The Healer's Heartache: Reforming Medicine from the Rough Ground.

Current and future clinicians are experiencing a malaise that is at once deeply felt yet difficult to articulate. While "burnout" and "moral injury" have come to dominate many accounts, these terms do not fully capture the experience of clinicians who struggle to name a sense of loss, lament, and love for a beloved profession. Therefore, we propose "heartache" as a more fitting description. While burnout comes from the world of industry and moral injury from the world of war, heartache comes from the world of love. Burnout depends on individuals to cultivate resilience, and moral injury encour

Treatment of spinal tuberculosis- Evolution in last 50 years.

Extrapulmonary tuberculosis (EPTB), including bone and joint involvement, was historically considered an extension of pulmonary disease and managed empirically with uniform treatment protocols. Over the past five decades, this perception has undergone a major transformation with EPTB now recognized as distinct clinical entity accounting for 15-25% of all TB cases, frequently affecting children, women, and immunocompromised individuals, often without pulmonary involvement, and contributing significantly to long-term morbidity. Spinal tuberculosis (STB) represents a unique form of EPTB character

A Case of a Cardiac Re-Rupture Caused by the Removal of Pericardial Patches Late after a Sutureless Repair for a Cardiac Rupture.

INTRODUCTION: Surgical treatment is the only reliable therapeutic option for a left ventricular free wall rupture (LVFWR). There are 2 major surgical techniques, namely, sutureless repair and sutured repair; a ventricular pseudoaneurysm is a late postoperative complication. We herein report a case of a re-ruptured left ventricular pseudoaneurysm after a sutureless bovine pericardial patch repair. CASE PRESENTATION: The patient was a 79-year-old woman who underwent a sutureless pericardial patch repair for a blow-out-type left ventricular rupture following an anterior myocardial infarction 10

From biological scaffold to multifunctional bioplatform: research progress and applications of decellularized extracellular matrix.

Decellularized extracellular matrix (dECM) is a biomaterial generated by the selective removal of cellular components from native tissues while preserving the three-dimensional ultrastructure and endogenous biochemical cues of the extracellular matrix. Owing to its favorable biocompatibility, low immunogenicity, and capacity to mimic tissue-specific microenvironments, dECM has been increasingly explored across a wide range of biomedical applications. Its role has expanded from a conventional scaffold for tissue repair to a multifunctional biomaterial platform integrating 3D bioprinting bioinks

Metagenomic Tools as Predictive Approaches to Decipher Soil and Rhizosphere Microbial Communities and Their Functions for Sustainable Agriculture.

The increasing demand for resilient and sustainable food production necessitates a comprehensive understanding of rhizosphere microbial communities and their functional roles in enhancing plant health, nutrient acquisition, stress resilience, and crop productivity, thereby supporting food security and sustainable agriculture. However, soil microbiomes are composed of complex and unculturable communities of beneficial microorganisms that drive critical ecosystem functions, which traditional culture methods cannot capture. This limits the understanding of their roles and impacts on plant health

Decodification for dyslexia and broader reading difficulties.

UNLABELLED: Dyslexia is often addressed through specialised intervention, yet growing scholarship in literacy and educational psychology also underscores the importance of contextualised and meaningful literacy experiences for students with reading difficulties. In this article, I argue the relevance of Paulo Freire's concept of decodification for reading instruction for students with dyslexia and broader reading difficulties. Drawing on the literature on dyslexia, background knowledge, culturally relevant text selection and experiences of emergent bilingual learners, I argue that decodified t

Production and evaluation of a novel GABA-producing almond yogurt: dietary modulation of stress.

Gamma-aminobutyric acid (GABA) is an inhibitory neurotransmitter recognized for its antihypertensive, anxiolytic, neuroprotective, and cognitive-enhancing properties. Functional foods enriched with naturally produced GABA have emerged as promising dietary interventions to modulate the microbiota-gut-brain axis and promote mental well-being. In the current study, a novel GABA-enriched plant-based almond yogurt was developed using the psychobiotic strain Enterococcus faecalis RIT BMS15-3, a human breast milk-derived isolate, as the starter culture. To the best of our knowledge, this is the first

Metaphyseal cones in septic revision total knee arthroplasty: A retrospective cohort study with minimum 2-year follow-up.

PURPOSE: Metaphyseal cones improve fixation and address bone loss in revision total knee arthroplasty (rTKA), but their use in septic revision remains controversial because of theoretical concerns about biofilm formation on porous surfaces, and evidence confined to the septic setting is scarce. We aimed to describe infection eradication, implant survival and functional outcomes and the metaphyseal microbiological environment after cone implantation in septic rTKA. METHODS: This multicentre retrospective single-arm cohort study included consecutive patients undergoing septic rTKA with at least

Incidence, Process of Care, and Outcomes of In-Hospital Cardiac Arrest in China: A Multicenter Prospective Registry Study.

BACKGROUND: In-hospital cardiac arrest (IHCA) is an acute event associated with high mortality. Although survival has improved in high-income countries, contemporary multicenter data from low- and middle-income countries remain scarce, limiting global benchmarking and the identification of system-level opportunities for improvement. METHODS: This cohort study used data from the BASIC-IHCA (Baseline Investigation of In-Hospital Cardiac Arrest) registry. Adult patients (≥18 years) who experienced IHCA and received cardiopulmonary resuscitation between July 1, 2019, and December 31, 2020, at 37

Clonal isolation of Borrelia miyamotoi identifies multiple infection phenotypes and potential novel virulence determinants.

Borrelia miyamotoi is an emerging tick-borne pathogen in the Northern Hemisphere that causes a relapsing fever-like illness known as Borrelia miyamotoi disease. Despite increased recognition of B. miyamotoi as a human pathogen, the mechanisms underlying infection and pathology remain poorly understood. During routine cultivation of B. miyamotoi LB-2001, our lab observed that bacterial cultures extensively passaged in vitro (over 70 passages) lost the ability to establish infection in severe combined immunodeficiency (SCID) mice. Clonal populations were isolated from both high- and low-passage

Current laboratory diagnostics for tuberculosis: from conventional approaches to next-generation molecular and immunological technologies.

Tuberculosis (TB) remains a major global public health concern despite advances in diagnosis and treatment. Early and accurate diagnosis is essential for timely treatment initiation, interruption of transmission, and effective management of drug-resistant TB.Although microscopy and culture remain the cornerstones of TB diagnosis, their limited sensitivity and long turnaround times have driven the integration of molecular and immunological methods. This review summarizes current TB diagnostic approaches, including conventional methods, nucleic acid amplification tests (NAATs), line probe assay (

"My feet are no longer swollen": Testing a co-designed podoconiosis protective footwear among rural female farmers in Rwanda.

Podoconiosis is a disabling neglected tropical disease (NTD) caused by prolonged occupational exposure to alkaline red clay soil, disproportionately affecting female farmers in Rwanda. Although preventable through the consistent use of protective footwear, such as farming boots, high costs and limited availability at local markets impede rural farmers' access to appropriate footwear. Among female farmers, gender norms and cultural expectations further constrain their ability to access and wear farming boots. This qualitative community-based participatory project aimed to test a women-centered

Neutrophil-derived oncostatin m contributes to endothelial cell dysfunction during Treponema denticola interaction.

Periodontitis (PD) is a common chronic inflammatory condition and a risk factor for cardiovascular diseases (CVD), yet underlying linking mechanisms remain unclear. The cytokine Oncostatin M(OSM) is elevated in both PD and CVD and has emerged as a potential mediator linking oral inflammation to vascular dysfunction. Neutrophils represent a prominent source of OSM during PD and OSM production is elevated by the periodontal pathobiont Treponema denticola (Td). This study investigated the role of exogenous and neutrophil-derived OSM in endothelial cell (EC) dysfunction and the contribution of het

Words Matter: The Language of Leadership in Nursing Professional Development.

Every day, nursing professional development specialists shape culture through hundreds of conversations. They welcome new graduate nurses, coach experienced clinicians, facilitate difficult debriefings, mentor emerging leaders, and provide reassurance during moments of uncertainty. Although these conversations may seem routine, each represents a leadership intervention. Every word has the potential to build confidence, strengthen trust, encourage learning, or unintentionally diminish it.

Embryo-derived extracellular vesicle miRNA cargo reflects developmental competence in porcine in vitro-produced embryos.

Extracellular vesicles (EVs) have emerged as important mediators of intercellular communication during embryo development and represent a promising source of non-invasive biomarkers of embryo developmental competence. This study characterized for the first time EVs released by porcine in vitro-produced embryos and compared the microRNA (miRNA) cargo of EVs derived from blastocysts (BL) and non-blastocysts (N-BL) to identify molecular signatures associated with developmental competence. Porcine embryos were produced by standard in vitro maturation, fertilization, and culture procedures. On day
