Wildlife-associated ecosystems in Algeria harbor internationally distributed ESBL-producing clones and plasmids, including a CTX-M-15 Klebsiella pneumoniae ST17 lineage associated with a conserved IncFIB(K) resistance plasmid.

UNLABELLED: Expanded-spectrum cephalosporin-resistant Enterobacterales (ESC-R-E) have disseminated beyond clinical settings, but their occurrence and genomic epidemiology in wildlife remain poorly characterized, particularly in low- and middle-income countries. ESC-R may result from the production of extended-spectrum β-lactamases (ESBLs) or AmpC-type cephalosporinases. We investigated the prevalence, resistance mechanisms, and population structure of ESC-R-E in free-ranging wildlife, captive wild animals, and zoo workers in Algeria. Between October 2021 and June 2023, 1,899 fecal samples were

Activity of oral β-lactams, tebipenem and ceftibuten, combined with novel oral β-lactamase inhibitors against broad-spectrum β-lactamase-producing Escherichia coli.

Oral ceftibuten and tebipenem, alone or combined with avibactam, ledaborbactam, or xeruborbactam, were evaluated against recombinant Escherichia coli producing 57 acquired β-lactamases. Tebipenem showed greater intrinsic activity than ceftibuten against extended-spectrum β-lactamase (ESBL) and AmpC producers. Whereas avibactam and ledaborbactam remained inactive against metallo-β-lactamases, xeruborbactam restored activity against most class B producers. Tebipenem-xeruborbactam demonstrated the broadest inhibitory spectrum and the lowest MIC values overall. This combination may represent a pro

Mechanisms of synergistic action between olaparib and KPT-330 in pancreatic cancer.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) is a highly aggressive and prognostically poor cancer driven by multifaceted mechanisms, notably immune evasion and drug resistance. The majority of PDAC patients present with unresectable locally advanced or metastatic disease, emphasizing a critical unmet need for effective therapies against advanced-stage pancreatic cancer. Despite the proven efficacy of the PARP inhibitor Olaparib against pancreatic cancer in patients carrying BRCA mutations, its practical use is limited by the development of acquired resistance. The clinical application

O-GlcNAcylation Regulation of SNAP29-Dependent Autophagy Activation Dictates Chemoresistance in Gastric Cancer.

Chemoresistance remains a major obstacle in gastric cancer (GC) treatment, particularly to standard fluoropyrimidine and platinum regimens. O-GlcNAc transferase (OGT) and O-linked β-N-acetylglucosamine modification (O-GlcNAcylation) are frequently elevated in cancers, yet their specific role in GC chemotherapy remains poorly defined. Contrary to its typical oncogenic function, this study demonstrates that low OGT/O-GlcNAcylation is associated with resistance to 5-fluorouracil (5-FU) or oxaliplatin (OXA) in GC. Clinically, high OGT expression correlates with better prognosis in patients receivi

Progress in the Mechanism of Heat Shock Proteins in Gastric Cancer: From Biomarkers to Therapeutic Targets.

BACKGROUND: Gastric cancer remains a major challenge in public health, with low early diagnosis rates and poor prognosis for advanced patients. Heat shock proteins (HSPs) are highly conserved molecular chaperones that maintain proteostasis and promote cell survival under stress. Aberrantly elevated HSPs in gastric cancer are closely linked to tumor initiation, invasion, metastasis, and poor prognosis. This review aims to comprehensively synthesize the latest advances concerning HSPs in gastric cancer across three core dimensions: their utility as diagnostic/prognostic biomarkers, their regulat

Tumor-Associated Mast Cells Drive Immunosuppression and Resistance to Anti-PD-1 Therapy in Hepatocellular Carcinoma Through CCL17/CCR4-mediated Regulatory T-Cell Recruitment.

This study investigated how tumor-associated mast cells (TAMCs) contribute to immunosuppression and anti-PD-1 resistance in hepatocellular carcinoma (HCC). TAMC infiltration as well as the levels of stem cell factor (SCF) and various chemokines were measured in clinical specimens obtained from 59 HCC patients. The recruitment and maintenance of TAMCs by HCC-conditioned medium were examined, with a focus on the role of SCF in these processes. In addition, the chemotactic effect of CCL17 (secreted by activated TAMCs) on CCR4-expressing regulatory T cells (Tregs) was evaluated. The functional imp

Prevalence of Y93H resistance-associated substitution in treatment-naïve hepatitis C virus genotype 1b patients: a systematic review and meta-analysis.

BACKGROUND: Hepatitis C virus (HCV) genotype 1b infection remains a leading cause of chronic liver disease despite advances in direct-acting antivirals (DAAs). Resistance-associated substitutions (RASs) in the non-structural 5 A (NS5A) region, particularly the Y93H mutation, have been associated with reduced antiviral susceptibility and variable treatment outcomes. The global baseline prevalence of this substitution in treatment-naïve populations has not been systematically characterized. METHODS: A systematic review and meta-analysis were conducted in accordance with PRISMA 2020 guidelines a

TRIM22 in cancer: a context-dependent E3 ligase at the interface of interferon, p53, and tumor plasticity.

TRIM22 is a TRIM family E3 ubiquitin ligase whose expression is strongly induced by interferons. For many years, it has been identified primarily as an antiviral effector, a role that remains well validated. A growing body of cancer-related work now forces us to expand that view considerably. Depending on tumor type, upstream signaling context, and the specific substrates available in a given cell, TRIM22 can either constrain malignancy or actively promote it. In breast, colorectal, ovarian, endometrial, hepatocellular, cervical, and gastric cancers, as well as in certain lung tumor subsets, T

The bacteriology of fallen leaves (Phylloplane) from council parks and green spaces in Northern Ireland: Clinical significance for at risk people living with cystic fibrosis (CF).

BACKGROUND: People living with cystic fibrosis (CF) may suffer from chronic respiratory infections due to acquired environmental organisms, ultimately leading in increased morbidity and mortality. Consequently, parents of children living with CF frequently ask their CF healthcare professional about sources and potential routes of transmission of environmental organisms to their child. This allows parents to make an informed decision regarding risk assessment/risk management of outdoor/ nature-based activities. Mitigations can then be put in place to help minimise or absolutely avoid exposure o

Changes in the microbial spectrum and antimicrobial resistance of bloodstream infections in intensive care units before and during the COVID-19 pandemic.

BACKGROUND: Bloodstream infections (BSIs) are major infectious conditions associated with high morbidity and mortality, particularly in intensive care units (ICUs). In critically ill patients, early and appropriate empirical antimicrobial therapy plays an important role in preventing adverse clinical outcomes. However, the increasing burden of antimicrobial resistance necessitates that empirical treatment strategies be continuously reassessed based on up-to-date local resistance data. This study aimed to compare the distribution of bacterial pathogens and antimicrobial resistance profiles of B

Mycobacterium tuberculosis complex Lineage 8 causes drug resistant pulmonary tuberculosis also in South Kivu, Eastern Democratic Republic of Congo.

BACKGROUND: Lineage 8 (L8) occupies a unique phylogenetic position as a sister clade to all other known Mycobacterium tuberculosis complex (MTBC) lineages. Although the initial identification of L8 strains was limited to Rwanda and Uganda, the complete geographical distribution of this lineage remains undetermined due to limited genomic surveillance across eastern and central Africa. METHODS: We performed whole genome sequencing (WGS) on MTBC isolates from rifampicin-resistant tuberculosis (TB) patients' sputum in Bukavu, South Kivu province, eastern Democratic Republic of Congo (DRC). High-throughput screening to identify inhibitors of the MTB CARD•RNA polymerase protein-protein interaction.

In 2023, tuberculosis (TB) once again became the world's deadliest infectious disease, overtaking COVID-19 as the leading cause of death from a single infectious agent, Mycobacterium tuberculosis (MTB). The World Health Organization (WHO) reports that the COVID-19 pandemic has reversed years of progress in providing essential TB services and reducing TB disease burden. In 2025, TB was responsible for an estimated 1.23 million deaths globally (95% uncertainty interval: 1.13-1.33 million), and there were 400 K cases of rifamycin resistant/multidrug resistant TB (RR, RIFR, MDR). While treatment s

Osteosarcoma Nanomedicine: From Resistance Reversal to Bone Reconstruction.

The clinical management of Osteosarcoma (OS) is primarily hindered by two major challenges: intractable multidrug resistance and the reconstruction of critical-sized post-surgical bone defects. Nanomedicine provides a promising paradigm to overcome these clinical limitations. Synthesizing over 100 pivotal studies published predominantly between 2021 and 2026, this review moves beyond treating OS nanotherapy and bone tissue engineering as independent fields, conceptualizing them instead as a contiguous clinical pipeline. We map the trajectory of nanoscale interventions based on OS-specific biol
