Clinical Practice Guideline (Update): Earwax (Cerumen Impaction).

Objective This update of the 2008 American Academy of Otolaryngology-Head and Neck Surgery Foundation cerumen impaction clinical practice guideline provides evidence-based recommendations on managing cerumen impaction. Cerumen impaction is defined as an accumulation of cerumen that causes symptoms, prevents assessment of the ear, or both. Changes from the prior guideline include a consumer added to the development group; new evidence (3 guidelines, 5 systematic reviews, and 6 randomized controlled trials); enhanced information on patient education and counseling; a new algorithm to clarify act

Check-list for the assessment of functional impairment in children with congenital aural atresia.

OBJECTIVES: Congenital Aural Atresia (CAA) is a deformity of the external ear and it is commonly associated with malformations of middle and inner ear and, in some cases, with other facial deformities. Very few assessment measures exist for evaluating the functional impairment in children with CAA. Purpose of this study is to introduce and describe an assessment Checklist, (nominated FOS Checklist) that covers feeding abilities (F), oralmotor skills (O), communication/language development (S) in children with CAA. FOS wants to offer a range of assessment providing a profile of the child in com

Comprehension of texts by deaf elementary school students: The role of grammatical understanding.

BACKGROUND: The aim of this study was to analyze how the reading process of deaf Spanish elementary school students is affected both by those components that explain reading comprehension according to the Simple View of Reading model: decoding and linguistic comprehension (both lexical and grammatical) and by other variables that are external to the reading process: the type of assistive technology used, the age at which it is implanted or fitted, the participant's socioeconomic status and school stage. DESIGN: Forty-seven students aged between 6 and 13 years participated in the study; all pr

[Cholesteatoma of the external and middle ear in the childhood].

The objective of the present work was to enhance the effectiveness of diagnostics of cholesteatoma of the external and middle ear in the children. The study included 66 patients presenting with chronic suppurative otitis media and one child having cholesteatoma of the external auditory meatus. All the patients were examined with the use of otoendoscopy and CT of the temporal bones. It was shown that the frequent occurrence of acute suppurative otitis media, exudative suppurative otitis media, and adhesive otitis media is the risk factor of the development of cholesteatoma of the external and m

Sound transmission by cartilage conduction in ear with fibrotic aural atresia.

A hearing aid using cartilage conduction (CC) has been proposed as an alternative to bone conduction (BC) hearing aids. The transducer developed for this application is lightweight, requires a much smaller fixation force than a BC hearing aid, and is more convenient to use. CC can be of great benefit to patients with fibrotic aural atresia. Fibrotic tissue connected to the ossicles provides an additional pathway (termed fibrotic tissue pathway) for sound to reach the cochlea by means of CC. To address the function of fibrotic tissue pathway, BC and CC thresholds were measured in six ears with

Sophono Alpha System and subtotal petrosectomy with external auditory canal blind sac closure.

Recently, a new acoustic device, the so-called Sophono Alpha System, has been introduced into clinical practice. The aim of this study was to assess Sophono Alpha System hearing aids in ten patients suffering from recurrent chronic middle ear disease who underwent subtotal petrosectomy. Presence of mixed hearing loss with bone conduction thresholds better than or equal to 45 dB was present in each patient. Audiometric tests were performed before and after Sophono implantation and using a conventional bone conduction hearing aid (hearing glasses). Speech audiometry data (speech recognition thre

Air-bone gap component of inner-ear origin in audiograms of cochlear implant candidates.

BACKGROUND: Experimental studies have shown that creating a window in the bony cover of the cochlea and vestibular parts of the inner ear, with preservation of membranous and middle-ear functions, induces an air-bone gap (ABG). This study sought to determine if a similar mechanism explains the ABG frequently observed in audiograms of cochlear implant candidates. METHOD: The study group included 47 candidates for a cochlear implant (94 ears) attending a university-affiliated tertiary medical center who had an ABG component in the audiogram in the absence of external or middle-ear abnormalities

Békésy's contributions to our present understanding of sound conduction to the inner ear.

In our daily lives we hear airborne sounds that travel primarily through the external and middle ear to the cochlear sensory epithelium. We also hear sounds that travel to the cochlea via a second sound-conduction route, bone conduction. This second pathway is excited by vibrations of the head and body that result from substrate vibrations, direct application of vibrational stimuli to the head or body, or vibrations induced by airborne sound. The sensation of bone-conducted sound is affected by the presence of the external and middle ear, but is not completely dependent upon their function. Me

Otologic and audiology aspects of microtia repair.

Congenital abnormalities of the outer ear pose a reconstructive challenge for plastic surgeons and otologists. Many patients with microtia of the auricle have concurrent atresia of the external auditory canal. The hearing loss associated with canal atresia can have long-lasting effects if not treated promptly and appropriately. The diagnosis and workup for canal atresia requires an otologic evaluation. Audiologic and radiologic evaluations direct treatment, which varies depending on unilateral or bilateral presence of atresia. Rehabilitation of hearing loss can be performed via hearing aids, b

Computed tomographic analysis of outer calvarial thickness for osseointegrated bone-anchored hearing system insertion.

INTRODUCTION: Osseointegrated bone-anchored hearing systems (BAHSs) are a useful tool in auditory rehabilitation for single-sided deafness and conductive/mixed hearing loss. They rely on adequate osseointegration of titanium implants, which can be limited by calvarial thickness. This study examines adult computed tomographic (CT) temporal bone normative data for calvarial thickness in the areas commonly recommended for BAHS insertion and identifies hazards that may compromise the osseointegration process. METHODS: Prospective analysis of 100 consecutive adult helical CT scans. Calvarial thick

Speech perception for adult cochlear implant recipients in a realistic background noise: effectiveness of preprocessing strategies and external options for improving speech recognition in noise.

BACKGROUND: Although cochlear implant patients are achieving increasingly higher levels of performance, speech perception in noise continues to be problematic. The newest generations of implant speech processors are equipped with preprocessing and/or external accessories that are purported to improve listening in noise. Most speech perception measures in the clinical setting, however, do not provide a close approximation to real-world listening environments. PURPOSE: To assess speech perception for adult cochlear implant recipients in the presence of a realistic restaurant simulation generate

Active middle ear implants in patients undergoing subtotal petrosectomy: new application for the Vibrant Soundbridge device and its implication for lateral cranium base surgery.

OBJECTIVE: The functional outcome of ossiculoplasties in chronic ear and lateral cranium base surgery depends on the presence of a ventilated middle ear space and is guided by the existence or absence of ossicular remnants. In patients with poorly ventilated middle ears, after multiple previous operations, missing stapes suprastructure, or after partial temporal bone resection for tumor removal, restoration of conductive hearing is not possible. The direct placement of a vibrating floating mass transducer (FMT) onto the round window membrane with obliteration of the surgical cavity is a new op

Clinical practice guideline: cerumen impaction.

OBJECTIVE: This guideline provides evidence-based recommendations on managing cerumen impaction, defined as an accumulation of cerumen that causes symptoms, prevents assessment of the ear, or both. We recognize that the term "impaction" suggests that the ear canal is completely obstructed with cerumen and that our definition of cerumen impaction does not require a complete obstruction. However, cerumen impaction is the preferred term since it is consistently used in clinical practice and in the published literature to describe symptomatic cerumen or cerumen that prevents assessment of the ear.

Analysis of surfaces using constrained regression models.

We present a study of the relationship between the changes in the shape of the human ear due to jaw movement and acoustical feedback (AF) in hearing aids. In particular, we analyze the deformation field of the outer ear associated with the movement of the mandible (jaw bone) to understand its effect on AF and identify local regions that play a significant role. Our data contains ear impressions of 42 hearing aid users, in two different positions: open and closed mouth, and survey data including information about experienced discomfort due to AF. We use weighted support vector machines (WSVM) t

Partial withdrawal of deeply inserted cochlear electrodes: observations of two patients.

Three patients implanted in our department received the preformed Clarion S-Series cochlear implant with the electrode Positioning System (EPS). The EPS is a device designed to bring the electrode array closer to the modiolus and deeper into the cochlea. Two of these patients still complained because they were perceiving too low pitch sounds, and because of the presence of echoes and poor discrimination after 3 years of implant use and many tuning sessions. We hypothesized that the electrode array was too deeply inserted and could be stimulating overlapping populations of neurons in the low fr
