Pregnancy and tuberculosis.

There is no solid evidence that pregnancy has an adverse effect on tuberculosis. With early diagnosis and prompt, adequate chemotherapy, the outcome of pregnancy in a woman with tuberculosis is likely to be good. Routine therapeutic abortion is not indicated. Data in the literature do not support the notion that pregnancy is a major risk factor for the development of tuberculosis, although no well-designed studies have been conducted. Screening of pregnant patients for tuberculosis should be based on consideration of other proved risk factors not on the fact of pregnancy. Preventive therapy sh

Histiocytic hemophagocytosis in miliary tuberculosis.

The signs, symptoms, and bone marrow findings of the recently described virus-associated hemophagocytic syndrome was seen in a patient we treated. The manifestations of this syndrome may not be specific to viral infections and may possibly occur in other infectious processes. Autopsy findings in the present case showed it to be associated with disseminated miliary tuberculosis. Concomitant viral infection was excluded. To our knowledge, only one similar patient has previously been described.

Extrapulmonary tuberculosis revisited: a review of experience at Boston City and other hospitals.

During a 10-year period, 136 patients with extrapulmonary tuberculosis were seen at Boston City Hospital and other hospitals affiliated with Boston University School of Medicine. Review of these cases revealed that the prevalence of extrapulmonary tuberculosis was declining less rapidly than that of pulmonary disease. Extrapulmonary disease represented 4.5% of all new cases of active tuberculosis and tended to occur in older patients than in previous reports. Sites of involvement included lymph nodes, blood, genitourinary tract, bone and articular sites, the meninges, peritoneum, adrenal gland

Antituberculous agents. Isoniazid, rifampin, streptomycin, ethambutol, and pyrazinamide.

Effective antituberculous drugs have radically improved the prognosis of the patient with active tuberculosis. Surgical therapy is rarely needed, and sanitoriums have largely vanished. Triple-drug therapy may be indicated initially for cavitary pulmonary disease, meningitis, miliary disease, and moderate to severe renal disease. Short-course therapy twice or three times weekly with isoniazid and rifampin may be used in cavitary pulmonary disease and probably in these other serious infections as well. Isoniazid alone is adequate for prophylaxis. The major cause of therapeutic failure is noncomp

[A case of generalized tuberculosis].

A 52 years old female with suspected malignancy was hospitalized because of fever and lymph node swelling. In spite of lymph node biopsy, bone marrow aspiration, blood culture and other diagnostic procedures, no clue to definite diagnosis was obtained. During the course of hospitalization, abscesses with fistulae appeared and tubercle bacilli were demonstrated in the pus from a subcutaneous abscess. Thus, the diagnosis of tuberculosis was established. Finally the patient died of massive bleeding as a result of perforation into thoracic aorta of the zone of necrosis of esophageal wall. It is to

Active tuberculosis undiagnosed until autopsy.

In this report 21 patients in whom tuberculosis was the primary cause of death, but which was not diagnosed until necropsy, are reviewed. Of the 21 deaths, 11 were due to pulmonary tuberculosis and 10 to miliary tuberculosis. Proper evaluation of the following factors might have led to the correct diagnosis in many of the patients: A family history of tuberculosis, prior pleurisy, a gastrectomy, diabetes mellitus or end-stage renal failure; all can be associated with an increased incidence of tuberculosis. A negative tuberculin skin reaction does not exclude the presence of active tuberculosis

[Cryptic miliary tuberculosis. Two case-reports (author's transl)].

The term cryptic miliary tuberculosis designates a particular form of hematogenous tuberculosis in which the usual diagnostic criteria, especially the roentgenographic ones, are lacking. Two cases are reported. Initial clinical manifestations were prolonged fever, poor general condition, low blood pressure, low serum sodium, mild hepatic biological disorders, and myocarditis. No other radiological or biological features, suggestive of tuberculosis were found. In the first patient, the various visceral tuberculous localizations occurred under specific treatment, although the latter was otherwis

Mycobacterial infections in renal transplant recipients. Seven cases and a review of the literature.

During an 11-year period, 1,069 patients received renal allografts at the University of Minnesota Hospital, Minneapolis, and infections developed in seven (0.65%) due to mycobacteria (Mycobacterium tuberculosis and M kansasii). The primary infection was in joint or subcutaneous tissue in six patients and pulmonary (miliary) in one. Infections in joint or skin shared common features regardless of the species of Mycobacterium and usually mimicked acute pyogenic bacterial infection; all responded to antimycobacterial drugs. The clinical manifestations in our patient in miliary tuberculosis were c

Management of extra-pulmonary tuberculosis (excluding miliary and meningeal) in south and west Wales (1976-8).

In a retrospective survey of the management of extrapulmonary tuberculosis lymph node and genitourinary tuberculosis were found more commonly than bone and joint or gynaecological disease. Only 29% of patients received 18 moths' chemotherapy while 31% received nine to 12 months' treatment with rifampicin and isoniazid regimens and 34% had short-course chemotherapy with other regimens. Five patients were not offered any chemotherapy after diagnosis, and in five patients the diagnosis was overlooked because of administrative errors. One patient died from tuberculosis (renal). [Results of thoracoscopies in the diagnosis of diseases of the lungs and of the pleura (author's transl)].

The diagnostic results of 125 thoracoscopies in patients aged 17-85 years are analysed retrospectively. The condition for relevant diagnosis by thoracoscopy is an intrapleural space permitting the installation of a pneumothorax of sufficient size for tissue-biopsy under direct vision. The results are largely dependent upon the choice of suitable cases. Macroscopic appearance and histological examination made it possible in 100 of 125 cases to make a definite diagnosis or to exclude it. Thoracoscopy is a suitable diagnostic method especially for the following indications : pleural disorder of u

Mycobacterium bovis infection of the conjunctiva.

An elderly woman from Minnesota had bilateral bulbar conjunctival nodules that proved to be caused by Mycobacterium bovis. She was found to have active military tuberculosis; only pulmonary and ocular involvement was confirmed. She had a history of tuberculosis but no recent exposure or intercurrent illness. The case was unusual in that the endogenous infection caused bilateral bulbar symptoms but no palpable lymphadenopathy and no other organ involvement. This case demonstrates many of the basic tenents on the course of tuberculosis in humans. Triple chemotherapy was administered, and the pat

It is suggested that in veiw of the ubiquitous presentations and incidence of tuberculosis in this environment, more vascular studies need be don

Late generalized tuberculosis: a clinical pathologic analysis and comparison of 100 cases in the preantibiotic and antibiotic eras.

1. The clinical and pathologic findings in 100 patients with late generalized tuberculosis (LGT) are described and a comparison made between the findings occurring in the preantibiotic era with those in the early antibiotic period. The clinical presentation of LGT as seen in a general hospital has changed. Whereas, in the preantibiotic era, LGT was often the primary disease, occurring principally in young adults and frequently associated with pulmonary symptoms, in the antibiotic era, LGT commonly occurred together with and was frequently obscured by other diseases, often afflicted the elderly

Miliary tuberculosis.

Twenty-six cases of miliary tuberculosis were studied in retrospect. The mean age of the patients was 62 years. Eighteen patients suffered from another underlying chronic disease. Nine had been treated with corticosteroids or cytotoxic agents. A limited manifestation of tuberculosis had been previously verified or suspected in ten cases. Fever was present in 85% of the patients, frequently combined with fatigue or abdominal pain. Serum alkaline phosphatase was elevated in 81% of the cases. Minor haematological abnormalities (anaemia, etc.) were found in 16 cases and pancytopenia, stimulated ly

[Nieman-Pick disease revealed by a pulmonary miliary tuberculosis (author's transl)].

A diagnosis of Niemann-Pick disease was made in a 26 years old man with chronic pulmonary miliary tuberculosis and splenomegaly. It was confirmed by the drop of sphingomyelinase level in leucocytes and fibroblasts of the skin. The authors showed the presence of foamy cells with sphingomyelin in the spleen and bone marrow. They underlined the value of dosing sphingomyelinase in leucocytes for diagnosis purposes. They also observed blue histocytes in the spleen and bone marrow, next to the foamy cells which are not specific of Niemann-Pick disease and can be found in numerous other affections. T

Investigation of Nigerians with diffuse radiographic pulmonary shadowing.

During a five-year period, 45 Nigerians with radiographic appearances of diffuse pulmonary infiltration with at least six sputum specimens negative on culture for tubercule bacilli were investigated at University College Hospital, Ibadan. The value of pulmonary function studies and open or closed lung biopsy in discovering the cause of diffuse lung disease was examined. Cryptogenic fibrosing alveolitis was the most common cause (13 cases). Other diagnoses included malignant disease (11 cases), sarcoidosis (three cases), progressive systemic sclerosis (three cases), miliary and non-caseating tu

[Clinically unrecognised tuberculosis in autopsy material (author's transl)].

Among 18,724 unselected postmortems performed between 1955 and 1975 811 (4.33%) cases of post-primary tuberculosis were found among which 370 (45.6%) were active and 441 (54.4%) inactive. Occurrence of death involved tuberculosis in 32% of all cases discovered at autopsy. The age group over 50 years was mainly affected and males were clearly more frequently involved, particularly in pulmonary tuberculosis. 333 cases of tuberculosis (41.1%) had been recognised clinically and had mainly consisted of chronic pulmonary forms. In the older age group the proportion of cases diagnosed prior to death

Tuberculosis in immunosuppressed patients.

11 patients who developed tuberculosis while on long-term immunosuppressive-drug therapy are described. The indications for immunosuppressive therapy were varied, but all patients received high doses of corticosteroids with azathioprine in addition in 2 cases and chlorambucil in a third. The diagnosis was delayed in all cases because of suppression of symptoms, so that 4 patients died, 3 directly as a result of tuberculosis. 3 of the remaining patients were critically ill at the time of diagnosis but recovered with antituberculosis chemotherapy; whereas the other 4, although not critically ill

First tuberculosis prevalence survey in KwaZulu.

The results of a prevalence survey undertaken during 1974 are presented. A stratified 3-stage sampling technique was used to select 3 789 people from 10 randomly chosen sites in KwaZulu. One Heaf test and 2 Mantoux tests were performed on each of 1 769 children under 18 years of age, of whom 28,7% were found to have BCG scars. Of the radiographs taken of persons 15 years of age and older, 893 were found to be negative, 25 (2,7%) disclosed active tuberculosis (TB) and 3 (0,3%) miliary TB. Of the 1 136 sputum specimens collected and examined by fluorescence microscopy, 15 (1,3%) were found to be
