The effects of prenalterol, a selective beta 1-adrenoceptor agonist with potent cardiac positive inotropic properties have been investigated on regional myocardial blood flow (RMBF) (microspheres) and contractile function (ultrasonic crystals) during partial circumflex coronary artery stenosis in 8 open-chest anaesthetized dogs. Prenalterol was investigated at two intravenous doses: 5 micrograms kg-1, which increased myocardial contractility (dP/dt max: +29%) more than heart rate (+12%, up to 150 beats min-1) and 20 micrograms kg-1 which induced almost similar increases in contractility (+35%)

We studied 15 anesthetized dogs with 4-day-old anterior wall infarctions caused by previous ligation of the left anterior descending coronary artery. Left circumflex artery (LCA) stenosis induced an average 41% decrease in collateral-dependent flow to the surviving anterior wall (epicardium) and only a 10% decrease to the normal posterior epicardial wall. Ideal physiological form of pacing responds to body's needs for varying cardiac output indipendent of the requirement to synchronize with atrial activity. The atrial synchronous pacemakers are superior to the ventricular inhibited fixed rate pacing mode. Alternative methods of sensing physiological demands with single chamber ventricular pacing that produce a physiological rate response to exercise are described. System based on QT sensing and pacing system responsive to changes in respiratory rate have undergone successful clinical trials. Hemodynamics of rate responsive pacing are described.

Involvement of the heart is common in familial amyloidosis with polyneuropathy (FAP) and is manifested by disturbances of atrioventricular and intraventricular conduction and dysfunction of the sinus node. Fifty consecutive patients with metastatic carcinoma who underwent cryohypophysectomy were studied. Of these, 26 had breast cancer, 19 had prostatic cancer, one had malignant melanoma, one had cancer of the kidney, and three had metastatic adenocarcinoma from an unknown primary tumour. A case of multiple pyarthroses due to Staphylococcus aureus occurring in a severe classical hemophiliac is presented. Successful management depended on drainage of the infected joints and a prolonged course of antibiotics. Several criteria are suggested for recognition of hemophiliacs who might benefit from joint aspiration as a diagnostic tool for the presence of septic arthritis.

Eleven patients with bifascicular block complicating anteroseptal acute myocardial infarction were studied to determine the effect of prophylactic permanent pacing; eight of them also had transient high grade atrioventricular block during the acute phase of the infarction.
