[Auditory neuropathy--new disease].

Auditory neuropathy, a disorder of neural synchrony, is a retrocochlear hearing disorder identified by absence of auditory brainstem responses (ABR) and presence of evoked acoustic otoemissions (EOA). Registration of emissions shows normal function of external cilliary cells. Patients with auditory neuropathy have difficulties to understand speech especially in presence of beckground noise. Normal conversation requires the support of orofacial reading. The authors presents electrophysiological and behavioral tests useful in diagnostic process. An option in treatment of auditory neuropathy is c

[Ear canal narrowing: a case report].

BACKGROUND: ear canal narrowing is the thinning or closing of the ear canal. This can damage the sound transmition from the external to the internal ear, which in turn may cause a false hearing loss. AIM: to alert the professionals of the area on how to avoid a miss diagnosis. METHOD: a 47 year old woman, submitted to two audiologic evaluations: one without avoiding ear canal closing and one avoiding it. RESULTS: a decrease of up to 40 dB was observed in the results of the second evaluation. CONCLUSION: in order to obtain reliable results it is extremely important to verify the presence of

Glutamatergic clock output stimulates melatonin synthesis at night.

INTRODUCTION: Otomycosis is a fungus-causing superficial infection of the external auditory canal that is common in otolaryngology practice. Though it is a worldwide infection, but is more prevalent in warm and humid regions. There has been rise in the occurrence of otomycosis in the last few years because of the extensive usage of antibiotic eardrops. Other influencing reasons for the occurrence of otomycosis is swimming, an immunocompromised host (e.g.: DM, AIDs) pregnancy, post-canal wall down mastoidectomy, tympanic membrane perforation, hearing aids and self-inflicted injuries. METHODS:

Endothelial nitric oxide synthase affects the progression of autosomal dominant polycystic kidney disease.

BACKGROUND/AIM: The phenotypic variability of autosomal dominant polycystic kidney disease (ADPKD) cannot be explained only by various mutations of two known genes (PKD1 and PKD2), but the influence of other unknown factors should also be considered. Impairment of endothelial function has been observed in ADPKD patients. Our study tried to verify the hypothesis that endothelial nitric oxide synthase (ecNOS) gene polymorphisms in intron 4 could have some relevance to the progression of ADPKD. METHODS: We examined 155 Czech patients with ADPKD (80 males, 75 females; mean age 43.4 +/- 14.7 years

Congenital malformations of the ear and cochlear implantation in children: review and temporal bone report of common cavity.

The objective of this review is to analyze aspects of congenital malformation of the ear in relation to cochlear implantation in children. Having briefly described the in utero development of the ear and the classification of types of external, middle and inner ear malformation, five practical aspects of these malformations are discussed. It seems likely that the combination of bilateral profound sensorineural deafness with bilateral microtia severe enough to make a surgical approach to the cochlea difficult will be extremely uncommon. No such cases have been reported, although Klippel-Feil de

Imaging and clinical evaluation of isolated atresia of the oval window.

BACKGROUND AND PURPOSE: Congenital causes of hearing loss in children commonly are encountered, and imaging aids in diagnosis as well as presurgical evaluation. Atresia of the oval window not associated with atresia of the external auditory canal (EAC) is a rare cause of congenital hearing loss in children. We present the clinical and imaging findings in children with isolated oval-window atresia. METHODS: Atresia of the oval window was defined as the absence of the structure with the presence of a bony plate superimposed between the vestibule and middle ear. The bony plate is within the expe

[First implantation of a totally implantable electronic hearing aid in patients with inner ear hearing loss].

For the majority of patients with sensorineural hearing loss (SNHL) many available hearing aids often do not achieve satisfactory results. For these patients partially implantable hearing devices have been developed, allowing distortion-free hearing and speech intelligibility that may be superior to conventional hearing aids. The external parts of partial implants, however, may result in a patient's stigmatization. Furthermore, they do not use the acoustic properties of the external auditory canal. Recently, we published the successful development of the first totally implantable hearing devic

Cochlear implantation in the presence of chronic suppurative otitis media.

Nine patients are presented who underwent cochlear implantation in the presence of chronic suppurative otitis media. Four had a simple tympanic membrane perforation, four had a pre-existing mastoid cavity and one had cholesteatoma in the ear chosen for implantation. Patients with a simple perforation had a staged procedure with myringoplasty followed by cochlear implantation after an interval of three months. Patients with cholesteatoma or with an unstable mastoid cavity were also staged. A mastoidectomy or revision mastoidectomy was performed with obliteration of the middle ear and mastoid us

Earlens tympanic contact transducer: a new method of sound transduction to the human ear.

This article is an initial report on a new method of transducing sound to the human ear. The report describes the shortcomings of conventional acoustic amplification devices, the potential advantages afforded by the Earlens system (ReSound Corp., Redwood City, Calif.), a description of the system, and preliminary clinical results. The system is composed of two elements: a transducer that is composed of a magnet mounted on a thin, conical silicone platform and a device that generates a magnetic field which causes the Earlens to vibrate. The Earlens transducer is placed on the umbo area of the t

Osseointegration and growth effects of temporal bone percutaneous pedestals.

The percutaneous temporal bone pedestal has shown significant utility for the attachment of bone-anchored hearing aids, attachment of cosmetic auricular prostheses, and as connector between external sound processors and implanted cochlear implants. The biological acceptance of these implants by temporal bone hosts is affected by many factors including the maturity of the bone, the design of the pedestal-fixation system, and the pedestal-construction material. The first phase of this study evaluated the effect of the pyrolized graphite pedestal fixation on maturing temporal bones. Pedestals wer

Unilateral aural atresia in childhood: case selection and rehabilitation.

The classic management of children and adolescents with unilateral aural atresia, with or without microtia, is to assess audition, to exclude cholesteatoma formation both clinically and radiologically, to offer amplification in unilateral cases, and to consider surgical repair in bilateral cases. However, evidence indicates that children with unilateral hearing loss from any cause are at risk for delayed language development, attention deficit, and poor school performance. Favourable factors for reconstruction of hearing include a normal bone line with good word discrimination, a normal or rec

Distribution of Fos-like immunoreactivity in the auditory pathway of the Sprague-Dawley rat elicited by cochlear electrical stimulation.

Fos-like immunoreactivity (FLI) was mapped in the auditory pathway of Sprague-Dawley rats in response to unilateral electrical stimulation of the cochlea implanted with two stimulating electrodes. Densely packed FLI neurons were widely distributed in the dorsal cochlear nucleus (more ipsilaterally than contralaterally), while FLI neurons were rare in the posteroventral cochlear nucleus and virtually absent in the anteroventral cochlear nucleus. Sparse FLI was detected in the superior olivary complex, the pontine nuclei and the ipsilateral dorsal nucleus of the lateral lemniscus, whereas the co

Listener-assessed intelligibility of a hearing aid self-adaptive noise filter.

A principal complaint of hearing aid users is the inability to understand speech in the presence of competing background noise. Efforts at reducing the adverse effects of external noise on amplification systems have centered on manually adjusted or low frequency activated high-pass filtering; however, there has been recent interest in the potential of self-adaptive noise reduction systems. In this study we investigate the performance of normals and three groups of sensorineural loss subjects under five noise conditions with a self-adaptive noise filter incorporated in a conventional hearing ai
