonary responses to capsaicin instillation to the laryngeal lumen and their reflex mechanisms in rats.

Cardiorespiratory effects of capsaicin (CAPS) solution instilled into the larynx and the reflex mechanisms were investigated in rats with spontaneous breathing or under artificial ventilation. The first challenge with CAPS (100 micrograms/ml, 20 microliters) markedly inhibited spontaneous breathing due to a considerable prolongation of expiration time (TE) (1785% of control) in all rats. Circulatory changes such as hypertension (mean systolic blood pressure-210 mmHg) and bradycardia (10.5% decrease in heart rate) were also elicited by the 1st challenge with CAPS. These changes were largely red

Vibroacoustic stimulation of the fetus entering the second stage of labor.

OBJECTIVE: To evaluate the fetal heart rate (FHR) response to vibroacoustic stimulation of fetuses entering the second stage of labor as a predictor of neonatal outcome. METHODS: Three hundred sixteen cases and 316 controls were studied during the second stage of labor. All cases had vibroacoustic stimulation on entering the second stage of labor using an electronic artificial larynx. For control patients, the artificial larynx was not activated. The patients were stratified into groups based on the quality of FHR response: acceleration (n = 124), acceleration followed by deceleration (n = 12

Improved LAboratory Prototype ELectrolarynx (LAPEL): using inverse filtering of the frequency response function of the human throat.

The electrolarynx (EL) provides a valued means of verbal communication for people who have lost their larynx. Existing ELs have some drawbacks such as harsh, raucous, and unpleasant sound and the presence of background noise. This study presents an experimental analysis of two commercial ELs and describes the development and testing of an improved LAboratory Prototype ELectrolarynx (LAPEL) which more accurately simulates the sound of a natural larynx and has lower background noise. This natural sound is obtained by determining the frequency response function (FRF) of the tissue of the human ne
