Tuberculosis: frequency of unusual radiographic findings.

The changing pattern of the radiologic presentation of tuberculosis was noted in a review of 100 consecutive cases of pulmonary tuberculosis. Active tuberculosis was not suggested as a radiologic diagnosis in 43% of the patients. Of the 100 cases, only 66 had characteristics of reactivation tuberculosis (i.e., minimal upper lobe infiltrate or more advanced cavitary disease). Of the other 34 patients, 18 had less usual presentations, making the radiographic diagnosis difficult. The remaining 16 patients had radiographic patterns quite unusual for tuberculosis, including masslike densities resem

Radiographic spectrum of pleuropulmonary tularemia.

Pleuropulmonary disease was seen in 50 of 62 patients (81%) with proven tularemia. Radiographic findings included patchy subsegmental air space opacities (74%), hilar lymphadenopathy (32%), and pleural effusion (30%). Less common manifestations were air space opacification of an entire lobe or segment, cavitation, oval opacities, pericardial effusion, linear opacities and septal lines, apical and miliary disease resembling tuberculosis, a mediastinal mass, empyema with bronchopleural fistula, and residual cystic changes, calcification, and fibrosis. Pleuropulmonary tularemia may be easily misd

Tuberculous peritonitis in Manitoba.

Between Jan. 1, 1971 and June 30, 1976 the authors diagnosed tuberculous peritonitis in 17 patients. The basis for the diagnosis was a positive culture for Mycobacterium tuberculosis from the peritoneal fluid or nodules (nine patients) or the presence of caseating granulomas in biopsy specimens of the peritoneum (eight patients). Fifteen of the 17 patients were women. Eleven were North American Indians and eight of them suffered from alcoholism. The predominant symptoms of abdominal pain, progressive abdominal distension and vomiting, and abdominal tenderness on physical examination were prese

Clinical and roentgenographic spectrum of pulmonary tuberculosis in the adult.

In order to define the roentgenographic manifestations of pulmonary tuberculosis in the adult, we reviewed a 12 month experience with newly diagnosed patients with active pulmonary tuberculosis in two Boston hospitals. Of 88 patients, 30 (34 per cent) presented with roentgenographic manifestations other than those associated with "usual" postprimary disease. At least 12 patients (13.5 per cent) had primary tuberculosis, including two subjects over 60 years of age. Six patients (6.8 per cent) had disease confined to the lower lung fields, eight patients (9 per cent) had tuberculomas and four (4

[The influence of BCG-vaccination on the tuberculosis in children (Hamburg 1950-1971) (author's transl)].

Between 1950 to 1971 6 758 cases of active tuberculosis were registered among children under 15 years of age in Hamburg. 33 of them died, mostly due to tuberculous meningitis. In spite of a general decrease in tuberculosis during that period a relative increase of cases of pleurisy, nodular tuberculosis and tuberculous infiltrations of the lung were seen. On the other hand, cases of tuberculous meningitis and miliary tuberculosis as well as cases of glandular tuberculosis decreased. Rarely BCG-vaccinated children suffered from miliary tuberculosis, tuberculous meningitis and pleurisy. A statis

Antituberculosis agents: isoniazid, rifampin, streptomycin, ethambutol.

Effective antituberculosis drugs have radically improved the prognosis of the patient with active tuberculosis. Surgical therapy is rarely needed, and sanatoria have largely vanished. Initially, triple-drug therapy is indicated in cavitary pulmonary disease and severe renal disease and it is generally used in miliary tuberculosis and meningitis. Use of one of the three drugs may be discontinued after there is evidence that the bacillary population has been decreased. Two-drug therapy is indicated for other active disease. Isoniazid alone is adequate for prophylaxis. The major cause of therapeu

[Old and new aspects of childhood tuberculosis (author's transl)].

During the preceding 8 years 50 children were treated for active tuberculosis at the University of Innsbruck Department of Pediatrics. None of them were vaccinated against tuberculosis. Three children died; one of them from a fulminant miliary tuberculosis, two others from tuberculous meningitis; both of them were neurosurgically explored due to massive increase of intracranial pressure before a definite diagnosis was established. Two other patients, out of eleven with Tb-meningitis, are suffering permanent damages. Each one of the fatal cases and those with permanent sequelae could have been

Miliary tuberculosis following homograft valve replacement.

Postoperative septicaemia with infective endocarditis is a recognized complication of open-heart surgery, in particular homograft or prosthetic replacement of cardiac valves. Several infective organisms, both bacterial and fungal, have been incriminated but infection due to tubercle bacilli has not, to our knowledge, been reported. The clinicopathological features of this condition are discussed. During a five-year period, over 800 homograft replacements in the aortic and/or mitral positions have been performed at Harefield Hospital. Seven cases of miliary tuberculosis following homograft valv

Miliary tuberculosis in two infants after nursery exposure: epidemiologic, clinical, and laboratory findings.

Two infants born 4 days apart, and cared for in the same nursery, developed miliary tuberculosis at 2.5 and 5.5 months of age, respectively. A search for a source case among household contacts failed to reveal anyone with active tuberculosis. An epidemiologic study of nursery personnel revealed a nurse's aide with cavitary tuberculosis. Examination of 1,647 infants (98.9 per cent of all infants cared for in the nursery during the 10.5-month period preceding the discovery of the source case) failed to reveal any other infected children. Recommendations are made for tuberculosis control programs

Miliary tuberculosis presenting with hyponatremia and thrombocytopenia.

A 74-year-old woman with miliary tuberculosis had moderately severe hyponatremia due to inappropriate secretion of antidiuretic hormone (SIADH) and very severe thrombocytopenia without other hematologic abnormalities. She was treated with isoniazid, rifampin, ethambutol, prednisone, vincristine and fluid restriction and recovered completely. The SIADH may have been a response by the posterior pituitary to a decrease in intravascular volume resulting from the extensive pulmonary disease or associated hypoxia, or the tuberculous lung may have released ADH or an ADH-like substance. The thrombocyt

[Bone marrow's histopathology in tuberculosis of the hematopoietic organs (author's transl)].

Through the reports of the literature, the histopathologic lesions of the bone marrow in tuberculosis of the hematopoietic organs are analysed. Whether the lesions are typical or not, it is necessary to have a bacteriologic proof of tuberculosis. Near these lesions, bone marrow's cellularity is often greater or lower, reticular fibers are often increased and sometimes reticulin fibrosis is marked. Other necrosis and granulomatous inflammation are discussed. Contribution of macrophage is described. The relationships between tuberculosis and hematologic diseases are reported.

[Anticonvulsant drugs and liver damage (Author's transl)].

540 autopsies of institutionalized epileptics who had been treated with anticonvulsant drugs were evaluated as to type and frequency of pathological changes in the liver and compared with a control series of 270 nonepileptics. A specific liver-toxicity of antiepileptic drugs, or combination of drugs, could not be established. Thus, only in 3 cases, one of which was fatal, was a relationship found between anticonvulsant drug therapy and liver damage. Therefore, there seems to be no reason to discontinue anticonvulsant therapy or to reduce the usual dose for fear of causing liver damage. On the

[Epidemiology of Tuberculosis in Children from 1968 til 1973 in Schleswig-Holstein (author's transl)].

In Schleswig-Holstein 271 children (131 boys, 140 girls) were treated for tuberculosis between january 1968 and June 1973; 84% were aged 5--14 years, 14% between 1--4 years and 29% between 0--1 year. Each year, nearly the same number of infections occurred in the various districts of the country. In 246 children a primary form of tuberculosis was diagnosed (in 11 cases with pleurisy). 11 patients tuberculous meningitis, 3 patients miliary tuberculosis and 5 patients another form of the disease, 18 of 271 patients (7%) had been vaccinated against tuberculosis as newborns; in 4 of these children

Abdominal tuberculosis: demonstration of tubercle bacilli in tissues and experimental production of hyperplastic enteric lesions.

Material obtained from 35 cases of abdominal tuberculosis waps studied and an attempt was made to demonstrate tubercle bacilli by special staining of histological sections, culture and guinea-pig inoculation. Acid fast bacilli could be demonstrated in 80 per cent of cases. The remaining cases were also possibly due to tuberculous infection because granulomas with Langhans's type of giant cells and caseation or calcification of lymph nodes were present. The bacilli were demonstrated in bowel tissue showing non-specific histology in 1 case. In 2 other cases with non-specific lesions in the bowel

Ethical problems in the management of myelomeningocele and hydrocephalus. The Milroy Lecture 1975.

Within the last generation the application of major advances in drug therapy, intensive care, transfusion techniques, surgery, anaesthesia, and radiotherapy, together with a vast expansion of knowledge due to increased investigative facilities, have led to an unprecedented, dramatic and beneficial increase in the number of persons who would previously have died, or lived with severe handicaps, but can now be cured. Techniques have also been developed to prolong the lives of many people who are now able to enjoy several extra years of productive and meaningful existence, and to alleviate and im

[Bone marrow biopsy in the diagnosis of hematopoietic tuberculosis].

The authors report 6 cases of tuberculosis of the bone marrow, in which the diagnosis was made by biopsy. Three of these cases, included peripheral blood abnormalities, such as pancytopenia, whereas in the three other, nothing special was found on routine blood examination. All the patients, except one who died from intercurrent infection, became cured thanks to anti-tuberculosis treatment. Marrow biopsy thus permits early diagnosis, miliary tuberculosis, even when usual radiological and bacteriological investigations are negative.

Radiological patterns of pulmonary tuberculosis in Nigeria.

Two hundred and sixteen patients with pulmonary tuberculosis were reviewed. Childhood and adult patterns of tuberculosis as noted in other countries were recognised. However, some children were seen with an adult pattern most probably due to a reinfection. There were also adults with the childhood pattern most probably due to a failure in the localisation of the primary intra-pulmonary parenchymal lesions with subsequent haematogenous spread. The prognosis in children with the adult pattern, and in adults with the childhood pattern was poor. The lesions (noted in this study) were severe and wi

In addition, it made it possible to assess the involvement of intraabdominal organs which cannot be judged clinically or biochemically and may not be parallel to changes in the thoracic X-ray. Extensive liver sarcoidosis was revealed in 12 and required treatment independent of other organ manifestations. Miliary exudative or cavernous pulmonary tuberculosis in 15 cases revealed gross non-specific changes in one, miliary foci of the liver surface in 13 and coarse nodular infiltration in one. Fatal infections in protein-calorie malnourished children with thymolymphatic atrophy.

The clinicopathological features of 25 children who died with protein-calorie malnutrition were studied. All but four subjects were found at necropsy to have nutritional thymectomy and all but 3 died of infectious diseases. The infectious agents were chiefly intracellular micro-organisms including miliary tuberculosis, Herpes simplex, varicella, measles, Pneumocystis carinii, and Plasmodium falciparum. Staphylococcal infections, salmonellosis, shigellosis, strongyloidiasis, and hookworm were other significant infectious agents. Nutritionally acquired defective immunity, especially cell-mediate
