Treatment of low-pressure hydrocephalus using a novel shunt valve technique: applying Bernoulli's equation to fluid statics. Technical note.

Low-pressure hydrocephalus (LPH) is the presence of persistent ventriculomegaly with low to normal intracranial pressure. Patients with LPH respond to subzero drainage, which consists of external ventricular drainage at levels below the external auditory meatus. Multiple treatment modalities have been described in the literature, but due to low intracranial pressures, weaning the external ventricular drain can take weeks to even months. This poses a relevant gap in the knowledge of treatment for LPH. The authors describe a new technique of placing a programmable differential pressure valve bel

Temporary Drainage of Cerebrospinal Fluid for Diagnosis and Treatment of Hydrocephalus.

Lumbar puncture is a frequently used procedure in the diagnosis and treatment of CSF disorders. Specialized tests such as the lumbar tap test and CSF infusion study are often used in the assessment of patients with possible idiopathic normal pressure hydrocephalus (iNPH), but their utility in this setting is questionable. The Extended Lumbar Drainage (ELD) test is a better test for ruling out possible shunt responsiveness in iNPH but still has limitations. External ventricular drainage is an important temporizing measure in acute hydrocephalus particularly in the presence of possible infection

Chitosan-based adhesive patch with anti-inflammatory, pro-healing, and immunomodulatory properties for dural defect repair.

Dural defects and subsequent cerebrospinal fluid (CSF) leakage are common complications in spinal and cranial surgeries. However, current clinical patches often fail to achieve long-term adhesion and effective sealing, as the presence of CSF can weaken interfacial bonding and ultimately result in adhesion failure. Herein, a new class of chitosan-based adhesive patch (PLLA/CP) is designed by constructing an adhesive hydrogel layer containing chitosan-dopamine (CS-DA) on the surface of fibrous poly(L-lactic acid) (PLLA) dissipative layer. Benefitting from electrostatic crosslinking within the hy

Haemoclip repair technique: A preliminary evaluation of a safe and practical method for dura mater closure in haemilaminectomy.

PURPOSE: In the surgical management of spinal tumours, haemilaminectomy is frequently the preferred approach due to its minimal invasive nature and the ability to preserve stability. However, the repair of the dura mater defect is technically very difficult and a challenging problem for neurosurgeons because of the high incidence of complications related to postoperative cerebrospinal fluid (CSF) leakage. This study presents a novel haemoclip-assisted technique that has been developed for the purpose of providing safe and rapid dura repair in spinal tumour surgery involving haemilaminectomy.

Clinical effect of radical debris evacuation with a flexible neuroendoscope on patients with ventriculitis: a case series.

BACKGROUND: Ventriculitis is a severe infectious disease affecting the entire ventricular system and is associated with high mortality and morbidity, particularly due to the subsequent development of hydrocephalus. Although most patients are treated with external ventricular drainage or ventricular irrigation, the impact of radical debris evacuation on clinical outcomes has not been well examined. CASE PRESENTATION: This study included 11 Japanese patients with ventriculitis treated at Tohoku University Hospital or Kohnan Hospital between January 2000 and December 2020. Of these, eight were m

Ventricular catheter tissue obstruction and shunt malfunction in 9 hydrocephalus etiologies.

OBJECTIVE: Hydrocephalus is a neurological disorder with an incidence of 80-125 per 100,000 births in the United States. The most common treatment, ventricular shunting, has a failure rate of up to 85% within 10 years of placement. The authors aimed to analyze the association between ventricular catheter (VC) tissue obstructions and shunt malfunction for each hydrocephalus etiology. METHODS: Patient information was collected from 5 hospitals and entered into a REDCap (Research Electronic Data Capture) database by hydrocephalus etiology. The hardware samples were fixed, and each VC tip drainag

An Unusual Case of Escherichia Coli Meningitis in an Immunocompetent Adult.

Spontaneous meningitis caused by Gram-negative bacilli is rare in adults. It typically occurs after a neurosurgical procedure or head injury but may also be related to the presence of a neurosurgical device, cerebrospinal fluid (CSF) leak syndrome, or seen in immunosuppressed patients. Escherichia coli (E. coli) is the leading cause of Gram-negative bacilli meningitis. We describe the case of a 47-year-old man who was hospitalized for spontaneous, community-acquired E. coli meningitis, which is unusual to see in an immunocompetent adult. CSF analysis was consistent with bacterial meningitis; h

Improving Surgical Outcomes in Middle Cranial Fossa Cerebrospinal Fluid Leak Repair.

BACKGROUND: Spontaneous cerebrospinal fluid (CSF) leaks of the temporal bone occur in the absence of known iatrogenic or traumatic injury and without underlying congenital malformation. Holistic treatment of the condition requires meticulous perioperative evaluation of the patient to mitigate underlying risk factors. Similarly, surgery for CSF leaks requires careful consideration of all potential routes of CSF leakage to improve short- and long-term surgical outcomes. OBJECTIVE: To describe our surgical technique in this article, which addresses most of the potential routes of spontaneous CSF

Safety and Feasibility of Steroid-Eluting Stent as a Bolster in Endoscopic Anterior Skull Base Reconstruction.

BACKGROUND: With a rising incidence of cerebrospinal fluid (CSF) leaks, endoscopic endonasal CSF leak repair is increasingly performed. Current approaches utilize a variety of materials including free mucosal grafts and vascularized flaps, but post-op leaks continue to be reported. Steroid-eluting bioabsorbable stents (SES) are used during functional endoscopic sinus surgery for chronic rhinosinusitis to reduce inflammation and scarring while maintaining patency of sinus ostia. OBJECTIVE: The aim of this study is to assess the feasibility of SES as a graft/flap bolster for endoscopic endonasa

Intra-operative identification of a temporal bone fracture line during cochlear implant surgery: a case report.

BACKGROUND: Temporal bone fractures are divided into otic capsule sparing and otic capsule involving fractures. In the latter, hearing loss, facial nerve paralysis, cerebrospinal fluid leak and meningitis have been reported to occur. The impact of hearing loss can be devastating, especially when occurring in children, with significant risk to speech development and sound localization. In the event of hearing loss, early rehabilitation is therefore of paramount importance. Identification of an intra-operative fracture line with available images and the outcome of such cases has not been reporte

Impact of Postural Changes on Sinonasal Pressure After Endoscopic Endonasal Surgery.

BACKGROUND: Postoperative management following endoscopic endonasal surgery (EES) is important to prevent cerebrospinal fluid leak and preserve the integrity of the nasoseptal flap. No consensus regarding an optimal posture in the postoperative period has been established. We hypothesized that sinonasal pressure (SNP) can represent intracranial pressure affecting the sellar floor in the absence of the sellar bone after surgery. This study provides evidence for the effect of postural changes and recommends optimal posture to reduce SNP following EES. METHODS: The authors conducted a retrospect

Prophylactic use of dural tenting sutures in elective craniotomies in adults-is it necessary? A study protocol for a multicentre, investigator- and participant-blinded randomised, parallel-group, non-inferiority trial.

BACKGROUND: In the early days of neurosurgery, extradural haemorrhages (EDHs) contributed to a high mortality rate after craniotomies. Almost a century ago, Walter Dandy reported dural tenting sutures as an effective way to prevent postoperative EDH. Over time, his technique gained in popularity and significance to finally become a neurosurgical standard. Yet, several retrospective reports and one prospective report have questioned the ongoing need for dural tenting sutures. Dandy's explanation that the haemostasis observed under hypotensive conditions is deceiving and eventually causes EDH ma

A multicenter retrospective study of heterogeneous tissue aggregates obstructing ventricular catheters explanted from patients with hydrocephalus.

BACKGROUND: Implantation of ventricular catheters (VCs) to drain cerebrospinal fluid (CSF) is a standard approach to treat hydrocephalus. VCs fail frequently due to tissue obstructing the lumen via the drainage holes. Mechanisms driving obstruction are poorly understood. This study aimed to characterize the histological features of VC obstructions and identify links to clinical factors. METHODS: 343 VCs with relevant clinical data were collected from five centers. Each hole on the VCs was classified by degree of tissue obstruction after macroscopic analysis. A subgroup of 54 samples was analy
