Keratoconus is characterized by the presence of corneal distortion (secondary to thinning of the apex) and either Fleischer's ring (found in 57% of patients) or Vogt's striae (found in 44% of patients). Often the only presenting symptom is decreased visual acuity. Steep keratometric readings are not usually diagnostic; 18% of patients with keratoconus have readings flatter than 45.00 D. Treatment requires correction of acuity with spectacles or contact lenses. More advanced cases are generally managed with rigid gas-permeable contact lenses. Corneal scarring occurs in approximately 43% of case

We conducted a retrospective analysis of the management of 118 eyes of 66 new patients presenting with keratoconus at the University of Florida from 1987 through 1992. Eyes were ultimately managed by one of three methods: glasses or no correction; contact lenses; or penetrating keratoplasty. The outcome of each management method was determined by evaluating initial and final vision and keratometry for each group. Twenty-one eyes received glasses or required no correction. Rigid gas permeable lenses, Dura-T style PMMA lenses, and specialty design gas permeable lenses were used to successfully f

After photorefractive keratectomy (PRK) using excimer lasers (193 nm) many patients report the presence of halos around light sources at night. However, halos are not unique to PRK patients, as they are a common observation in myopic contact lens wearers. We present an objective measurement of the halos using a computerized technique. The patient fixated on a red cross within a white circle in the center of a video monitor which served as the halo source. The screen surrounding the circle was not illuminated. The operator controlled the movement of the white spot and moved the spot toward the

In a retrospective analysis, we examined 30 consecutive cases of penetrating keratoplasty in which a double running 10-0/11-0 nylon suture technique was used and the 10-0 suture was adjusted early in the postoperative period to reduce astigmatism. When the response to suture adjustment was inadequate, the presence of the 11-0 suture allowed for early (ten to 18 weeks) removal of the 10-0 suture. Rapid visual recovery (12.3 +/- 0.95 weeks; mean +/- standard error) and low levels of final astigmatism (2.66 +/- 0.24 diopters) were achieved. Visual acuity was 20/20 to 20/40 in 25 of the 28 patient

Recent technological advances have had a major impact on military aviation. While modern methods of providing visual information via electro-optics/visionics systems have extended the aviator's operational envelope, these devices are becoming increasingly incompatible with spectacle wear. Since close to 22 percent of Army aviators are ametropic (spectacle wearing), alternative means of providing a refractive error correction need to be investigated. One alternative being considered is the use of a contact lens correction. Since November 1988, the U.S. Army Aeromedical Research Laboratory (USAA

While good visual acuity and refractive results after epikeratoplasty for aphakia have been reported, particularly for adults and older children, limited detailed information on the nature of the resulting vision is available. We have evaluated the visual performance of seven aphakic subjects corrected by epikeratoplasty by measuring contrast sensitivity, with and without the presence of glare. These results were compared with those for other aphakic corrections, namely spectacles (n = 5 eyes), contact lenses (n = 5 eyes), or intraocular lens implantation (n = 5 eyes). There were statistically

A series of 67 cases of epikeratophakia is presented with an average time from surgery of 12.2 months. For aphakia there was a delay in the recovery of vision, but by nine months 83% of 57 patients achieved an acuity equal to, or within 1 line of, the preoperative value. 57% were corrected to within 3 dioptres of emmetropia, but in the latter part of the series 75% were within this range. Astigmatism and reduced contrast sensitivity, especially in the presence of glare, were important complications. For keratoconus, 86% of seven patients with over two months of follow-up achieved a spectacle c

Authors refer their favourable experiences with the application of soft contact lenses used after the performed scleroplastic operation described by Pivovarov. Soft contact lenses were applied in 54 patients where myopia was operated. In 90% of patients the daily use of contact lenses was more than 8 hours. These patients tolerated the contact lenses even better than the patients with myopia but without operation. Compared with the correction with spectacles, the visual acuity was significantly increased in the group using the contact lenses. The visual acuity 6/6-6/9 was achieved in the group

Aniseikonia, although present in isometropia, is associated principally with anisometropia and is widely thought to be the result of differences in the retinal image size presented to each eye. This assumes that equating retinal image size results in congruous cortical images. To test this assumption 1 refractive and 17 axial anisometropic subjects were examined to determine the aniseikonia present when corrected with spectacles and contact lenses. Contrary to many previous predictions, based upon Knapp's Law, the presence of equal retinal images resulted in larger aniseikonias than observed w

There exists some controversy regarding the effects of active horizontal head rotation on vergence error. We used a conventional, Ogle-type, subjective fixation disparity apparatus to measure vergence error at near over a full range of horizontal head-rotation frequencies. Change in vergence error rarely exceeded 10 min arc. This finding demonstrates the adequacy of the vergence system to maintain relatively accurate bifixation at near and to allow for continuous maintenance of binocular sensory-motor integrative ability, even in the presence of system perturbations.

The basic concepts of impact resistance are reviewed. Also discussed are the research on materials, treatment and testing procedures, mechanisms of fracture and the factors affecting impact resistance. Research has shown that the strength of a lens treated or untreated is well below the theoretical strength of glass. The impact resistance of a lens is altered by the presence of breaks in the surface of a lens. The drop ball test as mandated by the FDA is essentially a control test. Its use as a means of evaluating the performance of a lens has been open to question. Researchers have suggested

We treated 16 patients aged 1 1/2 to 11 years with myopic anisometropic amblyopia with contact-lens correction of refractive error and occlusion. The degree of visual improvement compared favourably with that reported with the use of spectacles. There were no complications from contact lenses, and only one patient required anesthesia for fitting. Contact lenses appear to be more satisfactory than spectacle lenses in the management of myopic anisometropic amblyopia in regard to cosmesis, comfort, and treatment compliance. Patients with myelinated retinal nerve fibers and unilateral severe myopi

The correlation between lacrimal lens power changes and changing posterior apical radii was examined for a sample of 20 eyes wearing Bausch and Lomb Soflens contact lenses. Power changes caused by in situ lens flexure were calculated for principal meridians and both calculated in situ contact lens powers and over-refraction corrections were subtracted from the patient's spectacle corrections to determine lacrimal lens powers in each meridian. A significant correlation (p less than 0.01) was found in the flatter meridians for B, F, J, and N series lenses. To support the validity of the study a

The first 4 symptoms of binocular confusion concern primarily daytime vision and the fifth symptom concerns nighttime vision. Even with a conventional contact lens correction, symptoms of binocular confusion occur, especially with high optical aniseikonia for near or far distance. In addition, diplopia occurs in the presence of strabismus. The important result of our research is the fact that unilateral aphakes are severely disturbed by the 5 symptoms of binocular confusion described above. These disturbances can be eliminated completely with an appropriate spectacle-contact lens combination b
