Assessing Medication Confounders Before Determination of Brain Death/Death by Neurologic Criteria: A Primer, with Opioids as a Worked Example.

PURPOSE: To provide a primer on how clinicians, particularly pharmacists, can systematically assess whether a medication may confound determination of brain death/death by neurologic criteria (BD/DNC), using pharmacokinetic and pharmacodynamic principles that can be applied to any agent with the aid of tertiary references, and using opioids as a worked example. SUMMARY: All major guidelines require exclusion of central nervous system (CNS) depressant effect before BD/DNC determination, yet none provides a general method for deciding when a given medication has cleared. Rather than cataloging

Patient and clinical characteristics associated with general anesthesia for cesarean delivery at a Texas level IV maternal care center: a single-center retrospective study.

BACKGROUND: The primary aim of our study was to determine which patient and clinical characteristics were associated with general anesthesia (GA) for cesarean delivery (CD), and the secondary aim was to compare neonatal cord blood gas pH between patients who did and did not have GA for CD. METHODS: We used a retrospective observational study design. Patients who had CD at Baylor Scott & White Medical Center - Temple from July 1, 2022, to June 30, 2024, were included in the study. We defined GA as placement of an endotracheal tube or supraglottic airway. RESULTS: Of 1729 patients who had CD d

Successful systemic fibrinolysis in pulmonary embolism with a thrombus-in-transit.

Right heart thrombus-in-transit with acute pulmonary embolism (PE) is a rare but potentially lethal clinical finding. Optimal management remains controversial, particularly when major contraindications or risk factors coexist. We present a 72-year-old male admitted to the emergency room with leading diagnosis acute coronary syndrome who was later diagnosed with acute PE and a massive, mobile right heart thrombus along with a concomitant ascending aortic aneurysm. Due to high surgical risk, systemic fibrinolysis was performed under close monitoring. Follow-up echocardiography demonstrated compl

Sequencing-based molecular epidemiology of mesophilic Aeromonas in dairy environments and water systems.

This study investigated the prevalence, virulence gene distribution, antimicrobial resistance, and molecular epidemiology of mesophilic Aeromonas species from milk, dairy farm environments, and drinking water systems in Kashmir, India. Aeromonas prevalence varied between sample sources, with environmental samples having the highest detection rate (51.96%), followed by water sources (46.60%) and milk (36.15%). Following pasteurization and disinfection, prevalence in milk samples was significantly reduced, demonstrating the efficacy of hygiene measures in minimizing contamination. Higher detecti

Phytolectin Potentiates Copper Sulfide Nanoparticles Anticandida Activity Through Enhanced ROS Production.

Fungal infections caused by Candida albicans are challenging to treat with antifungal drugs due to the emergence of antimicrobial-resistant strains, necessitating the development of innovative therapeutic methods. The present study demonstrates that cationic copper sulfide nanoparticles (cCuS) synthesized using [2-((N-(2-hydroxyethyl)palmitamido)methyl)-1-methylpyridin-1-ium iodide] exhibit increased efficacy against C. albicans when delivered using Jacalin, a lectin from jackfruit that binds to cell surface glycans. The requirement of cCuS to kill was reduced twofold when delivered through Ja

Regulating Fe3+ Migration and AlO4-Associated Hydroxyl Evolution via Solid-State Ion Exchange for Enhanced Glycerol Ketalization over Y Zeolite.

Solid-state ion exchange (SSIE) method generates minimal wastewater and provides high ion-exchange rates, making it an emerging strategy for tuning zeolitic structures and properties. However, the mechanistic understanding involving ionic diffusion in zeolites remains unclear. In this work, we used Y zeolite as the proof-of-concept case, demonstrating that FeY catalysts prepared via the SSIE method exhibit superior catalytic activity for ketalization of glycerol and cyclohexanone. As one of the key findings, SSIE induces significantly less structural damage to the zeolite framework than ion ex

Phase Selectivity in LnSrCoO4 (Ln = La-Gd) Using Hydrogen-Free and Hydride Reduction Methods.

The n = 1 Ruddlesden-Popper perovskite LaSrCoO3.5-δHx has been explored as an oxygen ion conductor and catalyst due to its ability to contain both oxygen vacancies and anionic hydrogen. Here, we compare the phase selectivity and formation of LnSrCoO3.5-δHx from LnSrCoO4 (Ln = La, Pr, Nd, Sm, Eu, and Gd) using two distinct topochemical methods, CaH2 hydride reduction and the recently reported amalgam-based hydrogen-free reduction─AxAlGa, with A = Li, Na, and K─with a focus on the A = Na case in particular. These methods are coined as amalgam-based for their inspiration and similarities to sodiu

Exploring gender beyond labels: Uncovering diverse gender groups with latent profile analysis.

Gender is a variable of great interest to couples and family researchers who frequently use gender labels in individual, dyadic, and systems-level analyses. Labels are unsatisfactory representations of gender as a whole, as they may obfuscate nuanced gender experiences that impact health disparities for both transgender and cisgender respondents. However, there is a lack of consensus on how to model the multidimensionality of gender in quantitative analysis. The present study seeks to assess whether latent profile analysis can identify subgroups that could be used to observe patterns in dispar

The strain behind the smile: Temporal dynamics between parental burnout and emotional labor.

Parents routinely engage in emotional labor to meet caregiving demands by faking emotions (surface acting) or by genuinely cultivating expected ones (deep acting). The association between emotional labor and parental burnout is an emerging question, yet predominantly cross-sectional evidence leaves directionality and temporal dynamics unclear. We therefore conducted two studies spanning different parent samples and timescales. Study 1 applied models from the cross-lagged panel family to a four-wave design with 4-month intervals among 1,373 U.S. parents of young adolescents. From an individual-

[Local anesthetic systemic toxicity-Part 2: treat and prevent : Acute management including lipid rescue and preventive strategies].

In cases of local anesthetic systemic toxicity (LAST) early recognition, the immediate establishment of adequate oxygenation and hemodynamic stability and adherence to specific considerations regarding resuscitation are of critical importance. Intravenous lipid emulsion treatment remains a central component of therapeutic management in this context. Preventive measures aimed at avoiding LAST rely less on isolated interventions than on an integrative, multimodal concept that encompasses both human factors and procedure-specific risk reduction strategies. These measures include, among others, th

Green-synthesized Nanoparticles as Emerging Antidepressant and Anxiolytic Candidates: A Critical Review of Preclinical Evidence and Translational Prospects.

The high incidence of depression and anxiety disorders worldwide, with figures of depression > 300 million and anxiety > 264 million, poses a serious challenge to health and wellness. Treatment is still limited by existing pharmacotherapies with delayed onset, adverse side effects, poor entry into the blood-brain barrier (BBB), and resistance in approximately 30% of patients. Green-synthesized nanoparticles (NPs) obtained via phytochemicals, microorganisms, and biomolecules are a promising sustainable route for neurotherapeutic applications. This critical review explores preclinical studies on

Estimated plasma volume status (ePVS) as a prognostic marker in polytrauma: predicting 30-day mortality and coagulation support needs.

Estimated plasma volume status (ePVS), derived from a simple formula based on haematocrit and haemoglobin values, has been associated with an unfavourable prognosis in many cardiovascular conditions, demonstrating a correlation with volume overload and the haemodynamic status of the patient. Indeed, ePVS was demonstrated to estimate the degree of congestion and was independently associated with cardiovascular outcomes, rehospitalization, and death in patients with heart failure (HF), confirming its prognostic value also in inflammatory pathologies. Due to its easy and immediate availability ev

Context-dependent effects of social isolation and behavioural recovery in two populations of Lymnaea stagnalis.

Social context is a key modulator of behaviour and cognition across taxa, yet the effects of social isolation (SI) and subsequent recovery remain poorly characterised in invertebrate systems. Here, we developed a longitudinal behavioural procedure to examine how SI and social reintroduction (SR) influence feeding, exploration, and operant conditioning behavioural assays in the pond snail Lymnaea stagnalis. To capture biologically relevant variation, we compared two populations with differing environmental backgrounds: a long-established laboratory strain (W) and a "wild" population (TC1). Isol

Microplastic contamination in surface water and sediments of Lake Canaçari, Central Amazon, during the dry season.

Contamination by microplastics is an emerging environmental problem that is widely distributed across continental aquatic environments and may cause ecological impacts. This study evaluated the occurrence, concentration, and chemical and morphological characteristics of microplastics in surface waters and sediments from Lake Canaçari, comparing two areas with different land-use patterns within a floodplain system in the Central Amazon. Microplastics were detected in all sampled areas. In surface water, concentrations ranged from 0 (no microplastic particles detected in one sample) to 1,000 mic
