[Clinical significance and research status of small intestinal limb length in Roux-en-Y gastric bypass].

Roux-en-Y gastric bypass (RYGB) is an important bariatric and metabolic surgical procedure, and the design of intestinal limb lengths is a key factor affecting weight loss outcomes, metabolic improvements, and postoperative nutritional status. Currently, there are no established international standards for the optimal lengths of the biliopancreatic limb, alimentary limb, and common channel. This article reviews the evolutionary history of RYGB and its common classification, with a focus on analyzing the impact of the lengths of the alimentary limb, biliopancreatic limb, common channel, and tot

Outcomes Following Gastric Bypass Reversal for Refractory Complications: A UK Tertiary Centre Retrospective Case Series.

BACKGROUND: Gastric bypass is an effective and safe metabolic bariatric surgery, yet patients develop refractory long-term complications, post-bypass problems that persisted despite appropriate medical, endoscopic and/or surgical treatment, and that resulted in ongoing symptom burden and nutritional compromise, prompting consideration of reversal. Outcomes following reversal remain poorly characterised. The aim of this study is to describe indications, operative techniques and short- to medium-term nutritional and symptomatic outcomes following gastric bypass reversal in a UK tertiary bariatri

SADI-S Versus Distal OATB (SASI): Does the Duodenal Pathway Matter in Ileal-Level Single-Anastomosis Surgery?

BACKGROUND: Single anastomosis duodeno-ileal bypass with sleeve gastrectomy (SG) (SADI-S) and distal one anastomosis transit bipartition (dOATB) represent two distal single-anastomosis metabolic procedures sharing similar anatomical configurations but differing fundamentally in their approach to duodenal transit. Both combine SG with an omega-loop anastomosis to the distal ileum approximately 250-300 cm from the ileocecal valve. However, SADI-S positions the anastomosis on the duodenal bulb, achieving complete duodenal exclusion, while dOATB performs a prepyloric antral anastomosis, preserving

Application of Virtual Twin PBPK Models in Individuals with Obesity via CYP3A4 Phenotyping Using Endogenous Biomarker Data.

Plasma 4β-hydroxycholesterol (4β-OHC) normalized for the levels of its parent cholesterol (4β-OHC/C) is an endogenous biomarker for hepatic CYP3A4. This study evaluated CYP3A4 activity longitudinally in individuals while their obesity status was changing. 4β-OHC/C was measured pre-surgery (n = 54) and 2 years after Roux-en-Y gastric bypass (n = 30) and used as an input for creating virtual twins of each patient within physiologically-based pharmacokinetic (VT-PBPK) models, alongside other available data (e.g., demographics). Additionally, individual CYP3A4 abundance quantified from liver biops

Oral Fluconazole Absorption in a Complicated Post-Roux-en-Y Gastric Bypass Patient: A Case Report.

Introduction: Roux-en-Y gastric bypass (RYGB) is a surgical weight-loss procedure that combines gastric restriction with malabsorption for weight reduction. There is currently a paucity of data to guide pharmacists and healthcare providers in optimizing oral medication therapy in patients with a history of bariatric surgery. Fluconazole is a commonly used triazole antifungal with high oral bioavailability. The literature is limited regarding how RYGB affects oral fluconazole absorption. Case: We investigated fluconazole oral bioavailability in a patient with a history of RYGB status-post revis

Long-term home parenteral nutrition in chronic intestinal failure following metabolic and bariatric surgery and its clinical outcomes: A descriptive cohort study.

BACKGROUND: Metabolic and bariatric surgery is one of the most efficacious treatments for obesity. The increasing incidence of referrals to intestinal rehabilitation programs and the necessity for long-term parenteral nutrition because of bariatric surgery complications have gained attention, yet data remain limited. Our objectives were to (1) assess parenteral nutrition-related complication rates of patients requiring long-term parenteral nutrition because of bariatric surgery complications and (2) compare outcomes between types of metabolic and bariatric surgery. METHODS: This was a descrip

Possible familial predisposition in superior mesenteric artery syndrome with surgical management: A case report.

RATIONALE: Superior mesenteric artery (SMA) Syndrome is a rare gastrointestinal disorder caused by compression of the third part of the duodenum between the SMA and the aorta due to a reduced aortomesenteric angle. While most cases are sporadic, familial patterns suggest a possible genetic predisposition. This report presents a case of SMA syndrome in a 16-year-old male, with a familial occurrence, raising suspicion of possible genetic correlation. PATIENT CONCERNS: A 16-year-old male presented with chronic food intolerance, bilious vomiting, early satiety, and epigastric pain for over 4 year

[A Case of Ascending Colon Cancer with Duodenal Fistula Treated by Gastrojejunostomy Combined with"Separation Surgery of the Right-Sided Colon"].

An 80-year-old man presented with fever and vomiting. Abdominal CT and upper and lower gastrointestinal endoscopy revealed ascending colon cancer with duodenal invasion, resulting in the formation of a duodenocolic fistula. The patient's general condition was poor, with an Eastern Cooperative Oncology Group-performance status(ECOG-PS)of 3, rendering the patient unsuitable for radical resection and chemotherapy. However, the patient strongly desired to resume oral intake and was discharged from the hospital. As a palliative measure, gastrojejunostomy and"separation surgery of the right-sided co

Usefulness and description of the intestinal bypass technique in children with short bowel syndrome: report of a Mexican cohort.

BACKGROUND: Short bowel syndrome (SBS) is one of the most frequent causes of intestinal failure, needing parenteral nutrition to maintain an energy-protein and water-electrolyte balance. At the Hospital Infantil de México Federico Gómez (HIMFG), the formation of two stomas is a technique used for intestinal rehabilitation, where the use of residue through the bypass technique (BT) helps to maintain gastrointestinal functionality, water-electrolyte, and nutritional stability. This study aimed to describe the technique of using intestinal residue through BT as a treatment strategy in intestinal

Intestinal Calcium Absorption Decreases After Laparoscopic Sleeve Gastrectomy Despite Optimization of Vitamin D Status.

CONTEXT: Laparoscopic sleeve gastrectomy (LSG), now the most commonly performed bariatric operation, is a highly effective treatment for obesity. While Roux-en-Y gastric bypass is known to impair intestinal fractional calcium absorption (FCA) and negatively affect bone metabolism, LSG's effects on calcium homeostasis and bone health have not been well characterized. OBJECTIVE: We determined the effect of LSG on FCA, while maintaining robust 25-hydroxyvitamin D (25OHD) levels and recommended calcium intake. Individuals with type 2 diabetes have higher density of small intestinal neurotensin-expressing cells.

Neurotensin (NT) is a gastro-intestinal hormone involved in several pathways that regulate energy and glucose homeostasis. NT was hypothesized to act in synergy with incretin hormones to potentiate its anti-diabetic effects. Additionally, circulating NT levels were shown to rise after bariatric surgery-induced weight loss. Knowledge of NT-secreting cells distribution along the small intestine and its variation according to diabetes status could provide insights on NT role in mediating type 2 diabetes (T2D) improvement after bariatric surgery. So, our aims were to characterize NT-expressing cel

Serum Versus Fecal Calprotectin Levels in Patients with Severe Obesity Before and 6 Months After Roux-Y-Gastric Bypass: Report of the Prospective Leaky-Gut Study.

INTRODUCTION: Obesity is associated with low-grade inflammation, including intestinal inflammation based on fecal or serum calprotectin (FC-SC) measurement. Roux-en-Y gastric bypass (RYGB) improves obesity-related parameters. However, the association between FC-SC levels and postoperative course and the link with metabolic and inflammatory phenotypes before and after RYGB remains unclear. METHODS: We determined SC levels in 48 patients before (T0) and 6 months after (T6M) RYGB. We then analyzed postoperative changes in FC-SC levels and the relationship with inflammation and metabolic status.

Role of Proximal Intestinal Glucose Sensing and Metabolism in the Blood Glucose Control in Type 2 Diabetic Rats After Duodenal Jejunal Bypass Surgery.

BACKGROUND: Although gastric surgery can significantly improve blood glucose homeostasis in type 2 diabetes mellitus (T2DM), its mechanism remains unclear. This study evaluated the role of intestinal glucose sensing, glucose transport, and metabolism in the alimentary limb (A limb) of T2DM rats after duodenal jejunal bypass (DJB) surgery. METHODS: A T2DM rat model was induced via a high-glucose high-fat diet and low-dose streptozotocin injection. The diabetic rats were divided into two groups: the DJB surgery (T2DM-DJB) group and the sham surgery (T2DM-Sham) group. Wistar rats were used as wi

Risk of Reopening of the Mesenteric Defects After Routine Closure in Laparoscopic Roux-en-Y Gastric Bypass: a Single-Centre Experience.

PURPOSE: Small bowel obstruction (SBO) due to internal hernias (IH) is a well-recognised complication after laparoscopic Roux-en-Y gastric bypass (LRYGB). Routine closure of the mesenteric defects (MDs) is recommended to reduce the risk of IH and subsequent SBO. However, data about the rates of reopening of the MDs after LRYGB is scarce. The main aim of this study was to evaluate the risk of reopening of the MDs after routine closure during LRYGB. The secondary objective was to determine any risk factors associated with the reopening of the MDs. METHODS: Data of all patients who underwent reo
