Viral Respiratory Infections: COVID-19.

On March 11, 2020, the World Health Organization declared COVID-19 a pandemic. By late 2024, COVID-19 had become endemic, with regional surges caused by new variants and declining population immunity. Case fatality rates have decreased because of vaccination and the emergence of less virulent variants, but long COVID persists as a public health concern. Because of structural issues, including those related to employment, housing, health care access, and mistrust, minority and low-income individuals are at higher risk for COVID-19 infection and mortality. Preventive strategies include shared de

Facilitators and barriers to decentralized Long COVID platform clinical trials using repurposed drugs.

Long COVID is an emerging chronic condition that results in substantial impairments in quality of life, physical function, and ability to maintain gainful employment. There are no FDA-approved treatments for Long COVID, and recent institution-based clinical trials have not yielded effective treatments. Current standard of care involves trial-and-error drug repurposing that warrants evaluation in rigorous clinical trials. Here we describe potential advantages of decentralized repurposing drug trials as an alternative or complementary approach to institution-based trials. Additionally, we descri

Pentosan polysulfate sodium limits pulmonary inflammation in a SARS-CoV-2 mouse model.

UNLABELLED: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection remains prevalent globally, causing severe pulmonary inflammation and mortality in up to 10% of patients. While the introduction of effective vaccines has substantially reduced the number of severe coronavirus disease 2019 (COVID-19) cases, there are currently few available therapeutics that have been developed. Pentosan polysulfate sodium (PPS) has been shown to possess potent anti-inflammatory activity in influenza infection and allergen-induced rhinitis models and was recently demonstrated to inhibit SARS-CoV

Large-scale prospective evaluation of co-folding across hundreds of Mac1-ligand complexes and three virtual screens.

Accurate prediction of ligand-bound protein complexes and ranking them by affinity are central problems in drug discovery. While deep-learning co-folding methods can help address these challenges, their evaluation has been hampered by the difficulties in assessing independence from training data and insufficiently large test sets. Here, we test the ability of co-folding methods to predict the structures of 551 ligands, 489 of which are of sufficient quality for evaluation, bound to the SARS-CoV-2 NSP3 macrodomain (Mac1) that were determined after the training cutoff dates. AlphaFold3 (AF3), Bo

Combined elevated C-reactive protein and detectable plasma RNA has predictive value for hospitalization during an outpatient COVID-19 convalescent plasma clinical trial.

BACKGROUND: Laboratory biomarkers that predict progression from mild COVID-19 to hospitalization are not well defined. OBJECTIVE: To evaluate the performance of C-reactive protein (CRP) and plasma SARS-CoV-2 RNA for predicting hospitalization among mild COVID-19 outpatients enrolled in a randomized controlled trial of COVID-19 convalescent plasma (CCP). METHODS: CRP and plasma SARS-CoV-2 RNA were measured in 487 outpatient trial participants screened before March 2021. Weighted analyses accounting for sampling probabilities were used to estimate positive predictive values (PPVs) and sensitiv

Antibody Affinity Maturation by Yeast Mating against a Diverse Antigen Library Enhances Antibody Resistance to Viral Mutations.

Antibodies suffer from resistance mutations in their targets, which diminishes their efficacy. Most antibody library screens use a single antigen yet should ideally use many antigens to ensure that the hits have broad-binding capacity. As a test case, we used the receptor-binding domain (RBD) of SARS COV-2 and CR3022 as the starting antigen/antibody pair. We screened a CR3022 library against a large panel of RBDs (94) and isolated a hit (C8) that has improved affinity for 95/102 tested RBDs. C8 has improved affinity to variants unseen in the library screening, indicating that C8 is more resist

Searching for Novel COVID19 Treatments: Towards Covalent TMPRSS2 Inhibitors.

We present the design, synthesis and biophysical and biochemical evaluation of new guanidinium derivatives with potential as covalent inhibitors of Transmembrane Protease Serine 2 (TMPRSS2), aiming to find therapies against COVID-19. TMPRSS2 is a key protease for SARS-CoV-2 entry into host cells, making it a promising drug target since blocking viral entry, viral replication is stopped. We have designed five families of guanidinium derivatives with different scaffolds that would allow establishing covalent interactions with the target. The proposed structures were optimised for better binding

Expression of the SARS-CoV-2 nucleocapsid (N) gene in Chlorella vulgaris via electroporation-mediated transformation.

The microalga Chlorella vulgaris is a unicellular photosynthetic microorganism with the potential of serving as a host organism for foreign genes. Utilising C. vulgaris as a host organism to express the SARS-CoV-2 structural protein, such as the nucleocapsid (N) gene, is a sustainable approach with high scalability. Here, we genetically engineered C. vulgaris with the N gene of SARS-CoV-2. Initially, a recombinant plasmid, pCAMBIA1303 harbouring the SARS-CoV-2 N gene, was transformed into C. vulgaris via electroporation. Subsequently, the transgenic C. vulgaris was cultivated in BG11 medium co

Viral interference with SARS-CoV-2 during the COVID-19 pandemic: An individual participant data meta-analysis.

Viral interference between SARS-CoV-2 and other respiratory virus infections has been implicated, but symptom-based testing in observational studies may bias estimations. We used individual participant data from ten cohort studies conducted during pandemic years 2020-2024 to study virus interference. Respiratory samples were PCR-tested for viruses including SARS-CoV-2. The primary analysis was based on samples collected from cohort participants regardless of respiratory symptoms. A secondary analysis included only symptom-based testing data. We estimated odds ratios for co-occurrence of SARS-C

Intranasal immunization expressing S1 antigen can confer functionally sterile protection against an intranasal SARS-CoV-2 challenge in mice.

Currently approved COVID-19 vaccines have shown significant protective efficacy against disease onset and progression after SARS-CoV-2 infection. However, systemic immune responses induced by intramuscular vaccination have limited impact on protection of viral transmission. Intranasal vaccination is an important strategy for induction of mucosal immune responses for protection of viral transmission. Here, we investigated the potential of intranasal immunization expressing a SARS-CoV-2 spike (S) antigen to prevent SARS-CoV-2 infection in a mice model. Intranasal immunization with a Sendai virus

More Cases, Less Autonomy? The Paradox of Modern Orthopaedic Residency Training.

Orthopaedic residents may be logging more operations than prior generations, but concerns regarding their readiness for independent practice persist. This apparent contradiction reflects a limitation of case counts: participation does not measure autonomy, clinical ownership, judgment, or the ability to manage complications. The COVID-19 pandemic exposed the vulnerability of surgical training, but declining operative autonomy predates the pandemic. Simultaneously, reimbursement pressure, throughput demands, expanding team-based care, and increasing subspecialization have changed how residents

Time series evidence on air quality transitions under urban shock during COVID-19 shutdown in Delhi-NCR, India.

The COVID-19 pandemic and the associated nationwide lockdown provided an unprecedented natural experiment to investigate how abrupt reductions in anthropogenic activities influence urban air quality. Although several studies have quantified changes in pollutant concentrations during the lockdown, the nonlinear temporal transitions and persistence of these changes after reopening remain poorly understood. This study investigates the evolution of air quality across Delhi-NCR, India, one of the world's most polluted metropolitan regions, by examining three distinct phases associated with the COVI

Pulmonary malignant lymphoma requiring surgical lung biopsy for diagnosis following coronavirus disease 2019 infection: a case report.

BACKGROUND: Pulmonary involvement by peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS), is an exceedingly rare extranodal manifestation with nonspecific radiological and clinical features. Its computed tomography (CT) findings-including ground-glass and consolidative opacities and, in some areas, a crazy-paving appearance-overlap with those of resolving infectious pneumonia and drug-induced lung injury, so that pulmonary lymphoma is readily mistaken for these more common conditions and the diagnosis is often delayed. CASE DESCRIPTION: A 62-year-old man with a smoking history and

Human β defensin type 2 based novel antimicrobial peptide prediction using both molecular dynamics simulation and artificial intelligence methods.

The global health crisis triggered by SARS-CoV-2 has highlighted the urgent need for effective treatments. Although several anti-viral drugs targeting SARS-CoV-2 have been developed, there remains considerable interest in exploring natural antimicrobial peptides (AMPs) secreted by the human innate immune system as alternative therapeutic agents. Human β defensin type 2 (hBD-2) has been found to bind to the receptor-binding domain (RBD) of SARS-CoV-2 and block the virus entry. In order to design effective anti-virial drugs based on hBD-2, 32 block-mutants (mutation at more than one site simulta

Comparing two case definitions for including severe acute respiratory infection patients in influenza and COVID-19 vaccine effectiveness studies among older adults using the test-negative design: VEBIS hospital network, seven European countries, 2023/24.

INTRODUCTION: Two severe acute respiratory infection (SARI) case definitions (CDs) are often used in test-negative design (TND) hospital-based vaccine effectiveness (VE) studies. SARI-CD1 requires ≥1 symptom (cough, fever, shortness of breath, anosmia, ageusia, or dysgeusia); SARI-CD2 requires both fever and cough. Using VEBIS hospital network end-of-season 2023/24 data, we compared the effect of these CDs on influenza and COVID-19 VE estimates in Europe. METHODS: Using the TND, we restricted analysis to seven countries recruiting patients using SARI-CD1; we created a subset of SARI patients

How Swedish healthcare workers managed the early weeks of the COVID-19 pandemic: sensemaking across organisational, interpersonal and individual levels.

BACKGROUND: In times of crisis the need to make sense of the situation becomes particularly evident. The perspective of sensemaking captures how individuals seek to clarify and understand what is going on when they encounter ambiguous, unexpected or confusing events, such as the COVID-19 pandemic. Sensemaking may occur at different but mutually interrelated organisational, interpersonal and individual levels. During the COVID-19 pandemic, healthcare workers (HCWs) faced multifaceted challenges related to their work. This study aimed to explore how HCWs managed their daily work during the early
