Association Between the Modified Frailty Index-5 and Clinical Outcomes in Burn Patients.

Frailty is a clinical state characterized by increased vulnerability to stressors, leading to an increased risk of adverse health outcomes. The modified Frailty Index (mFI-5), based on 5 comorbidities-diabetes mellitus, heart failure, pulmonary disease, hypertension, and totally or partially dependent functional health status-has shown predictive value for treatment outcomes and mortality. The primary objective of this study was to evaluate differences in clinical outcomes between patients with burn injuries who have mFI-5 scores of 0, 1, and ≥ 2. The secondary objective was to determine wheth

A Game-Changing 20 Years: Progress and Future Directions in Lung Cancer Screening.

In the past two decades, lung cancer screening (LCS) with low-dose computed tomography (LDCT) has emerged as one of the most effective strategies for reducing lung cancer mortality. Landmark trials, including NLST and NELSON, demonstrated mortality reductions exceeding 20%, establishing LDCT as the standard of care for early detection in high-risk populations. Currently, 13 countries have implemented national or regional LCS programs, with additional nations preparing for rollout. Advances in risk-prediction models, volumetric nodule assessment, and structured management protocols have improve

Virtual myocardial blood flow and flow reserve from static PET imaging using artificial intelligence.

BACKGROUND: Quantitative myocardial blood flow (MBF) and myocardial flow reserve (MFR) provide incremental diagnostic and prognostic value in cardiac PET, but their widespread use is limited by the technical demands of dynamic imaging protocols. We evaluated the feasibility of using artificial intelligence (AI) to predict MBF and MFR from static and gated PET images, without the need for dynamic acquisition. METHODS: A machine learning (XGBoost) model was trained on 82Rb PET multi-center dataset using static perfusion imaging, injected dose, hemodynamic measures, clinical data and CT-derived

Baseline vagal tone and pain reactivity: linking autonomic function to endogenous pain inhibition in women.

INTRODUCTION: Conditioned pain modulation (CPM) is a laboratory test of the efficacy of endogenous pain inhibition. Although the vagus nerve is thought to contribute to analgesia via descending inhibitory pathways, its role in CPM remains unclear. In addition, as vagal tone fluctuates across the menstrual cycle due to hormonal changes, the menstrual cycle's influence on the vagal-CPM association warrants investigation. METHODS: Sixty-seven healthy females completed psychological questionnaires and underwent psychophysical assessments (1-minute tonic heat pain [THP] given alone and during CPM)

Mechanisms linking cytoplasmic decay of translation-defective mRNA to transcriptional adaptation.

Transcriptional adaptation (TA) is a genetic robustness mechanism through which mutant messenger RNA (mRNA) decay induces sequence-dependent up-regulation of so-called adapting genes. How cytoplasmically generated mRNA fragments affect nuclear transcription remains poorly understood. Using genome-wide CRISPR screens, we uncover ILF3 as an RNA binding protein connecting cytoplasmic mRNA decay and transcription during TA and show that it is required for a range of TA substrates. ILF3 is enriched at adapting genes' RNAs, and its artificial recruitment through dCas13 promotes gene expression. Usin

Survival After Left Ventricular Assist Device: A Society of Thoracic Surgeons National Intermacs Database Risk Model Assessment.

BACKGROUND: Several left ventricular assist device (LVAD) risk models exist; however, currently, only 1 LVAD is available. Therefore, we assessed prognostic accuracy of including data from other durable LVADs vs fully magnetic devices only in 3-year survival models. METHODS: All patients receiving primary continuous flow LVADs in The Society of Thoracic Surgeons National Intermacs (Interagency Registry for Mechanically Assisted Circulatory Support) Database (January 2013-April 2023) were included. Patients were stratified into 3 cohorts: cohort 1, fully magnetic devices only, 2018-2023 (n = 1

Heart failure induced by cancer therapies: focus on targeted agents, mechanisms, risk prediction, and clinical management.

Targeted therapies have revolutionized oncology but are accompanied by significant cardiovascular complications, with heart failure being a major dose-limiting toxicity. This review primarily focuses on heart failure induced by targeted anticancer agents, while also contextualizing findings with insights from classical chemotherapeutics and radiotherapy where they inform mechanistic understanding or combination regimen management. We detail the multifaceted pathophysiological mechanisms, which vary by drug class, including direct cardiomyocyte injury via HER2/ErbB signaling disruption, mitocho

Human metabolic heat production under varying atmospheric and oxygen pressures during submaximal cycling.

Accurate thermal management in hypoxic and altered-pressure environments is essential, as variations in ambient pressure and oxygen availability can modify human metabolic heat production. This study investigated human metabolic responses under seven atmospheric conditions (normobaric, hypobaric, and micro-hyperbaric) with varying oxygen concentrations. Nine healthy male participants performed submaximal cycling while minute ventilation, respiration rate, respiratory quotient, body temperature, heart rate, and blood lactate were measured, along with subjective thermal sensation and fatigue. Wearable Biosensing and Machine Learning for Data-Driven Training and Coaching Support.

Background: Artificial Intelligence (AI) and Machine Learning (ML) are increasingly integrated into sport and exercise through wearable biosensing systems that enable continuous monitoring and data-driven training adaptation. However, their practical value for coaching depends on the validity of biosensor data, the robustness of analytical models, and the conditions under which these systems have been empirically evaluated. Methods: A structured narrative review was conducted using Scopus, PubMed, Web of Science, and Google Scholar (2010-2026), synthesising empirical and applied evidence on we

Structure-Property-Function Evaluation of a β-Type Ti-Nb-Zr Alloy for Dental Implant Applications with Short-Term Clinical Validation.

Titanium-based alloys are widely used in dental implantology; however, the mechanical limitations of commercially pure titanium (cpTi) and unresolved concerns regarding stress shielding remain. This study evaluated the structure-property-function relationship of a novel β-type titanium-niobium-zirconium (Ti-Nb-Zr; TNZ) alloy for dental implant applications. Laboratory testing assessed the elemental composition, tensile properties, and fatigue resistance of the cpTi, compared with modified Grade 4 cpTi (MG4T). In parallel, a randomized, single-blind, controlled clinical trial was conducted over

Smart Devices and Multimodal Systems for Mental Health Monitoring: From Theory to Application.

Smart devices and multimodal biosignal systems, including electroencephalography (EEG/MEG), ECG-derived heart rate variability (HRV), and electromyography (EMG), increasingly supported by artificial intelligence (AI), are being explored to improve the assessment and longitudinal monitoring of mental health conditions. Despite rapid growth, the available evidence remains heterogeneous, and clinical translation is limited by variability in acquisition protocols, analytical pipelines, and validation quality. This systematic review synthesizes current applications, signal-processing approaches, an

Digital Medicine in the Management of Heart Failure: From Reactive Care to Predictive, Pathophysiology-Driven Strategies.

BACKGROUND: Heart failure (HF) is a progressive, multisystem syndrome characterized by recurrent decompensation, high hospitalization rates, and substantial mortality. Conventional HF management is mainly episodic and often fails to detect worsening conditions in advanced disease. Digital medicine and remote patient monitoring (RPM) hold promise for advancing HF care by enabling earlier detection, proactive action, and personalized care. METHODS: We conduct a narrative review to summarize evidence from randomized clinical trials, real-world registries, and emerging digital health technologies

Lipid metabolism drives dietary effects on T cell ferroptosis and immunity.

Ferroptosis, a major mechanism of non-apoptotic programmed cell death, critically regulates the homeostasis and functionality of peripheral CD4+ and CD8+ T cells1-6. Here we demonstrate that in mouse, resistance of T cells to ferroptosis depends critically on the composition of standard rodent diets, and that dietary effects on ferroptosis (DEFs) have a crucial role in regulation of T cell homeostasis and immune responses. DEFs are microbiota-independent and are driven by variations in dietary polyunsaturated and monounsaturated fatty acids (PUFAs and MUFAs) that lead to variations in abundanc

Stacked Ensemble Model With Explainable AI for Early Detection of Heart Disease.

Heart disease (HD) is still one of the most common causes of death around the world. Early detection is very important, but it is often hard to do because the symptoms are not specific and the models are not very clear. We propose a two-layer stacked ensemble that combines four base learners-Support Vector Machine, K-Nearest Neighbors, Naïve Bayes, and Decision Tree-with Logistic Regression as a meta-learner. This ensemble was trained on a balanced sample from five public HD datasets. The model outperforms standard baselines with 93.69% accuracy, 93% F1-score, and 94.7% AUC. To ensure that pre

Artificial Intelligence-Assisted Point-of-Care Ultrasound for Evaluating Left Ventricular Ejection Fraction: A Systematic Review of Prospective Observational Studies.

Artificial intelligence (AI) embedded in point-of-care ultrasound (POCUS) could reduce operator dependence in left ventricular ejection fraction (LVEF) assessment and accelerate bedside decision making by non-cardiologists and non-radiologists. Prospective comparative studies evaluating real-time AI-assisted POCUS for LVEF across point-of-care settings were systematically reviewed. The PubMed/MEDLINE, Embase, and Cochrane databases were searched from inception to June 11, 2025. The authors included prospective analytical observational studies in intensive care unit (ICU), emergency department,

Deep learning-based multi-view echocardiographic framework for comprehensive diagnosis of pericardial disease.

AIMS: Pericardial disease spans a wide spectrum from small effusions to life-threatening tamponade or constriction. Transthoracic echocardiography (TTE) is the main diagnostic tool, but its interpretation is limited by operator dependence and incomplete functional assessment. Existing deep learning (DL) models focus mainly on effusion detection, lacking broader evaluation. METHODS AND RESULTS: We developed a DL-based framework that performs sequential assessment of pericardial disease: (i) morphological features, including effusion amount (normal/small/moderate/large) and pericardial thickeni
