Anterior segment mass in miliary tuberculosis: a confusing scenario.

Tuberculosis (TB) is one of the most prevalent diseases in the Indian subcontinent. Ocular TB presenting as iris mass is rare; furthermore, invasion of anterior segment without choroidal invasion is even more scarce. Presented here is a case report detailing the condition of a patient in late adolescence who presented with diminution of vision and pain in the left eye. Slit lamp biomicroscopy revealed a nodular vascular mass emerging from the iris. Subsequently, the patient underwent a fine needle aspiration biopsy from the anterior chamber, which yielded a positive result for acid-fast bacill

Isolated nodular hepatic tuberculosis in an immunocompetent patient following dexamethasone treatment for COVID-19.

Hepatic tuberculosis is a rare extrapulmonary manifestation of pulmonary or miliary tuberculosis, typically seen in immunocompromised individuals. Isolated nodular hepatic tuberculosis is exceptionally uncommon in immunocompetent individuals and often mimics other hepatic lesions such as hepatocellular carcinoma. We present a 35-year-old male who developed isolated nodular hepatic tuberculosis following high-dose dexamethasone treatment for COVID-19. Imaging and fine-needle aspiration cytology suggested hepatocellular carcinoma, prompting surgical hepatic segmentectomy. Postoperative histopath

Epidemiological and clinical analysis, and outcomes of tuberculosis co-infection among people living with HIV in Türkiye (2014-2024) ClinSurv HIV cohort: A large case series.

BACKGROUND: Tuberculosis (TB) is one of the most common opportunistic infections in people living with HIV (PLHIV). Mycobacterium tuberculosis may cause more TB in all stages of HIV infection than in the general population, with the incidence of TB and the spread of pulmonary TB to other organs increasing as the CD4 count decreases. OBJECTIVE: In this HIV cohort study, we aimed to evaluate the clinical features, diagnosis, and prognosis of TB among PLHIV in Türkiye. MATERIALS AND METHODS: We conducted a retrospective cohort study to analyze clinical outcomes and identify determinants of mort

Miliary Lung Lesions Mimicking Tuberculosis: A Case of Metastatic Lung Adenocarcinoma.

Miliary patterns on chest imaging frequently prompt urgent consideration of disseminated infectious diseases such as tuberculosis (TB), particularly in patients from endemic regions. However, non-infectious aetiologies like metastatic malignancy must also be considered. We report the case of a 39-year-old previously healthy male with miliary pulmonary lesions initially treated empirically for disseminated TB. Despite therapy, his condition deteriorated until biopsy results confirmed metastatic lung adenocarcinoma. This case highlights the diagnostic challenges in TB-endemic regions and undersc

Spontaneous Resolution of Miliary Pulmonary Nodules Following Intravesical Bacillus Calmette-Guérin (BCG) Therapy: A Case Report and Literature Review.

Intravesical Bacillus Calmette-Guérin (BCG) immunotherapy is a well-established treatment for non-muscle-invasive bladder cancer. Although it is typically associated with local irritative symptoms, rare systemic and pulmonary complications can occur, including hypersensitivity pneumonitis and miliary tuberculosis. We report the case of a 70-year-old man who developed diffuse miliary pulmonary micronodules and ground-glass opacities after his 12th BCG instillation. Despite imaging findings suggestive of disseminated infection, the patient remained clinically stable, without fever, hypoxia, or s

Metastatic Lung Adenocarcinoma Mimicking Miliary Tuberculosis: A Case Report.

Lung adenocarcinoma (LA) can present with a wide range of morphological patterns and may mimic disseminated infectious diseases such as miliary tuberculosis (TB), posing significant diagnostic challenges and potentially delaying appropriate treatment. We report a case of metastatic pulmonary adenocarcinoma that was initially misdiagnosed as miliary TB. A 35-year-old nonsmoking male presented with a progressive dry cough and shortness of breath. Chest imaging revealed diffuse bilateral micronodules and a pericardial effusion, raising suspicion for miliary TB. Although acid-fast bacilli smears a

Barriers to Care Leading to Fatal Consequences: A Case of Tuberculosis Sepsis Amid Policy Change.

Mycobacterium tuberculosis is an acid-fast bacterium with a diverse range of clinical manifestations and is considered rare in Western countries. Tuberculosis (TB) remains a global health concern and can pose several challenges in diagnosis and treatment. Paired with changing immigration policies, immigrant populations can face several barriers to healthcare. This case illustrates the impact of immigration policy on clinical outcomes and how government policies can act as a barrier to care. We present a case of a 26-year-old Guatemalan man with no known past medical history who came to the eme

Isolated splenic tuberculosis mimicking a solid splenic neoplasm in an immunocompetent patient: A surgical case report.

INTRODUCTION AND IMPORTANCE: Tuberculosis is the leading cause of death by an infectious agent globally, especially among immunocompromised patients. Extrapulmonary tuberculosis (EPTB) can occur as a sequela of pulmonary tuberculosis through lympho-hematogenous or miliary spread, or can occur in isolation. EPTB rarely occurs in isolation in a single organ, especially in immunocompetent patients. Limited data exist on the incidence of isolated splenic tuberculosis in immunocompetent patients. CASE PRESENTATION: This report highlights a case of a 45-year-old immunocompetent female with a one-mo

Asymptomatic Miliary Tuberculosis in a Patient With Polymyalgia Rheumatica.

Miliary tuberculosis (TB), a severe form of TB caused by lymphohematogenous dissemination from a Mycobacterium tuberculosis focus, usually presents with systemic symptoms including fever and malaise. Here, we report the case of an 87-year-old woman treated with low-dose prednisolone and methotrexate for polymyalgia rheumatica, incidentally diagnosed with small miliary nodules in her lungs on chest computed tomography without any symptoms. Moreover, the patient reported being full of energy. The acid-fast bacterial culture and polymerase chain reaction test from various sites, including the tra

Radiologic Deception: A Case Report of Tuberculosis with Nodular Onset and Cystic Conclusion.

Cystic lung disease is a rare and underrecognized manifestation of pulmonary tuberculosis (TB), often masquerading as other diffuse cystic lung disorders and complicating timely diagnosis. We present the unusual case of a 25-year-old woman who developed progressive dyspnea and pleuritic chest pain shortly after initiating antituberculous therapy. Initial imaging revealed classical miliary nodules without cystic changes. However, within a month of treatment, follow-up high-resolution CT unveiled an unexpected transformation of numerous thin-walled cysts clustered in the upper lobes, accompanied

CT Imaging Features of Pulmonary Sarcoidosis: Typical and Atypical Radiological Features and Their Differential Diagnosis.

Sarcoidosis is a chronic, idiopathic, multisystemic inflammatory disease characterized by non-caseating granulomas, most commonly affecting the lungs and mediastinal lymph nodes. Radiological imaging plays a fundamental role in the diagnosis, assessment of disease extent, and differentiation from other pulmonary conditions. This narrative review offers a comprehensive overview of the imaging features of pulmonary sarcoidosis, focusing on both typical patterns-such as bilateral hilar lymphadenopathy, perilymphatic nodules, and upper lobe-predominant infiltrates-and atypical manifestations-inclu

Miliary tuberculosis with hypercalcaemia-induced nephrogenic diabetes insipidus in an infant.

Miliary tuberculosis (TB) is a rare but severe manifestation in infancy. Hypercalcaemia, well documented in granulomatous diseases like TB and sarcoidosis in adults, is infrequently reported in children. Nephrogenic diabetes insipidus induced by hypercalcaemia is a known complication. A female infant with right-sided neck swelling for 1 month (fine-needle aspiration showed tubercular lymphadenitis), on four-drug anti-tubercular therapy (ATT), admitted with complaints of polyuria, polydipsia and progressive weight loss over the last 2 weeks. Her father was on ATT for pulmonary TB. Investigation

Many-Faced Imitator: A Case of Acinar Adenocarcinoma of the Lung Resembling Miliary Tuberculosis.

The Great Imitator is a term used to describe different conditions that resemble other diseases. These include conditions such as syphilis, tuberculosis, lupus, systemic mycoses, and sarcoidosis, just to name some of which have fallen into this category of diseases that present with multi-system involvement, and a myriad of signs and symptoms that can be mistaken for other pathologic processes. A high index of clinical suspicion is necessary to analyze the information obtained from history of present illness, physical exam, and information obtained from available tests (i.e., serology, microbi

Beyond the Lungs: A Case Report of Disseminated Tuberculosis With Multisystem Involvement.

Tuberculosis (TB) is an infectious disease caused by acid-fast bacillus pertaining to the Mycobacterium tuberculosis complex. Pulmonary TB is the most common presentation, resulting either from primary infection or reactivation of latent disease. In rare cases, wide dissemination of M. tuberculosis can occur, usually by hematogenous or lymphatic route, leading to multiorgan involvement and potentially life-threatening conditions known as disseminated TB. We present the case of a 55-year-old man who presented to the emergency department (ED) with complaints of inflammatory polyarthralgia and my

A Perplexing Case Highlighting the Diagnostic Conundrum of Miliary Tuberculosis Mimicking Sarcoidosis and Progressing Into Hemophagocytic Lymphohistiocytosis.

Establishing the diagnosis of miliary tuberculosis (TB) can be challenging due to the heterogeneous clinical presentations and low sensitivity of diagnostic tests. Miliary TB shares overlapping clinical, radiological, and histopathological features with other chronic granulomatous diseases, such as sarcoidosis, often posing a significant diagnostic challenge for clinicians. A 36-year-old male from Haiti presented with a four-month history of recurrent fever, dry cough, night sweats, and weight loss. Chest imaging revealed innumerable widespread miliary nodules throughout the lungs bilaterally,

A Pathogen's Whisper: Reactivation of Quiescent Membranous Nephropathy by Disseminated Tuberculosis.

Primary membranous nephropathy (MN) is an autoimmune glomerular disease commonly associated with anti-PLA2R antibodies, with relapses typically attributed to spontaneous immune reactivation. We report the first documented case of a relapse of primary, PLA2R-positive MN that was temporally and immunologically linked to disseminated tuberculosis (TB) infection. A 42-year-old man, previously in complete remission, developed severe nephrotic syndrome and acute kidney injury unresponsive to standard immunosuppressive regimens. Concomitant diagnosis of miliary TB was confirmed by culture and imaging

Incidental Primary Hepatic Tuberculosis Mimicking Malignancy in a Patient With Hepatolithiasis: A Case Report of a Rare Surgical Condition.

Primary hepatic tuberculosis (TB) is an uncommon and diagnostically challenging condition, particularly in the absence of miliary or pulmonary disease. It can closely mimic hepatobiliary malignancies both clinically and radiologically, often leading to misdiagnosis and unnecessary surgical interventions. We report a case of a 45-year-old man who presented with right upper quadrant abdominal pain, fever, and jaundice. His medical history included open cholecystectomy with resection and anastomosis of the transverse colon, and endoscopic retrograde cholangiopancreatography for a choledochocolic

Recent advances in nanomedicine for the diagnosis and therapy of thyroid disorders.

INTRODUCTION: Inappropriate antibiotic prescribing drives antimicrobial resistance. Although the Swiss Society of Infectious Diseases has introduced national guidelines for common infectious diseases starting from 2019, it remains unclear whether family physicians and paediatricians adhere to them and what factors influence their prescriptions. The aims of this study were to assess whether Swiss family physicians and paediatricians make appropriate antibiotic choices in accordance with national guidelines and to identify physician- and patient-related factors associated with the prescribing of
