SARS-CoV-2 seroprevalence and extent of infection under-ascertainment in seven regions of Uzbekistan: a population-based serosurvey, May-July 2022.

Accurate estimation of SARS-CoV-2 transmission was limited in many settings by incomplete diagnostic testing and case reporting. We conducted a population-based cross-sectional serosurvey in seven regions of Uzbekistan between May and July 2022 to estimate SARS-CoV-2 seroprevalence, describe self-reported vaccination coverage, and assess the implications for routine surveillance. A two-stage cluster sampling design was used to randomly select polyclinic catchment areas and registered residents aged ≥5 years. Survey weights accounted for selection probabilities, non-response, and regional popul

Hyperinflammation and immunomodulation in influenza, SARS-CoV-2 and syncytial respiratory virus infection in adults: from innate immune responses and biomarkers to clinical and therapeutic implications.

Influenza, SARS-CoV2-2 and syncytial respiratory virus infection in adults are major causes of morbidity and mortality. However, the pathophysiological mechanisms underlying these infections remain incompletely understood. Disease severity results from an interaction between viral factors and host responses. Lung damage and respiratory failure are produced by hyperinflammation. Understanding these mechanisms is necessary to improve risk stratification and may guide therapeutic decisions. C- reactive protein (CRP), IL-6, ferritin, and ApoH are relevant biomarkers which can identify inflammatory

Analysis of Virologic and Clinical Predictors for Hospitalization, Supported by Machine Learning, in a Phase 3 Molnupiravir COVID-19 Treatment Trial.

Machine learning (ML) is increasingly being used in clinical medicine. The objective of this study was to identify factors predictive of hospitalization, with the support of ML, in outpatients with mild-to-moderate COVID-19. Unvaccinated adults with mild-to-moderate COVID-19 were randomized 1:1 to molnupiravir or placebo in MOVe-OUT (NCT04575597). In this post hoc analysis, various factors were evaluated among participants who progressed or did not progress to hospitalization. ML was used as an unbiased method to evaluate multiple factors for their association with hospitalization in placebo-t

Variant escape site mapping and characterization of SARS-CoV-2 nucleocapsid rapid antigen test antibodies.

Rapid antigen tests (RATs) were a critical tool for diagnosing severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection during the height of the pandemic, but their performance depends on antibodies that remain robust to viral evolution. Since most commercial RATs target the viral nucleocapsid protein, mutations in this domain can compromise binding and test performance. To systematically define this risk, we used deep mutational scanning (DMS) to map escape mutations for 82 antibodies across 43 commercially available tests, generating the most comprehensive escape database of SA

Genomic surveillance of COVID-19 in Hong Kong (2020-2023): Analysing SARS-CoV-2 variants, mutations, and their impact on viral entry.

We analysed 15,982 quality-controlled SARS-CoV-2 genome sequences from Hong Kong, obtained from NCBI and GISAID between 2020 and 2023, to characterize lineage dynamics, mutational profiles, demographic distributions, and variant-specific viral entry. Genomic surveillance identified 543 lineages and 5,982 unique mutations. Lineages A, B, and B.4 dominated the initial epidemic waves, whereas Delta and Omicron emerged and predominated in later waves. Omicron BA.1 and BA.2 exhibited distinct distributions across gender and age groups, while JN.1 harboured several novel mutations. Molecular simulat

SARS-CoV-2 nucleocapsid protein hijacks host transcription by targeting eEF1A1.

SARS-CoV-2 variants display substantial genetic and phenotypic diversity, yet the molecular mechanisms underlying their effects on host gene regulation need further investigation. We performed a transcriptomic analysis of host responses to multiple SARS-CoV-2 variants, including Delta and Omicron. Despite their divergence, all variants induced a conserved transcriptional program in human lung epithelial cells characterized by activation of interferon regulatory factors (IRFs), immediate-early genes, and downstream cytokines. We identify the viral nucleocapsid (N) protein as a central driver of

Severity-dependent and sex-biased dysregulation of A-to-I RNA editing in COVID-19 reveals CTSS as a key regulatory target.

SARS-CoV-2 infection causes diverse clinical outcomes, with men facing higher risks of severe disease. Adenosine-to-inosine (A-to-I) RNA editing mediated by ADAR enzymes governs innate antiviral immunity, yet its contribution to COVID-19 severity and sex-biased disease remains unclear. We performed epitranscriptomic profiling on 126 blood samples from non-COVID controls and COVID-19 patients spanning different disease severities. We detected 1,466 differentially RNA-edited sites in 514 genes linked to clinical outcomes. Although ADAR expression increased in COVID-19 patients overall, it was st

Higher risk of Long COVID among patients with autoimmune rheumatic diseases and immunomodulatory therapy: a retrospective cohort analysis of national US data.

BACKGROUND: Little is known about the prevalence of and factors associated with Long COVID in patients with autoimmune rheumatic diseases (AIRDs). METHODS: We conducted a retrospective cohort study using data from the National Clinical Cohort Collaborative (N3C). We included adults with a first documented SARS-CoV-2 infection between September 2021 and December 2023, with follow-up through December 2024. Long COVID was defined as ICD-10-CM code U09.9 or a Long COVID clinic visit occurring 28 to 365 days after first infection. We compared patients with AIRD with matched non-AIRD controls using

The Association between Vitiligo and Infections: A Systematic Review.

Vitiligo is an acquired depigmentation disease with an unknown etiology. Several theories on its etiology have been proposed, including autoimmune mechanisms, oxidative stress, genetic factors, environmental factors, and lack of melanocyte growth factor. Recent studies have demonstrated a possible correlation between microbial infections and the development or consequences of vitiligo. This study aims to investigate the potential association between microbial infections and vitiligo, focusing on identifying any explainable correlations between these two. A systematic search was performed on al

Changes in birth certificate-reported prevalence of congenital anomalies at live birth during the COVID-19 pandemic: a nationwide study of 26 million United States live births.

BACKGROUND: Although the majority of studies have indicated that maternal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and coronavirus disease 2019 (COVID-19) vaccination are not associated with neonatal birth defects, it is relevant to examine whether the disease burden changed during the COVID-19 pandemic, as other influencing factors may have also changed. We aimed to compare the birth certificate-reported prevalence of congenital anomalies at live birth before and during the pandemic. METHODS: Using public birth data, all live births from 2016 to 2022 in the Unit

In vivo antiviral efficacy and safety of 5-hydroxymethyltubercidin against DENV2 and SARS-CoV-2.

Several newly emerging and re-emerging viral infections significantly affect human health and the global economy each year. As many of these viruses lack treatment options other than symptomatic management, the development of effective preventive and therapeutic measures is urgently needed. We previously identified 5-hydroxymethyltubercidin (HMTU) as a broad-spectrum antiviral ribonucleoside analog and demonstrated its in vitro potent antiviral activity against positive-sense single-stranded RNA viruses, including dengue virus (DENV) and severe acute respiratory syndrome coronavirus 2 (SARS-Co

Heparan Sulfate Oligosaccharides Modulate SARS-CoV-2 Spike-Host Recognition.

Heparan sulfate (HS) is a key host-factor mediator of SARS-CoV-2 attachment and an attractive target for antiviral intervention, yet the synthesis of structurally defined HS oligosaccharides remains challenging. Here, we show that a defined HS hexasaccharide, heparin-dp6, efficiently inhibits RBD-ACE2 binding and exhibits superior antiviral activity compared with longer HS mimetics. Electronic-structure analyses suggest that the strong recognition capacity of HS arises from sulfate and carboxylate groups, which create highly negative electrostatic surfaces and serve as dominant hydrogen-bond a

Antiviral Antibody Fc Dysfunction Is Linked to Increased IgG1 Fucosylation in Long COVID.

BACKGROUND: Persistent SARS-CoV-2 antigen is a leading hypothesis for post-acute sequelae of SARS-CoV-2 infection (PASC), commonly known as Long COVID. Impaired Fc-mediated antiviral antibody function could contribute to inefficient clearance of infected cells and viral antigen despite preserved antibody levels. Therefore, we tested whether PASC is associated with altered Fcγ receptor-mediated antibody function and differences in IgG1 Fc fucosylation, a major regulator of Fc effector function. METHODS: Serum samples from RECOVER-Adult participants with prior SARS-CoV-2 infection (99 PASC, 139

Identification of Human Nectin-4 and Signaling Lymphocyte Activation Molecule/SLAM as Potential Receptors for the Spike Protein of SARS-CoV-2 and its Variants.

INTRODUCTION: SARS-COV2 Spike protein is critical for virus entry as well as proinflammatory responses in host cells. The Spike epitopes form heteromeric complexes with HLA-DR alleles and Toll-like Receptors 2, 4, and 8. The Receptor Binding Domain of the Spike protein binds with Angiotensin Converting Enzyme2 (ACE2) as the first known host receptor. In this context, the aim is to identify a host cell surface protein as a potential receptor for the SARS-CoV-2 family that not only allows viral entry but also triggers inflammation. METHODS: Virus Spike Receptor Binding Domain (RBD) sequences, h

Bidirectional Synergistic Interaction Between Influenza Virus and SARS-CoV-2 in the Postpandemic Era: A Modeling Study.

INTRODUCTION: Influenza and COVID-19 are major respiratory diseases whose co-circulation dynamics are not fully understood. Surveillance data from the postpandemic period, free from the confounding effects of nonpharmaceutical interventions (NPIs), provide a unique opportunity to study their interactions. This study aimed to (1) estimate the direction and strength of the epidemiological interactions between influenza virus and SARS-CoV-2 and (2) quantify the impact of these interactions on the cumulative disease burden. METHODS: We developed a deterministic influenza virus × SARS-CoV-2 12-com

Berberine as a Multitarget Therapeutic Agent: Insights from Molecular Docking Studies.

Natural compounds are crucial in pharmacological research for modulating numerous biological processes. This is particularly helpful in the case of complex diseases and in overcoming challenges (e.g., drug resistance and adverse effects). Berberine, an isoquinoline alkaloid extracted from plants such as Berberis, Coptis, and Hydrastis species, exhibits a wide range of activities, including antidiabetic, anticancer, anti-inflammatory, antimicrobial, and antiviral. However, due to its low oral bioavailability (poor absorption, high first-pass metabolism, along with rapid elimination), further me

Effectiveness of Remdesivir in individuals with confirmed Omicron variant: a meta-analysis of cohort studies.

INTRODUCTION: The emergence of the Omicron variant (B.1.1.529) of SARS-CoV-2 represents a new challenge due to its high transmissibility and potential resistance to existing treatments. While Remdesivir, an FDA-approved antiviral, has been widely used in managing COVID-19 cases, its efficacy against the Omicron variant remains uncertain and warrants further investigation. This systematic review and meta-analysis evaluates the effectiveness of remdesivir in individuals infected with the Omicron variant. METHOD: A systematic search of MEDLINE (PubMed), Embase, Cochrane Library, Google Scholar,

The impact of China's non-pharmaceutical interventions on economic growth during COVID-19: a provincial-level panel analysis.

This study examines the economic impacts of non-pharmaceutical interventions implemented in China during the COVID-19 pandemic, utilizing a provincial-level panel dataset from 2020Q1 to 2022Q4. We employ a pooled OLS model with panel-corrected standard errors (PCSE) to address cross-sectional dependence, serial correlation, and heteroskedasticity, while a fixed-effects instrumental variable (FE-IV) model is used to resolve endogeneity from reverse causality. The analysis reveals that NPIs significantly reduced quarterly GDP growth, with coefficients ranging from -0.078 to 0.154 across models.
