Leveraging local knowledge for crisis management: a practice-based approach to managing uncertainty in healthcare during COVID-19.

BACKGROUND/AIM: Crises like the COVID-19 pandemic are inherently uncertain, dynamic and generate broader consequences on organisations, challenging traditional crisis management approaches. Conventional approaches often neglect the mechanisms and processes frontline practitioners enact in their local practices to adapt effectively. This study explores how healthcare professionals (HPs) at a university hospital centre developed and mobilised local knowledge to rapidly respond to the evolving conditions of the COVID-19 pandemic. METHODS: We conducted an interpretive single case study at a desig

Heparan Sulfate Oligosaccharides Modulate SARS-CoV-2 Spike-Host Recognition.

Heparan sulfate (HS) is a key host-factor mediator of SARS-CoV-2 attachment and an attractive target for antiviral intervention, yet the synthesis of structurally defined HS oligosaccharides remains challenging. Here, we show that a defined HS hexasaccharide, heparin-dp6, efficiently inhibits RBD-ACE2 binding and exhibits superior antiviral activity compared with longer HS mimetics. Electronic-structure analyses suggest that the strong recognition capacity of HS arises from sulfate and carboxylate groups, which create highly negative electrostatic surfaces and serve as dominant hydrogen-bond a

Antiviral Antibody Fc Dysfunction Is Linked to Increased IgG1 Fucosylation in Long COVID.

BACKGROUND: Persistent SARS-CoV-2 antigen is a leading hypothesis for post-acute sequelae of SARS-CoV-2 infection (PASC), commonly known as Long COVID. Impaired Fc-mediated antiviral antibody function could contribute to inefficient clearance of infected cells and viral antigen despite preserved antibody levels. Therefore, we tested whether PASC is associated with altered Fcγ receptor-mediated antibody function and differences in IgG1 Fc fucosylation, a major regulator of Fc effector function. METHODS: Serum samples from RECOVER-Adult participants with prior SARS-CoV-2 infection (99 PASC, 139

Identification of Human Nectin-4 and Signaling Lymphocyte Activation Molecule/SLAM as Potential Receptors for the Spike Protein of SARS-CoV-2 and its Variants.

INTRODUCTION: SARS-COV2 Spike protein is critical for virus entry as well as proinflammatory responses in host cells. The Spike epitopes form heteromeric complexes with HLA-DR alleles and Toll-like Receptors 2, 4, and 8. The Receptor Binding Domain of the Spike protein binds with Angiotensin Converting Enzyme2 (ACE2) as the first known host receptor. In this context, the aim is to identify a host cell surface protein as a potential receptor for the SARS-CoV-2 family that not only allows viral entry but also triggers inflammation. METHODS: Virus Spike Receptor Binding Domain (RBD) sequences, h

Bidirectional Synergistic Interaction Between Influenza Virus and SARS-CoV-2 in the Postpandemic Era: A Modeling Study.

INTRODUCTION: Influenza and COVID-19 are major respiratory diseases whose co-circulation dynamics are not fully understood. Surveillance data from the postpandemic period, free from the confounding effects of nonpharmaceutical interventions (NPIs), provide a unique opportunity to study their interactions. This study aimed to (1) estimate the direction and strength of the epidemiological interactions between influenza virus and SARS-CoV-2 and (2) quantify the impact of these interactions on the cumulative disease burden. METHODS: We developed a deterministic influenza virus × SARS-CoV-2 12-com

Berberine as a Multitarget Therapeutic Agent: Insights from Molecular Docking Studies.

Natural compounds are crucial in pharmacological research for modulating numerous biological processes. This is particularly helpful in the case of complex diseases and in overcoming challenges (e.g., drug resistance and adverse effects). Berberine, an isoquinoline alkaloid extracted from plants such as Berberis, Coptis, and Hydrastis species, exhibits a wide range of activities, including antidiabetic, anticancer, anti-inflammatory, antimicrobial, and antiviral. However, due to its low oral bioavailability (poor absorption, high first-pass metabolism, along with rapid elimination), further me

The impact of China's non-pharmaceutical interventions on economic growth during COVID-19: a provincial-level panel analysis.

This study examines the economic impacts of non-pharmaceutical interventions implemented in China during the COVID-19 pandemic, utilizing a provincial-level panel dataset from 2020Q1 to 2022Q4. We employ a pooled OLS model with panel-corrected standard errors (PCSE) to address cross-sectional dependence, serial correlation, and heteroskedasticity, while a fixed-effects instrumental variable (FE-IV) model is used to resolve endogeneity from reverse causality. The analysis reveals that NPIs significantly reduced quarterly GDP growth, with coefficients ranging from -0.078 to 0.154 across models.

Sustained type I interferon activity and N-antigenemia form a vicious cycle during severe COVID-19.

BACKGROUND: Although antigenemia is associated with poor outcomes in respiratory viral infections, the underlying mechanisms driving viral bloodstream dissemination remain not fully understood. Herein, we aimed to characterise SARS-CoV-2 nucleocapsid (N)-antigenemia kinetics in patients with severe COVID-19. We also sought to identify host and viral determinants associated with viral dissemination through a multiparametric integrated analysis and assess the associated risk of death. METHODS: This observational study included hospitalised patients with severe COVID-19 across three independent

Assessing the FAIRness of survey data collected during the COVID-19 pandemic in Germany.

This study investigates the FAIRness (Findability, Accessibility, Interoperability, and Reusability) of survey data collected during the COVID-19 pandemic in Germany. During the pandemic, surveys played a crucial role in capturing public attitudes and health-related behaviors, but researchers often faced a trade-off between rapid data collection and implementing high-quality survey designs. Using metadata from the "Survey Data Collection during the COVID-19 Pandemic" (SDCCP) project, which documents 717 surveys conducted in Germany, we assessed the FAIRness of surveys published with a persiste

Long COVID in Primary Care: A Practical Approach to Evaluation and Management.

Long COVID is a multi-system, heterogeneous condition that manifests as new or worsened symptoms lasting at least 3 months following acute SARS-CoV-2 infection, often despite normal results on routine diagnostic testing. Common manifestations include fatigue, post-exertional malaise, cognitive impairment, and orthostatic intolerance. Primary care clinicians are uniquely positioned to identify and manage Long COVID due to their longitudinal relationships with patients and holistic understanding of their patients' health. However, diagnostic uncertainty, time constraints, and limited access to s

Leukocyte cell population data predict 30-day mortality in community-acquired pneumonia: A multicenter prospective study.

Early identification of patients with community-acquired pneumonia (CAP) at increased risk of death remains essential, but rapid and low-cost prognostic biomarkers are still needed. Cell population data (CPD), automatically generated during routine complete blood count (CBC) testing, reflect leukocyte morphology and functional activation. This study aimed to assess whether CPD parameters predict 30-day mortality in hospitalized patients with CAP. This multicenter, observational, prospective study included consecutive adults admitted with CAP to three teaching hospitals in northern Spain betwee

Genomic epidemiology of Omicron (BA.1) as a driver of Pakistan's fifth SARS-CoV-2 wave.

The emergence of the SARS-CoV-2 Omicron variant led to Pakistan's Omicron wave from December 2021 to March 2022. This study investigated the molecular epidemiology of the early Omicron wave in Pakistan through genomic surveillance and travel history-informed Bayesian phylogeographic inference. PCR-based genotyping and whole-genome sequencing shows a shift from the BA.1 lineage's early replacement of Delta as Pakistan's dominant lineage in December 2021, until it was replaced by BA.2 predominance by March 2022. We identify rare spike protein mutations and the absence of key mutations in circula
