Signal or noise? Evaluating commonly used attribution methods for explaining deep neural networks in electrocardiogram classification.

AIMS: Attribution-based explainability methods are widely used in electrocardiogram (ECG) analysis to interpret predictions from 'black-box' deep neural networks (DNNs). To be useful in clinical applications, attribution methods must produce explanations that are both clear and reflective of the model's inner workings. This study evaluates 12 attribution methods in DNN-based ECG classification. METHODS AND RESULTS: We analysed 12 attribution methods using a dataset of 873 710 median beat ECGs spanning nine diagnostic classes. Methods were applied to convolutional neural network-based models t

Cerebrovascular CTA radiomics for objective collateral grading in acute ischemic stroke.

OBJECTIVE: Collateral circulation is a key determinant of functional outcome after large vessel occlusion (LVO) and informs thrombectomy decisions. However, collateral grading is rater-dependent and error-prone. We developed an automated cerebrovascular radiomics pipeline to establish objective collateral scoring on computed tomography angiography (CTA). MATERIALS AND METHODS: We retrospectively analyzed admission CTAs from 343 LVO patients in the MR CLEAN trial, split into training/validation (n = 274) and testing (n = 69) sets. Vessel regions of interest were segmented using nnU-Net models

MicroRNAs in Heart Failure Pathogenesis and Progression: Mechanistic Control, Biomarker Potential, and Translational Perspectives.

Heart failure (HF) remains a leading cause of morbidity and mortality worldwide and is driven by complex, interconnected pathophysiological processes, including maladaptive cardiac remodeling, fibrosis, hypertrophy, metabolic dysregulation, and cardiomyocyte loss. MicroRNAs (miRNAs), small non-coding RNAs that act as key post-transcriptional regulators of gene expression, have emerged as important coordinators of these processes across cardiomyocytes and non-myocyte cardiac cell populations. In addition to altered expression patterns, accumulating evidence indicates that miRNA activity is dyna

Electrocardiographic phenotypes of a representative subset of the French general population: ECGs at inclusion in the CONSTANCES cohort.

BACKGROUND: Large electrocardiogram (ECG) dataset analyses have emerged as potential game-changers in the field of personalized predictive medicine. ECG parameters have been described in cohorts of apparently healthy subjects and from primary care but seldom in community-based representative populations. AIMS: To describe the ECG phenotypes of a representative subset of the adult French general population. METHODS: ECGs recorded at inclusion in the CONSTANCES cohort were automatically analysed using the Glasgow diagnostic algorithm. Extreme values and abnormal statements were adjudicated to

An explainable artificial intelligence framework for ischemic heart disease prediction using enhanced squirrel search feature selection.

Clinical decision-making depends on the ability to anticipate ischemic heart disease (IHD) accurately and interpretably. Despite their excellent accuracy, machine learning models are not often accepted in medical applications due to their black-box nature. To diagnose IHD, this study suggests an Explainable Artificial Intelligence (XAI) architecture that combines explainable models with the Enhanced Squirrel Search Optimization (ESSO) method for feature selection. The proposed Enhanced Squirrel Search Optimization (ESSO) introduces adaptive exploration mechanisms to efficiently identify an opt

Intraoperative Transesophageal Echocardiography in Cardiac Surgery: Clinical Applications, Decision-Making, and Impact on Outcomes.

Intraoperative transesophageal echocardiography (TEE) has become a central component of modern cardiac surgery by providing real-time, high-resolution assessment of cardiac anatomy and physiology throughout the perioperative period. Beyond its traditional diagnostic role, intraoperative TEE directly influences surgical and anesthetic decision-making by enabling immediate detection of unexpected pathology, residual lesions, and hemodynamic instability. Accumulating evidence demonstrates that TEE-guided intraoperative assessment frequently leads to modification of surgical strategy or anesthetic

AI-Derived LA Volume Index, LA/RA and LA/LV Volume Ratios From Coronary Artery Calcium Scans Predict Long-Term Atrial Fibrillation and Stroke.

BACKGROUND: The AI-CVD initiative aims to extract actionable insights from coronary artery calcium (CAC) scans beyond the traditional CAC score. Although AI-derived cardiac chamber volumes predict atrial fibrillation (AF) and stroke, the long-term prognostic value of chamber ratios is less established. We evaluated the predictive value of AI-derived left atrial volume index and related chamber ratios (left atrial [LA]/right atrial [RA], LA/left ventricular) from routine CAC scans for incident AF and stroke, and their incremental value beyond established risk scores. METHODS: Pooled participan

Measuring the quality of AI-generated clinical notes: A systematic review and experimental benchmark of evaluation methods.

High-quality clinical documentation is essential for safe and effective care, yet its production remains time consuming and prone to error. Large language models (LLMs) have shown potential for supporting clinical note generation, but their clinical adoption depends on how the quality of generated text is assessed, and current evaluation practices vary widely. We systematically searched Ovid Medline and Scopus on 10 April 2025 for peer-reviewed studies that used LLMs to generate clinical notes and reported an evaluation of text quality. Screening followed PRISMA guidelines, and the protocol wa

Exhaled breath condensate based breath analyser: Real-time thermofluidic standardisation of hydrogen peroxide for longitudinal COPD monitoring: a pilot investigation.

Chronic obstructive pulmonary disease (COPD) remains a leading cause of morbidity and mortality worldwide, yet current monitoring strategies based on clinic spirometry and symptom reporting often fail to detect early exacerbations. Exhaled breath condensate (EBC) offers a non-invasive alternative; however, substantial variability in biomarker concentrations has limited its clinical translation. This study aimed to quantify longitudinal variability of EBC hydrogen peroxide (H2O2) in healthy volunteers and COPD participants and to determine whether real-time correction for physiological sampling

Beyond CABG vs. PCI: Contemporary and Future Coronary Revascularisation from Historical Evolution to Artificial Intelligence, Robotics, and Hybrid Strategies.

Coronary artery bypass grafting (CABG) and percutaneous coronary intervention (PCI) are the two dominant revascularisation strategies for obstructive coronary artery disease, yet their relative roles continue to shift because they address coronary pathophysiology differently with ever-evolving techniques. PCI has advanced through iterative improvements, including balloon angioplasty, bare-metal stents, and drug-eluting stents, with contemporary outcomes increasingly driven by procedural optimisation using intracoronary imaging and physiology-guided lesion selection rather than device category

Risk-Guided Atrial Fibrillation Screening With Artificial Intelligence-Enabled Electrocardiogram Models: A VITAL-AF Trial Analysis.

BACKGROUND: Screening for atrial fibrillation (AF) may lead to earlier detection and initiation of preventive measures. Current AF screening approaches using a guideline age-based threshold of ≥65 years have shown limited yield. OBJECTIVES: In an AF screening trial, we assessed whether the screening effect was larger among individuals at elevated AF risk using validated clinical and electrocardiogram (ECG)-based artificial intelligence (AI) risk models. METHODS: VITAL-AF was a cluster-randomized trial of patients aged ≥65 years treated at 1 of 16 primary care practices affiliated with Massac

MEDiT: A mask-enhanced diffusion transformer model for fetal heart rate signal generation.

Fetal Heart Rate (FHR) signals are widely used in fetal monitoring and maternal-fetal health assessment. However, their acquisition is constrained by ethical limitations, motion artifacts, and signal loss, resulting in limited data availability for intelligent clinical analysis. To address these, this paper proposes a deep generative Data Augmentation (DA) framework for FHR synthesis. Specifically, we propose a Mask-Enhanced Diffusion Transformer model (MEDiT) to model various missing data scenarios and generate high-fidelity FHR signals in data-scarce clinical settings. First, a mask-enhanced

Stylet-driven vs non-stylet-driven leads for conduction system pacing: Insights from the NCDR Electrophysiology Device Implant Registry.

BACKGROUND: Conduction system pacing (CSP), including His bundle pacing and left bundle branch area pacing, is a growing alternative to traditional right ventricular or biventricular pacing. The choice between stylet-driven leads (SDLs) and non-stylet-driven leads (NSDLs) remains operator-dependent, with limited comparative data. OBJECTIVE: The purpose of this study was to compare characteristics and outcomes of SDLs and NSDLs for CSP using data from the National Cardiovascular Data Registry. METHODS: We analyzed 11,412 CSP lead implants from April 2021 to December 2023 in the National Cardi

Immediate Postoperative Pacing Dependency Is Not Associated With Worse Mid-Term Survival in Redo Mitral Valve Surgery.

Risk factors (RF) for permanent pacemaker (PPM) placement in cardiac surgery include valvular operations and redo operations. We aim to identify RF for PPM placement prior to discharge (DC) specifically among redo mitral valvular operations, and to analyze outcomes in this population. Our retrospective review considered 943 patients undergoing redo MV operations between 01/2002 and 12/2021, excluding patients experiencing in-hospital death within 48 hours of their operation and patients with existing PPM or implanted cardiac defibrillator (ICD). Patients (n = 742) were categorized based on whe

Randomized Trial of Left Bundle Branch Pacing vs Right Ventricular Pacing in Vulnerable Cardiac Function.

BACKGROUND: Right ventricular pacing (RVP) is associated with an increased risk of pacing-induced cardiomyopathy (PICM) in patients with a high pacing burden. Left bundle branch pacing (LBBP), a more physiological pacing modality, may better preserve cardiac function. OBJECTIVES: This randomized trial aimed to evaluate the clinical outcomes of LBBP vs RVP in patients with a high pacing burden with high risk of cardiac dysfunction. METHODS: In this prospective, multicenter, randomized controlled trial, 160 patients with a high pacing burden with high risk of cardiac dysfunction were randomly
