Factors Influencing Maternal and Neonatal Mortality in South Asia and Southeast Asia (2010-2023): A Comparative Panel Data Analysis.

Background Maternal and neonatal mortality remains a critical public health challenge in low- and middle-income countries, particularly in South Asia and Southeast Asia. Despite progress, regional disparities persist, and the influence of socioeconomic and health system factors varies across contexts. This study aimed to comparatively assess determinants of maternal mortality ratio (MMR) and neonatal mortality rate (NMR) across these regions. Methods An ecological longitudinal panel study was conducted using data from the World Bank for 19 countries (2010-2023). Outcomes included MMR and NMR,

Assessing disaster-smart school governance to advance an integrated evaluation framework.

UNLABELLED: This research examined the governance factors influencing disaster-smart school governance (DSSG) and suggests a comprehensive evaluation framework to enhance resilience within educational systems. Despite global commitments outlined in the Sendai Framework and the comprehensive school safety framework, governance outcomes in school-based disaster risk reduction (DRR) remain inconsistent, primarily due to fragmented institutional capacity and a lack of empirical clarity regarding the significance of various governance dimensions. The research employed a quantitative design using pa

Non-evacuation to cyclone shelters and the imperative of implementing disaster-resilient community-based housing and shelter plans.

UNLABELLED: Despite considerable progress in cyclone monitoring, early warning dissemination, evacuation planning and strengthening community confidence in alerts, one unresolved issue remains: why many residents opt not to evacuate during life-threatening events. A wide body of research has explored the factors influencing the hesitation of Bangladesh's coastal communities to seek refuge in cyclone shelters during emergencies. Drawing on primary data obtained through in-depth interviews, focus group discussions (FGDs), participant observation and field visits, this study examines residents' c

Delisting Strategies in Highly Sensitised Kidney Transplant Candidates.

HLA antigen delisting has recently emerged as a new strategy to expand transplant opportunities in the population of broadly sensitised patients who have poor chance to receive a transplant offer through allocation systems due to present donor specific anti-HLA antibodies (DSA). Delisting involves removing selected HLA antigens from the unacceptable antigen list when are considered less immunogenic and/or the corresponding antibodies are considered lower risk, thereby increasing access to transplant offers. Current approaches recommend a stepwise strategy, initially targeting lower-risk antibo

Early Lower Pulmonary Arterial Diastolic Pressure Monitored With Pulmonary Arterial Catheters Is Associated With Improved Survival During VA ECMO for Patients With Acute Cardiogenic Shock: A Propensity-Matched Study.

BACKGROUND: Venoarterial (VA) extracorporeal membrane oxygenation (ECMO) is used to support refractory cardiogenic shock (CS) but may increase peripheral afterload. Invasive hemodynamic measurement and cardiopulmonary congestion during VA ECMO may correlate with survival, but the precise mechanisms underlying this benefit are uncertain. OBJECTIVES: The aim of this study was to determine whether invasive hemodynamic monitoring with a pulmonary arterial catheter (PAC) and early on-support pulmonary artery diastolic pressure (PAD) are associated with survival during VA ECMO. METHODS: Adult pati

Building Moral Community in an Age of Polarization: A Model for Reconciling Rights of Conscience in Healthcare Organizations.

Responding to conscientious objection and participation declarations to various morally contentious medical services requires a careful, inclusive, and comprehensive approach in reconciling the right of conscience of involved parties. In this paper, we make a case that the traditional discourse of whether or not to accommodate conscience-based requests does not adequately support complex, team-based healthcare systems in addressing emerging areas of moral controversy in medicine, particularly in this era of polarization. We describe how key parties in our organization developed an inclusive an

Cooperation in binary mixtures of active colloids.

The complex interactions underlying collective motion in biological systems give rise to emergent behaviors such as flocking, sorting, and cooperative transport. These dynamics often involve species with different motilities coordinating movement to optimize navigation and survival. Synthetic analogues based on active colloids offer a controlled platform to explore such behaviors, yet most experimental realizations remain limited to monodisperse systems or mixtures of passive and active particles. Here, we investigate dense binary mixtures of active Janus colloids with two different metal caps

Pediatric Rural Interfacility Transfers by Emergency Medical Services.

OBJECTIVES: Interfacility transports (IFTs) are critical for the care of pediatric patients in the United States (US), particularly in rural areas that may have limited pediatric specialty resources. Given the paucity of data describing rural pediatric IFTs, we aimed to characterize pediatric rural IFTs in the US. METHODS: We conducted a retrospective, observational study using the 2023 National Emergency Medical Services Information System dataset. Rural IFTs involving patients aged >1 day to <18 years were included and stratified into the following age groups: infant (1 day to <1 year), tod

Probabilistic T-spherical hesitant fuzzy decision support for selecting advanced tertiary wastewater treatment technologies using comprehensive distance based ranking method.

Pharmaceuticals, microplastics, per- and polyfluoroalkyl substances (PFAS) and endocrine-disrupting compounds are increasingly discharged in treated wastewater, are not effectively removed by conventional secondary treatment, and have important implications for the environment and human health. Advanced tertiary treatment is needed for safe reuse of urban non-potable water, but the choice among competing technologies is a complex decision involving technical, economic, environmental and regulatory considerations, as well as uncertainty and reluctance in expert evaluations. To address this issu

Knowledge, attitudes, and practices of livestock veterinarians on antibiotic use and resistance: a cross-sectional study.

Antimicrobial resistance (AMR) is a pressing global health threat that requires integrated efforts under the One Health approach. Together with human medicine and agriculture, veterinary medicine is key to mitigating AMR emergence and spread. Veterinarians work at the forefront as public health agents and their antibiotic prescribing decisions may be influenced by several factors. This cross-sectional study aimed to assess the knowledge, attitudes, and practices (KAP) of livestock veterinarians regarding AMR and antibiotic use. A questionnaire-based study was conducted in Portugal, from Septem

Psychophysical Characterization of Osseoperception Across Upper-Limb Bony Landmarks.

Somatosensory feedback is essential for fine motor control, yet it remains a challenge to restore it in assistive technologies such as prostheses or exoskeletons. This study investigates osseoperception-the perception of vibrotactile and auditory sensations through bone conduction- a feedback approach that has unique advantages but is rarely considered in literature. We have systematically assessed the psychometric properties of this method across four anatomical sites: the pisiform bone (PB) at the wrist, the olecranon (OL) at the elbow, the clavicle (CL) at the shoulder, and the mastoid proc

From danger to resolution: targeting purinergic signaling in CNS diseases.

The central nervous system (CNS) is a dynamic immunological environment, where specialized interfaces, immune reservoirs, and metabolic clearance pathways, such as the glymphatic system, orchestrate immune surveillance and homeostasis. Within this intricate network, purinergic signaling, mediated by nucleotides like ATP and adenosine, emerges as a critical regulator of immune responses, inflammation resolution, and tissue repair. Adenosine, drives immunosuppression and immune exhaustion, shaping the balance between neuroprotection and neurodegeneration. Purinergic signaling influences a divers

Management of dental-origin gastrointestinal foreign bodies: a proposed site-based, risk-stratified framework for multidisciplinary care.

OBJECTIVES: To propose a risk-stratified, site-specific framework tailored to the biomechanical hazards of dental-origin gastrointestinal foreign bodies (DGIFBs). MATERIALS AND METHODS: We conducted a narrative review with expert consensus-based synthesis, evaluating leading gastroenterological and surgical consensus statements on aerodigestive and gastrointestinal emergencies. Dedicated guidelines for DGIFBs are currently scarce. Consequently, we systematically extrapolated management principles for sharp, complex objects and integrated them with clinical data on dental device complications.

Early fixation of traumatic mandible fractures is associated with decreased morbidity and resource utilization.

BACKGROUND: There is no clear guidance on when isolated mandibular fractures should undergo surgical fixation. Multiple studies have assessed the timing of mandibular fracture repair, with a consensus that earlier repair does not change outcomes. We aimed to challenge this consensus and identify a data-driven time for surgical fixation. We hypothesized that delayed fixation would be associated with worse outcomes. METHODS: Data on adult patients with closed mandible fractures who underwent surgical fixation were abstracted from the Trauma Quality Improvement Program database from 2017 to 2023

Navigating global health futures under the Shared Socioeconomic Pathways.

The Shared Socioeconomic Pathways (SSPs) provide valuable insights to understand how future socioeconomic changes could alter the magnitude and pattern of climate-related risks across sectors, which might facilitate or hinder adaptation and mitigation responses. Many sectors have created SSP narrative extensions to specify insights for their domains. In this Viewpoint, we created health narrative extensions for the SSPs, considering socioeconomic and demographic drivers and population health and health-system drivers. The projected health-relevant trajectories diverge substantially over the co
