Promoting Older Adults' Participation in Fall Prevention Activities.

Falls are a leading cause of injury and death among older adults, yet a persistent gap exists between available evidence-based prevention strategies and their real-world uptake. Social determinants of health shape fall risk as well as the accessibility and effectiveness of prevention interventions. Cohort evidence demonstrates that disparities in outcomes are intersectional and emerge at the community level, challenging current risk assessment tools and one-size-fits-all prevention approaches. Closing this gap requires further research, validated equity-informed assessment tools, and intervent

Ursolic acid: context-dependent redox activity on oxidative stress and its toxicological implications.

Oxidative stress is a major contributor to xenobiotic-induced tissue injury, mitochondrial dysfunction, inflammation, ferroptosis, and carcinogenesis. Ursolic acid (UA), a naturally occurring pentacyclic triterpenoid, has emerged as a context-dependent regulator of redox homeostasis. This review summarizes current evidence regarding the dual effects of UA on oxidative stress and its toxicological implications. Under non-malignant conditions, UA generally exerts antioxidant and cytoprotective effects in the majority of experimental models examined. Reduced reactive oxygen species (ROS) accumula

From Participant to Passenger: Workers' Experiences of Recovery During a Workers' Compensation Claim.

BACKGROUND: Social insurance systems provide essential financial support to cover living expenses and medical treatment needs during times of work incapacity. However, many ill and injured workers experience the administrative processes involved in accessing these systems as difficult to navigate, which can stifle recovery and slow return to work. The present study centres workers' lived experiences to better understand facilitators and barriers to recovery, and to identify solutions grounded in workers' perspectives. METHODS: Twenty Australian workers with work-related injuries, health, and

Occupational Therapy Referral Pathways for Upper Limb Conditions in Tygerberg, South Africa: Stakeholder Perspectives.

Effective referral pathways to occupational therapy are essential for continuity of care and recovery following upper limb injury or condition. Although challenges within South Africa's public healthcare referral systems are recognised, patients' experiences remain underexplored. This study explored patients' experiences of occupational therapy referral pathways within the Tygerberg substructure and identified factors influencing access to care. A qualitative descriptive design was used, with semistructured interviews conducted with seven purposively sampled participants. Data were analysed th

Integrated bioinformatics analysis uncovers TNFSF10 as a key panoptosis-related gene in diabetic tubular injury.

Diabetic kidney disease (DKD) is a major cause of chronic kidney diseases. Panoptosis has emerged as a key contributor to renal injury. However, the specific panoptosis-related genes (PRGs) that drive diabetic tubular injury remain unknown. TNFSF10, a known regulator of apoptosis and inflammation, has been linked to podocyte injury in DKD, but its role in diabetic tubular injury remains unknown. To address this, we integrated weighted gene co-expression network analysis, machine learning algorithms, and clinical data from the Nephroseq database to identify PRGs associated with DKD. The biologi

Tetrahedral Framework Nucleic Acid-Based Small RNA Delivery: Prospects and Challenges in Bone and Cartilage Regeneration.

Bone and cartilage defects represent major clinical challenges, with an increasingly urgent need for effective repair and regeneration. In recent years, small RNA-based regulatory strategies, such as microRNAs (miRNAs) and small interfering RNAs (siRNAs), have emerged as promising candidates for promoting bone and cartilage regeneration. However, achieving efficient and safe targeted delivery remains a critical challenge. Tetrahedral framework nucleic acids (tFNAs), a type of three-dimensional DNA nanomaterial with precise structure and excellent biocompatibility, serve dual functions as both

The Role of Mast Cells and Their Potential as a Therapeutic Target For Neuroinflammation.

Neuroinflammation is increasingly recognized as a central mechanism underlying the onset and progression of numerous neurological disorders, including neurodegenerative, autoimmune, and cerebrovascular diseases. Although microglia and astrocytes are traditionally considered the primary mediators of inflammatory responses within the central nervous system (CNS), accumulating evidence has identified mast cells (MCs) as important contributors to neuroimmune regulation. Strategically located at the interface between the peripheral immune system and the CNS, MCs can rapidly respond to a variety of

Causes of Admission, Prescribing Patterns, and Outcomes Among Trauma Patients Visiting the Department of Trauma and Emergency at a Tertiary Care Hospital in Bihar, India: A Cross-Sectional Study.

Introduction Trauma is one of the most common, preventable causes of mortality globally. Its burden is rising, particularly in low- and middle-income nations, and hence, it is being acknowledged as an epidemic. The factors responsible for the high burden of trauma and trauma-related morbidity and mortality in resource-limited countries need to be identified and quantified. Additionally, there is a paucity of data from our setting on prescribing patterns among trauma victims. We, therefore, aimed to evaluate the burden, attributes, and outcomes of trauma as well as the prescribing patterns amon

Oral Contraceptives, Concussion History, and Contact-Sport Exposure: Multimodal MRI and Brain Injury Biomarkers in Female Athletes.

Neuroimaging and blood-based biomarkers are increasingly utilized to identify long-term effects of concussion and contact-sport exposure (CSE), but female athletes are underrepresented in this work, despite potential sex-related differences. Early research indicates that widely used oral contraceptives (OCs) influence brain structure and function and concussion-related biomarkers. This study investigated whether current oral contraceptive (OC) use moderates associations of concussion history and CSE with magnetic resonance imaging (MRI) measures and brain injury biomarkers in female athletes.

Proposal to refine the mechanistic classification of chronic pain and corresponding clinical symptomatology.

The management and treatment of patients suffering from chronic pain must be based on a precise understanding of the pathophysiological mechanisms underlying the syndrome. However, it cannot be defined solely by the presence or absence of disease or injury affecting either non-neuronal tissues or the neuronal structures of the somatosensory system, as these elements do not always accurately or causally reflect the underlying mechanisms of the pain experienced by patients. The current taxonomy of chronic pain classically includes three main mechanisms: nociceptive, neuropathic, and nociplastic.

Acute External Iliac Artery Occlusion After Posterior Total Hip Arthroplasty: A Case Report.

Acute limb ischemia is a rare but potentially limb-threatening complication following total hip arthroplasty. Although vascular injuries have been described in the literature, proximal arterial occlusion at the level of the external iliac artery after a primary posterior total hip arthroplasty, to our knowledge, has not been previously reported. We present the case of a 74-year-old woman who developed acute limb ischemia approximately 3 h after undergoing an primary posterior total hip arthroplasty without any intraoperative complications. On postoperative examination, she was noted to have di

When Stroke Is Not a Stroke: A Case Report of Crimean-Congo Hemorrhagic Fever Presenting With Focal Neurological Deficits.

Crimean-Congo hemorrhagic fever (CCHF) is a potentially fatal tick-borne viral zoonosis with a wide geographic distribution, a significant case-fatality rate, and a risk of healthcare-associated transmission. Although CCHF commonly presents with nonspecific febrile illness followed by hemorrhagic manifestations, neurological symptoms may occur and can mimic acute stroke, creating diagnostic challenges in the emergency department. We report the case of a 72-year-old male farmer in the United Arab Emirates who presented with acute slurred speech, confusion, and right-sided facial droop, promptin

Clinical immunogenicity in rAAV gene therapy: Insights and implications.

Recombinant adeno-associated viruses (rAAV) provide prolonged therapeutic protein expression in multiple target organs after single administration, delivering significant clinical benefits with a well-characterized safety profile, and can be further supported by active immune management strategies. Systemic rAAV gene therapy is associated primarily with transient hepatocellular injury, while complement-mediated thrombotic microangiopathy and immune-related cardiac events occur in specific contexts; DRG-associated neurotoxicity remains predominantly nonclinical with only isolated suspected huma

Iron-Driven Ferroptosis in Diabetic Kidney Disease: From Molecular Mechanisms to Targeted Therapeutic Translation.

Diabetic kidney disease (DKD) remains a leading cause of end-stage renal disease worldwide, and current standard treatments cannot fully halt its progressive decline in renal function. Dysregulated iron metabolism and its downstream induction of ferroptosis - an iron-dependent, lipid peroxidation-driven form of regulated cell death - have now been recognized as a key pathogenic driver of renal injury in DKD. This narrative review systematically synthesizes up-to-date evidence on how aberrant iron uptake, storage and export in renal cells trigger ferroptosis, disrupt antioxidant defenses, and p

Electroactive biomaterials modulating ion channels in nervous system regeneration.

Traumatic injury to the central and peripheral nervous systems disrupts the electrophysiological microenvironment through persistent ionic dysregulation and maladaptive ion channel remodeling, creating conditions that are fundamentally hostile to endogenous repair. Conventional biomaterials, while offering structural support, remain electrically inert and unable to reconstitute the bioelectric signaling context essential for functional neural regeneration. Here, we review the emerging class of electroactive biomaterials, encompassing conductive and piezoelectric platforms that are engineered t

Acute type B aortic dissection after ruptured thoracoabdominal aortic aneurysm repair resulting in thoracoabdominal branch endoprosthesis collapse and malperfusion.

We report the case of a patient who developed an acute Stanford type B aortic dissection 4 days after thoracoabdominal branch endoprosthesis repair for a ruptured thoracoabdominal aortic aneurysm. The dissection caused complete endograft collapse and profound malperfusion. Emergent salvage endovascular repair successfully re-expanded the graft; however, the patient ultimately died because of the severity of the systemic injuries. This case highlights the potential for acute type B aortic dissection to occur in a recently stented thoracic aorta segment following thoracoabdominal branch endopros

Carbon dioxide as a promising alternative to iodinated contrast media for lower extremity endovascular procedures (Review).

Carbon dioxide (CO2) angiography has emerged as an alternative contrast agent for arterial endovascular procedures, particularly in patients at increased risk of complications from iodinated contrast media (ICM). The present review evaluates the clinical circumstances in which carbon dioxide (CO2) angiography can reduce the use of iodinated contrast media (ICM) during arterial endovascular procedures, with particular attention to renal-risk reduction, image quality, anatomical limitations, and procedure-related complications. A narrative literature review was performed using PubMed, EMBASE, th

Reintubation in the ICU: A Prospective Observational Study of Its Impact on Recovery Outcomes and Mortality.

Background Extubation is an important step in the recovery of critically ill patients, but some patients deteriorate after removal of the endotracheal tube and require reintubation. Extubation failure can prolong intensive care and is associated with poor outcomes. This study examined the factors associated with reintubation and the clinical outcomes of patients who failed planned extubation. Methods We prospectively followed adult patients undergoing planned extubation in the adult ICU of a tertiary care hospital between June 2025 and June 2026. Patients were observed for 72 hours after extub
