Neck and jaw dysfunctions in somatosensory tinnitus: Clinical insights and implications.

OBJECTIVE: Tinnitus is known to be influenced by many different factors, such as hearing loss and stress or anxiety. In about 25% of tinnitus sufferers, dysfunctions of the cervical spine or temporomandibular area influence the tinnitus percept, then known as somatic or somatosensory tinnitus (ST). Previous research has provided diagnostic criteria for ST that are focused on medical history, but it is not yet clear if we can also recognise patients with ST based on the type of cervical spine or temporomandibular dysfunctions that are present. This cross-sectional study, therefore, aims to inve

Functional near-infrared spectroscopy (fNIRS) biomarkers of tinnitus severity within tinnitus subtypes.

Tinnitus, the perception of sounds not present externally, can significantly impair quality of life. Currently there are no clinically available objective measures to enable monitoring of tinnitus-related changes in brain activity. Our previous work demonstrated sensitivity of a non-invasive imaging technique called functional near-infrared spectroscopy (fNIRS) to changes in brain activity associated with tinnitus presence and severity. In this study we aimed to assess whether fNIRS features could quantify tinnitus severity on a continuous scale, a requirement for effective monitoring of treat

[Expert consensus on a novel classification system, surgical management strategies, and prognostic evaluation for congenital ossicular chain malformations based on stapes footplate mobility and suprastructural integrity (2026 edition)].

Congenital ossicular chain malformation (COCM) is a congenital developmental disorder that impairs hearing function. It most commonly presents unilaterally, either as an isolated anomaly or in association with a variety of systemic syndromes; in severe cases, it may coexist with malformations of the auricle and external auditory canal. Owing to its insidious onset and the presence of an intact tympanic membrane, affected children are frequently misdiagnosed with secretory otitis media. Bilateral COCM, if left untreated in a timely manner, can hinder speech and cognitive development, while spec

The auricular cough reflex: lateralization, prevalence, procedural determinants, and the role of examiner handedness.

BACKGROUND: The auricular cough reflex (Arnold's reflex) is a vagally mediated somato-visceral response triggered by stimulation of the external auditory canal (EAC). While classically considered rare, its clinical relevance in otologic procedures remains underexplored. OBJECTIVE: This study aimed to evaluate the prevalence, lateralization, and potential risk factors of the Arnold reflex during routine otologic examinations, with particular attention to examiner handedness, hearing aid use, and EAC inflammation. METHODS: In this prospective observational study, 404 consecutive patients under

Mechanically evoked tinnitus after cochlear implantation with preservation of residual hearing.

In the recent past, inclusion criteria for cochlear implant surgery expanded to patients with ever more residual acoustic hearing in the low frequencies. By applying the meticulous hearing preservation surgical strategy and specifically designed atraumatic electrode arrays, residual hearing can be preserved to a meaningful extent in a large majority of patients. In this paper, we describe two female patients suffering from mechanically evoked tinnitus after hearing preservation cochlear implantation surgery with MEDEL flex electrodes. The occurrence of audible perceptions through mechanical st

Activation region overlap visualization for image-guided cochlear implant programming.

Objective. The cochlear implant is a neural prosthesis designed to directly stimulate auditory nerve fibers to induce the sensation of hearing in those experiencing severe-to-profound hearing loss. After surgical implantation, audiologists program the implant's external processor with settings intended to produce optimal hearing outcomes. The likelihood of achieving optimal outcomes increases when audiologists have access to tools that objectively present information related to the patient's own anatomy and surgical outcomes. This includes visualizations like the one presented here, termed the

Intraoperative recordings of electromyogenic responses from the human stapedius muscle.

Following surgical implantation of a cochlear implant, the external processor must be programmed individually, which is typically achieved by behavioral measurements, with the user indicating the perceived loudness between the threshold and the maximum comfort levels (MCLs). However, the stapedius reflex measurement could be used instead, as the basis for programming the fitting maps. The electrically evoked stapedius reflex threshold is known to have a high correlation with the MCLs and thus is used as an objective tool to determine the MCL, especially in children and non-cooperative patients

Tinnitus, Use and Evaluation of Sound Therapy, Current Evidence and Area of Future Tinnitus Research.

Tinnitus is one of the otologic symptoms seen in persons with ear pathologies. It is defined as an acoustic sensation in the ear without a corresponding external acoustic stimulus. Tinnitus is said to be generated from the higher auditory centres because of reduced feedback from the peripheral or neural component but can be an outcome of irritation at different levels of the auditory system. Several treatment modalities have been tried in the management of tinnitus, either as a single treatment or as part of combination therapy. They range from lifestyle or dietary modification, medications, s

Posterior semi-circular canal electrode misplacement in Goldenhar's syndrome.

Literature documents the incidence of electrode misplacement within the range of 0.2% to 5.8% with the superior SCC as the most common site, followed by the vestibule. In this report, we present the finding of electrode misplacement in the posterior SCC in a child with Goldenhar syndrome which was subsequently corrected. This child with bilateral congenital profound SNHL presented for unilateral cochlear implant surgery. Intraoperatively, the lateral SCC bulge, stapes, oval window, round window niche and pyramid were noted absent, leading to a surgical decision in favour of a subtotal petrosec

The external ear morphology and presence of tragi in Australian marsupials.

Multiple studies have described the anatomy and function of the external ear (pinna) of bats, and other placental mammals, however, studies of marsupial pinna are largely absent. In bats, the tragus appears to be especially important for locating and capturing insect prey. In this study, we aimed to investigate the pinnae of Australian marsupials, with a focus on the presence/absence of tragi and how they may relate to diet. We investigated 23 Australian marsupial species with varying diets. The pinnae measurements (scapha width, scapha length) and tragi (where present) were measured. The inte

The effect of anatomical variables and use of the Lifts system on hearing outcomes after implantation of an active transcutaneous bone conduction device in bilateral congenital conductive hearing loss.

BACKGROUND: Malformations of the temporal bone present different challenges to the implantation of a transcutaneous active bone conduction device, such as Bonebridge (Med-el, Innsbruck, Austria). This study aims to describe the benefits of high-resolution computed tomography (HRCT) in preoperative assessment and to analyze whether characteristics of the mastoid process, intraoperative compression of the dura or sigmoid sinus, and the use of the Lifts system, lead to differences in audiological performance after implantation. METHODS: We examined 110 cases of congenital microtia. The structure

[Complications caused by hearing aid mold impression material - clinical case and literature review].

The procedure of taking an impression of the external auditory canal for the manufacture of individual earplugs in the ear canals can be associated with complications in some cases. The article describes the clinical case of ingestion of the impression mass during taking the impression of the external auditory canal in the patient with chronic otitis media. The presence of pronounced hearing loss has become the basis for the selection of a hearing aid. As a result, during taking a cast of the external auditory canal, the material fell into the middle ear cavity. Despite contacting specialists,

A technical review and evaluation of implantable sensors for hearing devices.

Most commercially available cochlear implants and hearing aids use microphones as sensors for capturing the external sound field. These microphones are in general located in an external element, which is also responsible for processing the sound signal. However, the presence of the external element is the cause of several problems such as discomfort, impossibility of being used during physical activities and sleeping, and social stigma. These limitations have driven studies with the goal of developing totally implantable hearing devices, and the design of an implantable sensor has been one of

On the design of a MEMS piezoelectric accelerometer coupled to the middle ear as an implantable sensor for hearing devices.

The presence of external elements is a major limitation of current hearing aids and cochlear implants, as they lead to discomfort and inconvenience. Totally implantable hearing devices have been proposed as a solution to mitigate these constraints, which has led to challenges in designing implantable sensors. This work presents a feasibility analysis of a MEMS piezoelectric accelerometer coupled to the ossicular chain as an alternative sensor. The main requirements of the sensor include small size, low internal noise, low power consumption, and large bandwidth. Different designs of MEMS piezoe
