Lymphocyte loss and plasmacytosis are associated with IL-6- and TNF-producing cells in the spleens of fatal COVID-19 cases.

BACKGROUND: The spleen undergoes changes during acute and chronic infections, which may contribute to immune dysregulation and disease aggravation. In fatal cases of COVID-19, pronounced splenic changes are noted. However, the role played by these alterations in patient mortality remains poorly understood. Objectives: We aim to characterize structural alterations and changes in splenic cell populations in fatal COVID-19 cases, as a potential substrate for immune dysfunction associated with bacterial coinfection and mortality in severe infectious diseases. METHODS: In this study, we characteri

Diagnostic performance of upper airway sampling sites for SARS-CoV-2 and influenza testing.

UNLABELLED: Early diagnosis of upper respiratory infections is essential to control infectious disease transmission within the community and to initiate relevant antiviral treatments. Nonetheless, variable sensitivities of sampling sites are often overlooked. We conducted a clinical trial at a COVID-19 outpatient test center, where healthcare workers collected nasopharyngeal, throat, nasal swabs, and saliva specimens. Each specimen was tested by RT-PCR for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), influenza virus A and B, and respiratory syncytial virus (RSV). Clinical info

Depression and anxiety mediate the relationship between COVID-19 stay-at-home orders and tobacco and marijuana use.

BACKGROUND: The COVID-19 pandemic had notable impacts on the mental health of the U.S. population. There were concerns about how the pandemic affected substance use in the population. The overall objective of this study was to assess whether COVID-19 Stay-At-Home (SAH) orders, an ambient ecological stressor, as well as the severity of depressive and anxious symptoms, can explain tobacco and marijuana use. METHODS: Data come from the first seven waves of the Understanding America Study, a nationally representative longitudinal web-based panel study. A total of 7,554 persons participated in the

Fatigue in Post-COVID-Condition is accompanied by hypoperfusion of right-occipital areas.

BACKGROUND AND PURPOSE: A proportion of individuals recovering from COVID-19 continue to experience persistent symptoms, including fatigue and cognitive difficulties - a syndrome commonly referred to as Post-COVID condition (PCC), which affects an estimated 2-10% of cases. In this study, we evaluated cerebral blood flow (CBF) to better understand the pathophysiological mechanisms underlying PCC. MATERIALS AND METHODS: In this prospective, monocentric study, we analyzed clinical and cerebral blood flow (CBF) data from a cohort of 55 patients who met the WHO diagnostic criteria for Post-COVID c

Clinical Symptom Patterns as Predictors of SARS-CoV-2 Infection in Healthcare Workers in Puerto Rico.

Coronavirus disease 2019 (COVID-19), caused by SARS-CoV-2, has posed major risks for healthcare workers (HCWs) worldwide. This study assessed the prevalence of infection and its relationship with demographic and clinical characteristics among HCWs at the University of Puerto Rico Adult Hospital. A total of 132 individuals were enrolled, of whom six tested positive (4.55%). The study population was predominantly female (78.8%) with a mean age of 41 years, and although men showed higher odds of infection (OR = 3.98), the difference was not significant. Symptom presence was strongly associated wi

Colorimetric tetra-combo loop-mediated isothermal amplification assay for efficient SARS-CoV-2 detection.

Coronavirus disease 2019 (COVID-19), which is caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has spread rapidly and caused a pandemic in 2020. Both pharmaceutical approaches, such as vaccinations and non-pharmaceutical approaches such as social distancing through lockdown and international border restrictions had been implemented to control the outbreak and the transmission of SARS-CoV-2. However, the prolonged implementation of these measures negatively impacted the population and global economy, while the continuously emerging SARS-CoV-2 variants led to breakthrough i

Livedoid Vasculopathy in a Female Patient With a History of COVID-19 Infection During Pregnancy: A Case Report.

Livedoid vasculopathy is a rare, chronic thrombo-occlusive disease characterized by recurrent livedoid skin changes, atrophic white plaques, and ulceration. It most commonly affects the dermal vessels of the distal lower extremities, ankles, and feet, bilaterally. While it is commonly associated with hypercoagulable states, systemic autoimmune diseases, and malignancies, we report a case of a previously healthy 31-year-old female with a history of moderate COVID-19 infection during pregnancy, two years before the onset of livedoid skin changes. The patient presented to a hematologist with a tw

A Proposed Scale to Assess Magnesium Status Using Serum Calcium and Magnesium Ratios.

Background/Objectives: Reliable markers of human magnesium (Mg) status are needed. Methods: Current Mg studies report ratios between serum Mg and calcium (Ca) using four interchangeable expressions (i.e., molar or weight calculations of Mg/Ca or Ca/Mg). We propose a scale using ratios of serum Mg and Ca to assess Mg status, unified for all four expressions. We explore its application for case studies and published research. Results:Case Studies (4)-the proposed serum Mg/Ca scale showed better Mg diagnostic value than serum Mg alone. Published Studies-A. The proposed Mg/Ca scale's "depleted Mg"

Generalized morphea profunda following COVID-19 messenger ribonucleic acid vaccination.

Morphea is an uncommon inflammatory disorder characterized by progressive sclerosis of the skin and soft tissues. We describe the novel occurrence of generalized morphea profunda arising in close temporal association with the COVID-19 messenger ribonucleic acid vaccination series. An 80-year-old man presented with numerous areas of firm, tight skin across his trunk and extremities with associated itching and burning. He denied characteristic symptoms of systemic sclerosis. Physical examination revealed numerous indurated plaques, some with porcelain-white or sclerotic yellow centers, on the bi

Evaluating Google Trends as a proxy for symptom incidence: insights from the winter COVID-19 infection study in England 2023/24.

Google Trends is used in research and surveillance as a proxy for community infection incidence. Signals are difficult to validate, as most surveillance biases towards severe outcomes and certain demographics.Using Winter COVID-19 Infection Study (WCIS) data in England, symptom prevalence is estimated via generalized additive model with multilevel-regression and poststratification. Symptom duration was estimated using interval censored time delay modelling, converting prevalence to incidence. Google Trends and WCIS incidence and growth rates were compared using cross-correlation.Google Trends

Immunotherapies for postural orthostatic tachycardia syndrome, other common autonomic disorders, and Long COVID: current state and future direction.

Postural orthostatic tachycardia syndrome (POTS), neurocardiogenic syncope, and orthostatic hypotension are the most common autonomic disorders encountered in clinical practice. The autoimmune etiology and association of these conditions with systemic autoimmune and inflammatory disorders, autonomic neuropathy, and post-acute infectious syndromes, including Long COVID, suggest that immunotherapies should be considered as a therapeutic option, at least in a subset of patients. However, the treatment of common autonomic disorders has traditionally included pharmacologic and non-pharmacologic sym

Presenteeism Among Health Care Personnel With COVID-19.

IMPORTANCE: Presenteeism-defined as continuing to work during an illness-poses a public health risk in the workplace and is especially hazardous within health care institutions where vulnerable patients may be exposed to nosocomial infections. Understanding the frequency and characteristics of health care personnel (HCP) who report presenteeism while ill with COVID-19 may help mitigate SARS-CoV-2 spread in hospitals and other health care institutions. OBJECTIVES: To determine the frequency of presenteeism among HCP with symptomatic COVID-19, and to evaluate the demographic, occupational, and

Pulmonary embolism in a post-COVID-19 patient: a critical diagnostic challenge.

The most severe clinically recognized complication of venous thromboembolism (VTE), pulmonary embolism (PE), can be difficult to diagnose due to its nonspecific symptoms. The overlapping clinical symptoms of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection that causes Coronavirus 2019 (COVID-19) and PE can make it difficult to differentiate between one and the other. Therefore, PE diagnosis can be delayed or missed in patients with COVID-19, resulting in critical consequences for patient safety and outcomes. In situ pulmonary thrombosis and pulmonary embolus are distinct thrombotic phenotypes in critically ill patients with COVID-19 Acute Respiratory Distress Syndrome.

BACKGROUND: Pulmonary embolism (PE) is thought to originate from distal thrombosis. However, in hyperinflammatory conditions, such as COVID-19, thrombosis in the lung vasculature may occur independently of peripheral thrombosis, denoted as in situ thrombosis (IST). OBJECTIVES: We hypothesize that IST results from a dysregulated immune response involving both T-helper type 1 (Th1) and non-Th1 cell upregulation and can be distinguished from PE by histologic, serologic, and radiologic features. METHODS: This study included critically ill patients who succumbed to COVID-19 and from whom pulmonar

Expanding the clinical spectrum of Cernunnos/XLF deficiency: a literature review of a rare cause of severe combined immunodeficiency including a novel case.

BACKGROUND: Severe combined immunodeficiency (SCID) is a congenital immunodeficiency characterized by significant numerical or functional defects in T lymphocytes and is often accompanied by B lymphocyte dysfunction. It presents early in life with severe, recurrent opportunistic infections. Early diagnosis and hematopoietic stem cell transplantation (HSCT) are vital for patient survival. Cernunnos/XLF deficiency is an autosomal recessive form of SCID caused by mutations in the NHEJ1 gene, which plays a critical role in repairing DNA double-strand breaks. First described in 2006, this condition

Prevalence and trajectories of post-COVID-19 neuromuscular conditions: A systematic-review and meta-analysis.

INTRODUCTION: Neuromuscular diseases (NMDs) are a significant component of the post-acute sequelae of COVID-19. However, their long-term prevalence and trajectories remain poorly defined. This systematic review and meta-analysis aimed to determine the long-term prevalence in COVID-19 survivors of fourteen specific NMDs and related symptoms: cranial nerve diseases, Guillain-Barré syndrome, small fiber neuropathy, (poly)radiculopathies, (poly)neuropathies, plexopathies, motor neuron disease, myasthenia gravis, Lambert-Eaton syndrome, neuropathic pain, sarcopenia, myalgia, myalgia associated with
